1.Efficacy of recombinant human growth hormone in the treatment of dwarfism in children at different ages and its effect on bone metabolism, insulin-like growth factor-1 and vitamin D3

Objective:To investigate the effects of recombinant human growth hormone on bone metabolism, insulin-like growth factor-1 (IGF-1) and vitamin D3 in dwarfism children at different ages.Methods:Sixty children with dwarfism who received treatment in Shaoxing Women's and Children's Health Care Hospital from January 2018 to January 2019 were included in this study. They were divided into group A (age at 4-9 years, n = 32) and group B (age at >9-13 years, n = 28) according to different ages. Two groups of children were treated with recombinant human growth hormone for 6 months. Total effective rate in two groups was recorded. Before and after treatment, bone metabolism index, serum IGF-1 and 25-hydroxyvitamin D3 levels were measured in each group. Results:Total effective rate in the group A was significantly higher than that in the group B [90.62% (29/32) vs. 67.85% (19/28), χ2 = 4.838, P < 0.05]. Before treatment, there were no significant differences in serum levels of calcium, phosphorus, zinc, IGF-1 and 25 hydroxyvitamin D3 between groups A and B (all P > 0.05). After treatment, serum levels of calcium, phosphorus, zinc, IGF-1 and 25 hydroxyvitamin D3 in the group A were (1.99 ± 0.53) mmol/L, (1.76 ± 0.14 ) mmol/L, (88.97 ± 6.89) μmol/L, (325.57 ± 15.29) ng/L, (89.47 ± 15.58) ng/L, respectively,which were significantly higher than those in the group B [(1.71 ± 0.55) mmol/L, (1.65 ± 0.15) mmol/L, (85.22 ± 6.76) μmol/L, (312.29 ± 13.88) ng/L, (80.11 ± 15.31) ng/L, t = 2.005, 2.936, 2.121, 3.502, 2.340, all P < 0.05]. Conclusion:Recombinant human growth hormone has better curative effect on dwarfism in children at 4-9 years old than in children at 9-13 years old, and it can effectively improve bone metabolism, IGF-1, vitamin D3 and other indicators.

Thyroid cancer is one of the most common malignant tumors in the human endocrine system . It is one of the common diseases in head and neck ,thyroid and breast surgery .Its incidence rate is increasing year by year .With the development of ultrasonography and fine needle aspiration biopsy ,as well as the auxiliary exami-nation of gene detection technology ,the detective rate of early diagnosis of papillary thyroid microcarcinoma ( PT-MC) is getting higher and higher .Compared with conventional surgery , highlighting the advantages of minimally invasive surgery , endoscopic and ultrasound guided percutaneous ablation and other new surgical methods are gradually applied in clinical treatment .Combined with the new guideline ,the present paper reviews the progress in the diagnosis and treatment of papillary thyroid microcarcinoma .
3.Analgesic Effect of Different Extraction Process of Taohong Siwu Decoction and Its separating Prescription

Objective To compare the analgesic effect of Semen Persicae (SP, Taoren), Flos Carthami (FC, Honghua), Siwu Decoction (SD) and Tao Hong Siwu Decoction (THSD) in dysmenorrhea model and the acetic-acid-induced pain model. Methods The dysmenorrhea model and the acetic-acid-induced pain model were established, and the pain threshold was compared in different mice models. Results On dysmenorrheal model, the analgesic effect of water extract supernatant and ethanol suspension of FC, SP polysaccharide, water extract supernatant and ethanol extract of SD, SD and THSD polysaccharide differed from that of blank control. On acetic-acid-induced pain model, the analgesic effect of water extract supernatant and ethanol suspension of FC, FC polysaccharide, ethanol extract of SP, SP polysaccharide, SP+ FC water extract, SP+ FC ethanol extract, SP+ FC polysaccharide, SD ethanol extract, SD polysaccharide, THSD water extract and ethanol extract, and THSD polysaccharide differed from that of blank control. Conclusion FC water extract and ethanol extract, SP ethanol extract, THSD polysaccharide, SD ethanol extract and SD polysaccharide have the best analgesic action, and then come with THSD water extract and ethanol extract, SP + FC ethanol extract, and SP + FC polysaccharide.
4.A clinic pathological study of skip metastases of the mediastinal lymph node in lung cancer

Objective To investigate the pathologic characteristics of skip metastases of mediastinal lymph node in lung cancer. Methods Of 398 lung cancer patients who underwent radical resection and extensive lymph node dissection. 47 patients were diagnosed as with skip N 2 metastases, and their mediastinal lymph nodes were analyzed. Results The skip N 2 metastases of lung cancer appeared as single group or multi-group. The commonest metastatic site was the 7th group lymph node (28.4%), then the 4th, 3rd and 5th group, rating 24.2%, 15.8% and 10.5%, respectively. For cases with squamous cell carcinoma or adenocarcinoma, no any skip N 2 metastasis occurred for the tumor less than 1 cm in diameter, for tumor greater than 1 cm in diameter, the skip N 2 metastasis increased with tumor size. 5.Surgical Treatment of Esophageal Stricture Caused by Corro-sive Burn

This paper presents the experience of surgical treatment of 32 patients with esophageal stricture caused by corrosive burn. The modes of surgical treatment in this series included: instrumental dilatation in 11 cases, reconstruction with resection of strictured esophagus in 19 (pharyngogas-trostomy in 6, cervical esophagogastrostomy in 12 and colon interposition in 1) and reoperation for esophageal restenosis in 2, There was no operative morality. All patients were fol- lowed-up and 94% of them resumed their normal diet post-operatively. The author recommend that dilatation can be applied to patients with localized esophageal stricture or slight annular stricture, and should done early. Resection of the esophageal stricture and esophagal reconstruction can be used in patients with extensive or severe stricture. There are many ways to esophageal reconstruction. The optimal one should meet the following requirements: isoperistaltic segment without tension and rich in blood supply, it should be easy to perform and result in no restenosis.
6.Investigation of Needlestick Injury in Medical care workers

OBJECTIVE To study the risk factors and control measures for needle injury,so that to provide relevant information for epidemiological studies of needle injury.METHODS The Needle injury of the worker′s age,type,location,operation links,workload and the spread exposed source were investigated,in the medical staff from Jan 2005 to Dec 2007 including 155 cases of needle injury(125 medical staff).RESULTS The survey showed that the risk factors of needle injury were short working age,lack understanding of needle injury,needles being putted cap back again,naked needles and busy working.CONCLUSIONS The control measures for Needle injury are to strengthen occupational training,dispose contaminated needles timely,avoid needles being naked,forbid needles being putted cap back again,and assign people reasonably.
7.Ginsenosisde Rg3 inhibits HIF-1? and VEGF expressions in acute leukemia bone marrow stromal cells

Objective To investigate the effect of ginsenoside Rg3 on HIF-1? and VEGF expressions in acute leukemia bone marrow stromal cells(BMSCs)and the possible underlying mechanism.MethodsBMSCs from normal health individuals and acute leukemia patients were cultivated.The appropriate time and concentration of ginsenoside Rg3 on BMSCs' proliferation were assessed by MTT.The BMSCs were treated with ginsenoside Rg3,and mRNA levels of HIF-1? and VEGF were detected by RT-PCR.In addition,the expre-ssion of HIF-1?,VEGF,p-Akt and p-ERK proteins were measured by Western blot analysis and immunofluorescence assays.ResultsAfter the addition of ginsenoside Rg3,MTT showed that the appropriate concentration and time was 40 ?g/ml and 24 h respectively.HIF-1? and VEGF expressions at mRNA and protein levels were reduced significantly(P
8.Preoperative regional chemotherapy in advanced gastric carcinoma

Purpose:To study the efficacy of preoperative selective intra arterial chemotherapy in advanced gastric carcinoma. Methods:Between August 1999 and November 2001, a total of 98 hospitalized patients with advanced gastric carcinoma who underwent selective intra arterial regional chemotherapy prior to surgery were recruited in this study. 60 patients with advanced gastric carcinoma on whom surgery were performed did not receive any preoperative regional anti cancer drugs between 1997 and 1998 and served as controls. The surgical resection rate and histologic investigations were compared. Results:The resection rate was 78% in regional chemotherapy group and 68% in control group. Tumor cell necrosis was investigated by postoperative pathological study and revealed that the response rate in regional chemotherapy group was 67%. Only 8 cases in control group showed mild focal necrosis. Conclusions:Preoperative regional chemotherapy in patients with advanced gastric carcinoma may enhance resection rate and destroy large numbers of tumor cells vigorously. The preliminary results are satisfying.
9.Magnetic resonance imaging reflects articular cartilage degeneration caused by proteoglycan depletion in the rabbit knee

Objective To study the appearance of magnetic resonance imaging (MRI) in early stages of cartilage degeneration and to detect its values.Methods Intra articular injection of 5 units of papain causing a reversible loss of cartilage proteoglycan in the New Zealand rabbit knees.Rabbits were scanned with magnetic resonance imaging,using a 0 3 T Hitachi magnet with 16 cm coil.Gradient echo sequences and spin echo sequences were performed in the sagittal planes at 0,24,48 and 72 hours after intra injection of papain.Then signal intensity and thickness of cartilage were measured.The proteoglycan content was measured biochemically and histochemically.Results The cartilage thickness and signal intensity decreased significantly in treated knees compared with control knees at 24 and 48 hours ( P 0 05) after injection of papain.These changes on magenetic resonance (MR) images were consistent with the changes of proteoglycan concentration measured by biochemical analysis and histochemical staining of cartilage.Conclusion It is possible for MRI to detect the early stages of cartilage degeneration.
10.Comparison of clinical efficacy of penehyclidine hydrochloride and atropine in the treatment of acute organophosphorus pesticide poisoning

Objective To compare the clinical effect of penehyclidine hydrochloride and atropine in the treatment of acute organophosphorus pesticide poisoning.Methods 86 patients with organophosphorus pesticide poisoning were selected and randomly assigned to the observation group and the control group according to the digital table,43 cases in each group.The two groups were given conventional treatment,while the control group was treated with atropine,the observation group was given penehyclidine hydrochloride.The disappearance time of main symptoms, rescue success rate and adverse reactions of the two groups were observed.Results After salvage treatment, the rescue success rate of the observation group was 97.8%,which of the control group was 88.4%,the difference was statistically significant (x2=1.433,P<0.05).The disappear time of M like symptoms,N like symptoms and central nervous system symptoms in the observation group was significantly shorter compared with the control group (all P<0.05).The number of drugs,dosage,cholinesterase recovery time and hospitalization time between the two groups had statistically significant differences (P<0.05).The incidence rates of blurred vision,restlessness,heart rate and urinary retention in the control group were significantly higher than those in the observation group (all P<0.05).Conclusion Penehyclidine hydrochloride for acute organophosphorus pesticide poisoning can significantly reduce the incidence of symptoms,shorten the disappearance time of symptoms,reduce hospitalization time,improve the efficacy of rescue,reduce the incidence of adverse reactions, it is safe,effective and has great clinical significance.
