1.Human umbilical cord blood mesenchymal stem cell transplantation improves the liver function of liver cirrhosis rats

BACKGROUND: The feasibility and the mechanism of human umbilical cord blood mesenchymal stem cel transplantation for the treatment of liver cirrhosis need to be discussed in-depth. OBJECTIVE: To observe the effect of human umbilical cord blood mesenchymal stem cel transplantation through portal vein on the liver function and tissue pathological changes of the rats with liver cirrhosis. METHODS: Carbon tetrachloride was used to prepare rat model of liver cirrhosis. After the success of modeling, the rats in the cel transplantation group received portal vein injection of 1 mL 5-bromo-2-deoxyuridine -labeled human umbilical cord blood mesenchymal stem cells (5×106), the model group was injected with the same volume of PBS; the normal rats received 1 mL human umbilical cord blood mesenchymal stem cel transplantation via the portal vein were as the control group. At 4 weeks after transplantation, the rat tail vein blood and liver tissue were obtained for testing. RESULTS AND CONCLUSION: At 4 weeks after cel transplantation, compared with the model group, levels of serum alanine aminotransferase, aspartate aminotransferase and total bilirubin in the cel transplantation group were significantly decreased, while the albumin level was increased significantly (P < 0.01); the liver cel inflammatory necrosis, steatosis and liver fibrosis were improved significantly (P < 0.05 or P < 0.01). Immunohistochemistry and immunofluorescence staining showed that human umbilical cord blood mesenchymal stem cel colonization could be seen in the rat liver tissues of the cel transplantation group and control group, but the number of 5-bromo-2-deoxyuridine-positive cells in cel transplantation group was significantly larger than that in the control group. Reverse transcription-PCR test result showed that the expressions of cytokeratin 18 and albumin mRNA could be observed in the rat liver tissue of the cel transplantation group, but no expression could be seen in the control group. It is visible that human umbilical cord mesenchymal stem cells can improve liver function and pathological damage of liver cirrhosis rats in a certain extent, which may relate with the intrahepatic homing colonization and hepatocyte-like cel differentiation of the transplanted cells in the liver cirrhosis rats.
2.The value of spiral CT in diagnosis of blunt duodenum trauma

Objective To elevate the diagnosis value of spiral CT in blunt duodenum trauma. Methods 9 cases of blunt duodenum trauma proved by operation or clinical were analysed retrospectively. Results 9 cases of blunt duodenum trauma were all diagnosed accurately before operation. The main CT signs were duodenal wall thick- ening; duodenal wall hematoma; free retroperitonea air; free retroperitouea fluid and retoperitonea hematoma. Conclu- sion Spiral CT scan have important clinical value in diagnosis of blunt duodenum trauma.
3.Significance of preserving anterior thoracic nerve and intercostobrachial nerve during breast-conserving operation for breast cancer

Objective To study the method and significance of preserving anterior thoracic nerve(ATN) and intercostobrachial nerve(ICBN) during breast-conserving operation for breast cancer.Methods A total of 382 cases with breast-conserving operation for breast cancer in recent 5 years were reviewed.In 312 cases the opeeration was performed by preserving ATN and ICBN,and in 70 cases the ATN and ICBN were not preserved.The patients were followed-up postoperatively.Results Among the patients whose operation was performed by preserving ATN and ICBN,skin sensation of armpit and upper arm was normal in 80.4%(251/312) and abnormal in 19.6%(61/312) of patients,but in most of them,it recovered in 2-3 months.Among the patients with out preservation of ATN and ICBN,skin sensation was normal in only 16 cases(22.9%),and had varying degrees of abnormal sensation in 54 cases(77.1%).Conclusions Preservation of ATN and ICBN during breast-conserving surgery for breast cancer is feasible,can decrease the incidence of hypoesthesia of axilla and upper arm and atrophy of pectoralis muscles,and the quality of life of patients after operation is improved.
4.Complications of reoperation of thyroid carcinoma and management

Objective To investigate the causes , surgical methods and complications of reoperation of thyroid carcinoma and management .Methods Clinical data of patients with thyroid carcinoma in Department of General Surgery of Tang Du Hospital during Jan .2007 to Apr.2012 were retrospectively analyzed .The causes and complications were studied.Results Postoperative complications happened to 5 cases(13.51%), among whom 2 cases had temporary recurrent laryngeal nerve palsy and they recovered after 2 to 6 months , 2 cases had temporary hypocalcemic convulsions and the symptom resolved after one-week treatment with intravenous calcium gluconate , and 1 case had bucking and recovered after one week of treatment .Conclusion For patients with thy-roid disease,detailed preoperative examination , intraoperative frozen pathological examination , intraoperative explo-ration and the right operation method are the key factors to prevent thyroid carcinoma reoperation .Careful dissection and gradually sever of upper pole of thyroid lobe reduce complications of thyroid carcinoma reoperation .

With comparison of three different methods for the marking of amines compound, an optimal deri vatization method was selected.5-(2-Hydroxyethyl) benzoacridine (HBA) reacts with coupling agent N,N'-carbonyldiimidazole(CDI) to form an activated amide intermediate 2-(benzoacridin) ethyl-imidazole-1-carbox-ylate(BAEIC).BAEIC, which is dual-sensitive probe, reacts preferably with amino compounds at 80 ℃ in the presence of 4-dimethylaminopyridine(DMAP) catalyst in N,N-dimethylformamide(DMF) solvent to give the corresponding sensitively fluorescent derivatives with an excitation maximum at λ_(ex) of 280 nm and an emis sion maximum at λ_(em) of 510 nm.BAEIC-amine derivatives simultaneously exhibited high ionization potential with percent ionization (changing from 5.62% to 58.08% in aqueous acetonitrile and from 2.14% to 56.58% in aqueous methanol.Derivatives were not only sensitive to fluorescence but also to MS ionizable potential.The fluorescence detection limits(5/iV = 3) were 0.12-0.59 μg/L.The online APCI-MS detection limits were 1.9-14 μg/L(S/N=5).

Objective To study the mechanism of SW480 cell line death caused by transfection of retroviral vector-G1CEACDNa.Methods Plasmid G1CEACDNa was transferred into the SW480 cell line using liposomes method. RT-PCR was performed to examin the expression of CD gene indrectly.The cell growth curve was measured by means of cell counting. When the CD+SW480 cells were exposed to 5-FC (1mmol/L), the growth inhibition rate and the "bystander effect" were detected by MTT method.The ultrastructure was observed by electron microscope.Apoptosis was verified by flow cytometer .Results The product of RT-PCR showed a band at 1.5kb on the photo of electrophoresis. The growth of CD+ SW480 cells was inhibited 24h after administrating 5-FC,and the inhibition rates at 72h,96h,120h were 30.0%,50.0% and 80.0%,respectively.Apoptosis cells in different phases and apoptotic bodies in the field of electron microscope were observed. Meanwhile ,a few cells showed necrosis.Flow cytometer verified that a few cells appearred apoptosis 48h after exposed to 5-FC (1mmol/L), the apoptosis rate and the necrosis rate at 72h,96h were 20.2%,30.7% and 19.6%,21.1% respectively.Conclusions The death mechanism of SW480 cells transfected with G1CEACDNa followed by 5-FC treatment includes both necrosis and apoptosis.Apoptosis is possibly the bystander effect.

Objective To investigate the effect of thermochemotherapy with 5 fluorocytosine(5 FC) on human colorectal carcinoma cell lines SW480 transfected with carcinoembryonic antigen (CEA) tissue specific cytosine deaminase (CD) in vitro.Methods Recombinant retroviral vector G1CEACDNa, in which CD gene was controlled under the CEA promoter, was introduced through liposome technique to the human colorectal carcinoma cell line SW480, and then the cells were selectively cultured in G418. The proliferated colonies were treated with the combined therapy of 5 FC and hyperthermia at a 43℃ over 30 min for 3 times. RT PCR was performed to analyze the expression of CD gene in target cells after thermochemotherapy. Results The CD gene expressed steadily in the target cells after 3 times of hyperthermic treatment. Objective To study the expression of high mobility group box chromosomal protein 1(HMGB-1) in the skin lesions of patients with lupus erythematosus and investigate the role of HMGB-1 in the pathogenesis of lupus erythematosus. Methods Immunohistochemical assay and Western-blot were used to test the expression of HMGB-1 in skin lesions from 20 discoid lupus erythematosus (DLE) patients, 25 systemic lupus erythematosus (SLE) patients and 20 healthy controls. Results In healthy controls, H MGB-1 was mainly expressed in the nucleus of keratinocytes. In skin lesions of DLE patients, HMGB-1 was mainly expressed in mononuclear cells of dermis, but the percentage of positive keratinocytes in epidermis of lesion area was decreased than that of healthy controls (t=11.315, P<0.01). In skins that were not involved,HMGB-1 was also expressed in the nucleus of keratinocytes and there was no difference in the percentage of positive keratinocytes between DLE and healthy controls (P>0.05). By Western-blot, there was no stati-stical significance in total protein between DLE and healthy controls (t=0.681, P>0.05). In SLE, besides the mononuclear cells, HMGB-1 could be detected both in the cytoplasmic and extracellular space in dermis,while the HMGB-1 nuclear expressions in keratinocytes'of epidermis were decreased than those of DLE (t=6.821, P<0.01), and in un-involved skin, HMGB-1 was also expressed in the nucleus of keratinocytes and there was no difference in the percentage of positive keratinocytes with healthy controls (P>0.05). The total protein was increased in SLE than that of healthy controls and DLE patients (t=15.494, P<0.01 ; t=13.221, P<0.01, respectively). The intensity of HMGB-1 was con'elated with SLEDAI and proteinuria (r=0.565, P<0.01,OR=1.027, P<0.05, respectively). Conclusion Compared with healthy controls, there is translocation and alteration of HMGB-1 expression in patients with lupus erythematosus, which indicates that HMGB-1 may be involved in the inflammation of lupus erythematosus.
9.Clinical significance of the expression of D-amino acid oxidase gene in hepatocellular carcinoma tissues

Objective To study the clinical significance of the expression of D-amino acid oxidase(DAO) in hepatocellular carcinoma(HCC) tissues.Methods The gene expression profiles and related clinical data of 214 HCC cases were collected.Univariate and multivariate analyses were conducted to investigate the association between DAO expression levels,clinical traits,the prognosis.Results Univariate analysis indicated that there were a lower level of blood alpha fetoprotein(AFP,P=0.001),a smaller number of nodules(P=0.042),a better TNM stage(P=0.014),a lower metastasis risk(P=0.001) and a better prognosis(P=0.011)in the samples with high DAO expression.Multivariate analysis also indicated a lower AFP level(OR:0.162,95%CI:0.078-0.336) and metastasis risk(OR:0.140,95%CI:0.069-0.284) in the samples with high DAO expression,as well as a better prognosis(OR:0.833,95%CI:0.700-0.992).Conclusion DAO expression was associated with blood AFP levels,metastasis risk and prognosis in HCC,and its high expression was a protective factor for HCC.
10.Analysis of the clinical effect of biliary-enteric anastomosis and biliary stent to palliative treatment of malignant obstructive jaundice

Objective To analyze the clinical effect of biliary-enteric anastomosis and biliary stent to palliative treatment of malignant obstructive jaundice.Methods A total of 40 patients with inoperable malignant obstructive jaundice were enrolled in this study,and 20 patients were performed biliary stent placement (stent group),simultaneously 20 patients were performed biliary-enteric anastomosis (operation group).The fatality rate after operation,the level of total bilirubin before treatment and after treatment for 4,7,14 d,the rate of hyperpyrexia,nausea and vomiting,postoperative recurrence of jaundice were compared between two groups.Results There were no dead in two groups.The level of total bilirubin was decreased after treatment,and there was no significant in two groups before treatment and after treatment for 4,7,14 d (P > 0.05).The rate of hyperpyrexia in operation group was significantly lower than that in stent group (0 vs.4/20) (P < 0.05).Conclusion For palliative treatment of malignant obstructive jaundice,the biliary-enteric anastomosis should be performed first if there is no significant contraindication.
