Objective To study the treatment effect of endoscopic sinus surgery in the elderly patients with chronic sinusitis and nasal polyps.Methods The results of endoscopic sinus surgery in 60 elder patients were analysed retrospectively.Results The total response rate was 93.4%(56/60),curative rate was 76.7%(46/60),improvement rate was 16.7%(10/60) with over one year followed-up.Conclusion The endoscopic sinus surgery is also applicable to patients with chronic sinusitis and nasal polyps.

ObjectiveTo investigate the predictive value of N-terminal pro-brain natriuretic peptide(NT-proBNP) level on the capacity load of patients with maintenance hemodialysis (MHD).MethodsOne hundred and twenty patients undergoing MHD were divided into two groups according to blood pressure levels:bad blood pressure control group with 72 patients and good blood pressure control group with 48 patients.NT-proBNP levels were detected before and after dialysis,and cardiothoracic ratio,left ventricular mass index ( LVMI ),swelling index,number of occurrences of low blood volume and ultrafiltration volume were measured and compared between two groups.The correlation between NT-proBNP and the above indicators was analyzed.ResultsThe systolic blood pressure,diastolic blood pressure,NT-proBNP before dialysis,cardiothoracic ratio,LVMI,swelling index and number of occurrences of low blood volume in bad blood pressure control group respectively were (159.7±18.2) mm Hg (1 mm Hg =0.133 kPa),(95.3±14.8) mm Hg,(11 837.13±1374.26) ng/L,0.61±0.12,(174.36±21.35) g/m2,2.93±1.72,12cases/week,while the indicators in good blood pressure control group were ( 131.6±17.3) mm Hg,(86.5±13.2) mm Hg,(7649.54±1106.18) ng/L,0.52±0.11,( 151.83±18.94) g/m2,1.02±1.48,26 cases/sweek.There were significant differences between two groups (P ＜ 0.01 ).It was showed that NT-proBNP difference between before and after dialysis had significant positive correlation with ultrafiltration volume(P＜ 0.05).NT-proBNP before dialysis had a significant positive correlation with systolic blood pressure,diastolic blood pressure,cardiothoracic ratio,LVMI and swelling index (P ＜ 0.05 or ＜ 0.01 ),while had obvious negative correlation with the number of occurrences of low blood volume(P ＜ 0.01 ).ConclusionsNT-proBNP level of MHD patients has close correlation with blood pressure,cardiothoracic ratio,swelling index,number of occurrences of low blood volume and ultrafiltration volume.Detecting the level of NT-proBNP can better evaluate the changes of blood volume in dialysis patients,adjust dialysis prescription in time,and ensure the adequacy of dialysis.It has important significance to improve the quality of life of MHD patients.

Long non-coding RNAs (lncRNAs) have gradually become a research hotspot in recent years. The abnormal expression of some lncRNAs has been proven to promote tumorigenesis and progression. Among the lncRNAs, metastasis-associated lung adenocar-cinoma transcript 1 (MALAT-1) has attracted considerable attention in various tumor studies, particularly lung cancer. Therefore, the mechanism in which MALAT-1 regulates the cellular activities of tumors should be analyzed. This review mainly discusses the molecu-lar structure, biological function, and role of MALAT-1 in regulating cell proliferation, tumor metastasis, cell cycle, apoptosis, and so on. Furthermore, the role of MALAT-1 in epigenetic regulation is appropriately illustrated to provide a novel perspective in literature on the prevention and treatment of tumors.

Mycoplasma pneumonia (MP) is one of common pathogens of community-acquired pneumonia in children. In recent years, the incidence of MP pneumonia (MPP) shows a trend of increasing. The report of severe refractory MPP was more and more. Glucocorticoids has anti-inlfammatory, anti-allergic and restraining immune response pharmacological effects and is the most effective anti-inlfammatory drug currently. In this paper, pathogenesis of the lower respiratory infection by MP and the therapeutic effects of glucocorticoid were reviewed.

In clinical treatment, it is found that certain patients always have some positive reaction points those are relevant with low back pain in the abdomen area. When the simple treatment on the low back is ineffective, the efficacy could be significantly improved if acupuncture or tuina is performed at the abdomen areas, which is called "regulating yin to treat yang", or "treating the back from abdomen". In this paper, with the diagnosis and treatment method of "treating the back from abdomen" for low back pain as principal line, the detailed manipulation is explained for low back pain that is induced by TCM meridian diseases or modern anatomy, which could open the methods for clinical treatment of low back pain and enrich the therapeutic options.

Objective:To explore diagnostic value of 6min walking test (6MWT) for coronary heart disease (CHD) . Methods :A total of 192 patients suspected of CHD ,including 123 men and 69 women ,who were treated in our hos‐pital ,were selected .After hospitalization ,all patients received 6MWT and coronary angiography (CAG) examina‐tion .With CAG results as the gold standard for diagnosing CHD ,sensitivity ,specificity ,accuracy ,positive predic‐tive value and negative predictive value of 6MWT diagnosing CHD were calculated ,and they were compared be‐tween men and women .Detections of 6WMT among groups of different diseased number of vessels were compared . Results :With CAG results as the standard ,accuracy ,specificity ,sensitivity ,positive predictive value and negative predictive value of 6MWT diagnosing CHD were 71.35% ,80.58% ,60.67% ,72.97% and 70.34% respectively . Compared with female patients ,there were significant rise in accuracy (57.97% vs .77.24% ) ,sensitivity (39.29%vs .70.49% ) ,specificity (70.73% vs .83.87% ) and positive predictive value (47.83% vs .81.13% ) , P<0.05 or<0.01 ,and significant reductions in false positive rate (29.27% vs .16.13% ) and false negative rate (60.71% vs . 29.51% ) in male patients (P<0.05 or <0.01) ,there was no significant difference in negative predictive value be‐tween two groups (P>0.05) .Compared with single - and double -vessel coronary disease ,there was significant rise in true positive rate (43.75% ,54.05% vs .100% ) of triple - or multi -vessel coronary disease diagnosed by 6MWT , P< 0.01 both . Conclusion:The 6min walking test possesses high specificity and sensitivity diagnosing CHD ,and its diagnostic value is higher for male patients and patients with serious condition .Its procedures are sim‐ple with low cost and no damage to patients ,which can be used as adjunctive examination for CHD diagnosis and ex‐tended in clinic .

Objective To investigate the effect of Zhonghui Tangmaikang on clinical symptom, blood sugar and lipid of pre-diabetes patients. Method Seventy-two patients of pre-diabetes were divided into 2 groups randomly (36 patients of Zhonghui Tangmaikang group, 36 patients of metformin hydrochloride group), the course of treatment was 12 weeks. Objective By means of control test,to compare the surgical trauma and perioperative recovery between video-assisted thoracoscopic surgery(VATS) and conventional thoracotony for lobectomy.Methods Eightysix non-small cell lung cancer patients who had undergone operations from April 2010 to April 2011 were chosen.According to operative mcthods,the patients were divided into two groups:video-assisted thoracoscopic surgery group(VATS) and open chest group(OT).The protein and hormone level of serum Cortisol(Cor),growth hormone (GH),triiodothyronine (T3),tetraiodothyronine (T4),C-reactive protein (CRP) were selected as indexes to investigate the surgical trauma on non-small cell lung cancer patients.The duration of operation,quantity of lymph node dissection,chest tube duration,postoperative hospital stay,postoperative complications were chosen as indexes to measure operation result and perioperative recovery.Results Under the condition that patients undergone the same operation level(lobectomy + Systemic lymph node dissection),there was no ststistical significance in preoperative level of serum Cor,GH,T3,T4,CRP.The postoperative concentrations of serum Cor,GH,T3,T4,CRP on the 1 d,2 d,3 d and 7 d were lower in VATS group,and there were obvious statistical significance(P ＜0.01).And there were obvious statistical significance in duration of operation (P ＜ 0.01),postoperative hospital stay(P ＜ 0.01),quantity of lymph node dissection (P =0.477).There were no obvious statistical significance in chest tube duration (P =0.508),postoperative complications (P =0.787).Conclusions VATS lobectomy is able to achieve a same result for radical treatment outcone and safety to routine open chest lobectomy in patients.Compared witt routine open chest lobectomy,VATS lobectomy has less surgical trauma,much rapid recovery,it should be recommended for patients with non-small cell lung cancer.
