1.Influence of trimetazidine on prognosis of patients with X syndrome

Objective:To explore the influence of trimetazidine (TMZ)combined traditional routine drugs on progno-sis of patients with cardiac syndrome X.Methods:A total of 80 patients with cardiac syndrome X were randomly and equally divided into routine treatment group and TMZ group (received TMZ 20mg,three times/d based on routine medication).Both groups were treated for 12 weeks.Patients of two groups received treadmill exercise test and car-diac function examination before and after treatment,and the results were statistically compared.Results:Com-pared with routine treatment group after treatment,there were significant rise in total exercise time [(7.90±1.45) min vs.(9.35±1.70)min]and the time ST segment depressed 1mm [(5.30±1.43)min vs.(6.78±2.00)min], and significant reduction in ST segment depression extent [(0.89±0.30)mm vs.(0.61±0.20)mm],P <0.05 all;for cardiac function,there were significant rise in stroke volume [(67.99±11.77)ml vs.(74.05±7.58)ml]and left ventricular ejection fraction [(50.13±11.05)% vs.(56.02±9.52)%]in TMZ group,P <0.01 all.Conclusion:TMZ can significantly improve prognosis of patients with cardiac syndrome X.

Microcapsule technology is a high and new technology under the focused research and development in 21st century, which has been used in many research fields. However, its researches on preparation field should be paid more attention to, especially the research direction of TCM drug delivery system, which is of great value for development and application. This article reviewed the application progress of microcapsule technology in pharmaceutic preparation field, and discussed the strategic significance of the researches on TCM microcapsule preparation, with a purpose to provide research ideas and paths for the researches on the new formulations of traditional Chinese medicine.
3.The Correlation Analysis of CHGA and ATGL in Patients of Type 2 Diabetes Mellitus with Nonalcoholic Fatty Liver Disease

Objective To investigate the relationship between plasma levels of chromogranin A (CHGA) and adi-pose triglyceride lipase (ATGL) in patients with type 2 diabetes (T2DM) combined non-alcoholic fatty liver disease (NAFLD). Methods The plasma levels of CHGA and ATGL were assayed by enzyme-linked immunosorbent assay (ELISA) in T2DM patients with NAFLD (group A, n=74), T2DM without NAFLD (group B, n=76), and normal group (group NC, n=75). The correlation between CHGA, ATGL and other metabolic index was analyzed. Results The plasma level of CHGA was significantly higher in group A (83.15±9.46) and group B (70.90±2.75) than that of group NC (46.74±8.15, P<0.01), and the level of CHGA was significantly higher in group A than that of group B (P<0.01). The plasma level of ATGL was sig-nificantly lower in group A (21.36±13.42) and group B (40.29±22.83) than that of group NC (72.30±26.41, P<0.01), and the level was lower in group A than that of group B (P<0.01). There was a negative correlation between the plasma CHGA, AT-GL and carbohydrate oxidation rate in group A. There was a positive correlation between fasting insulin (FINS), insulin resis-tance index (HOMA-IR), free fatty acid (FFA) and fat oxidation rate in group A. There was a negative correlation between plasma level ATGL and body mass index (BMI), FINS, cholesterol (TC), triglyceride (TG) and HOMA-IR, meanwhile, it was positively correlated with FFA. The multiple stepwise regression analysis showed that FINS, ATGL and FFA were indepen-dent variables for CHGA. The Logistic regression analysis showed that plasma levels of CHGA, ATGL and FFA were the in-dependent predictors of T2DM with NAFLD. Conclusion The plasma levels of CHGA and ATGL are closely correlated with substance and energy metabolism, and the interaction between them may play an important role in the pathogenesis of T2DM with NAFLD .
4.Association of Plasma Omentin-1 Levels with Adiponectin and Inflammatory Cytokines in Diabetic Patients with Fatty Liver

Objective To investigate the relationship of omentin-1 with adiponectin and inflammatory cytokines in type 2 diabetes (T2DM) patients with nonalcoholic fatty liver disease (NAFLD). Methods The serum levels of omentin-1 and adiponectin were assayed by enzyme-linked immunosorbent assay (ELISA) in patients of T2DM with NAFLD (group A, n=63), T2DM without NAFLD (group B, n=63)and normal control group (group C, n=70). At the same time the biochemical markers and inflammatory marker, such as tumor necrosis factor (TNF)-α, high-sensitivity C-reactive protein (hs-CRP) and interleukin 6(IL-6) were detected in three groups. The correlation analysis and multiple regression analysis were used to de-tect the association of omentin-1 with adiponectin and inflammatory markers. The logistic regression was used to analyze fac-tors influencing NAFLD in patients with T2DM. Results The serum levels of omentin-1 and adiponectin were significant-ly lower in group A [ (27.02±2.82)μg/L and (11.98±3.63) mg/L] than those of group B [(31.52±2.81)μg/L and (15.85±3.28) mg/L] and group C [(35.92±2.80)μg/L and (19.88±3.44) mg/L], and there were significantly lower levels of them in group B than those of group C (P<0.01). The plasma omentin-1 level was positively correlated with adiponectin and high density li-poprotein (HDL-C) in group A. Also the plasma omentin-1 level was negatively correlated with TNF-α, IL-6, fasting blood glucose (FBG), homeostasis model assessment of insulin resistance (HOMA-IR), visceral adipose tissue, waist, waist-to-hip ratio (WHR) and free fatty acid in group A (P<0.05 or P<0.01). Multiple stepwise regression analysis showed that adipo-nectin, TNF-αand IL-6 were independent factors influencing the level of plasma omentin-1. Logistic regression analysis showed that omentin-1 was one of independent factors influencing T2DM combined with NAFLD (P<0.01). Conclusion The incident of NAFLD in T2DM patients is related to the lower level of omentin-1, which may be influenced by adiponectin and inflammatory factors.
5.Prediction of the Th/B cell epitopes on HA of influenza virus(H1N1)and antigenicity analysis

Objective:To predict Th/B cell epitopes in HA of influenza virus(H1N1)and analyze antigenicity of the candidate epitopes in order to develop epitope-bacterin by the way of bioinformatics.Methods:The HA amino acid sequences of infiuenza virus(H1N1),which the viral infection was prevalent recently,were downloaded from Genbank.The Th/B cell epitopes were predicted and analyzed by bioinformatics methods.Then,specificity and conservation of the candidate epitopes were estimated.Finally,antigenicity of the candidate epitopes was identified by influenza virus(H1N1)positiVe serum samples of mice.Results:Three Th/B cell epitopes containing HA_(73-87),HA_(125-139),HA_(188-205) were acquired Two of the candidate epitopes were in a relatively conserved domain of HA1,and a deal of 2006-2009 influenza virus(H1N1)isolates contained the sequences.Moreover,the candidate epitopes were showedin a distinct antibody combining reactivity with the influenza virus (H1N1)positive serum of mice,which inferred the predicted epitopes to be functional ones.Conclusion:The selected epitopes are able to be functional HA Th/B cell epitopes of influenza virus(H1N1).Our study also establish the foundations for the further research of influenza virus infectlon and immunity mechanism,the recognition of influenza virus(H1N1)functional epitope and the development of epitope vaccines.

7.Clinical and genetic analysis of 46, XY partial gonadal dysgenesis caused by DHX37 mutation: One case report and literature review

One case of 46, XY partial gonadal dysgenesis due to a congenital defect of DEAH-box RNA helicase 37(DHX37) was reported. The clinical and genetic data of a boy who was admitted to the Department of Endocrinology and Metabolism, the First Affiliated Hospital of Henan University of Science and Technology due to ambiguous external genitalia in September 2020 were collected and analyzed. This 3-month-old male patient showed a micropenis, bilateral cryptorchidism, 46, XY karyotype, a decrease in testosterone, anti-Müllerian hormone, inhibin B, an increase in follicle stimulating hormone. Testis biopsy indicated gonadal dysgenesis. The proband harbored a de novo heterozygous mutation in the DHX37 gene c. 923G>A(p.Arg308Gln). DHX37 variants need to be considered for 46, XY gonadal dysgenesis.
8.Effects of Sacubitril Valsartan Sodium on Short-term Prognosis of Patients with Acute Anterior Myocardial Infarction Complicated with Acute Cardiac Insufficiency

OBJECTIVE:To investigate the effec ts of angiotensin receptor neprilysin inhibitor (ARNI)sacubitril valsartan sodium(SVS)on the short-term prognosis of patients with acute anterior myocardial infarction (AAMI)complicated with acute cardiac insufficiency. METHODS :A total of 80 patients with AAMI and Killip grade Ⅱ-Ⅳ of cardiac function ,who met the inclusion criteria ,were randomly divided into ARNI group and control group ,with 40 patients in each group. Both groups were given the same basic standardized drug treatment ,vital signs support treatment and percutaneous coronary intervention treatment at the same time. On this basis ,ARNI group was given SVS tablet orally ,with initial dose of 25 mg each time ,twice a day ; thereafter,gradually adjust the dose to 200 mg each time ,twice a day. Control group was given Enalapril maleate tablets orally , with an initial dose of 5 mg each time ,twice a day ;thereafter,gradually adjust the dose to 10 mg each time ,twice a day. Both groups took medicine for a long time ,and were followed up after 1,3 and 6 months of medication to the clinic. The levels of N-terminal pro-B-type natriuretic peptide (NT-proBNP), soluble growth stimulation expressed gene 2 protein (sST2) and echocardiography indexes were compared between 2 groups before and after medication. The 6-minute walking test (6MWT)and the incidence of cardiogenic readmission events were recorded in 2 groups after medication. RESULTS :Compared with before treatment,the indexes of the two groups were significantly improved at 1,3 and 6 months after treatment (P<0.05). Compared with control group ,the levels of NT-proBNP and sST 2 in ARNI group decreased significantly (P<0.05),the levels of left ventricular ejection fraction and 6MWT increased significantly(P<0.05),and the left ventricular end systolic diameter and left ventricular end diastolic diameter decreased significantly,after 3 and 6 months of treatm ent(P<0.05). However ,there was no significant difference in the velocity ratio of peak E to peak A ,pulmonary artery pressure ,right ventricular end diastolic diameter and the incidence of cardiogenic readmission events between 2 groups(P>0.05). CONCLUSIONS :For patients with AAMI complicated with acute cardiac insufficiency , compared with enalapril ,SVS can significantly improve the cardiac function (especially the left ventricular systolic function ), reduce the inflammatory reaction of cardiomyocytes ,protect cardiomyocytes ,so as to improve the short-term prognosis of patients.
