Objective This paper presents formula for determining the sample size required in case-control study to estimate vaccine efficacy VE with adjusting the precision of confidence interval.Methods Formula to calculate sample size is derived from the principle of relative width of the confidence interval.Results Prespecified relative width may regulate and control the precision of confidence interval and may reflects magnitude of the sample size.Conclusion Sample size calculated by formula will assure that the investigator understand accurately the degree of point estimates of VE close to the true value of VE in the population.
2.Relationship Between the Levels of Platelet Surface EMMPRIN, GPVI and the Stability of Coronary Plaque in Patients With Acute Coronary Syndrome

by lfow cytometry. According to CAG morphology, the plaques included 3 types as Type I, Type II and TypeⅢ. The patients also received coronary CT angiography (CTA), upon CTA value, the plaques were classiifed by soft plaque, ifbrous plaque and calciifed plaque. Expressions of platelet surface EMMPRIN and GPVI among different groups were compared.
Results:①Compared with Control group, ACS and SAP groups had increased expressions of EMMPRIN (5.82 ± 0.81 and 3.45 ± 0.48) vs (1.35 ± 0.15) and GPVI (16.22 ± 5.27 and 8.20 ± 2.87) vs (4.14 ±1.17); the expressions in ACS group were higher than those in SAP group, allP<0.05.②In ACS group, compared with Type I plaque, the patients with Type II and TypeⅢ plaques had elevated levels of EMMPRIN (6.35 ± 1.05 and 4.09 ± 0.67) vs (2.45 ± 0.27) and GPVI (19.50 ± 4.55 and 10.81 ± 2.33) vs (5.89 ± 1.28); the expressions in patients with Type II plaque was higher than those with Type III plaque, allP<0.05.③In ACS group, compared with calciifed plaque, the patients with soft and ifbrous plaques had the higher levels of EMMPRIN (6.18 ± 1.01 and 3.87 ± 0.56) vs (2.43 ± 0.25) and GPVI (19.14 ± 4.27 and 11.08 ± 1.94) vs (5.96 ± 0.99); the expressions in patients with soft plaque was higher than those with ifbrous plaque, allP<0.05.④In CAD patients, EMMPRIN expression was negatively related to plaque type (95% CI: -0.359 to -0.206, β: -0.211) and positively related to clinical type (95% CI: 0.893-1.034, β: 0.893); GPVI expression was negatively related to plaque type (95% CI: -1.222 to -0.586, β: -0.181) and positively related to clinical type (95% CI: 3.576-4.164, β: 0.960).
Conclusion: Expression levels of platelet surface EMMPRIN and GPVI were closely related to the stability of coronary plaque, both of them were the risk factors for severe coronary lesions. EMMPRIN and GPVI may have certain predictive value for early diagnosis of arteriosclerosis in ACS patients.
3.Drug resistance and MLST of Campylobacter jejuni from human and avian sources in Jinshan District of Shanghai from 2021 to 2022

ObjectiveTo understand the current drug resistance status and bacterial multilocus sequence typing (MLST) of human and avian Campylobacter jejuni in Jinshan District, Shanghai. MethodsFecal samples were collected from diarrhea patients in the annuity mountainous area from 2021 to 2022, and poultry and related samples were collected from 2 poultry farms in the Jinshan area for detection of C. jejuni. Minimal inhibitory concentration (MIC) drug sensitivity test was performed on the detected C. jejuni, and some strains were selected for whole genome sequencing and MLST analysis. ResultsA total of 823 samples of diarrhea disease were collected, and 32 strains of C. jejuni were detected, with a detection rate of 3.89%. Out of 600 poultry related samples, 62 strains of C. jejuni were detected, with a detection rate of 10.33%. Human multidrug resistance reached 93.75% (30/32), while avian multidrug resistance reached 100.00%(62/62). The top four drug resistance rates of human and avian C. jejuni were azithromycin (100.00% from humans and 100.00% from birds), naphthoic acid (93.75% from humans and 87.10% from birds), ciprofloxacin (90.63% from humans and 98.39% from birds), and tetracycline (84.38% from humans and 98.39% from birds). The relatively low resistance strains of human derived C. were erythromycin, chloramphenicol, and thalithromycin. The relatively low resistance strains of avian C. jejuni were erythromycin, clindamycin, and flufenicol. MLST analysis showed that the selected 16 strains of bacteria were divided into 9 ST types, among which the evolutionary relationship of avian C. jejuni was relatively concentrated, while human C. jejuni was relatively dispersed. It was found that one strain of avian C. jejuni was closely related to two strains of human C. jejuni. ConclusionsC. jejuni infection is severe in patients with diarrhea in this region, with a detection rate second only to salmonella and Vibrio parahaemolyticus. C. jejuni infection in poultry is relatively common, and both are highly resistant. Therefore, monitoring and control should be strengthened. MLST analysis shows new ST types in both avian and human sources of C. jejuni, indicating the emergence of new mutations that require continuous monitoring to avoid the epidemics caused by new strains. The isolated strains with close genetic relationships between avian and human sources reveal the evidence of the spread of C. jejuni from poultry to humans. Therefore it is necessary to strengthen the monitoring of C. jejuni in relevant samples from breeding farms.
4.Drug resistance and molecular typing characteristics of Klebsiella pneumoniae in meat food and diarrhea samples in a local area

ObjectiveTo determine the drug sensitivity and molecular typing characteristics of Klebsiella pneumoniae isolated from meat and diarrhea samples in a local area. MethodsSeventy-one strains of K.pneumoniae were isolated from 118 meat food (chicken and pork) randomly sampled in the markets in Jinshan District, Shanghai, 2020‒2021, and 1 499 diarrhea samples from outpatient diarrhoea patients in hospitals in the same district. Then drug susceptibility testing was conducted by micro-broth dilution method, and sequence identity was determined by pulsed field gel electrophoresis(PFGE). ResultsThe overall detection rate of K.pneumoniae in meat was 11.86% (14/118), with detection rate 20.93% (9/43) in chicken and 6.67% (5/75) in pork. The difference in detection between meats was statistically significant (χ2=5.317,P<0.05). The detection rate of K.pneumoniae in diarrhea samples was 3.80% (57/1 499). Furthermore, the isolated strains showed the highest resistance to ampicillin at 76.06%. The multi-drug resistant strains included 5 of human origin (8.77%) and 2 of foodborne origin (14.28%). Additionally, 1 foodborne imipenem-resistant strain was detected. A total of 71 strains of K.pneumoniae were found to have 70 banding types, with similarity ranging from 39.4% to 100%, suggesting genetic diversity. ConclusionK.pneumoniae isolated from foodborne and diarrhea samples showed multi-drug resistance in Jinshan District, . with scattered PFGE banding patterns. It is recommended to strengthen the monitoring of this pathogen in the population and animal food, and be alert to the emerging multi-drug resistant strains and risk of food chain transmission.

Objective To study the changes of the putamen metabolites with magnetic resonance spectrum(MRS),and to explore possible underlying unrecongnised aetiological factor and pathophysiology mechanism in the central nervous system of the patients with Tourette' s syndrome.Methods Twenty-two cases of Tourette' s syndrome,and twenty-two gender and age-matched subjects ( the control subjects ) were performed on a clinical 3.0 T MRI system.Proton prob-voxel spectroscopy imaging (1H-MRS) was obtained from two sides of the putamen.The metabolites included N-acetylaspartate (NAA),creatine and phosphocreatine ( Cr),choline-containing compounds (Cho),and myoinositol ( MI ).The value of the NAA,Cr,Cho,and MI were calculated by integration of their peaks.The ratios of NAA/Cr,Cho/Cr,ML/Cr were calculated respectively.Repeated measures analysis of variance ( ANOVA ) was used to test both the value of NAA/Cr,Cho/Cr,MI/Cr of the putamen for group difference,with group as between-subjects factor and side as within-subjects factor.Results The NAA/Cr ratio in patients ( left:1.29 ± 0.13 ; right:1.34 + 0.15 ) was significantly lower than that in the control subjects ( left:1.50 ± 0.08,T =1.962,P <0.05 ; right:1.52 ± 0.11,T =1.865,P < 0.05 ).There was no significant difference in the Cho/Cr and MI/Cr ratio between both groups.Conclusion The abnormalities of the structure and(or) function in the putamens of patients may be the one of the underlying anaetiological factors and pathophysiology mechanisms of the Tourette' s syndrome.
7.A finger-taping switch task functional magnetic resonances imaging study in the patients with Tourette's syndrome

ObjectiveTo investigate the central nervous system mechanisms of active of movement switch in the patients with Tourette' s syndrome( TS),and to explore the possible underlying distinct neural networks for tic trigger and generation. Methods Participants were 14 patients with TS and 14 age- and gender-matched healthy volunteers with no history of physical,psychiatric or neurological disease: All patients were assessed with functional magnetic resonances imaging (fMRI)of the brain during the intermittent performance of finger-tapping switch tasks,Blood-oxygen-level dependent-fMRI was performed using a 3.0 Tesla MR.The area over which the activation was distributed was calculated,and the activation volumes were also compared between the patients with TS and the control subjects.ResultsThe regions activated in the patients with TS and in the volunteers were similar in several brain regions,including contralateral precentral and postcentral gyrus,contralateral mesia pre-front gyrus,contralateral cingulate gyrus,contralateral insula and ipsilataral cerebellum. There were also many different activation areas between the patients and the control subjects. The patients with TS demonstrated more significant and extended activation in the contralateral pre- and postcentral gyrus than the healthy volunteers.The volume of the left pre- and postcentral gyrus of the TS patients was (8.024 ±0.071 ) cm3,while the volume of the left pre-and postcentral gyrus of the control subjects was ( 6.480 ± 0.026) cm3 ( t =3.026,P < 0.01 ) ; The volume of the right pre- and postcentral gyrus was (6.192 ± 0.019) cm3 in the TS cases,while there was (5.608 ±0.037) cm3 in the control subjects (t =2.752,P <0.05).There were significant differences in the volumes of bilateral pre- and postcetral gyrus between the TS and control subjects. The activations of conralateral thalamus without contralateral insula were found in the patients with TS. Conversely, the contralateral insula activation without thalamus activation could be found in the healthy volunteers.ConclusionThe thalamus might play an important role in the aetiological and physiopathologic mechanisms of the TS. The thalamus along with the parietal cortex,cingulate cortex and insular cortex appear to constitute a distinct neural network for tic trigger and generation.
8.Clinical Application of Artificial Intelligence Recognition Technology in the Diagnosis of Stage T1 Lung Cancer.

BACKGROUND:
Lung cancer is the cancer with the highest morbidity and mortality at home and abroad at present. Using computed tomography (CT) to screen lung cancer nodules is a huge workload. To test the effect of artificial intelligence in automatic identification of lung cancer by using artificial intelligence to find the lung cancer nodules automatically in the chest CT of 1 mm and 5 mm thick.
METHODS:
5,000 cases of T1 stage lung cancer patients with 1 mm and 5 mm layer thickness were respectively labeled and learned by computer neural network, the algorithm of forming pulmonary nodules was carried out. 500 cases of chest CT in T1 stage lung cancer patients with 1 mm and 5 mm thickness were tested by artificial intelligence formation, and the sensitivity and specificity were compared with artificial reading.
RESULTS:
Using artificial intelligence to read chest CT 500 in 5 mm, the sensitivity was 95.20%, the specificity was 93.20%, and the Kappa value of two times repeated read was 0.926,1. For 1 mm chest CT 500 cases, the sensitivity is 96.40%, the specificity is 95.60%, and the Kappa reads two times is 0.938,6. Compared with 5 doctors, the same CT sets with 1 mm thickness were read. The detection rates of artificial intelligence and artificial reading were similar to those of lung cancer nodules and negative control read films, and there was no significant difference between them. In the comparison of the same CT slices with 5 mm thickness, the number of detection of lung cancer nodules by artificial intelligence is better than that of artificial reading, and the sensitivity is higher, but the number of false messages is increased and the specificity is slightly worse.
CONCLUSIONS
The automatic learning of early lung cancer chest CT images by artificial intelligence can achieve high sensitivity and specificity of early lung cancer identification, and assist doctors in the diagnosis of lung cancer.

9.Preoperative predictors of pancreatic mucinous cystic neoplasm with invasive carcinoma

Objective To investigate the predictors for the preoperative diagnosis of pancreatic mucinous cystic neoplasm with invasive carcinoma (MCN-IC).Methods Clinical data of 132 patients with pancreatic mucinous cystic neoplasm ( MCN ) who underwent surgery and were pathologically diagnosed in Shanghai Changhai Hospital and General Hospital of Xinjiang Military Region from August 2000 to December 2013, including gender, age, medical history, clinical presentations, laboratory examinations and imaging findings and etc , were retrospectively analyzed .All cases were classified into two groups:MCN with noinvasive carcinoma ( MCN-nIC, including MCN with low-or intermediate-grade dysplasia and MCN with high-grade dysplasia ) and MCN-IC.The univariate and multivariate logistic regression was used to analyze the differences on laboratory examinations and imaging findings and the like to identify the predictors for the preoperative diagnosis of MCN-IC.Receiver operator characteristic ( ROC) curve was used to evaluate fitting performance and Hosmer-Lemeshow test was performed to evaluate goodness of fit .Results Of the 132 patients, 115 (87.12%) were MCN-nIC, 17(12.88%) were MCN-IC.Univariate analysis identified old age(≥60 years), abdominal pain, anorexia, GLU elevated, CEA≥5 ng/ml, CA19-9≥37 U/ml, unclear border of tumor , thick wall (>2 mm) , presence of mural nodules and absence of the septa as independent predictors for MCN-IC. Multivariate analysis identified old age (≥60 years), abdominal pain, CA19-9≥37 U/mL, unclear border of tumor, presence of mural nodules and absence of the septa as the predictors for MCN -IC.The maximal area under ROC ( AUC) was 0.947, which indicated that the fitting performance of the model was satisfactory and the goodness of fit was better (P=0.056).Conclusions MCN-IC had a generally low prevalence .Old age (≥60 years), abdominal pain, CA19-9≥37 U/ml, unclear border of tumor, presence of mural nodules and absence of the septa may predict the diagnosis of MCN-IC.
