1.The clinical application of CT-guide percutaneous biopsy in the pulmonary nodules

Objective To explore the diagnosis value of CT-guide percutaneous biopsy in the pulmonary nodules.Methods A total of 101 cases of the pulmonary nodules with unknown causes who underwent Germany-made auto-spring-gun spiral CT-guide percutaneous pulmonary biopsy were studied.Results The accuracy of centesis was 100%.97 cases were succeeded in getting diagnosis,the diagnosis rate was 96.0%.Among them,there were 61 cases of lung cancer,4 cases of malignant pleural mesothelioma,10 cases of metastatic carcinoma,16 cases of tuberculosis,6 cases of inflammatory pseudotumor.Pneumothorax occurred in 8 cases,hemoptysis occurred in 3 cases.The rate of complication was 10.9%.Conclusion CT-guide percutaneous biopsy to the pulmonary nodules is a safe and effective diagnostic means,which has a high diagnosis raze with few complication.
2.18F-FDG PET/CT features of spinal tuberculosis

Objective To investigate the diagnostic value of 18F-FDG PET/CT features of spinal tuberculosis.Methods 18 F-FDG PET/CT imaging manifestation of 125 cases (70 males,55 females,average age 44.3 years) with osseous or mixed vertebra bone destruction from January 2006 to June 2014 were analyzed retrospectively.The patients included 32 cases with spinal tuberculosis (50 lesions,study group) and 93 cases with other diseases (150 lesions,control group).The corresponding FDG PET/CT features were analyzed by two classification logistic regression (Forward:LR),then FDG PET/CT features with statistical significance in diagnosis of spinal tuberculosis were obtained.The sensitivity and specificity of those FDG PET/CT features were calculated.Results Odds ratio (OR) of FDG PET/CT features with statistical significance in diagnosis of spinal tuberculosis from high to low were paraspinal cold abscess (20.790),cold zone(10.528),intervertebral disc involvement(5.394),continuous vertebral involvement(3.493).The diagnostic sensitivity and specificity were 22.0% (11/50),99.3% (149/150) forcold abscess,70.0% (35/50),90.0% (135/150) for cold zone,82.0% (41/50),83.0% (125/150) for intervertebral disc involvement,82.0% (41/50),78.0% (117/150) for continuous vertebral involvement,respectively.Spinal tuberculosis often had two or more signs.The continuous vertebral involvement + intervertebral disc involvement was most common,with the diagnostic sensitivity and specificity of 78.0% (39/50) and 88.7% (133/150).The diagnostic sensitivity and specificity of continuous vertebral involvement+intervertebral disc involvement+cold zone were 58.0% (29/50),96.0% (144/150),and those of four features conourrence were 14.0% (7/50),100% (150/150),respectively.Conclusion Regression analysis shows that FDG PET/CT features with statistical significance in diagnosis of spinal tuberculosis are paraspinal cold abscess,cold zone,intervertebral disc involvement,continuous vertebral involvement,which have high diagnostic sensitivity and specificity.
3.Effect of Psychological Intervention on First Episode Schizophrenia in Quality of Life and Social Function

Objective To explore the effect of psychological intervention on first episode schizophrenia in quality of life and social function.Methods 120 inpatients with first episode schizophrenia were randomly divided into the observed group (60 cases) in which patients were treated with risperidone plus psychological intervention and the control group (60 cases) in which patients were treated with risperidone only. All patients had been treated for 8 weeks and followed up for 6 months after discharge. The assessments included the Positive and Negative Symptoms Scale(PANSS), WHO, QOL-100 and Social Disability Screening Schedule(SDSS) before the treatment and at the end of the follow-up.Results The scores of PANSS, SDSS and QOL-100 in both groups did not show significant difference before the treatment(P>0.05). At the end of follow-up, the total score, negative score and positive score of PANSS in the observed group were lower than that in the control group(P<0.01), while the total score of QOL-100, areas of mentality, dependence, social relation factor scores and the scores of SDSS in the observed group were higher than that in the control group(P<0.05 or P<0.01). Conclusion Psychological intervention can help to improve the symptom of first episode schizophrenia and enhance their quality of life and social function.

BACKGROUND: Recently the prevalence rate of mass in mammary gland increases significantly than before. To use which kind of technology is not only related with the post-operation recurrence, but also related with the reconstruction of their confidence. Therefore, how to reduce the trauma physically and psychologically in patients to the maximal degree has become the chief factor to choose technology by surgeons.OBJECTIVE: To observe the effects of transverse rectus abdominis musculocutaneous (TRAM) flap, latissimus dorsi musculocutaneous flap with breast prothesis, local flap and simple breast prothesis after skin-sparing mastectomy with immediate breast reconstruction.DESIGN: Case analysis.SETTING: Department of Plastic Surgery, Shenzhen People's Hospital,Second Clinical Medical College of Jinan University.PARTICIPANTS: A total of 15 ductal carcinoma in situ (DCIS) and huge breast benign tumor patients were enrolled into Shenzhen People's Hospital between October 1997 and May 2004, aged from 20 to 46 years. They all got disease in single breast, and 8 of them had preoperative diagnosis of DCIS, T1N0M0 and 7 of them had huge breast benign tumor. Operation program was informed to the patients before operation so as to gain their agreement.METHODS: Skin-sparing mastectomy was performed with circular shape incision at areola and using TRAM flap, latissimus dorsi musculocutaneous flap with breast prothesis, local flap and simple breast prothesis to fill breast. Slight skin of musculo-cutaneous flap could compensate the nipple and areola when performing reconstruction. At the reoperation, nipple was made at the transferred flap.MAIN OUTCOME MEASURES: To observe the patients with or without recurrence or transfer after operation and local healing of wound.RESULTS: With the cooperation of general surgeons the operation was done in 15 cases and 14 cases of them were followed up for more than 1 year. The reconstructed breasts were with good symmetry with the opposite one, normal skin sensation, natural shape and inconspicuous scar.CONCLUSION: This is an excellent alternative method for mastectomy and breast reconstruction based on the avoidance of recurrence of breast cancer in severe operation.
6.Twist induces drug resistance in human lung cell line A549 by up-regulating MRP3

Objective To investigate the molecular mechanism of Twist induced-drug resistance in human lung cell line A549 cells.Methods The sensibility of A549 cells over-expressed Twist to cisplatin was measured by CCK8 assay. The expressions of Twist,MRP1,MRP2 and MRP3 were detected by RT-PCR. Western blotting was used to detect the expressions of Twist and MRP3 in A549 cells over-expressed Twist.The correlation between Twist and MRP3 expressions in 30 cases of lung cancer tissues was detected with immunohistochemistry. Results The viability of A549 cells over-expressed Twist was significant better than the control group under cispla-tin(32,64,128 μmol/L)treatment(0.79 ± 0.039,0.63 ± 0.048,0.46 ± 0.039 vs.0.53 ± 0.039,0.41 ± 0.043, 0.27 ± 0.063,respectively). After knockdown of MRP3 expression,it reversed drug resistance induced by Twist. Clinically,the expressions of Twist and MRP3 in lung cancer tissues were significantly relevant(P < 0.05). Conclusions Twist induces the drug resistance of human lung cell line A549 to cisplatin via up-regulating the expression of MRP3.
7.Comparison of sinus tarsi and lateral L-shaped approaches in medial wall reduction and calcaneal alignment in treatment of calcaneal fractures

Objective To compare the minimally invasive sinus tarsi approach and lateral intensive L-shaped approach in the therapeutic effects concerning medial wall reduction and calcaneal alignment for cal-caneal fractures. Methods A retrospective analysis was conducted of the 52 patients with calcaneal fracture who had been treated at Department of Foot & Ankle Surgery, Guangzhou Orthopaedic Hospital from January 2010 to December 2014. They were 39 men and 13 women, 28 to 46 years of age ( average, 40.4 years ). Of them, 26 were treated via the sinus tarsi approach ( minimally invasive group ) and the other 26 via the con-ventional lateral extensile L-shaped approach ( conventional group ) . X-ray axial films of the calcaneus were taken pre-operatively and post-operatively to evaluate the medial wall reduction and calcaneal alignment. The American Orthopedic Foot Ankle Society ( AOFAS ) ankle-hindfoot scale was adopted to assess the therapeutic effects. Results The average follow-up period for this cohort was 18 months ( from 12 to 24 months). The post-operative varus angle was 7.41°± 5.17°for the minimally invasive group and 8.01°± 5.33°for the con-ventional group; the correction of varus angle was 6.60°± 6.23°for the minimally invasive group and 8.57°± 6.64°for the conventional group; the good to excellent rate of medial wall reduction was 42.3% ( 11/26 ) for the minimally invasive group and 53.8% ( 14/26 ) for the conventional group; the AOFAS score was 89.5 ± 7.0 for the minimally invasive group and 86.2 ± 8.2 for the conventional group. There were no statistically signifi-cant differences between the 2 groups in all the above comparisons ( P > 0.05 ). Conclusion The mini-mally invasive sinus tarsi approach can be a fine choice for treatment of calcaneal fractures, because it leads to no differences in medial wall reduction, postoperative varus angle and postoperative correction of varus angle, compared with the conventional lateral extensile L-shaped approach.
8.Symptomatic tarsometatarsal osteoarthritis after open reduction and screwing of Lisfranc injuries

Objective To explore the outcomes of open reduction and internal fixation ( ORIF ) with transarticular screws for Lisfranc injuries and the postoperative incidence of symptomatic tarsometatarsal os-teoarthritis ( OA ) . Methods This retrospective study involved 28 patients who had been treated surgically at our institution between January 2009 and January 2015 for Lisfranc injuries. They were 18 males and 10 females, with an average age of 36. 1 years ( from 19 to 54 years ) . According to the Quenu-Kuss classifica-tion, 5 patients had type-A injury, 10 type-B injury ( 4 cases of type-B1 and 6 ones of type-B2 ) , and 13 type-C injury ( 8 cases of type-C1 and 5 ones of type-C2 ) . The patients underwent ORIF with screws for the 1st to the 3rd tarsometatarsal joints and ORIF with Kirschner wires for the 4th to the 5th tarsometatarsal joints within 2 weeks. After the Kirschner wires were removed 8 to 10 weeks postoperatively, progressive weight-bearing began. Functional outcomes were assessed according to the American Orthopaedic Foot and Ankle Society ( AOFAS ) midfoot scores and visual analog scale ( VAS ) at final follow-ups. Results The mean duration of follow-up was 29. 9 months ( from 26 to 72 months ) . AOFAS scores revealed one excellent case, 22 good ones and 5 poor ones with an excellent to good rate of 82. 1%. The mean VAS score was 2. 8. Radiographic evidence of OA was noted in 20 patients ( 71. 4%, 20/28 ) , in 18 of whom ( 90. 0%) symp-tomatic OA was observed. There was no significant difference ( P=0. 399 ) in the incidence of symptomatic OA either between the patients with anatomic reduction ( 60. 9%, 14/23 ) and those without anatomical re-duction ( 80. 0%, 4/5 ) . Conclusions ORIF with transarticular screws can lead to good therapeutic outcomes for Lisfranc injuries. The incidence of symptomatic OA may not be related to the injury type or re-duction quality.
9.Selective intra-arterial cold saline infusion combined with intravascular reperfusion for acute ischemic stroke with large artery occlusion: a preliminary safety and feasibility study

Objective To preliminarily investigate the safety and feasibility of intra-arterial cold saline infusion combined with intravascular reperfusion for acute ischemic stroke with large artery occlusion. Methods From March 2016 to March 2018, consecutive acute ischemic stroke patients with large artery occlusion within 8 h after onset admitted to the Department of Neurology, the People's Hospital of Longhua District, Shenzhen and recanalized successfully after endovascular treatment were enrolled. After recanalization, cold saline was infused through the guiding catheter via the ipsilateral guilty vessel (10 ℃, 33 ml/min for 30 min). Results A total of 20 patients were enrolled, including 15 males. Their median age was 67 years (interquartile range, 53-80 years). Fifteen patients were treated with thrombolysis. A median onset-to-needle time was 300 min (interquartile range, 260-360 min). During the infusion of cold saline, the lowest rectal temperature was only decreased 0. 1 ℃, but within 5 min after completion of perfusion, it returned to the temperature before perfusion. Complications associated with intra-arterial hypothermia were not observed. The median National Institutes of Health Stroke Scale score was significantly decreased from 21 (interquartile range 15-55) before needle to 15 (interquartile range 10-16; Z = -4. 549, P < 0. 001) at discharge. Conclusion Selective intra-arterial cold saline infusion combined with intravascular reperfusion for acute ischemic stroke with large artery occlusion is safe and feasible.
10.Adverse effects of 0.01% atropine sulfate eye drops on myopic children

Objective To observe the adverse effects of 0.01% atropine sulfate eye drops on myopic children.Methods A prospective non-randomized controlled trial was conducted.Ninety-nine myopic children (99 right eyes) were randomly assigned to experimental group(55 cases) and control group(44 cases).Myopic children of experimental group wore completely corrected frame glasses,while 0.01% atropine sulfate eye drop was dropped into each eye once a day before going to bed.Myopic children in control group only wore completely corrected frame glasses.The follow-up time was 4 months.Best corrected visual acuity (BCVA),intraocular pressure (IOP),change of pupil diameter (PD) and amplitude of accommodation,symptoms of discomfort after medication were observed.This study followed the Helsinki declaration and was approved by Ethic Committee of the First Affiliated Hospital of Zhengzhou University.Informed consent was signed by the parents of each patient.Results No significant differences were found in best corrected distance visual acuity (BCDVA),best corrected near visual acuity(BCNVA) and IOP between the two groups before and 4 months after treatment (BCDVA:Fgroup =3.880,P =0.112;Ftime =27.220,P =0.413;BCNVA:Fgroup =5.200,P =0.311;Ftime =38.200,P =0.116,IOP:Fgroup =12.350,P=0.214;Ftime =22.300,P =0.146).After 4 months treatment,the PD was (6.99 ±0.64) mm in the experimental group,which was significantly higher than that before treatment(P<0.001).The PD of the experimental group was higher than that of the control group after 4 months treatment,and the difference was statistically significant (P<0.01).The amplitude of accommodation in the experimental group was 14.01 ±3.98 after 4 months treatment.which was 1.20 D lower than that before treatment,the difference was statistically significant (P<0.01).The amplitude of accommodation of the experimental group was lower than that of the control group after 4 months treatment,the difference was statistically significant (P < 0.01).Four cases (8%) appeared photophobia symptoms.Two cases and 2 cases appeared photophobia for 1 week or 2 weeks,respectively.However this symptom would relieve after wear sunglasses or sunshade caps in outdoor activities.One case (2%) appeared ocular itching and swelling after 1 month treatment,but disappeared after withdrawal.There were no other uncomfortable symptoms such as near sight blurring.Conclusions In mainland of China,the BCDVA,BCNVA and IOP of myopic children are stable;the PD is dilated and the amplitude of accommodation is decreased slightly after 4 months treatment of 0.01% atropine,and 10% children appear photophobia,but do not affect their study and life.
