1.Clinical observation on precaution effect of ephedrine on protamine's untoward reaction

Objective To investigate precaution effect of ephedrine on protamine's untoward reaction. Meth-ods Forty patients, who were scheduled to have mitml valve replacement operation were randomly divided into two groups: ephedrine group(group A, n = 20) and control group (group B, n = 20). 50ml solution, mixed with protamine and 5% glucose,was injected through internal jugular vein in five minutes to neutralize the effect of heparin. In group A,3mg ephedrine was added into neutralization solution while in group B nothing was added. Arterial pressure(AP) , heart rate(HB) peripheral hemoglobin oxygen saturation(SpO2) and airway resistance(AR) were measured before injection and 1,3,5,10 minutes after injection in both groups. Results In group A, the AP, HR, SpO2 and AR had no significant difference before and after injection (P >0.05) ;while in 6 eases of group B, the AP, HR, SpO2 and AR have significant difference before and after injection. Conclusion Ephedrine can significantly improve protamine's untoward reaction, and neutralize the effect of heparin.
2.Study on abnormal myocardial motion of left ventricle by quantitative tissue velocity imaging

Objective To observe the velocity of left ventricular wall motion features of normal subjects and patients with abnormal regional wall movement of left ventricle with the quantitative tissue velocity imaging(QTVI),and to know whether variances exit between the velocity of left ventricular myocardial motion of the normal subjects and the abnormal subjects with coronary artery disease(CAD) and myocardial ischemia. The practical value of QTVI on myocardium movement of left ventricle was evaluated. Methods The segmental left ventricular wall motion velocity was measured on 27 normal subjects and 21 patients with CAD and myocardial ischemia,including three waves,peak velocity of systolic S wave,diastolic E wave,A wave, and time velocity integrate of S wave(S TVI) was used as measurement parameters.The parameters were analysed between the two groups. Results The velocity and S TVI were gradually decreased from base to apex in the same wall in the control group.There were some abnormal segmental movements in the abnormal group. S wave ,E wave and S TVI of diseased segments in the abnormal group were much lower than those of control group ( P 0.05 ). Conclusions QTVI can quantitatively measure the velocity in abnormal movement segment of ventrical wall of patients with CAD and myocardial ischemia.S wave,E wave and S TVI are better parameters of assessing systolic function of left ventricle.QTVI has clinical application in regional myocardial motion function evaluation.
3.The elderly nonalcoholic fatty liver disease prevalence and related factors analysis in Xuanwu community

Objective To explore the epidemiological characteristics of nonalcoholic fatty liver disease (NAFL)and relationship with main chronic diseases in Xuanwu district community of the elderly.Methods 6 779in the elderly physical examination,unified health questionnaire,physical examination,measurement of body mass index,blood pressure,fasting blood glucose,liver function,blood lipids,abdominal B-ultrasound to calculate prevalence of NAFL rate,and its related factors were analyzed.Results NAFL prealence was 38.7%,NAFL group of overweight and obesity,diabetes,high blood pressure,dyslipidemia was significantly higher than the control group.Conclusion NAFL rate was high,and it has close relationship with the metabolic syndrome,NAFL should be placed in community mainly chronic diseases of prevention and control category.
4.Study on the design and detection method of a novel blood pressure measurement system.

A novel blood pressure measurement system was designed which based on auscultatory method. And the electret sensor that embedded into the internal instrument can detect the Korotkoff-sound signal directly which is coupled by the cuff and transmitted in the cross connection. The BP values identification algorithm is based on combined detection of Korotkoff-sound and pulse signal, and the products of amplitudes and energies are calculated as the characteristic values of Korotkoff-sound, and the Korotkoff-sound phases are classified and detected by means of clustering of characteristic values, and then BP parameters are determined. The contrast test and statistical analysis showed good consistency and accuracy between the new BP detection method and conventional mercury sphygmomanometer.

5.Detection of the viscoelasticity of relaxed myocardium in vivo by intervention ultrasound indentation system

Objective To explore the viscoelasticity of relaxed myocardium in vivo through indentation method with an intervention ultrasound indentation system (IUIS). Methods Old myocardial infarction (OMI) models of canine were established by ligating left anterior descending branch of coronary artery for 3 months. The indentation creep tests were used respectively in OMI group and shame group (n=8, each) by IUIS in middle and advanced diastole stage in vivo. Test data were processed with three-parameter solid viscoelasticity model, and the viscoelastic parameters, such as instantaneous elastic modulus (E1), relaxation modulus (E∞), creep elastic modulus (E2) and viscous damping coefficient (η) in normal and infarcted myocardium were obtained and compared. Results All the parameters of E1, E∞, E2 and η increased obviously in OMI group than in sham group shown as follows: 27.81±6.74kPa vs. 6.78±2.43kPa; 17.87±3.59kPa vs. 4.52±1.56kPa; 49.54±14.35kPa vs. 16.82±12.37kPa and 1.97±0.78Pa.s vs. 0.66±0.40Pa.s. The differences were statistically significant (P<0.05). Conclusions IUIS is a feasible method to assess the viscoelasticity of relaxed myocardium in vivo. Three-parameter viscoelasticity model can be used to describe creep properties of relaxed myocardium. Both elastic modulus and viscosity resistance have increased in infarcted myocardium.
6.Mental fatigue assessment based on physiological signals.

OBJECTIVETo assess mental fatigue by noninvasive monitoring of the physiological signals.
METHODSThe changes in the physiological parameters including the electrodermal activity, heart rate and heart rate variability were analyzed in 14 subjects performing the reaction-time tasks when fatigue and changes in the reaction time occurred.
RESULTSThe average skin conductance level, average heart rate, and heart rate variability parameters including the total power density, percentage of the very low power density, percentage of high power density all differed significantly between the sober state and the mental fatigue state.
CONCLUSIONMonitoring the physiological parameters including the electrodermal activity, heart rate and heart rate variability is a noninvasive, effective and practical approach to mental fatigue assessment.

7.Serum monocyte chemoattactant protein-1 level and spinal tuberculosis susceptibility

BACKGROUND:The monocyte chemoattactant protein-1 gene polymorphism is associated with spinal tuberculosis susceptibility. OBJECTIVE:To investigate the association between serum monocyte chemoattactant protein-1 expression level and spinal tuberculosis susceptibility in Han population of Hunan province. METHODS:The patients with spinal tuberculosis and the healthy volunteers were recruited in Xiangya Hospital of Central South University from December 2004 to December 2010. The empty peripheral venous blood 2 mL were col ected from the subjects in early morning, then the monocyte chemoattactant protein-1-362 genotypes were detected by polymerase chain reaction and DNA sequencing technology. And the serum monocyte chemoattactant protein-1 level was detected by enzyme linked immunosorbent assay technology. The ROC curve was used for diagnostic tests to calculate diagnostic threshold value of serum monocyte chemoattactant protein-1 level to spinal tuberculosis susceptibility, and to analyze the diagnostic titer. RESULTS AND CONCLUSION:208 patients with spinal tuberculosis and 210 healthy volunteers were included. The serum monocyte chemoattactant protein-1 level of the spinal tuberculosis patients was significantly higher than that of the healthy volunteers [(134.58±51.63) ng/L vs. (39.18±17.45) ng/L, P<0.01]. The serum monocyte chemoattactant protein-1 level could not be affected by gender, but over-expressed in patients with monocyte chemoattactant protein-1-362-CC genotypes. The serum monocyte chemoattactant protein-1 level higher than 101.65 ng/L indicated that the patients might suffered from spinal tuberculosis (sensitivity:85.5%, specificity:94.3%, Youden index:0.799, area under curve of ROC:0.946, 95%confidence interval:0.916-0.975, P<0.01). The serum monocyte chemoattactant protein-1 level may be associated with spinal tuberculosis susceptibility in Han population of Hunan province, highly expressed serum monocyte chemoattactant protein-1 can be used as one of the indicators for the diagnosis of spinal tuberculosis.
8.Protective effect of salidroside pretreatment on rabbit heart after limb ischemia/reperfusion

Objective: To observe the protective effect of salidroside pretreatment on rabbit heart after limb ischemia/reperfusion (I/R). Methods: A total of 24 New Zealand rabbits were randomly and equally divided into sham operation group, I/R + placebo group (I/R group)and salidroside pretreatment group(salidroside group). Before establishment of rabbit models of limb I/R, salidroside group received salidroside injection via ear marginal vein, and I/R group received saline injection, once a day for three days. After model establishment, echocardiography was used to evaluate rabbit cardiac function of each group 4h after reperfusion, including left ventricular end-systolic dimension (LVESd), left ventricular end-diastolic dimension (LVEDd), left ventricular ejection fraction (LVEF) and left ventricular fractional shortening (FS). Blood sample was taken to measure serum cardiac troponin I (cTnI) and tumor necrosis factor (TNF)-α. Some ventricular tissue homogenates were taken to measure levels of superoxide dismutase (SOD) and malonyl diadehyde (MDA). Then heart sample was taken to receive pathological examination. Results: Compared with I/R group 4h after reperfusion, there were significant decrease in LVESd [(0.69±0.07) mm vs. (0.62±0.05) mm] and significant increase in LVEF [(64.6±3.4) % vs. (72.1±3.6) %], FS [(34.2±3.2) % vs. (41.7±3.4) %] (P<0.05 all), but these indexes of salidroside group were all no significant different than those of sham operation group (P>0.05). Compared with I/R group, there were significant decrease in cTnI [(5.24±0.34) μg/ml vs. (1.06±0.12) μg/ml], MDA [(8.92±2.18) μmol/L vs. (6.79±1.43) μmol/L] and TNF-α [(37.43±10.02) pg/ L vs. (19.73±6.31) pg/ L], and significant increase in SOD level [(16.61±3.75) U/ml vs. (22.26±4.73) U/ml] in salidroside group (P<0.05 all). Pathological results indicated that injury degree in salidroside group was significantly attenuated than that of I/R group. Conclusion: Salidroside pretreatment could protect cardiac function and relieve rabbit cardiac injury after limb ischemia/reperfusion.
9.The design of data transfer protocol for home and community tele-medical system.

This article describes a design of physiological signal storage format and transfer protocol for the tele-medical system between home and community. The protocol is based on ASCII character, with frames as its basic structure. There are two kinds of frames: control frames and data frames. Control frames can start and stop data transfer, confirm the order, and ask for start. There are seven kinds of data frames, according to the different data types. Data is transferred in data frames. The protocol described in this article is simple and extensible. The design target has been accomplished in real system.

10. Arthroscopic autologous scapular spine bone graft transplant for shoulder recurrent instability

Objective:
To investigate the clinical outcomes and radiological results of arthroscopic autologous scapular spine bone graft transplant to treat shoulder recurrent instability.
Methods:
Data of 27 patients diagnosed as shoulder recurrent instability with the bone defect of 10%-15% from July 2016 to August 2018 who were treated by arthroscopic autologous scapular spine bone graft transplant were retrospectively analyzed. There were 20 males and 7 females with an average age of 30.8 years old (range, 19-50). The bone loss of the glenoid was 10%-15%. The time between the first dislocation and the surgery was 24.1±15.8 months. The patients were treated with arthroscopic autologous scapular spine bone graft transplant. Postoperatively the affected shoulder was immobilized by the abduction brace for 6 weeks, after that the passive motion was applied. Strengthening exercise began at 10-12 weeks and sports was allowed after 6 months. Constant-Murley score and the Disabilities of Arm, Shoulder and Hand (DASH) score were used to evaluate the shoulder function, and visual analogue score (VAS) score was used to evaluate the degree of pain. Computed tomography scans were obtained one week post-operation and at the latest follow-up, from which the length, width, height and volume of the bone graft were measured and the absorption rate of the bone graft was calculated. The subjective satisfaction degree of patients at the latest follow-up was also recorded.
Results:
All 27 patients were followed up for 19.8 months (range, 13-39 months). No infection or neurovascular injury was identified. At the latest follow-up, the Constant-Murley score was 85.15±5.62 (range, 76-94), the DASH score 13.39±5.51 (range, 3.19-21.95) and the VAS score 1.29±0.45 (range, 1-2), thus all of those were improved significantly compared to those of pre-operation. At the latest follow-up, the anterior flexion was 153°±24°, lateral rotation by side 38°±21°, internal rotation 70°±21°, and abduction was 139°±18°. At the latest follow-up, the absorption rate of the bone graft was 46.1%±20.6% (range, 24.0%-71.7%). Among all the 27 patients, 19 patients considered the outcome as very good, and 6 patients considered as good, 2 patients fair.
Conclusion
Arthroscopic autologous scapular spine bone graft transplant could successfully treat shoulder recurrent instability with glenoid bone loss at 10%-15%. This technique could achieve satisfactory clinical results, improve glenohumeral stability, decrease the re-dislocation rate.
