Objective To constuct and identify a specific siRNA targeting mucins (MUC1) expression vector.Methods A pair of oligonucleotide completing and coding hairpin MUC1-siRNA synthesized were inserted into pGC silencerTM U6/Neo/RNAi vector.Double enzyme digestion,PCR test and DNA sequencing were used to identify the recombinant plasmid.Then,special MUC1-siRNA was transfected into cholangiocarcinoma cells with Lipofectamine TM 2000.In vitro,MUC1 mRNA and protein expression was tested by RT-PCR and Western Blot respectively.Results Double enzyme digestion,PCR test and DNA sequencing confirmed that the MUC1 specific siRNA expression vector was constructed successfully.After transfection,the expression of MUC1 mRNA and protein in QBC939 (experimental group) decreased significantly (P
3.Effects of acupuncture intervention at different stages on urinary function reconstruction of neurogenic bladder after spinal cord injury.

OBJECTIVETo explore the effects of early intervention of acupuncture on urinary function reconstruction of neurogenic bladder after spinal cord injury, so as to find the optimal treatment timing of acupuncture.
METHODSFifty-three patients of neurogenic bladder after spinal cord injury were randomly divided into a prior-to-catheter group (25 cases) and a posterior-to-catheter group (28 cases). For the prior-to-catheter group, one week before the removal of catheter, acupoints on the abdomen and back such as Guanyuan (CV 4), Zhongji (CV 3), Baliao (Shangliao (BL 31), Ciliao (BL 32), Zhongliao (BL 33), Xialiao (BL 34), Jiaji (EX-B 2) and acupoints at distal end such as Zusanli (ST 36) and Sanyinjiao (SP 6) were selected; after the catheter removal, simple method. of water column was used to measure bladder capacity pressure to classify the type of bladder, and then different acupuncture manipulations were given; intermittent catheterization was performed if condition was allowed. The posterior-to-catheter group applied identical treatment after removal of catheter as the prior-to-catheter group. The treatment was given 5 times per week; after one-month treatment, five aspects including the time of first effective urination, time of establishing reflex urination, average residual volume of urine, time of residual urine less than 100 mL and quality of life (QOL) were evaluated.
RESULTSAfter treatment, the time of first effective urination, time of establishing reflex urination, average residual volume of urine and time of residual urineless than 100 mL in the prior-to-catheter group were all superior to those in the posterior-to-catheter group (all P<0. 05) ; the score of QOL was improved in two groups after treatment (both P<0. 01), but the difference between two groups was not statistically significant (P>0. 05); each index of urination function was not significant among patients with different injured segments of spinal cord (all P>0. 05).
CONCLUSIONThe early intervention of acupuncture (prior to catheter) has obvious improving function on establishing balanced bladder in patients with neurogenic bladder after spinal cord injury, which is superior to acupuncture intervention after removal of catheter, however, the effects of different injured segments on establishing balanced bladder are not different.

4.Study on characteristics of Haemophilus influenzae strains by phenotyping and pulsed-field gel electrophoresis genotyping

objective To investigate the epidemiological and molecular typing features of the pathogenic Haemophilus influenzae(H.influenzae)by biotyping,serotyping and pulsed-field gel electrophoresis(PFGE).Methods A total of 273 invasive isolates of H influenzae were collected from the pediatric patients with pneumonia at Chengdu Children Hospital of Sichuan province from 1988 and 2004 to 2007.The idenbfication of H.influenzae strains were done according to the laboratory standard methodology described by Manual of Clinical Microbiology(American).All strains were biotyped according to Kilian's classification with the API[R]NH system.And serotyped by a slide agglutination assay with type a to f specific antlaerum as described by Pittman.PCR method for identification of H.influenzae were performed as described by Falla.One hundred of 273 strains were analyzed by PFGE as described by Saito with some modifications.The resuIts of PFGE were analyzed by Bionumerics soft(Version 4.0,Applied Maths BVBA,Belium).Restilts 78.2%of 273 cases occurred under 1 years old.Eight biotypes were found among the 273 H.influenzae isolates.17.6%(48/273)of all isolates belonged to biotype Ⅰ,43.6%(119/273)were biotype Ⅱ,22.7%(62/273)were biotype Ⅲ,7.3%(20/273)were biotype Ⅳ,5.9%(16/273)were biotype Ⅴ,0.4%(1/273)were biotype Ⅵ,1.8%(5/273)were biotype Ⅶ and 0.7%(2/273)were biotype Ⅷ.respeetively.99.6% of all 273 isolates were nontypeable.There was only one isolate was serotvpe f Ninty-six PFGE genotypes were obtained in this study.One hundred strains demonstrated a variety of genomic Datterns by PFGE.The most isolates of the flame PFGE genotype(type 35)was 3 isolates.Each of93 PFGE genotypes was represented by only a single isolate.The genotypes distribution didn't correlate with the time distribution of the strains were isolated.Conclusion Nontypeable H.influenzae primarily caused acute Dneumoma in children under 1 years old.They mostly belonged to biotype Ⅰ,Ⅱ and Ⅲ biotypes.The nontypeable H.influenzae strains appeared to more heterogeneous patterns by PFGE genotyping.Genotyping may helP understand the molecular characteristics of outbreak and endemicity according to the results of PFGE.PFGE genotyping proved to have a much stronger discriminatory power than either serotyping or biotyping.Our findings suggest that PFGE analysis is useful for the epidemiologieal study of H.influenzae infections.
5.Surgical versus non-surgical treatment of advanced intrahepatic cholangiocarcinoma: A comparative study

Objective To compare the survival outcomes between operative versus non-operative treatment of advanced intrahepatic cholangiocarcinoma.Methods This is a retrospective study.The data from 122 patients with intrahepatic cholangiocarcinoma treated at the Hunan People's Hospital,the Hepatobiliary Hospital and the Oncology Department from January 2012 to October 2017 were retrospective studied.87 patients who underwent radical surgery (anatomical hepatectomy + regional lymph node dissection) formed the operation group;35 patients who were treated with chemotherapy and/or radiotherapy and/or biological targeted therapy formed the non-operative group.The general characteristics of the two groups including age,sex,ALT,AST,CA19-9,liver function,Child's classification,AJCC staging,tumor number,vascular (hepatic artery,portal vein) invasion and regional lymph node metastasis rates were compared.The overall survival of the two groups was compared.Results There were no significant differences in age,sex,ALT,AST,CA19-9,liver function,Child's classification,AJCC staging,tumor number,vascular (hepatic artery,portal vein) invasion and regional lymph node metastasis rates (P>0.05).The overall survival of the operation group was significantly longer than that of the non-operative group (P<0.05).The mean overall survival for the 2 groups of patients were 32 months and 15 months respectively.The 1-year survival rates were 74.8% and 58.7%,and the 3 year survival rates were 42.4% and 6.5%,respectively.The 5 years survival rates were 12.3% and 0,respectively.Conclusion Operative treatment resulted in better median survival,as well as 1-,3-and 5-year survival rates than non-operative treatment for patients with advanced intrahepatic cholangiocarcinoma.
6.Diagnosis and treatment of intrahepatic stone complicated with atrophy-hypertrophy complex

Objective To explore the typing and surgical methods on intrahepatic stone complicated with atrophy-hypertrophy complex.Methods The clinical data of 32 cases of intrahepatic stone complicated with atrophy-hypertrophy complex from January 2014 to December 2015 in Hunan Province Peopole's Hospital were retrospectively analysed.Results These 32 cases of atrophy-hypertrophy complex accounted for 1.9% of admitted hepatolith patients.We divided the intrahepatic stone complicated with atrophy-hypertrophy syndrome into 5 types,and type-Ⅱ the most common.We performed operation combined with fiber choledochoscope to examine,smash and remove the stone.This methods could reduce the rate of residual stone to 6%.The liver will be partly atrophy with stones inside it,and the residual liver will become hypertrophy.Stones of intrahepatic bile duct lead to the change of the position of porta hepatis and straitness of primary bile duct,and the bile cannot smooth out.there will be lithogenesis and relapsing cholangitis.Patients were treated by intrahepatic biliary double-opening drainage,and followed-up for 12-36 months.There were no death cases,and the total effective rate was 94%.Conclusions Intrahepatic stones complicated with atrophy-hypertrophy complex could be diagnosed by CT scan.Selecting proper operation method to treat atrophy-hypertrophy complex may decrease residual stones and improve the quality of life.
