1.Diagnostic value of procalcitonin and C-reactive protein in bronchial asthma complicated with respiratory infection

Objective To explore the diagnostic value of serum procalcitonin(PCT) and C-reactive protein (CRP) in using of antibiotics in patients with acute bronchial asthma attack.Methods Sixty cases with acute bronchial asthma attack were divided into allergen-induced group (18 cases),viral infectioninduced group (14 cases) and bacteria infection-induced group (28 cases).The serum PCT level was examined by immune luminous method.The CRP level was examined by enzyme linked immune scattered method.Results The serum PCT and CRP level was (25.7 ± ll.2)μg/L and (50.3 ± 17.5) mg/L in bacteria infection-induced group,which was significantly higher than that in viral infection-induced group and allergen-induced group [(0.7 ± 0.2) μ g/L,(6.1 ± 0.3) mg/L and (0.2 ± 0.1) μ g/L,(3.5 ± 0.4) mg/L,P < 0.05].There was no significant difference in the serum PCT and CRP level between viral infectioninduced group and allergen-induced group (P > 0.05).There was positive correlation between PCT and CRP in bacterial infection group (r =0.932,P < 0.05),while no correlation existed between PCT and CRP in viral infection-induced group and allergen-induced group (r =-0.021,-0.103,P>0.05).CRP≥10 mg/L andPCT ≥0.5 μ g/L was as positive criteria.The positive rate of serum CRP and PCT in bacteria infection-induced group was 89.3%(25/28) and 92.9%(26/28),which was higher than that in viral infection-induced group and allergen-induced group [28.6% (4/14),64.3% (9/14) and 22.2% (4/18),33.3% (6/18),P <0.05],and the positive rate of serum CRP and PCT in viral infection-induced group was higher than that in allergen-induced group,there was significant difference (P < 0.05).Conclusion The serum PCT and CRP level can correctly identify early bacteria infection patients with acute bronchial asthma attack.
2.Analysis on the group purchasing organizations in the United States and its implications for Chinese drug procurement

In this paper, basic characteristics of group purchasing organizations ( GPOs) and its implications for US health care system are analyzed using a literature research method. The main contents include the development process, type, basic functions, drug procurement processes, fee sources, characteristics and influence for the hospi-tals, supplies, regulators of GPOs. We suggest that GPOs have helped US health care providers save a lot of money and played an important role in the healthcare supply chain. We also suggest that market mechanisms should be intro-duced widely concerning the model innovation and improvement of domestic drug procurement. At the same time, government and the market should be coordinated properly. Moreover, scientific evaluation methods should be set in consideration of drug quality, prices and its economic practicality.
3.Curative effect of cephalosporins in the treatment of neonatal infectious pneumonia and its effect on intestinal microflora

Objective To investigate the curative effect of cephalosporins in the treatment of neonatal infectious pneumonia and the effect on intestinal microflora.MethodsA total of 124 cases of neonatal pneumonia in our hospital were divided into the cephalosporin group (40cases), the piperacillin group (38 cases) and the combined treatment group (46 cases), and 40 healthy neonates were selected as healthy group.The clinical efficacy was compared.The intestinal bacterial genus of the four groups was examined on the 5th day after treatment, including Enterobacter, Bacteroides, Enterococcus, Bifidobacterium and Lactobacillus.ResultsThe cure rates of the cephalosporin group, the piperacillin group and the combined group were 82.5%, 81.57% and 89.13%, respectively.The healing time of the above three groups was (5.3±0.2) d, (5.5±0.3) d and (5.2±0.3) d, respectively.Enterobacteriaceae, Bacteroides, Enterococcus and Lactobacillus were significantly more in the above three groups than the healthy group, and Bifidobacterium was fewer than healthy group (P<0.05).There was no significant difference between the cephalosporin group and piperacillin group.ConclusionThe curative effect of cephalosporins is similar to other antibiotics in the treatment of neonatal infectious pneumonia.The former can effectively relieve alteration of intestinal flora, combined with other drugs.Irrational use of antibiotics woll increase alteration of intestinal flora.
4.The expression of PPARγmRNA and protein in rats with acute necrotizing pancreatitis

Objective To investigate the changes of PPARr mRNA and protein expressions in rats with ANP. Methods 36 rats were randomly divided into sham group and ANP group. The rats were sacrificed 3 h, 6 h,12 h after ANP induction, the levels of serum amylase were measured, the pancreatic pathological changes were determined and the expressions of pancreatic PPARr mRNA and protein was examined by RT- PCR and immunohistochemistry. Results In ANP group, the level of serum amylase at 6h was (7170.83± 1635.59) U/L, the scores of pathological changes were 6.67±1.03 and 13.00±2.36, which were much higher than those of sham group (P<0.01) ; the PPARr mRNA expression was 0.18±0.05, and there were no obvious differences compared with that of sham group (0.22±0.03 ) ; PPARr protein expression was 4.17 ±0.98, which was significantly higher than that of sham group (1.83±0.71, P<0.05). Conclusions Inflammatory injury resulted in increased deactivation of pancreatic acinar PPARr, meanwhile PPARr gene expression was inhibited by feedback.
5.Prediction of schistosomiasis infection rates of population based on ARIMA-NARNN model

Objective To explore the effect of the autoregressive integrated moving average model?nonlinear auto?regressive neural network(ARIMA?NARNN)model on predicting schistosomiasis infection rates of population. Methods The ARIMA model,NARNN model and ARIMA?NARNN model were established based on monthly schistosomiasis infection rates from Janu?ary 2005 to February 2015 in Jiangsu Province,China. The fitting and prediction performances of the three models were com?pared. Results Compared to the ARIMA model and NARNN model,the mean square error(MSE),mean absolute error (MAE)and mean absolute percentage error(MAPE)of the ARIMA?NARNN model were the least with the values of 0.011 1, 0.090 0 and 0.282 4,respectively. Conclusion The ARIMA?NARNN model could effectively fit and predict schistosomiasis in?fection rates of population,which might have a great application value for the prevention and control of schistosomiasis.
6.Interaction of butylphthalide with rat and human liver CYP450 isoenzymes.

The work aims to study the drug metabolizing enzymes involved in the metabolism of butylphthalide and evaluate the induction and inhibition activities of butylphthalide on CYP450 isoenzymes by using in vitro (liver microsome incubation system of rats and human) and in vivo (CYP induced model of rats) method. Butylphthalide was incubated with selective inhibitors of CYP450, and its metabolic rate was determined to identify the metabolizing isoenzymes of NBP in rat (normal and induced rats) and human liver microsomes. The in vitro inhibition effect of butylphthalide on 6 main liver microsomal CYP450 isoenzymes was evaluated by using probe drugs; the induction and inhibition activities in vivo of butylphthalide on CYP450 isoenzymes were evaluated by NBP ig dosing (160 mg x kg(-1)) and iv dosing (20 mg x kg(-1)) in rats. After adding the specific inhibitors of CYP2C11, 2E1 and 3A 1/2 for rat, CYP2C19, 2E1 and 3A4/5 for human, the metabolism of NBP in rat and human liver microsomes were reduced 38.8%, 86.2%, 78.4% and 51.0%, 92.0%, 58.9% of control, respectively. The metabolic rates of NBP in CYP2E1 and 3A 1/2 induced rat liver microsomes were increased 25.5% and 68.9%. High concentration of NBP (≥ 200 μmol x L(-1), in vitro) could inhibit the activities of CYP1A2, 2C6, 2C11 and 2D2 in rats, and high concentration of NBP ( ≥ 15 μmol x L(-1), in vitro) could inhibit the activity of CYP2C19 in human. All the results indicated that NBP should be mainly metabolized by CYP2E1, 2C11 and 3A 1/2 in rats and CYP2E1, 2C19 and 3A4/5 in human. High concentration of NBP could inhibit human CYP2C19 in vitro. No significant induction/inhibition effects of NBP were observed on rat liver CYP450 isoforms after ig 160 mg x kg(-1) NBP or iv 20 mg x kg(-1) NBP.
7.Transformation of PPS induce M2 subtype to M1 macrophages

Objective:To study the positive expression rate of M2 subtype of macrophage cell surface molecules and the inflammatory factors of PPS in IL-4-induced M2 macrophage.Methods:The experiment was divided into 5 groups:blank control group, Model group,PPS groups(50 μg/ml,100 μg/ml and 200 μg/ml).The expression of CD206 and CD23 was used as bio-maker to confirm IL-4 induced macrophages by treating RAW264.7 with 20ng/ml of IL-4.IL-4 induced RAW264.7 cells were treated with PPS of 50μg/ml,100μg/ml and 200μg/ml for 24 h.Then the expression of CD206,CD16/32 and CD40 were analyzed by flow cytometry, and the mRNA expression of IL-1β,TNF-α,IL-10 and iNOS were detect by qRT-PCR.Results: After treated with IL-4,the positive rate of CD206 of RAW264.7 were high.After treated with PPS ,the rate of CD16/32 and CD40 in IL-4 induced RAW264.7 cells were high ,the expression of CD206 decreased,and the mRNA level of IL-1βand TNF-αincreased.Conclusion:RAW264.7 cells can be polarlized to M2 subtype macrophage by using 20 ng/ml IL-4.PPS enhances the mRNA of IL-1β,TNF-αand the expression of CD40, CD16/32 in IL-4-induced RAW264.7 cells .These results indicate that PPS can induce the M2 subtype to become M1 macrophages, can improve immune function of macrophages.
8.Clinical Observation of Electroacupuncture and Acupoint Injection in Treating Common Fibular Nerve Paralysis

Objective: To observe the clinical efficacy of combined electroacupuncture and nerve growth factor (NGF) injection at acupoints in the treatment of common fibular nerve paralysis and provide evidences for integrative Chinese & western medicine against diabetic peripheral neuropathy (DPN). Methods: Forty subjects were randomized into two groups and NGF injection; and control group was given herbal suffocation, oral Dibazol and compound vitamin B and Mecobalamin Injection. The clinical symptoms and nerve conduction velocity were observed and compared. Results: The cure rate was higher in treatment group than in control group (P<0.05); after treatment, the nerve conduction velocity was improved in both groups (P<0.01), with a significant improvement in treatment group than in control group (P<0.01). Conclusion: Combined electro-acupuncture and NGF injection at acupoints is quite effective in the treatment of common fibular nerve paralysis.
9.Simultaneous Determination of Contents of Asiaticoside, Tetrahydropulmatine and Saikosaponin d in Shenji Huwei Granules by HPLC

Objective To develop an HPLC method for simultaneous determination of contents of asiaticoside, tetrahydropulmatine and saikosaponin d in Shenji Huwei Granules. Methods The analysis was performed on a R&C C18 column (250 mm × 4.6 mm, 5 μm) by using the mobile phase of acetonitrile (A) - phosphate buffer (which used potassium dihydrogen phosphate 8.34 g, potassium phosphate 0.87 g dissolved by 1000 mL water) with gradient elution (0–15 min, 20%A; 15–30 min, 20%→40%A; 30–42 min, 40%A; 42–45 min, 40%→48%A; 45–50 min, 48%A; 50–70 min, 48%→50%A). The flow rate was 1.0 mL/min; the detection wavelength was set at 210 nm; the column temperature was maintained at 30 ℃. Results Asiaticoside, tetrahydropulmatine and saikosaponin d were in the linear ranges among 0.173–2.770 μg (r=0.9999), 0.021–1.320 μg (r=0.9992), 0.151–9.660 μg (r=0.9993), respectively. The average recovery rates of asiaticoside, tetrahydropulmatine, saikosaponin d were 96.25%, 97.02%, and 97.84%, respectively, and RSD were 2.31%, 4.51%, 1.87%, respectively. Conclusion This method is simple, with good separation effect and strong specificity, and can be used for simultaneous determination of contents of asiaticoside, tetrahydropulmatine and saikosaponin d in Shenji Huwei Granules, which provides references for perfection of quality control of Shenji Huwei Granules.
10.Serological Investigation of STIs in 138 Men Who Have Sex with Men

Objective To investigate serologic screening for sexually transmitted infections (STI) in men who have sex with men (MSM) and provide evidence for intervention in the population. Methods A questionnaire investigation was conducted in MSM. Serologic specimens were taken to detect antibodies to HIV, syphilis, herpes simplex virus (HSV)-2, hepatitis C virus, and hepatitis E virus, as well as HBsAg. Results A total of 138 MSM were screened. HIV antibody was negative in all of them. TPPA was reactive in 23 (16.7%) men, among which RPR was positive in 10 (7.2%). The antibodies to HSV-2-IgG, hepatitis C virus, and hepatitis E virus, and HBsAg were found in 7.8%, 0, 16.1% and 9.7% of the men, respectively. Conclusions STIs are identified in this group of MSM. It is important to provide preventive intervention for this population.
