1.Clinical Observation of Annao Tablets Combined with Flunarizine Hydrochloride Capsules in the Treat-ment of Vertebral Basilar Artery Insufficiency

Objective:To explore the efficiency of Annao tablets combined with flunarizine hydrochloride capsules in the treatment of patients with vertebral basilar artery insufficiency ( VBI) . Methods:Totally 294 patients with VBI were randomly divided into group A, B and C (n=98). Group A received flunarizine hydrochloride capsules, group B received Annao tablets, group C received flunari-zine hydrochloride capsules combined with Annao tablets, and the treatment course was 15 days. The symptom scores, clinical effica-cy, blood flow velocity of vertebral-basilar artery and serum Tau level were compared among the three groups. The adverse reactions oc-curred during the treatment were recorded. Results:After the treatment, the symptom scores and clinical total effective rate of group C was (10.05 ±2.23) and 96.94%, respectively, which were better than those in the other two groups (P<0.05). After the treat-ment, the cerebral blood flow velocity, cognitive function and serum Tau level were improved in the three groups (P<0. 05), while those in group C were better than those in the other two groups (P<0. 05). No obvious adverse reaction was observed in the three groups. Conclusion:Annao tablets combined with flunarizine hydrochloride capsules is effective and safe in the treatment of patients with VBI, and the mechanism is partly related with the reduction of serum Tau protein level.
2.Clinical Observation of Centro-square Needling at Shangxing (GV23) for Anxiety After Cerebral Stroke

Objective To observe the clinical efficacy of centro-square needling at Shangxing (GV23) (so called Ghost Hall point) in treating anxiety after cerebral stroke as a major treatment.Method Sixty patients were randomized into a treatment group and a control group. The two groups were both given Flupentixol and Melitracen tablets (Deanxit). In addition, the treatment group received centro-square needling as the major treatment, and the control group received ordinary needling. Hamilton Anxiety Scale (HAMA), Zung's Self-rating Anxiety Scale (SAS), and Barthel Index (BI) of Activities of Daily Living (ADL) were observed before and after the treatment for evaluating the therapeutic efficacy.Result There was a significant difference in comparing the HAMA score between the treatment group and control group after the treatment (P<0.05). There were no significant differences in comparing the total effective rate and recovery rate between the two groups (P>0.05).Conclusion The treatment group shows a significant efficacy in improving the anxiety symptoms; the effect in improving the anxiety symptoms is insignificant in the control group; the recoveryrate and total effective rate are equivalent between the two groups.
3. Determination of 25 kinds of minerals in Mailuoning Injection by inductively coupled plasma mass spectrometry with microwave digestion

Objective: To establish an inductively coupled plasma mass spectrometry (ICP-MS) with microwave digestion method for the determination of the amounts of trace elements (Mg, Ca, Fe, Cu, Zn, Mn, Al, B, Ba, Co, Cr, K, Li, Mo, Na, Ni, P, Pb, Sr, Th, Ti, V, As, Cd, and Hg) in Mailuoning Injection. Methods: The conditions of microwave digestion and test were studied respectively. After the microwave digestion, 25 kinds of minerals in the samples were determined by ICP-MS, and the methodology was investigated. Results: The optimal digestion conditions were 3-step slowly heating: 400 W 80℃ to heat up for 10 min and keep 5 min; 600 W 120℃ to heat up for 10 min and keep 5 min; 900 W 200℃ to heat up for 20 min and keep 20 min. The determination results of 25 kinds of mineral elements all have a good linear relationship, r≥0.9996. The RSD values of the precision, stability, and repeatability all met the demands of quantitative analysis. The recovery was 94.7%-106.1% and RSD was 0.34%-2.79%. Mg, Ca, Fe, Cu, Zn, Mn, Al, B, Ba, Co, Cr, K, Li, Mo, Na, Ni, P, Pb, Sr, Th, Ti, and V were detected in Mailuoning Injection, and there was no detection of As, Cd, and Hg. Conclusion: This method is simple, sensitive, and precise, and could satisfy the simultaneous determination of minerals in Mailuoning Injection.
4.A case of Parkes-Weber syndrome accompanied by pseudo-Kaposi's sarcoma

A girl who aged eight years and seven months presented with prunosus patches on the right buttock for 8 years,gradual unilateral enlargement of the right lower limb for more than 7 years,and multiple vegetations for 1 year.Dermatological examination showed nevus flammeus and multiple malodorous vegetations over the right lower limb with high skin temperature.The right lower limb was thicker and longer than the left lower limb.X-ray examination,magnetic resonance imaging and Doppler ultrasound examination revealed high-flow vascular malformations.Pathological examination of the vegetations showed vascular proliferation,fibroblast proliferation and erythrocyte extravasation.She was diagnosed as Parkes-Weber syndrome accompanied by pseudo-Kaposi's sarcoma.
5.Disinfection of Endoscope by EOW and Glutaraldehyde:A Clinical Observation

OBJECTIVE To know the disinfection efficacy of electrolyzed oxidizing water(EOW) and glutaraldehyde on clinical used gastroscope and enteroscope in our hospital and to analyze their stability and probable harm.METHODS The gastroscope and enteroscope before and after use on randomly selected outpatients and inpatients were examined on their contamination,and the efficacy of EOW was compared with that of glutaraldehyde.RESULTS After using EOW and glutaraldehyde to disinfect the gastroscope and enteroscope the former showed that its killing rate on commonly encountered pathogens for 1 min was 93.75-98.22%,for 3 min was 81.20-89.29%,and for 5 min was 100.00%;the latter showed that its killing rate for 3 min was 81.20-89.29%,for 5 min was 90.38-93.04%,and for 10 min was 99.92-99.96%.EOW was non-irritative to mucosa,and didn′t cause allergic reaction,but its stability was poor.The glutaraldehyle could bring some side effects,such as some allergy,and mucosa stimulation.CONCLUSIONS It only needs 3-5 min for EOW to kill polluted bacteria on gastroscope and enteroscope to attain the disinfection eligibility,but glutaraldehyde needs more than 10 min to get to the disinfection eligibility.The effect of EOW is strong,rapid,active and environment-safe,it is worthy of application,but it must be applied immediately as soon as possible after production.The use of glutaraldehyde must strengthen the measures of personal protection.
6.Significance of Flexion Priority in the Rehabilitation of Posttraumatic Stiffness of Elbow

ObjectiveTo evaluate the significance of flexion priority strategy and the principle of joint mobilization technique in the rehabilitation of posttraumatic stiffness of the elbow. Methods24 patients with posttraumatic stiffness of the elbow were divided into 2 groups, 12 patients in each group. The treatment group was treated with flexion priority strategy of joint mobilization technique only in the treatment of flexion contracture. In order to decrease the pain caused by joint mobilization training of extension and achieve the restoration of flexion as early as possible, the rehabilitation of extension was altered to self-exercise under the consultation of doctors combined with gentle passive traction by the therapist. The control group was treated with joint mobilization technique of both flexion and extension once a day. All the patients were measured the flexion range, extension range, and flexion-extension arc after 6 weeks of rehabilitation. Results6 weeks after rehabilitation, the treatment group got 124° of flexion (range 95°~135°), much better than the control group of 95° (range 80°~110°) (P<0.01). There was no significant difference of extension degrees between the treatment group (15°, range 10°~35°) and the control group (16°, range 10°~30°) (P>0.05). The final arc of flexion and extension in the treatment group had an increase of 53°, significantly greater than 30° in the control group(P<0.01). ConclusionFlexion priority strategy of elbow rehabilitation can significantly restore the flexion function of posttraumatic elbow stiffness.
7.Clinical research: why we must mater the essentials

Learning clinical research methods, we must master the essentials. Essentials are the theories, principles and methods that make a discipline a unique discipline, without which the discipline will no more exist. Essentials are those things that are long-lasting and reliable, those that address the most important issues, those that are most frequently used in practice, and those that normally should not be compromised. The core underlying theories of epidemiological research are causation and errors control. The essentials often have profound theoretical underpinnings and difficult to grasp. We will truly believe the theories and observe the principles only when we have truly grasped them.

9.A case of pediatric drug-induced hypersensitivity syndrome

A 10-year and 9-month-old female patient presented with skin rashes all over the body,fever and superficial lymphadenectasis for 18 days after an intravenous drip of fosfomycin.Skin examination showed generalized swollen erythema all over the body,whose surfaces were covered with a large number of sticky furfuraceous grey-white scales.Laboratory examination revealed markedly increased levels of alanine aminotransferase and aspartate aminotransferase,as well as an increased number of eosinophils.Histopathological examination of skin lesions showed infiltration of scattered lymphocytes in the superficial dermis,as well as around skin appendages.Immunohistochemical study demonstrated that the infiltrating lymphocytes mainly included T lymphocytes,and no atypical cells were observed.The patient was diagnosed with druginduced hypersensitivity syndrome.After the treatment with intravenous glucocorticoids,immunoglobulin and oral cyclosporine,favorable therapeutic effects were achieved.
10. Effectiveness of different skin protectants in nursing care of patients with incontinence

Objective Incontinence-associated dermatitis(IAD) is a common skin disease, but there is still lack of evidence on which kind of skin proctectant can provide effective prevention and cost reduction. The article aimed to explore the clinical effect and cost-effectiveness of two skin protectants based on structured skin care regimens in the nursing care of incontinent patients and provide evidence for effective prevention of IAD.Methods A total of 124 patients with incontinence were randomly selected and divided into observation group (62 cases) and control group (62 cases). On the basis of the structured skin care program which focused on incontinence management, cleansing and protection, the patients in the observation group were treated with the composite zinc oxide ointment for 7 days, while patients in control group were treated with ostomy powder combined with skin barrier film for 7 days. The main observation indicators were the incidence of IAD and the cost of nursing materials.Results All 62 patients in the observation group completed the study, while in the control group 2 patients fell off because of death and discharge from hospital and 60 patients completed the study. The per-protocol analysis showed that the incidences of IAD in two groups were 6.45% and 20.00%, the intention-to-treat analysis showed 6.45% and 22.58%, and there was significant difference between groups (P<0.05). The nursing cost of the observation group (239.71±48.55) was lower than that of control group (264.35±61.55), and the difference was statistically significant (P<0.05).Conclusion The use of the composite zinc oxide ointment based on a structured skin care regimen can improve the preventive effect of IAD and reducing costs.
