2.Expression of VEGF-C and VEGFR-3 in breast cancer tissue and its clinical significance

Objective To investigate the expression of vascular endothelial cell growth factor-C (VEGF-C) and its receptor--vascular endothelial cell growth factor receptor-3 (VEGFR-3) in breast cancer tissue and its correlation with metastasis of lymphonodi axillares.Methods Fifty-eight cases of breast cancer samples and 10 cases of tumor adjacent non-carcinomatous tissue were measured by immunohistochemical staining for expression of VEGF-C and VEGFR-3. Their expression rates were compared between axillary lymph node metastasis and non-metastasis groups.Results The positive rates of VEGF-C and VEGFR-3 as well as the number of VEGFR-3 positive vessels were significantly higher than those in lymph node non-metastasis group (87.5% vs 55.9%; 83.3% vs 38.2%; 8.54±2.54 vs 4.73±2.46, respectively).Conclusion The high expression of VEGF-C and VEGFR-3 as well as the increase in the number of VEGFR-3 positive vessels are related to lymph node metastasis of human breast cancer. VEGF-C/VEGFR-3 system may be a new specific target which contributes to blockage of lymphatic metastasis in breast cancer.
3.Value of absent or reversed diastolic flow in predicting hepatic artery thrombosis after liver transplantation

Objective To probe the value of absent or reversed diastolic flow in hepatic artery shown by Doppler sonography of recently transplanted liver in predicting subsequent hepatic artery thrombosis.Methods The records of 54 liver transplantations performed were retrospectively reviewed.The first Doppler study of every case was done within 24 h after transplantation and subsequently reviewed.Findings were categoried into one of three patterns: forward flow,no flow and reversed flow throughout diastole.Meantime,the flow status of portal vein of every case was studied by Doppler sonography.Results Of 54 liver transplants, 12 had aberrant diastolic flow in the hepatic artery after liver transplantation.Four had reversed flow and 8 showed no flow in diastole.In this group of 12 transplants,hepatic artery thrombosis was not developed.Conclusions Reversed or absent diastolic flow in hepatic artery of a recently transplanted liver has no correlation with subsequent hepatic artery thrombosis.
4.Application of serum cystatin C-based eGFR equations in renal function evaluation for kidney transplant recipients

Objective To compare the differences of the four estimating equations about MDRD,CKD-EPI eGFRcr,CKD-EPI eGFRcys,and eGFRcr-cysin evaluating the renal function of kidney transplant recipients,and explore the optimal equations to estimate the renal allograft function.Method Between January 2000 to March 2015,a total of 213 renal transplant recipients with data integrity were enrolled.eGFR was calculated using the equations:Modification of Diet in Renal Dis-ease [eGFR (MDRD),the creatinine-based equation (CKD-EPI eGFRcr),the CKD EPI cystatin C equation (CKD-EPI eGFRcys),and CKD EPI creatinine + cystatin C equation (CKD-EPI eGFRcr-cys) drafted by Chronic Kidney Disease Epidemiology Collaboration (CKD-EPI) in 2009 and 2012.We assessed the performance of these four eGFR equations by Pearson correlation analysis,Bland-Altman plot,and Cox regression analysis.Result The incidence of reduced kidney function was 49.3% using MDRD,43.7% using eGFRcr,49.8% using eGFRcys,and 52.1% using eGFRcr-cys,respectively.In recipients with eGFR ≥60 ml·min· 1.73 m2 estimated by MDRD,19.5% and 11% had eGFR <60 ml·min·1.73 m2 re-estimated by eGFRcys and eGFRcr-cys respectively.Serum cystatin C was significantly correlated with serum creatinine,MDRD,eGFRcr,eGFRcr-cys with the correlation coefficient being 0.67,0.61,0.65,and 0.77 respectively.Only the CKD staging based on eGFRcr,and eGFRcr-cys was positively related to renal graft loss risk.eGFRcr,and eGFRcr-cys could not be replaced each other,and only eGFRcr-cys showed strong correlation with the other three equations.Conclusion As compared with others,eGFR (CKD-EPI-Creat + CysC) is more precise in estimating GFR and evaluating the renal graft loss risk.
5.Noninvasive evaluation of hepatic fibrosis stages by contrast enhanced ultrasonography parameters

Objective To discuss the feasibility of non-invasive quantitative evaluation of hepatic fibrosis by contrast-enhanced ultrasonography (CEUS) parameters. Methods CEUS of liver and liver biopsy were performed in 86 patients. CEUS parameters including area under curve of portal vein/hepatic artery(Qp/Qa) ,perfusion intensity of portal vein/hepatic artery(Ip/Ia), decreasing rate of liver parenchyma perfusion (β) and perfusion time of portal vein (Tp) were calculated based on CEUS images. Differences of the four parameters among stages of hepatic fibrosis were analyzed by ANOVA and Spearman rank correlation test was applied to analyze correlation between parameters and hepatic fibrosis stages. Areas under receiver operating characteristic (ROC) curves were calculated to evaluate the diagnostic accuracy of parameters. Results Tp and β had increasing tend while Ip/Ia and Qp/Qa had declining tend from S0 to S4.They were significantly correlated with fibrosis stage( P <0.05=. The areas under ROC curves of Ip/Ia were 0.931 (≥S1) ,0.884(≥S2 ) ,0.820(≥S3 ) and 0.846(S4 ) respectively. The areas under ROC curves of Qp/Qa were 0.914(≥S1),0.813(≥S2),0.845(≥S3) and0.869 (S4) respectively. Conclusions CEUS parameters of liver parenchyma Ip/la, Qp/Qa, Tp and β are related to fibrosis stages. Ip/Ia and Qp/Qa provide a reliable,non-invasive method for evaluating fibrosis stages.
6.Research on the effectiveness between high flow nasal oxygen cannula and non-rebreather oxygen face mask in post-extubation patients

Objective To compare the clinical effects of high flow nasal cannula (HFNC) and non-rebreathing oxygen face mask (NRB) in post-extubation patients.Methods 88 critically ill patients with machinery ventilations were divided into HFNC group and NRB group randomly.Blood gas analysis and hemodynamic parameters were assessed 1 hour prior to extubation and 6 hours after extubation.The primary clinical outcomes measured were ventilation-free days,re-intubation patient numbers,length of stay in ICU (Intensive Care Unite),total duration of hospitalization and mortality.The scant of breath degree and comfortableness of patient were recorded according to the Visual analogue scale.The measurement data were described by mean ± standard deviation ((x) ± s) and analyzed with t test,enumeration data were described by number of cases and composition ratio and analyzed with X2test,P < 0.05 was considered to have statistical difference.Results There was no significant difference in clinical features between the two groups,The oxygenation index of HFNC group is significantly higher than that of NRB group after extubation [(251.4 ±43.9) vs.(201.7 ±60.7),P =0.037)].There were more ventilator-free days in the HFNC group than NRB group [(4.2 ± 2.1) vs.(3.4 ± 2.8),P =0.037)] and fewer patients required reintubation (P =0.028).The rate of ventilator associated pneumonia is also lower than NRB group (P =0.024).The patients' scant of breath feeling were obviously allevated comparing with the NRB group [(2.9 ± 1.1) vs.(3.7 ± 1.8),P =0.042)].The oxygenation index of NRB group significantly decreased after extubation [(242.9 ±68.4vs.201.7 ±60.7 P =0.048)].The two groups demonstrated similar hemodynamic patterns before and after extubation.And there were no statistically significant clinical differences in PaCO2,length of ICU stay,total duration of hospitalization or mortality.Conclusions Compared with NRB,HFNC is a more safe and effective clinical tool in the prevention and treatment of critical adult patients with extubation failure.
7.Effects of Resina Draconis on the Levels of Plasma Glucose, Insulin and Lipids in Rats

Objective:To investigate the effects of Resina Draconis on the levels of plasma glucose,insulin and lipids in normal and diabetic rats. Methods: Hyperglycemia rat models were induced by glucose or adrenaline and rat models with diabetes mellitus were induced by alloxan.The levels of plasma glucose, insulin and lipids (triglyceride, total cholesterol and HDL-C) were determined. Results: Resina Draconis had no obvious effects in decreasing the plasma sugar level in normal fasted rats but could decrease the plasma sugar level and improve the glucose-resistance in glucose-induced or adrenaline-induced hyperglycemic rats and alloxan-induced fasted diabetic rats and increase insulin secretion in both normal and diabetic rats. It could also decrease the plasma lipid level in alloxan-induced diabetic rats. Conclusion: Resina Draconise exerts a good therapeutic effect on hyperglycemia in diabetic rats and its mechanism may be related to the increase of the secretion of insulin.

9.Effect of extracorporeal shock-wave lithotripsy on residual stones after different methods of surgery

Objective To observe the effect of extraeorporeal shock wave lithotripsy (ESWL) on residual stones after different methods of surgery. Methods Clinical resources of 100 patients with residual stones after different methods of surgery treated with extracorporeal shock wave lithotripsy from May 2006 to May 2008 were retrospeetively studied. Of the 100 patients, ureteroscopic laser lithotripsy was used for 15 patients (Group Ⅰ) , ureteroscopic pneumatic lithotripsy was used for 25 (Group Ⅱ), mini-percutaneous nephrolithotomy (MPCNL) with holmium laser for 11 (Group Ⅲ), mini-percutaneous nephrolithotomy (MPCNL) with airpressure path lithotripterfor 12 (Group Ⅳ) , open surgery for the other 37 (Group Ⅴ). Results About 94% of the residual stones were shattered, and 86 % of the residual stones were cleared successfully. The clearance rate of residual stones from Group Ⅰ to Ⅴ was 100% , 100% , 81.8% , 83.3% , and 73.0% , respectively. The clearance rate of residual stones in Group Ⅰ + Ⅱ was higher than that of Group Ⅲ + Ⅳ and Group V (P<0.05). Conclusion Extracorporeal shock wave lithotripsy is good for the treatmentof residual stones after different methods of surgery, especially the management of residual stones after trans-urethral ureteroscope technique.
10.Quantitative evaluation of portal pressure by contrast enhanced ultrasonography

Objective To investigate the feasibility of non-invasive quantitative evaluation of portal pressure (Pp) by contrast enhanced ultrasonography (CEUS).Methods 18 portal hypertension patients (PTH group) were performed with CEUS within one week before splenectomy and pericardial devascularization,and 20 healthy volunteers as controls were recruited in this study (control group).Hepatic-right kidney sectionwas chosen to calculate the area under curve of portal vein/hepatic artery (Qp/Qa) and the perfusion intensity of portal vein/hepatic artery (Ip/Ia) through time intensity curves (TIC) of liver parenchyma generated from CEUS images.Pp was measured by intra-operative mesenteric vein catheter,and the correlation betweenPp and Qp/Qa,Ip/Ia were analyzed by Pearson correlation test.Results The levels of Qp/Qa and Ip/Ia in the PTH group were 2.28 ± 0.66 and 0.35 ± 0.14 respectively,which were both significantly declined than that in the controlgroup (5.72 ± 3.69 and 1.97 ± 0.17).In the PTH group,the correlation coefficient were-0.747 and-0.617,and the linear regression equations were Y =-83 X + 5.013 andY =-15X + 0.837,which indicated that Qp/Qa and Ip/Ia had significant correlation with Pp.Conclusions CEUS parameters,including Qp/Qa and Ip/Ia,are significantly correlated to Pp in portal hypertension patients,which indicate that CEUS could be a new non-invasive clinical method for evaluating Pp.
