1.The Effect of Eating Habits and Nutrient Intake on the Physical Growth Indices in Preschool Children.

The purpose of this study was to estimate the nutrient intake, dietary habits, and body indices among preschool children. The study subjects were 166 kindergarten children, aged 4 to 6 years. A measurement of the weight, height, chest circumference, and head circumference of the children was conducted. The general home environment and factors related to the eating habits of children were collected using a questionnaire that included information about physical activity, outdoor playing time, television watching, family income, and parents education and occupations. Underweight, overweight were defined based on a value of less than 110 and more than 140 on the Rohrer Index, or less than 90 and more than 120 on the weight-length index (WLI). The average Rohrer and WLI for the preschool children were 144.3 +/- 23.2% and 104 +/- 13.5%. Using the WLI, 12.7% of the preschool children were underweight, 62.6% were normal, and 24.7% were overweight or obese. On the Rohrer Index, 32.0% of the preschool children were underweight, 34.7% were normal, and 34.1% were overweight or obese. With regard to frequency of regularity of eating breakfast, 2.5% of preschool children skipped breakfast every morning. The risk of being overweight increased in preschool children who had faster eating times. The Rohrer and WLI were negatively related to faster eating times. The average daily energy intake was 1272 kcal, which corresponded to 79.5% of the Korean RDA. In particular, the average intakes of calcium, iron, and vitamin B2 were much lower than the Korean RDAs for each of those nutrients. Children whose mothers had occupations were at greater risk of decreased nutrient intake than those whose mothers were at home. The data presented in this study confirm that, in preschool children, undernutrition remains the nutritional problem of great concern in Korea, even though Koreans are starting to have worrisome rates of overweight. Therefore, during the early years of life, focus should remain on sustaining proper growth and development.

2.Dementia Mortality and Related Clinical Factors at a Nursing Home.

OBJECTIVE: This study was designed to evaluate the cause of mortality in patients with dementia, including Alzheimer's disease and multi-infarct dementia, and those without dementia. METHOD: Hospital records of geriatric patients (> or =65) in nursing home having dementia were reviewed retrospectively for 6 years. The total number of patients was 351 (the number of demented patients was 146 and those without dementia was 68). Of the ones with dementia, 146 had dementia of Alzheimer's type, 101 patients had vascular dementia, and 36 patients had non-specific dementia. This study focused on the different complications associated with Alzheimer's disease and cerebrovascular dementia. RESULT: 1) The average age at death of demented patient is higher than the non-demented population;however, there is no difference in age between Alzheimer's disease and vascular dementia. 2) There is no difference in time spent in the nursing home among the non-demented group, Alzheimer's disease and vascular dementia group from admission to death. The life expectancy of male patients with Alzheimer's disease after admission to the nursing home is significant shorter than their female counterpart. 3) Patients with vascular dementia have a higher incidence of hypertension, atherosclerotic disease, and diabetes than the Alzheimer's patients. 4) The main overall causes of the death (in nursing home patients) are infection and sepsis. In Alzheimer's disease's, sepsis is the major cause of death, while in vascular dementia, atherosclerotic disease is the number one cause. CONCLUSION: Knowledge of the causes of the death in demented patients may help the management of the possible complications associated with the different types of dementia. However, long-term research is then needed to analyze the survival rates of demented nursing home patients.

Background:
Nurses play a central role in the process of blood transfusion because they have the closest interactions with patients. Providing nurses with the appropriate knowledge and competency through education can help ensure transfusion safety. This study investigated the effectiveness of the first nationwide transfusion-related education for nurses by analyzing questionnaires for a self-assessment of competency on transfusion-related knowledge before and after the education and general evaluation for the educational program.
Methods:
The education program was composed of four lectures and was conducted in seven regions in South Korea. One hundred and ninety-two nurses participated, and the questionnaires for 170 nurses were analyzed.
Results:
The participants consisted of 90 nurses (53.0%) from tertiary hospitals, 23 (13.5%) from general hospitals, and 57 (33.5%) from other hospitals. The majority of the participants (103/170, 60.6%) were from hospitals with ≥500 beds, and 69.4% had a work period of ≥10 years. The scores for pre-/post-education self-assessment of competency were as follows: blood components, 3.03/3.73; pretransfusion testing, 2.86/3.64; management of transfusion, 3.18/3.84; and transfusion reactions, 3.11/3.78. In all categories, there was a significant increase in the score after the education program. The majority of participants (99.4%) provided a positive response regarding the necessity of a transfusion-related education program.
Conclusion
South Korea’s first attempt at transfusion-related education for nurses showed a strong positive effect by improving participants’ transfusion-related competency. Considering the important role of nurses in blood transfusion, the educational program should continue and be expanded in the future.

5.Test-Retest Reliability of Word Recognition Score Using Korean Standard Monosyllabic Word Lists for Adults as a Function of the Number of Test Words.

BACKGROUND AND OBJECTIVES: The purpose was to establish the test-retest reliability of word recognition score (WRS) using Korean standard monosyllabic word lists for adults (KS-MWL-A) recently developed based on the international standard for speech audiometry (ISO 8253-3:2012). SUBJECTS AND METHODS: Subjects consisted of 159 adults aged to 18 to 25 years with normal hearing sensitivity. WRSs were obtained in 2 dB steps from the level of speech recognition thresholds to the level of 86% correct responses or greater. After one or two weeks, retest was performed. Correlation, confidence interval (CI) and prediction interval (PI) were calculated for the reliability. RESULTS: Correlation coefficients were 0.88 for 50 test words, 0.76 for 25 and 0.61 for 10 words. Results also showed that 95% CIs and PIs were narrower for 25 and 50 test words than those for 10 test words. CONCLUSIONS: Korean WRS using the KS-MWL-A has high reliability for 25 and 50 test words, but relatively low for 10 words. It suggested that 95% CIs for each test words would be criteria for significant differences in WRS for groups and 95% PIs at each score of WRS could be utilized for a considerable difference for each individual at retest.

6.Assessment of Medical Trainnees' Knowledge and Their Educational Needs on Transfusion Medicine.

BACKGROUND: Transfusion is one of the most important treatments in patient care. It has been known that there is a lack of transfusion medicine knowledge of doctors in other countries, however it has not been investigated yet in Korea. In this study, we assessed the educational need and the knowledge on transfusion medicine for trainees to provide basic raw data for future education in transfusion medicine. METHODS: Transfusion is one of the most important treatments in patient care. It has been known that there is a lack of transfusion medicine knowledge of doctors in other countries, however it has not been investigated yet in Korea. In this study, we assessed the educational need and the knowledge on transfusion medicine for trainees to provide basic raw data for future education in transfusion medicine. RESULTS: Among trainees, 89.9% said they received more than one hour of transfusion medicine education at medical schools, but 57.3% did not receive transfusion medicine education after finishing medical school. Moreover, 76.4% and 86.5% of respondents stated that additional transfusion medicine education was needed during and after medical school respectively. Among respondents, 43.5%, 53.9%, and 28.1% reported their knowledge on blood products, pretransfusion testing, and transfusion reactions as low or very low. In the assessment of knowledge of transfusion medicine, the mean percentage of correct answers was 65.2%. The rates of correct answers (average) ranged from 25.8% to 80.9% (58.2%), 49.4% to 94.4% (75.7%) 70.8% to 89.9% (80.2%) in case of blood products, pretransfusion testing, and transfusion reactions respectively. CONCLUSION: Transfusion medicine education among trainees after finishing medical school was found to be insufficient, and additional education for transfusion medicine is demanded by trainees.

7.Current Status of Pretransfusion Tests and Equipments in Small and Medium Sized Hospitals in Korea.

BACKGROUND: The appropriate procedures and equipment for the pretransfusion test are fundamental to a safe blood transfusion. The present study aimed to assess the current status of procedures and equipment for pretransfusion tests at small- and medium-sized medical institutions, as well as to use this basic raw data to better manage blood transfusions at these institutions. METHODS: Offline and online questionnaire surveys were performed at institutions that used between 24 and 1,000 units of blood products in 2014. A total of 338 institutions participated, and the survey results were subsequently analyzed. RESULTS: Among 307 institutions where on-site ABO blood typing was performed, 15.0%, 2.1%, and 43.5% did not conduct ABO serum typing, RhD typing, and irregular antibody screening tests, respectively, and 12.8% only conducted the saline phase for crossmatching. Moreover, among 338 institutions, only 66.7% of blood banks had centrifuges, 84.5% had 37℃ incubators, 41.1% had slide view boxes; in addition, 66.1% and 18.6% had refrigerators and deep freezers, respectively, for blood storage. CONCLUSION: Certain small- and medium-sized institutions did not have the essential equipment required to operate as blood banks. Moreover, they also needed to improve their testing procedures. To address these issues, the initiation of systematic training programs and the employment of institutional strategies are necessary to enhance testing procedures and equipment, respectively.

8.Scientific Principles on the Revision and Addition of the Codex Nutrient Reference Values for Food Labelling Purposes.

There has been a need to update the current Codex nutrient reference values (NRVs) which had been established following the Helsinki Consultation in 1988. The main purposes of NRVs are to help consumers compare the nutrient content of different food products and estimate the usefulness of a food product in terms of its percentage contribution to the overall healthful diets. The establishment of Codex NRVs for labelling purposes is intended to facilitate the goals of protecting consumers' health and ensuring fair practices in food trade. When revising the Codex NRVs several issues are needed to discussed, which are: 1) Consideration of population groups whether to have one set of NRVs for general population or to have several sets of NRVs for each population groups, 2) Determination of scope of nutrients such as what nutrients to include and what reference values to choose and 3) Development of principles for establishing NRVs such as the selection of suitable reference values among many including recommended intake/recommended dietary allowances, estimated average requirement and upper levels to extract NRVs. This paper will discuss the above issues and the inputs and efforts that the Korean Nutrition Society had put into to carry out the project, "International Collaboration on the Revision and Addition of the Codex Nutrient Reference Values for Food Labelling Purposes," and to ascertain relevant scientific principles and evidences that are utilized for establishment and revision of the Codex NRVs.
