Objective To investigate the effect of different modes of one-lung ventilation ( OLV) on respiratory mechanics, hemodynamics and arterial oxygen tension ( PaO2) in pediatric patients. Methods Thirty-four ASA Ⅰ - Ⅱ patients (14 male, 20 female) aged 2-10 yr, weighing 8-26 kg undergoing abdominal surgery or operations on extremities were enrolled in this study. The patients were premedicated with intramuscular phenobarbital 2-3 mg ? kg-1 and atropine 0.015 mg ? kg-1 . In the operating room the patients were given intramuscular midazolam 0.25 mg? kg-1 and ketamine 2.5 mg ? kg-1 before intravenous line and EGG, NIBP and SpO2 monitoring were established. Anesthesia was induced with intravenous fentanyl 3-5 ?g kg-1 and vecuronium 0.1 mg? kg-1 and maintained with isoflurane inhalation supplemented with intermittent iv boluses of fentanyl and vecuronium. In patients aged over 5 yr Univent (Fuji Corpi) was inserted and left main bronchus was blocked during OLV. In patients less than 5 yr ordinary tracheal tube was inserted and was advanced into right main bronchus during OLV. Correct positioning of the tube was checked by fiberoptic bronchoscopy or auscultation. The test consisted of three steps : firstly two-lung ventilation (TLV) with volume-control mode; secondly OLV with volume-control mode;lastly OLV with pressure-control mode. During first and second step VT was set at 8-10 ml kg-1 and respiratory rate was adjusted to maintain PETCO2 between 4.5-6.0 kPa. During the third step (OLV with pressure-control mode) the inspiratory pressure was set according to the plateau pressure during step 1 and 2 (volume-control mode). Each step was maintained for 25 min before respiratory mechanics , hemodynamics and PaO2 were measured and recorded.Results During OLV with volume-control mode (second step) , peak pressure (Ppeak), plateau pressure (Pplat) and airway resistance (Raw) were significantly higher but dynamic compliance was significantly lower, cardiac output (CO) and stroke volume (SV) were significant lower but systemic vascular resistance (SVR) was significantly higher and PaO2 was significantly lower than those during first step (two lung ventilation with volume-control mode) (P

Objective: To investigate the changes of haemodynamics, pulmonary mechanics and blood gas in volume controlled and pressure controlled ventilatory patterns during one-lung ventilation (OLV). Methods: 20 patients with patent ductus arteriosus (PDA) underwent left thoracotomy PDA ligation with right OLV. The patients were divided into three groups: two-lung ventilation with volume controlled (TLV-VCV), one-lung ventilation with volume controlled (OLV-VCV), and one-lung ventilation with pressure controlled (OLV-PCV). After two-lung ventilation with VCV, one-lung ventilation was started by VCV and the ventilation mode was then switched to PCV. All measurements were made 25 min after initiation of the ventilation mode. The respiratory mechanics index was measured by side stream spirometry (SSS), including peak airway pressure (Ppeak), plateau pressure (Pplat), airway resistance (Raw), lung compliance (Cdyn) and inspiratory and expiratory minute ventilation (Mvi, Mve). Cardiac output (CO), stroke volume (SV), systemic vascular resistance (SVR), left ventricular ejectiontime (LVETi), and aortic blood flow acceleration (ACC) were also measured, by using the transesophageal Doppler (TED) monitor. 5.Distribution and change in antimicrobial resistance of pathogens from sputum of hospitalized children in a pediatric department in 3 consecutive years

Objective To realize the distribution and change in antimicrobial resistance of common pathogens causing acute lower respiratory tract infection (LRTI)in children,and provide evidence for rational use of antimicrobial agents. Methods Data about pathogens from children with LRTI in a hospital between January 2011 and December 2013 were ana-lyzed statistically.Results Of 934 isoalted pathogenic strains,728 (77.94%)were gram-negative bacteria,the major were Klebsiella pneumoniae (n =278),Escherichia coli (n =216),Enterobacter cloacae (n =85 ),and Pseudomonas aeruginosa (n=63).The isolation rate of gram-positive bacteria was 20.87%(n=195),the major were Staphylococcus aureus (n=132)and Streptococcus pneumoniae (n=49).Antimicrobial susceptibility testing results revealed that sensitive rate of gram-negative bacteria to imipenem,meropenem,and amikacin were all 100.00%,to ceftazidime/clavulanic acid and piperacillin/tazobactam were relatively low,to cephalosporins increased year by year.Sensitive rates of the main gram-positive bacteria to vancomycin were both 100.00%, to erythromycin and clindamycin were relatively low. Conclusion The major pathogenic bacteria causing LRTI in pediatric department are gram-negative bacteria,antimicrobial resistance rates increased year by year,management of antimicrobial use in children should be strengthened to prevent the occurrence of multidrug-resistant organism infection.

Objective To investigate the effect of sitagliptin in the treatment of elderly patients with type 2 diabetes mellitus complicated with coronary heart disease and its influence on Hcy,irisin and chemerin.Methods From January 2015 to June 2016,600 elderly patients with type 2 diabetes mellitus complicated with coronary heart disease in the Sixth People's Hospital of Cixi were selected and divided into observation group and control group,with 300 cases in each group.The control group received metformin treatment,the observation group was given sitagliptin combined with metformin.Before and after treatment,the blood FBG,HbA1c and BMI,Hcy,irisin and chemerin levels were detected.Results After treatment,the FBG,HbA1 c and BMI levels in the observation group were significantly lower than those in the control group [(6.28 ± 1.43) mmol/L vs.(7.03 ± 1.04) mmol/L,t =5.256;(6.17 ± 1.02) % vs.(7.02 ± 0.98) %,t =7.446;(21.03 ± 2.04) kg/m2 vs.(23.02 ± 1.23) kg/m2,t =14.469] (all P < 0.05).The level of Hcy in the observation group after treatment was significantly lower than that in the control group [(7.48 ± 1.03) μmol/L vs.(10.38 ± 1.74) μmol/L,t =17.868,P < 0.05].After treatment,the blood irisin level in the observation group was significantly higher than that in the control group [(3.28 ± 0.89) μg/mL vs.(2.43 ± 0.38)μg/mL,t =15.213,P < 0.05].After treatment,the chemerin level in the observation group was significantly lower than that in the control group [(210.38 ± 9.84)ng/mL vs.(231.38 ± 10.03)ng/mL,t =24.379,P < 0.05].Conclusion Sitagliptin in the treatment of elderly patients with type 2 diabetes mellitus complicated with coronary heart disease can effectively regulate the levels of Hcy,irisin and chemerin,and improve the curative effect.
8.Immunosuppression effects of MSCs transplantation in rat burn models

Objective To explore the immunosuppression effects,outcomes and clinical significance of mesenchymal stem cells (MSCs) transplantation in burn treatment by comparing the levels of WBC,C-reaction protein (CRP),interferon-γ(IFN-γ),interferon-o (TNF-α),interleukin-6 (IL-6),interleukin-10 (IL-10) between control burn models and models conducting MSCs transplantation.Methods After stripping Wharton's Jelly from the neonatal umbilical cord,MSCs was cultured and expanded.Burn models were constructed in male SD rats weighted at (200± 5) g,and randomly grouped for control and MSCs transplantation.The rats in transplantation group were injected subcutaneously with hUCMSCs (1.5× 106) at 24 h after burning.The content of WBC,CRP,IFN-γ,TNF-α,IL-6,IL-10 in blood samples at 0,1,2,3,5 and 7 d after burning were detected by enzyme-linked immunosorbent assay (ELISA).The ELISA results,the wound healing rate at 7,14,21 and 28 d,as well as wound healing time were compared and analyzed statistically by ANOVA between the two groups.Results WBC in control group increased significantly at 1 and 2 d,and CRP in control group increased substantially at 2 and 3 d.IFN-% IL-6 and IL-10 levels in serum showed increase on 5 d,and TNF-α arrived at its peak value at 7 d.By contrast,WBC,CRP,TNF-α,IL-6 and IL-10 of the MSCs transplantation group showed slightly increase after burning and the differences were convinced by statistical analysis,while IFN-γ showed little difference among the two groups.The MSCs transplantation group also showed significantly higher wound healing rate at 14,21,28 d and shorter wound healing time than that of the control.Conclusions MSCs transplantation can suppress the over-inflammatory response by significantly mediating the inflammatory cytokines as WBC,CRP,TNF-α,IL-6,IL-10 in burn.IFN-γseems not affected by MSCs transplantation.MSCs would be suitable to promote wound healing and repair in burn,which is achieved not only by differentiation and paracrine,but also by subtle immunoregulation.
9.Effects of Danqi Granules combined with Xianling Gubao Capsules on bone metabolism and bone mineral density in patients with postmenopausal osteoporotic vertebral compression fractures after surgery

Objective To observe the effects of Danqi Granules combined with Xianling Gubao Capsules on bone metabolism and bone mineral density (BMD) in patients with postmenopausal osteoporotic vertebral compression fractures (OVCF) after surgery. Methods 90 cases of postmenopausal women with OVCF were randomly divided into the control group and the observation group, 45 cases in each group. The two groups were treated by vertebroplasty (PVP), and after the surgery, the control group was treated with Xianling Gubao Capsules while the observation group was additionally given Danqi Granules. The serum markers of bone metabolism [(25 (OH) D, PINP, β-CTX] and BMD of L2-4 and left femoral neck were detected before and after treatment. Results The level of serum 25 (OH) D in the observation group after treatment was significantly higher while the levels of PINP and β-CTX were significantly lower than those in the control group (P<0.05), and the BMD of L2-4 and left femoral neck of the observation group were significantly higher than those of the control group (P<0.05). There was no significant difference in the incidence of adverse reactions between the two groups. Conclusion Danqi Granules combined with Xianling Gubao Capsules can significantly accelerate fracture recovery, and improve bone mineral density in the treatment of postmenopausal women with OVCF.
10.Effect of Bushenantai recipe on the expression of endometrial LIF in mice with embryonic implantation dysfunction.

In order to observe the effect of Bushenantai recipe on the expression of endometrial leukemia-inhibitory factor (LIF) in mice with embryonic implantation dysfunction (EID), 120 Kunming mice post coition were randomized into three groups: normal control group, model group and traditional Chinese medicine group (TCM group) (n=40 in each group). Uterus was collected on the pregnancy day (Pd) 4, 5, 6 after an intravenous injection of Evan's blue. The endometrium was dyed by Evan's blue and the mean points of response were observed on Pd 5. The expression of LIF mRNA and protein was detected by RT-PCR and immunohistochemistry respectively and analyzed statistically by image system. The results showed that the number of implantation sites in model group was remarkably less than in normal control group and TCM group. There was no significant difference between normal control group and TCM group. The expression of LIF mRNA and protein in model group was delayed. Bushenantai recipe could increase the expression of LIF mRNA and protein in endometria of mice with EID. It was suggested that Bushenantai recipe could improve embryo implantation of mice with EID by promoting the endometrial LIF expression and endometrial decidualization.
