1.Application of Quality Control Graphs in Blood Concentration Monitoring of Tacrolimus

OBJECTIVE:To increase the accuracy of blood concentration monitoring results of tacrolimus.METHODS:Blood concentrations of tacrolimus(60points for high and low quality control,respectively)were determined by enzyme-linked immunosorbent assay and the quality control graphs were drawn.The whole monitoring course underwent both prospective and retrospective quality control.RESULTS:The quality control monitoring results could be evaluated from the quality control graphs.CONCLUSION:Quality control can effectively increase the accuracy of blood concentration monitoring results of tacrolimus,which can also prevent and rectify systemic error.
2.Investigation of Pain for the Elderly with Dementia

Objective To investigate the incidence of pain for the.elderly with dementia in our country.Methods The Chinese version of Doloplus-2 was used to assess pain in 103 patients with dementia.Results One or more specific diseases may cause pain in 94 patients,degeneration of bone and joint top of the list;69 patients with pain score geater than or equal to 5 points,the incidence of pain as much as 67% .Older persons in different age groups the incidence of pain is different,with the age increased the incidence of pain increased;different older age groups pain scores had no significant difference.Conclusion Pain in patients with dementia should be of great importance to the medical staff and give full play to the nursing staff in pain management.
3.Retrospective investigation of pharmacologic management in elderly inpatients with chronic heart failure

Objective To understand the practical application of drug intervention in elderly patients with chronic heart failure (CHF) for providing evidences and data for a standard treatment. Methods The elderly CHF patients aged at least 60 years who were hospitalized from July 1973 toJuly 2007 in the Second Hospital of Tianjin Medical University and from January 1983 to December 2002 in Tianjin First Center Hospital were enrolled. The age, gender, etiology, heart function and drug therapy were retrospectively analyzed. The patients were divided into 4 groups: group 1973-,group 1980-, group 1990- and group 2000-2007. And the data were collected and analyzed by SPSS 13.0 software. Results A total of 4704 cases were enrolled, of which there were 2430 (51.7%)male and 2274 (48. 3%) female. Mean age was (71.3±-7. 1) years. The patients aged 80 years andover accounted for 12.8% (603/4704). The first three ranking causes were pulmonary heart disease,coronary heart disease and rheumatoid heart disease in group 1973- and 1980-. And those were coronary heart disease, pulmonary heart disease and rheumatoid heart disease in group 1990 and 2000-2007. The combined uses of angiotension converting enzyme inhibitors(ACEI) and β-receptor blocker,and of ACEI, β-receptor blocker and aldosterone antagonist were also increasing year by year. Patients with coronary heart disease, rheumatoid heart disease, pulmonary heart disease and dilated cardiomyopathy had a higher proportion of multiple drugs cotreatment. The differences in the frequency of all drugs use among four groups of CHF had a statistical significance (p<0.05). The usage of ACEI combined with β-receptor blocker and aldosterone antagonist, and ACEI combined withβ-receptor blocker, diuretic and digitalis was increasing along with the heart function becoming serious. The patients with a heart function of New York Heart Association (NYHA) Ⅱ or Ⅲ class received more β-receptor blocker. And ACEI was used at most in patients with NYHA Ⅲ class. The angiotension Ⅱ receptor blockers(ARB) was used at most in NYHA Ⅲ or Ⅳ class. Conclusions Nitrate, diuretic and digitalis are the main therapeutic agents in hospitalized patients with CHF in some regions of Tianjin. ACEI, β-receptor blocker and ARB are used in a rapid increment. There are still some problems such as standardizing the heart failure treatment and prevention in the elderly.
4.Interventional effect of emodin on the changes of sphingomylinase activity and ceramide content in experimental rabbit with atherosclerosis

BACKGROUND: It is reported that ceramide signal pathway may play an important role in the apoptosis of vascular endothelial cell and then lead to the progress of atherosclerosis, such as the formation of foam cells and the proliferation of smooth muscular cells.OBJECTIVE: To study the changes of the sphingomylinase activity and ceramide content in aorta of rabbits with experimental atherosclerosis and investigate the regulative effects and mechanism of emodin on them as compared with positive fenofibrate.DESIGN: Completely randomized controlled design.SETTING: Laboratory of Pharmacology & Toxicology, School of Pharmaceutical Science of Sun Yat-sen University.MATERIALS: The experiment was completed at the Laboratory of Pharmacology & Toxicology, School of Pharmaceutical Science of Sun Yat-sen University from July to December 2003. Totally 48 New Zealand male rabbits were selected. Forty animal models of atherosclerosis were made with high cholesterol feed, and the other 8 rabbits were selected as the normal controls. Model animals were divided randomly into model group, 5 mg/kg emodin group, 10 mg/kg emodin group, 20 mg/kg emodin group and 25mg/kg fenofibrate group with 8 in each group.METHODS: At the seventh weeks of model duplication, 5 mg/kg, 10 mg/kg and 20 mg/kg emodin were perfused in rabbits of emodin groups respectively, and 25 mg/kg fenofibrate was perfused in rabbits of fenofibrate group. Emodin and fenofibrate were diluted or suspensed with 2 mL saline once per day respectively. Rabbits in normal control group and model group were administrated with the same volume of saline for 4 weeks. The rabbits were raised separately and were fed with 135-150 g food per day.MAIN OUTCOME MEASURES: [1] The area of the lipid plaque in aortal intima; [2] the content of serum TC and TG; [3] SOD activity and MDA content; [4] SMase activity and CER content in aorta.RESULTS: Totally 48 rabbits entered the final analysis. [1] Area of the lipid plaque: Area of the lipid plaque was (48.87±15.5) % in the model group, which was larger than that in each emodin group (P < 0.05 or 0.01),especially larger than that in the 10 mg/kg emodin group (22.19±12.9)%while that in the fenofibrate group was similar to that in the model group (P > 0.05). [2] Content of serum TC and TG: The anrtal intima of control was smooth. Content of serum TC and TG in each emodin group were similar to those in the model group (P > 0.05), but those in the 25 mg/kg fenofibrate group were lower than those in the model group (P < 0.05). [3]Content of SOD and MDA in plasma: SOD activity of rabbits in each emodin group was higher than that in the model group (P < 0.05, P < 0.01),but the MDA activity in the 10 mg/kg and 20 mg/kg emodin group was lower than that in the model group (P < 0.05). The MDA activity in the25 mg/kg fenofibrate group was similar to that in the model group (P > 0.05).[4] Content of SMase and CER: Those in the model group were higher than those in the normal control group, but those in the 5, 10 and 20 mg/kg emodin groups were lower than those in the model group; those in the 25 mg/kgfenofibrate group were similar to those in the model group (P > 0.05). [5]Analysis of correlation: Content of SMase was in positive relation with blood cholesterol (r=0.542, P < 0.01), in positive relation with blood MDA (r=0.789, P > 0.01), and in negative relation with blood SOD(r=-0.936, P > 0.01); content of CER was in positive relation with blood cholesterol (r=0.433, P > 0.05), in positive relation with blood MDA (r=0.673, P > 0.01), and in negative relation with blood SOD (r=-0.876, P > 0.01).CONCLUSION: The study finds that emodin, despite its insignificant effects on decreasing TG or TC, can protect vascular endothelial cells and reduce the area of lipid-laden plague of aortal intima by antioxidation, inhibition of the sphingomyelinase activity and reduction of the content of ceramide. It is suggested that moderate dosage of emodin employed in the study may be most appropriate to atherosclerosis treatment.

6.THE STUDY OF INFLUENCE OF LONG-TERM SMOKING ON THE EXPRESSION AND MUTATION OF p53 AND K-ras GENE IN RAT LUNGS

Objective To observe the influence of long-term smoking on the expression of p53 and K-ras in rat lung tissues, and to study the relationship of smoking to the mutation of p53 and K-ras gene. Methods The model of Wistar rat smoking was built up. Seventy-two healthy male Wistar rats were divided into the experimental group and control group at random. The rats of the experimental group were compelled to smoke, and the rats of the control group were given the same condition as the experimental group was, without smoking. The rats of the experimental group were smoked for 6 months. At the end of each month, 6 rats were chosen from the two groups respectively, their lung tissues were sampled and immunohistochemistry was applied to observe the expression of p53 and K-ras in lung tissues. Finally, the mutation which might happen in the exon 5, 6, 7-8 of p53 and the exon 1 of K-ras was examined by PCR-SSCP. Results The p53 protein was expressed in cell nucleus and K-ras in cytoplasm. The positive ratio of protein expression was increased with the extension of smoking time. The mutation of p53 was increased as the smoking time extended. But the effect of smoking time was not that significant on the mutation of K-ras.Conclusion Smoking can strengthen the expression of p53 and K-ras protein and can also result in gene mutation. As the time of smoking extended, those phenomenons were tending to rise. That provided the theoretical evidence which can be used to judge the lesion of lung tissues caused by smoking and help the early diagnosis of smoking-related lung carcinomas. It is of great theoretical and application values.
7.Safety and Adverse Drug Reactions of Antongding

OBJECTIVE:To evaluate the adverse drug reaction(ADR) induced by Antongding and its safety in use.METHODS:According to the organ/system type recommended by WHO,34 cases with ADRs induced by Antongding were classified.The ADRs in this series were analysed concerning clinical manifestations,severity of symptoms and inducing mechanism and the safety of use of Antongding was discussed.RESULTS:8 organs/systems were involved in 34 cases with ADRs,of them,18 cases had moderate and severe ADRs(52.94%).In vitro experiments showed that Antongding could inhibit CFU-GM in man.CONCLUSION:Antongding is apt to induce ADRs,so it has potential risk in use.
8.Sophoridine inhibits NF-kappaB signaling pathway activation in kidney tissue of endotoxemia mice.

This study is to investigate the effects of sophoridine on NF-kappaB signaling pathway in kidney tissue of endotoxemia mice and the mechanism involved. BALB/c mice were challenged with lipopolysaccharide (LPS) caudal vein injection, then sophoridine was administered by intraperitoneal injection. Totally 50 mice were divided into 5 groups: control group, LPS model group, sophoridine treatment 12 mg x kg(-1) group, 6 mg x kg(-1) group and 3 mg x kg(-1) group. All animals were sacrificed at 6 hours after treatment. Kidney and blood samples were harvested. IKKbeta mRNA and TNF-alpha mRNA expression of renal tissue was measured by the reverse transcription polymerase chain reaction (RT-PCR), and phosphorylation IKKbeta protein (pIKKbeta) was detected by immunohistochemistry. NF-kappaB P65 protein expression and distribution of renal tissue were observed by Western blotting and immunofluorescence laser confocal microscopy. Serum TNF-alpha level was detected by radioimmunoassay. The results showed that the sophoridine significantly reduced the expression of IKKbeta mRNA and pIKKbeta protein, and inhibited the expression of NF-kappaB P65 protein and decreased the entry nuclear rate of NF-kappaB P65 in the renal tissue of endotoxemia mice. Thereby the renal TNF-alpha mRNA expression and serum TNF-alpha level were significantly reduced. These results suggest that sophoridine could inhibit inflammatory reaction induced by LPS through inhibiting activation of NF-kappaB signaling pathway.
9.The effect of bed exercise for patients with intracranial aneurysm undergoing neural interventional treatment

Objective To approach the effect of bed exercise for patients with intracranial aneurysm undergoing neural interventional treatment. Methods Ninety-six patients with intracranial aneurysm were divided into the test group(50 cases) and control group(46 cases).The control group received routine nursing, while the test group carried out bed exercise base on routine nursing. The incidence of complica-tions and changes of life quality were compared between the 2 groups. Results Lower incidence of com-plications took place in the test group than the control group(P < 0.05).But changes in pulse rate and blood were not significant (P > 0.05).The score of life quality in the physical domain and psychological domainwere not statistically different between the two groups (P < 0.01 ),but no evident difference was seen in the domain of social relationship and environmental domain (P > 0.05). Conclusions Bed exercise could al-leviated the anxiety and trepidation of patients with intracranial aneurysm,decrease the incidence of compli-cations such as body pain caused by absolute bed rest,thus improve patients' life quality.
10.The effect of rational emotive therapy on serf-esteem status of breast cancer patients

Objective To explore the effect of rational emotive therapy on self-esteem status of breast cancer patients.Methods 118 cases of breast cancer patients were randomly divided into the control group and the observation group with 59 cases in each group.The control group received routine care,and the observation group received rational emotive therapy on the basis of routine care.The self-esteem status of the two groups of patients was assessed before and after the intervention respectively with State Self-Esteem Scale (SSES).Results The score of behavior self-esteem,social self-esteem,appearance self-esteem and the total score of self-esteem of the patients in two groups before intervention showed no significant difference,but the above scores were higher in the observation group than those in the control group after intervention.Conclusions Rational emotive therapy can improve self-esteem status of breast cancer patients,thereby improve the mental health of the patients.
