1.Analysis of influencing factors for persevering in pelvic noor muscle exercise in puerperal women and nursing countermeasures

ObjectiveTo investigate the influencing factors for persevering in pelvic floor muscle exercise in puerperal women and nursing countenneasures.MethodsThe influencing factors for persevering in pelvic floor muscle exercise in 120 puerperal women before June 2010 were analyzed,and they were set as the control group.According to the analysis results,targeted nursing countermeasures were developed,comprehensive care approach that combined nursing intervention with health education were applied in 116 cases of puerperal women from June 2010 to June 2011,who were set as the observation group.The situation of recovery for maternal pelvic floor muscle tension was observed.ResultsThere were four main factors that affected maternal pelvic floor muscle exercise:pelvic floor muscle exercise is not inspected as post-natal visits items,obstetric nurses did not carry out necessary propaganda,did not understand the importance of pelvic floor muscle exercise and lack of knowledge among the puerperal women.The test results for pelvic floor muscle tension were as followed:in the observation group Ⅰ degree was in 9 cases,accounting for 7.76%; Ⅱ degree was in 31 cases,accounting for 26.72%; Ⅲ degree was in 76 cases,accounting for 65.52%.In the control group,I degree was in 35 cases,accounting for 29.17 %; Ⅱ degree was in 44 cases,accounting for 36.67%; Ⅲ degree was in 41 cases,accounting for 34.17%.The recovery effect of pelvic floor muscle tension in the observation group was obviously better than the control group,the difference was significant.ConclusionsFactors influencing the puerperal women in persevering in pelvic floor muscle exercise was various,personalized care and intervention according to the influencing factors can improve compliance of pelvic floor muscle exercise for puerperal women,contribute to the recovery of pelvic floor muscle tension.
2.Experimental study on biodistribution and PET imaging of 3′-deoxy-3′-~(18)F-fluorothymidine in murine model of lung carcinoma

Objective To investigate the biodistribution and positron emission tomography (PET) imaging of 3′-deoxy-3′-~ 18 F-fluorothymidine (~ 18 F-FLT) in a murine model of pulmonary carcinoma, and to evaluate the use of ~ 18 F-FLT as a new PET tracer for diagnosis of pulmonary malignant tumor. Methods 40 T739 mice bearing the pulmonary adenocarcinoma were randomly divided into five groups according to different tracers and time after their injection (n=8/group). The biodistribution of mice for ~ 18 F-FLT was measured with well-gamma detector at 30min, 60min, 90min, 120min after injection via the tail veins. The biodistribution of mice for ~ 18 F-FDG was examined at 60min after injection as controls. In addition, the PET imaging of mice was performed using two tracers. Results In the biodistribution study of ~ 18 F-FLT, considerable radioactive uptake in tumor was observed, and high radioactivity was showed in the kidney and spleen. The T/NT ratios of tumor/blood, tumor/muscle and tumor/lung was all above 2.0. The tumor PET images with ~ 18 F-FLT were clear, as well. Conclusions The uptake of ~ 18 F-FLT in pulmonary adenocarcinoma is higher than that in normal tissues, thus the pulmonary neoplasm could be identified accurately with PET imaging. Our preliminary study of ~ 18 F-FLT in lung carcinoma xenografts is satisfactory, and it provides a basis for further clinical study.
3.Studies on the expression of hematopoietic growth factor modulated by Egr-1 promoter sequence in transcriptional targeting by chemotherapeutic agents

Objective To explore the chemotherapeutic agents-induced modulating effects of Egr-1 promoter sequence on GM-CSF expression and its protective effect against injury to haematopoiesis due to chemotherapy.Methods Human GM-CSF cDNA and enhanced green fluorescent protein(EGFP) cDNA were linked to internal ribosome entry site(IRES) respectively,and then recombined to pCIneo vector containing Egr-1 promoter(Egr-EG).The vector was transferred into human bone marrow stromal cell line HFCL by lipofection,and the HFCL/EG cells were then finally constructed.MTT assay was performed to determine the effects of cisplatin(DDP),5-fluorouracil(5-FU),gemcitabine(GEM) and paclitaxel(PTX) on the survival rates of HFCL/EG and HFCL cells,and the IC50 of such agents against HFCL/EG and HFCL cells were calculated.The percentage of HFCL/EG cells which positively expressed EGFP was assessed by both flow cytometry and inverted fluorescence microscope.The effects of the active oxygen inhibitor N-acetylcysteine(NAC) on the expression of GM-CSF,which was modulated by promotor Egr-1,were detected by ELISA.The effects of HFCL/EG supernatant on CFU-GM after being exposed to chemotherapeutic agents were examined.Results With different sensitivity to DDP,5-FU,GEM and PTX,HFCL/EG cells were successfully constructed.The drugs showed higher IC50 value against HFCL/EG cells than HFCL cells(P

AIM: To investigate the apoptosis of implanted tumor of primary human gastric cancer cells in nude mice induced by genistein and the relation between this apoptosis and expression of bcl - 2 and bax. METHODS: Establishing a transplanted tumor model by injecting human primary gastric cancer cells into subcutaneous tissue of nude mice. The different doses of genistein (0.5mg/kg, 1mg/kg and 1.5 mg/kg ) were directly injected beside tumor body respectively,for six times at an interval of two days. Then changes of tumor volume were measured continuously and tumor inhibition rate of each group was calculated. We observed the morphologic alteration by electron microscope, measured the apoptotic rate by TUNEL staining method, detected the expression of apoptosis - regulated gene bcl - 2 and bax by immunohistochemical staining and RT- PCR. RESULTS: Genistein could significantly inhibit carcinoma growth when it was injected near the carcinoma. Genistein induced implanted tumors cells to undergo apoptosis with apoptotic characteristics by transmission electron microscope. The apoptosis index of above three groups was increased progressively. Positive rate of Bcl - 2 protein of above three groups was decreased progressively and positive rate of Bax protein of above three groups was increased progressively by immunohistochemical staining. The density of bcl -2 mRNA decreased progressively and the density of bax mRNA increased progressively with elongation of time by RT - PCR. CONCLUSION: Genistein is able to induce the apoptosis of transplanted tumor cells. This apoptosis may be mediated by down - regulating bcl - 2 and up - regulating bax mRNA and its protein.
6.Diagnostic value of Geneva score combined with rapid plasma D-dimer assay for pulmonary embolism

Objective To study the value of Geneva score assessment combined with simplified rapid plasma D-dimer assay(SRPDDA)for early diagnosis of pulmonary embolism(PE).Methods Clinical data of 658 suspected cases of PE hospitalized at Shanghai Pulmonary Hospital during January 1,1995 to November 30,2007 were retrospectively analyzed.The patients were divided into three categories based on their Geneva score assessment,highly suspected with equal to or greater than 9 scores,intermediately suspected with 5-8 scores and mildly suspected with equal to or less than 4 scores,respectively.Clinical diagnosis for PE of the patients was made based on their probability Geneva score developed by Wicki,SRPDDA,and both Geneva score assessment and SRPDDA,respectively,and compared with results of lung imaging.Results Finally,PE diagnosis was established in 267 cases.Sensitivity for Geneva score and SRPDDA to diagnose PE was 88.4%and 90.3%,with negative predictive value(NPV)of 88.9%and 90.2%and Youden Index(YI)of 51.7%and 51.4%,respectively.And,sensitivity of both Geneva score assessment combined with SRPDDA wag 97.8%,with NPV and YI of 97.3%and 53.8%,respectively,with a likelihood ratio(LR)for both positive test results of 2.62 and a LB for both negative test results of 0.04,respectively.Conclusions As a routine and non-invasive diagnostic test,Geneva score assessment,as well as SRPDDA,play a screening role in early diagnosis for PE,which can help to save unnecessary cost of imping,and complement each other so to improve accuracy of diagnosis and reduce misdiagnosis.
7.Resveratrol induces apoptosis in human primary gastric carcinoma cells

AIM: To investigate the apoptosis in primary gastric cancer cells induced by resveratrol, and the relation between this apoptosis and expression of bcl- 2 and bax. METHODS: In in vitro experiments, MTT assay was used to determine the cell gowth inhibitory rate. Transmission electron microscopy and TUNEL staining were used to quantitatively and qualitively detect the apoptosis of primary gastric cancer cells before and after the resveratrol treatment. Immunohistochemical staining and RT - PCR was used to detect the expression of apoptosis - regulated gene bcl - 2 and bax. RESULTS: Resveratrol inhibited the growth of primary gastric cancer cells in a dose - and time - dependent manner. Resveratrol induced primary gastric cancer cells to undergo apoptosis with typically apoptotic characteristics. TUNEL assay showed that after the treatment of primary gastric cancer cells with resveratrol for 24, 48, 72, 96 hours, the apoptotic indexs were 4.93% ± 0.19%, 16.74% ± 0.43%, 27.88% ±0.36%, 36.84% ± 1.07 % respectively. Immunohistochemical staining showed that after the treatment of primary gastric cancer cells with resveratrol for 24, 48, 72, 96 hours, the positive rates of Bcl - 2 proteins were 20.68% ± 0.49%, 10.84% ±0.33%, 6.80% ± 0.34%, 3.91% ± 0.15% and the positive rates of Bax proteins were 19.79% ± 0.98%, 30.74% ±0.85%, 40.14% ± 1.17%, 60.08% ± 1.64%. After exposed to resveratrol for 24 h, 48 h, 72 h and 96 h, the density of bcl- 2 mRNA decreased progressively with elongation of time and the density of bax mRNA increased progressively with elongation of time by RT- PCR. CONCLUSION: Resveratrol is able to induce the apoptosis in primary gastric cancer. This apoptosis may be mediated by down- regulation of Bcl- 2 and up- regulation of Bax.
8.In vitro balanced sustained-release of Panax notoginseng saponins controlled with various matrix materials.

To explore the influence of matrix materials in complicate ingredients on traditional Chinese medicine and investigate the excipients selection model based on balanced release characteristics of multicomponents, the influence of HPMC (K4M, K15M, K100M) and Carbomer (934P, 971P, 974P) was illustrated by testing in vitro release of ginsenoside-Rg1, ginsenoside-Rb1 and notoginsenoside-R1 in Panax notoginseng saponins (model drug, PNS). According to in vitro release results of PNS matrix tablets in water and artificial intestinal juice, the release curves were analyzed with Peppas equation and simulating factor (f). Significant differences in k value and n value among ginsenoside-Rg1, ginsenoside-Rb1 and notoginsenoside-R1 existed in various formulations. The release behaviors from various excipients could be described with Non-Fickian transport or super Case II transport pattern. The f2 values for ginsenoside-Rg1, ginsenoside-Rb1 and notoginsenoside-R1 in 971P matrix tablet containing 30% Carbomer 971P were 74.91, 53.45, 57.89 in water and 79.35, 55.51, 51.89 in artificial intestinal juice, respectively. The release profiles fit for the regulation of FDA. The result revealed that the balanced release rates of Rg1, Rb1 and R1 in 971P matrix tablet were obtained.
9.The mechanisms of drug resistance in prostate cancer.

Drug therapy is one of the efficient methods for prostate cancer treatment. However, drug resistance greatly hindered the treatment of prostate cancer patients. Herein, the mechanisms of drug resistance in prostate cancer have been exhaustively reviewed, and that can provide an alternative strategy and new targets for anti-prostate cancer therapy.
10.Comparisons of the clinical effectiveness between ultrasound-guided puncture drainage and open surgery in the treatment of pyogenic liver abscess

Objective To compare the clinical effectiveness between puncture drainage and surgery in the treatment of pyogenic liver abscess.Methods Clinical date of 81 patients with pyogenic liver abscess were retrospectively analyzed.Patients were divided into the ultrasound-guided puncture drainage group (48 patients) and open surgical drainage group (33 patients).The demographic data,laboratory examination,efficient rate,complication rate,mortality,time for body temperature returned to normal and hospital stays were compared between the two groups.Results Klebsiella pneumoniae was positive in 45.45% cases by blood culture,and in 62.50% cases by pus culture.There was no statistically significant difference in the effective rate and mortality (x2 =0.91,2.05,P > 0.05).For patients with puncture drainage hospital stay was (14 ± 5) days,significantly shorter than (17 ± 5) days in surgery group (t =-3.20,P < 0.05).Time to normal temperature was (5.1 ± 1.6) days in puncture drainage group,which was shorter than (6.0 ± 1.1) days in open surgery group (t =-2.85,P < 0.05).Postoperative complications were fewer in the puncture drainage group (6 cases) than open surgery group (10 cases) (x2=3.91,P < 0.05).Conclusions Ultrasound-guided puncture drainage for liver abscess is safe,feasible,effective of low complication rate for the treatment of pyogenic liver abscess.
