1.Oligomeric and polymeric proanthocyanidins from Hippophae rhamnoides seed

Objective The oligomeric and polymeric proanthocyanidins in different fractions from Hippophae rhamnoides seeds were investigated. Methods The monomeric and oligomeric fractions were analyzed by HPLC/ESI-MASS detection. The polymeric proanthocyanidins were fractionated by Sephadex LH-20 column chromatography and their chemical constructures were studied by acid-catalysed degradation in the presence of benzyl mercaptan. Results Four monomers were identified as catechin, epicatechin, gallocatechin, and epigallocatechin. Eight dimers including three dimeric procyanidins and one dimeric prodelphinidin, and four mixed dimers were detected and their structures were partly elucidated by the detection of the pseudomolecular ion and main ion fragmentations. The results also showed that polymers to be highly heterogenous with catechin, epicatechin, gallocatechin, and epigallocatechin all being components of both the extension and the terminal units. The extension units were mainly gallocatechin, whereas the terminal units were mainly gallocatechin and epigallocatechin for all fractions. The proanthocyanidins with a mean degree of polymerization (mDP) of 9.1, 13.2 and 17.0 represented the three major polymeric fractions. Conclusion Proanthocyanidins of H. rhamnoides seeds, predominantly being of prodelphinidin-type, are remarkably different from that of grape seeds.

Seat lumbar rotation technique is a commonly used technique,and the clinician is also a kind of clinical curative effect more apparent bone-setting reduction technique.However,the power of spinning technique,direction and position selection is based on the personal experience of the clinician,the lack of a unified,standardized scientific basis for support.Now the article reviews the studies of the rotation of the seat,including lumbar anatomy,biomechanical characteristics,clicks,mechanism of action,and safety.It will provide new ideas for standardizing operations research techniques.
3.Affect of quality of life during organ reserved pelvic floor reconstruction using polypropylene mesh for pelvic organ prolapse

Objective To investigate the polypropylene mesh used transvaginal Mesh for improve clinical efficacy and benefit the quality of life of pelvic organ prolapse( POP) .Methods 80 patients with POP were randomly divided into A group and B group,40 cases in each group.A group received vaginal mesh suspension surgery for treatment,B group used traditional vaginal hysterectomy and vaginal wall repair for treatment.The operation situations of the two groups preoperation and postoperation, the quality of life after 6 months were compared.Results In A group,the operative time[(83.32 ±21.34)min],blood loss[(87.43 ±33.52)mL],retention catheter time[(3.23 ± 0.54)d],postoperative hospital stay[(5.32 ±0.69)d]were significantly less than those in group B[(1 084.34 ± 27.88)min,(98.65 ±12.43)mL,(2.9 ±0.87)d,7.97d](all P<0.01),and the physiological function,physical function,bodily pain,general health,vitality,social functioning and mental health and other seven dimensions of A group were significantly improved,compared with group B(P <0.01).Conclusion Trans -vaginal Mesh has obvious therapeutic effect and improves patients'living quality on POP.It is worthy of clinical application.
4.Effect of low concentration Aβ1-42 monomer/oligomers and CORM-2 on livability of SN56 cells

Objective To observe the effect of low concentration Aβ1-42 monomer/oligomers and CORM-2 in different concentration on livability of SN56 cells. Methods SN56 cells were cultured in the 96-well plate with uniform concentration, and were divided into control group, Aβ1-42 group, Aβ1-42 + CORM-2 50μM group, and Aβ1-42 + CORM-2 100 μM group. Three lines of cells in Aβ1-42 group were cultured in the surroundings of 10nM,100nM and 1 μM Aβ1-42monomer/oligomers, respectively. Aβ1-42 + CORM-2 50μM group and Aβ1-42 + CORM-2 100μM group had the same culture condition as group Aβ1-42 ,except contain 50μM, and 100μM CORM-2, respectively. Control group didnt have any effect factor. Three days later,the livability of different groups was compared with MTT method. Results The livability of group Aβ1-42 with the increasing concentration of Aβ1-42 was (79.73 ±0.94)% ,(67.99 ±0.79)% ,(60.42 ±0.62)% , respectively. The higher the concentration of Aβ1-42 was,the lower the livability of SN56 cell was. The livability of group Aβ1-42 + CORM-2 50μM/100μM was( 75.15±0.096)%,(63.20 ±0.17)%, (55.33 ±0.19)%; (73.20 ±0.27)%, (64.34 ±0.11 )%, (54.17 ±0.12)% , respectively. Both were lower than group Aβ1-42. And different CORM-2 concentration had discrepancy in the ability of decreasing the cell livability. Conclusion Low concentration of Aβ1-42 can reduce the livability of SN56 cells, and higher concentration has more significant effect; CORM-2 in different concentration both can decrease the livability of SN56 cells,and there is a discrepancy in the intensity.

BACKGROUND: The researches of Pulling and Rotating Oblique Pulling Manipulation focus on clinical research and biomechanical changes of lumbar functional units, and the mechanical characteristics of manipulation are little reported.OBJECTIVE: To study the characteristics of the Pulling and Rotating Oblique Pulling Manipulation by digital method, so as to provide quantitative basis for inheritance, teaching and learning, promotion, and basic research of the manipulation.METHODS: The force-time curve and the force of the manipulators were tested and recorded with the multi-point membrane pressure measurement system. The kinetic parameters (the average load force, the average minimum force and the maximum impact force) were shown. The values of = the flip time, flip speed and impulse were measured by the test analysis system.RESULTS AND CONCLUSION: (1) The average load force was (145.86±34.80) N, duration was (1.43±0.46) s, the average minimum force was (72.24±13.87) N, the maximum impact force was (446.21±143.98) N, the flip time was (0.55±0.15) s, flip speed was (914.52±259.18) N/s, and the impulse was (256.21±82.30) N?s. (2) The rising slope of the impulse was (93.96±6.94), and the falling slope was (-82.70±26.10). (3) To conclude, the characteristics of manipulation from hand are analyzed in views of digitization, which provides an objective evaluation index for Lin's manipulation.
6.A study on thumb manipulative forces of cervical rotatory and local manipulation during cracking sounds

Objective To measure the thumb manipulative forces of Lin cervical rotation manipulation during cracking sounds,and provide quantitative basis for its inheritance,teaching and learning,promotion and basic research of the manipulation.Methods The thumb manipulative force of Lin cervical rotation manipulation and traditional cervical rotation manipulation during cracking sounds were tested and recorded with a multi-film pressure measurement system.Result There was statistically significant difference between the maximum thumb manipulative force of the hand used frequently and the hand used infrequently (P < 0.05).The maximum thumb manipulative force of Lin cervical rotational manipulation is lower than the traditional cervical rotation manipulation (P < 0.05).Conclusion Whether inIN traditional or Lin cervical rotation manipulation,the maximum thumb manipulative force of the hand used frequently is more than the hand used infrequently.Lin cervical rotation manipulation has better ergonomic characteristics of biological engineering,safer,more low energy consumption practices.
7.Effect of neoadjuvant chemotherapy on the postoperative pulmonary infection of patients with advanced gastric cancer.

OBJECTIVETo evaluate the effect of neoadjuvant chemotherapy on the postoperative pulmonary infection of patient with gastric cancer.
METHODSBetween January 2009 and January 2011, 139 patients with curable stage T2N2 or T3N2 gastric cancer were randomly assigned to group 1(69 cases, neoadjuvant chemotherapy with combined preoperative intraarterial infusion and intravenous chemotherapy before gastrectomy and D2 lymphadenectomy) and group 2(70 cases,gastrectomy and D2 lymphadenectomy alone). The morbidity of postoperative pneumonia in the two groups were recorded respectively.
RESULTSThe two groups were similar with respect to various clinical or pathological characteristics. All the patients underwent gastrectomy and D2 lymphadenectomy successfully. Eight patients in group 1 suffered from the postoperative pneumonia, accounting for 11.6%. Nine patients in group 2 suffered from the postoperative pneumonia, accounting for 12.9%. There was no significant difference between the two groups about the incidence of postoperative pneumonia.
CONCLUSIONSurgery following neoadjuvant chemotherapy with combined preoperative intraarterial infusion and intravenous chemotherapy is safe with similar morbidity of postoperative pneumonia to immediate surgery in patients with locally-advanced resectable gastric carcinoma.

8.Prevention and management of refeeding syndrome in patients with chronic critical illness.

Nutritional support is an important means to treat the patients with chronic critical illness for commonly associated malnutrition. Refeeding syndrome is a serious complication during the process, mainly manifested as severe electrolyte with hypophosphataemia being the most common. Refeeding syndrome is not uncommon but it is often ignored. In our future clinical work, we need to recognize this chinical situation and use preventative and treatment measures. According to NICE clinical nutrition guideline, we discussed the risk factors, treatment methods and preventive measures of refeeding syndrome in patients with chronic critical illness. We argued that for patients with high risk refeeding syndrome, nutritional support treatment should be initially low calorie and slowly increased to complete requirement. Circulation capacity should be recovered, fluid balance must be closely monitored and supplement of vitamins, microelement, electrolytes should be noted. After the emergence of refeeding syndrome, we should reduce or even stop the calorie intake, give an active treatment for electrolyte disorder, provide vitamin B, and maintain the functions of multiple organs.
