1.Keyword Network Analysis of Infusion Nursing from Posts on the Q&A Board in the Intravenous Nurses Café

Objectives:
Portal sites have become places to share queries about performing nursing and obtain expert know-how. This study aimed to analyze topics of interest in the field of infusion nursing among nurses working in clinical settings.
Methods:
In total, 169 user query data were collected from October 5, 2018 to December 25, 2021. This exploratory study analyzed the semantic structure of posts on the nurse question-and-answer board of an infusion nursing-related internet portal by extracting major keywords through text data analysis and conducting term frequency (TF) and term frequency-inverse document frequency (TF-IDF) analysis, N-gram analysis, and CONvergence of iteration CORrelation (CONCOR) analysis. Word cloud visualization was conducted utilizing the “wordcloud” package of Python to provide a visually engaging and concise summary of information about the extracted terms.
Results:
“Infusion” was the most frequent keyword and the highest-importance word. “Infusion→line” had the strongest association, followed by “vein→catheter,” “line→change,” and “peripheral→vein.” Three topics were identified: the replacement of catheters, maintenance of the patency of the catheters, and securement of peripheral intravenous catheters, and the subtopics were blood sampling through central venous catheter, peripherally inserted central catheter management, evidence-based infusion nursing, and pediatric infusion nursing.
Conclusions
These findings indicate that nurses have various inquiries in infusion nursing. It is necessary to re-establish the duties and roles of infusion nurses, and to develop effective infusion nursing training programs.
2.Single Percutaneous Retrograde Anterior Column Screw Fixation in a Minimally Displaced Transverse Acetabular Fracture - A Case Report -

According to the Letournel classification, a transverse fracture is the only elementary fracture pattern that breaks both the anterior and posterior border of the innominate bone. A transverse acetabular fracture separates the innominate bone into two segments: the iliac segment and the ischiopubic segment. Therefore, minimally displaced transverse fractures can be stabilized by purchasing both segments with a large-diameter single screw. Although it is not a stable internal fixation construct compared with plates and screws, it provides sufficient stability to promote early mobilization and early weight-bearing while minimizing the risk of secondary displacement and preventing secondary complications associated with prolonged bed rest and immobilization. The authors successfully treated a case of minimally displaced transverse acetabular fracture with percutaneous column fixation using a retrograde fashion of a single anterior column screw. This report discusses the case with a literature review and deliberates the usefulness of the procedure.
3.An Deletion/Insertion Polymorphism of the Angiotensin Converting Enzyme Gene in Ischemic Stroke Patients.

BACKGROUND: A deletion (D)/insertion (I) polymorphism of the angiotensin-converting enzyme (ACE) gene is known to be associated with hypertension, left ventricular hypertrophy, myocardial infarction. Cardiac diseases, such as atrial fibrillation, valvular heart disease, myocardiac infarction and coronary artery disease have been clearly associated with increasing the risk of ischemic stroke. We investigated the relationship between ACE gene deletion/insertion (D/I) polymorphism and the pattern of ischemic stroke. METHODS: The pattern of ACE genotypes in 59 stroke patients including symptomatic carotid artery territory cerebral ischemia were compared with 101 age-matched control subjects. In the stroke patients, the degrees of stenosis of bilateral cervical carotid arteries and their major intracranial tributaries were recorded according to duplex neck sonography and magnetic resonance angiography. DNA was extracted from peripheral blood and ACE I/D polymorphism is confirmed by PCR method. RESULTS: In the stroke patients, 25.4% showed the I I genotypes, 8.5% the ID genotypes and 66.1% the DD genotypes. In the control group, the frequencies of each genotype were 20.8%, 55.4% and 23.8%, respectively. The DD genotypes were more common in patients with ischemic stroke compared with the controls, but there was no significant association between ACE genotypes and sub-types of cerebrovascular disease. CONCLUSIONS: The deletion polymorphism in the angiotensin-converting enzyme gene may play a role in development of ischemic stroke.

4.Large Hill–Sachs Lesion Combined with a Rotator Cuff Tear in an Acute Traumatic Anterior Dislocation of the Shoulder in an Elderly Patient Treated with an Allogenic Iliac Tricortical Bone Graft

An indirect reduction through the bicipital groove and allogenic iliac tricortical bone graft was performed as a treatment for a large Hill– Sachs lesion with a rotator cuff tear in an acute traumatic shoulder joint dislocation in an elderly patient. Six months after surgery, the rotator cuff healing and bone union were confirmed. The patient also showed satisfactory clinical results with 95 points of the American Shoulder and Elbow Surgeons (ASES) shoulder score and active range of motion with 155° flexion, 120° abduction, and 70° external rotation and 30º internal rotation at 90º abduction. The surgical method can be considered for a large Hill–Sachs lesion with rotator cuff tear in an acute traumatic shoulder joint dislocation in elderly patients.
5.Factors Affecting Telescoping of Spiral Blade after Operation for Intertrochanteric Fracture with Proximal Femoral Nail Antirotation II

Purpose:
This study examined the factors influencing the telescoping of the spiral blade after surgery with the proximal femoral nail antirotation II (PFNA II) for intertrochanteric fracture patients.
Materials and Methods:
Between January 2017 and January 2022, the degree of telescoping of the spiral blade was measured in 178 patients who underwent surgery with PFNA II for intertrochanteric fractures. Several variables were investigated, including the patient's sex, age, neck shaft angle of the nail, number and horizontal length of the posteromedial bone fragment, degree of osteoporosis in the normal hip, AO Foundation/Orthopaedic Trauma Association classification, tip apex distance, position of the spiral blade in the femoral head, and the reduction status of whether the proximal bone fragment had been adequately reduced into the intramedullary canal of the distal femur. These factors were analyzed statistically to determine their impact on the telescoping of the spiral blade.
Results:
The transverse size of the bone fragment and the reduction state of the fracture significantly influenced the telescoping of the spiral blade after surgery.
Conclusion
Inadequate reduction of the proximal bone fragment inside the distal intramedullary cavity must be avoided to prevent excessive telescoping of the spiral blade following intertrochanteric fracture surgery. In addition, in cases where the transverse size of the posteromedial bone fragment is substantial, consideration should be given to either fixing the posteromedial fragment or delaying weightbearing during rehabilitation.
6.Association of Falls and Fear of Falling with Mortality in Korean Adults: The Dong-gu Study

This study evaluated the association between falls and the fear of falling (FOF) with the risk of all-cause mortality in Korean adults. The study enrolled 4,386 subjects aged 50 years and over who participated in the Dong-gu Study. Falls in the past year were categorized as yes or no. Injurious falls were defined as falls that resulted in fractures, head injuries, sprains or strains, bruising or bleeding, or other unspecified injuries. FOF was classified as low or high. The associations of falls and fall-related characteristics with mortality were assessed using Cox proportional hazards models. The average follow-up was 7.8 years. During this period, 255 men and 146 women died. In a fully adjusted model, falls in the past year were not associated with an increased risk of all-cause mortality (hazard ratio [HR] 1.16, 95% confidence interval [CI] 0.85–1.58), but a history of injurious falls was associated with an increased risk of mortality (HR 1.36, 95% CI 1.04–1.79). Compared with subjects without a FOF, subjects who were moderately or very afraid of falling had a higher mortality rate (HR 1.26, 95% CI 0.97–1.63). In conclusion, injurious falls and a high FOF increased the risk of all-cause mortality in Koreans. This study suggests that injurious falls and FOF can predict mortality in the general population.

7.Unilateral Pulmonary Edema: A Rare Initial Presentation of Cardiogenic Shock due to Acute Myocardial Infarction.

Cardiogenic unilateral pulmonary edema (UPE) is a rare clinical entity that is often misdiagnosed at first. Most cases of cardiogenic UPE occur in the right upper lobe and are caused by severe mitral regurgitation (MR). We present an unusual case of right-sided UPE in a patient with cardiogenic shock due to acute myocardial infarction (AMI) without severe MR. The patient was successfully treated by percutaneous coronary intervention and medical therapy for heart failure. Follow-up chest Radiography showed complete resolution of the UPE. This case reminds us that AMI can present as UPE even in patients without severe MR or any preexisting pulmonary disease affecting the vasculature or parenchyma of the lung.

8.Type 2 Myocardial Infarction Following Generalized Tonic-Clonic Seizure.

Myocardial infarction is diagnosed when blood levels of biomarkers are increased in the clinical setting of acute myocardial ischemia. Among the biomarkers, troponin I is the preferred biomarker indicative of myocardial necrosis. It is tissue specific for the heart. Myocardial infarction is rarely reported following seizure. We report a case of elevated troponin I in a patient after an episode of generalized tonic-clonic seizure. The diagnosis was type 2 myocardial infarction.
