2.Effects of TAE and TACE on HIF-1?,MVD and PCNA in rabbit liver carcinoma

Objective To investigate expression and clinical significance of HIF-1?、MVD and PCNA after TAE or TACE for rabbit liver carcinoma,and to identify the mechanism of hepatic carcinoma cell proliferation after the operation.Methods Thirty rabbits were implanted with liver VX2 tumors for 1 week.Then they were randomly divided into 3 groups.Control group had hepatic arterial infusion of 0.5 to 0.8 mL saline.TAE group was embolized with 0.5 to 0.8 ml UFLP.TACE group was embolized with 0.5 to 0.8 ml UFLP mixed with 2 mg THP.Two weeks after treatment,HIF-1?、MVD and PCNA expression was detected.Results Growth of the liver tumor was found in the 3 groups,and there were 15,11 and 3 new tumors found in the control group,TAE group and TACE group respectively.The expressions of HIF-1?and PCNA in control group were higher than those in TACE group,but significantly lower than those in TAE group(P

4.Comparison of the effect of nifedipine controlled-release tablet and nifedipine sustained-release tablet in the treatment of hypertension

Objective To investigate the clinical efficacy of nifedipine sustained-release formulation(SR)and nifedipine controlled-release(GITS)in the treatment of essential hypertension.Methods 160 patients with essential hypertension were randomly divided into two groups.The SR group was treated with SR 20 mg twice a day;GITS group was treated with GITS 30 mg once a day.All patients were given ambulatory blood pressure monitoring(ABPM)and observed the 24-hour average blood pressure,heart rate,changes in the trough/peak(T/P)values,and adverse reactions before and after treatment in two groups.Results The T/P value of systolic blood pressure(SBP)and diastolic(DBP)in the GITS group was 68.5%,74.6%,respectively,which were higher than those of SR group(57.2%,61.1%)(x2=6.32,7.05,all P<0.05);GITS group was significantly lower than the SR group in heart rate(t=4.17,P<0.05);Adverse reactions between two groups had no significant difference(x2=0.16,P>0.05).Conclusion SR and GITS can effectively lower blood pressure,but GITS is more efficient,stable,safe,small impact on the heart rate.
5.Delayed Bile Leakage after Laparoscopic Cholecystectomy:Report of 12 Cases

Objective To Discuss the causes,prevention and treatment of delayed bile leakage following laparoscopic cholecystectomy (LC). Methods From November 1994 to December 2007,totally 11 000 cases of LC were performed in our hospital,12 of them developed delayed bile leakage after the procedure. The mechanism,characteristics,treatment,and prognosis of the disease were analyzed retrospectively. Results Among the twelve cases of delayed bile leakage,eight cases showed perforation of the bile duct caused by thermoelectric effect,two cases had leakage of the aberrant bile-duct,and two cases were injured by unknown reasons. B-ultrasonography-guided drainage was performed in four of the patients,laparoscopic or open repair combined with T-tube drainage were carried out in three,abdominal puncture combined with endoscopic nasobiliary drainage (ENBD) was done in three,abdominal puncture with endoscopic retrograde biliary drainage (ERBD) was made in one,and open repair and bilio-jejunal anastomosis was performed in one. All patients were cured and discharged in 8 to 21 days with a mean of 15 days. No intra-abdominal infection,hemorrhage,recurrent bile leakage or intestinal leakage occurred after the operations. Re-examination by B-ultrasonography or cholangiopancreatography performed ten days after the drainage showed no stricture or dilation of the bile duct,and contrast media drained out fluently. The 12 cases were followed up for one year,none of them showed biliary stenosis or infections during the period. Conclusions Post-LC injury of the bile duct is mostly caused by thermoelectric effect. Gentle operation and separation strictly following the anatomical levels are the keys in prevention of postoperative delayed bile leakage; while endoscopy and interventional treatment are main methods for the diagnosis and treatment of the disease.
6.Exploration of enzyme-MnO2 nanosheets hybridization probe for sensitively colorimetric self-indicating of glucose

A colorimetric self-indicating probe for glucose was constructed by self-assembly of MnO2 nanosheets (MnO2 NSs) and glucose oxidase(GOD) in this paper.Under the weak acidic medium,glucose oxidase specifically catalyzes glucose into gluconic acid and hydrogen peroxide.The by-product of hydrogen peroxide could efficiently dissolve the MnO2 nanosheets,resulting into a significant decrease of the characteristic absorbance at 374 nm assigned to MnO2 NSs.Furthermore,the absorbance difference was linearly proportional to the concentration of glucose ranging from 1 to 20 μmol/L The fitted curve could be used for quantification of glucose with a correlation coefficient of 0.990 1.And the detection limit as low as 0.1 μmol/L could be reached based on the definition of three times of the deviation of the blank signal (3σ) and there was negligible interference with other co-existing amino acids,anions,cations and protein,which indicated high sensitivity and selectivity of the hybrid probe.The construction strategy of designated probe is readily generalized in principle for detection of numerous analytes in view of reactive property of MnO2 and the diversity of enzymes.
7.The application of Fluoredeoxyglucose positron emission tomography/CT to detect the recurrence and metastasis of colorectal cancer

Objective: To evaluate the clinical value of positron emission tomography/ computed tomography(PET/CT) with fluorine-18-labeled fluorodeoxyglucose(18F-FDG) in dectecting and diagnosing the recurrence and metastasis of colorectal cancer.Methods: The results of whole-body PET/CT of 68 patients with colorectal cancer were analyzed retrospectively.The average follow-up time was 15.8 months.The diagnosis of recurrence tumor and/or metastasis was based on pathologic examination,colonoscopy,multi-modality imaging and clinical follow-up.Results: Fifty-five of the 68 patients had recurrence and /or metastasis but no recurrence was found in the other 13 cases.The sensitivity,specificity and accuracy of PET/CT in detecting recurrent and metastatic tumor were 96.4%,76.9% and 92.6% respectively.PET/CT successfully detected one or several malignant insidious lesions in 8 cases with negative findings by CT and /or ultrasonography,and it revaeled more malignant lesions than CT and ultrasonography in 30.9%(17/55)of the patients.The clinical therapeutic strategies were changed in 11 case duo to the results of PET/CT,and the influence rate of PET/CT was 16.2%.The metastasis was mainly confined in the liver,lung,peritoneal cavity and retroperitoneal region.In the examination of the 24 patients with increased serum CEA,the positive rate of PET/CT was 91.7%.Conlusion: PET/CT can detect the recurrence and metastasis of colorectal cancer with high sensitivity and accuracy.
8.Effects of rhubarb monomers on apoptosis and proliferation of intestinal epithelial ceils of rats with sepsis

Objective To observe apoptosis and proliferation of intestinal epithelial cells and to explore the mechanism of protective effects of rhubarb monomers on intestinal epithelial cells in septic rats.Methods Eighty male Sprague-Dawley (SD) rats (230-250 g) under anesthesia and sedation were subjected to cecal ligation and perforation (CLP).After surgical preparation,rats were randomly (ramdom number) divided into 8 groups (n =10 each):a sham group (A) [normal saline (NS) gavage];a sepsis group (B) (NS gavage);group C (ip dexamethasone 0.5 mg/kg immediately after CLP) (C);and rhubarb monomer 100 mg/kg in NS treated groups including:rhein group (D),emodin group (E),3,8-dihydroxy-1-methyl-anthraquinone-2-carboxylic acid group (F),1-O-caffeoyl-2-(4-hydroxy-O-cinnamoyl)-D-glucose group (G),and 3,8-dihydroxy-1-methyl-anthraquione group (H).Animals were sacrificed 24 hrs after treatment.Intestinal histopathology,apoptosis (TUNEL) and proliferation of intestinal epithelial cells (proliferating cell nuclear antigen,PCNA) were measured.Multiple comparisons were carried out with one-way analysis of variance (ANOVA).Results Histopathology revealed injury to the intestinal mucosal villi induced by sepsis in group B compared with group A.The injury was significantly ameliorated in groups C,D,E,F,G,and H compared with group B.The apoptosis index in group B was significantly higher than that in group A (P < 0.05) and the apoptosis index in groups C,D,E,F,G,and H was significantly lower than that in group B (P < 0.05).The PCNA positive index in group B was significantly lower than that in group A (P < 0.05),but was significantly higher in groups C,D,E,F,G,and H than that in group B (P < 0.05).Conclusion Rhubarb monomers can promote the proliferation of mucosal cells and prevent apoptosis of intestinal mucosal cells.In addition,rhubarb monomers may play a role in protecting the intestinal barrier function.

The device of automatic detection of patients receiving X-ray irradiation part, intelligent adjustment in patients and shooting device between automatic tracking radiation protection device height. The device has the advantages of automatic adjustment, anti-radiation device, reduce the height of non-irradiated area X-ray radiation and improve the work efficiency. Testing by the professional organization, the device can decrease more than 90% of X-ray dose for patients with non-irradiated area.

10.Fixation of patella transverse fracture with absorbable materials: A biomechanical comparison with mental implant

BACKGROUND: Absorbable materials for internal fixation have some advantages such as simple surgery, rapid recovery and no need to second surgery, and have been accepted in the circle of orthopaedics, and had been applied in the clinic. However, the system experiment evidences are not reliable.OBJECTIVE: To investigate whether the absorbable material (the absorbable tension band, absorbable stick or screw) is an acceptable alternative to stainless steel wire with Kirschner wire by comparing their biomechanical properties in the fixation of patella transverse fractures.DESIGN, TIME AND SETTING: The randomized controlled study of biomechanical analysis was performed at the Laboratory of Institute of Biomechanics, Shanghai University from March 2007 to January 2008.MATERIALS: Eight fresh patellae specimens were collected, with the complete patella, articular capsule and patellar ligament (3.0-4.0 cm) and quadriceps femoris (4.0-5.0 cm), and then divided into two groups at random, AO steel wire group (n = 2) and absorbable tension band group (n = 6).METHODS: Eight fresh patellae specimens were divided into two groups at random, AO steel wire group (n = 2) and absorbable tension band group (n = 6) to create models of transverse fractures, which were separately fixed by absorbable tension band,steel wire tension band + Kirschner wire to measure their biomechanics function. If fracture gap reached 1.0 mm, the fixation failed.MAIN OUTCOME MEASURES: The changes in patellar ligament tensile force, quadriceps femoris tensile force, patellar displacement, quadriceps femoris displacement, patellar ligament displacement and patella tensile strain were measured under external load.RESULTS: Under external load, no significant difference was determined in total displacement between the absorbable tension band group and AO steel wire group (9%-10% difference). No significant difference was detected in patellar strain between absorbable tension band group and AO steel wire group (9%-10% difference). The patellofemoral joint force was similar between the absorbable tension band group and AO steel wire group. When internal fixation displaced 1 mm, the load was 587 N in the absorbable tension band group, and 650 N in the AO steel wire group (P > 0.05).CONCLUSION: The absorbable material is an acceptable alternative to stainless steel wire with Kirschner wire in the fixation of patella transverse fractures, with the advantages of good biological function and good stability at the broken ends of fractured bone.
