1.Posterior surface topography of early keratoconus patients

Objective To investigate the corneal posterior surface topography characteristics of keratoconus at early stages applying the Pentacam anterior segment analysis system. Methods The present study included 43 eyes of 43 patients with keratoconus at the subclinical stage (group A), 40 eyes of 40 suspected keratoconus patients (group B), and 143 normal eyes of 143 controls (group C). Based on an examination of the Pentacam anterior segment analysis system of each subject, a series of data, including the posterior surface refractive power, the posterior surface elevation were collected. The differences among the three groups, the correlation among indices, and the area under the receiver operating characteristic (ROC) curves were analyzed. Results The mean values of the posterior surface maximum refractive power in 3 groups were -6.2 D, Q=0.5;-5.6 D, Q=0.3;and -5.5 D, Q=0.3, respectively. The mean values of the posterior surface maximum elevation were 23 μm, Q=14;11 μm, Q=8.5;and 7 μm, Q=6, respectively. The posterior surface maximum refractive power and maximum elevation among three groups were statistically different. The area under ROC (AUR) of posterior maximum refractive power and the AUR of posterior maximum elevation were greater than that of other indices in the diagnosis of early keratoconus. Conclusions Based on the results from the Pentacam anterior segment analysis system, the changes of the posterior surface refractive power and elevation are important characteristics of early keratoconus.
2.Factors associated with knowledge of HIV/AIDS prevention and control among farmer workers with private corporations in Guangxi

Objective To explore the knowledge about HIV/AIDS and related factors associated among farmer workers with private corporations in Guangxi and to inform intervention programs.Methods Questionnaire survey was conducted among workers working with 10 private corporations,and data were analyzed using logistic regression.Results Correct answers to questions about HIV/AIDS transmission route accounted for over 85%,yet much less regarding knowledge about non-transmission routes and prevention methods.The factors associated with knowledge about HIV/AIDS included gender,age,nationality,registered living place and years of schooling.Conclusions HIV/AIDS-related education should be strengthened for farmer workers working for private corporations.

This article introduced the peritoneal dialysis nursing model of Home Dialysis Unit in the affiliated hospital of Colorado University,including:overall status,setting and distribution,responsibilities of staff,work model of PD (pre-dialysis education,catheter insertion,CAPD/CCPD training,clinic visit model,home visit model,etc),and explored the enlightenment of this model on PD nursing in our country.This model provided references for dialysis center distribution,staff arrangement,pre-dialysis education,home visit,increase of social support and so on.
4.Prophylactic treatment of ganciclovir to prevent and cure cytomegalovirus infection after renal transplantation: a systematic review

Objective To assess the efficacy of ganciclovir to prevent and cure cytomegalovirus (CMV) infection after renal transplantation. Methods We searched PubMed, EMBASE, Cochrane Library, SCI, China Academic Journals Full-text Databases, Chinese Biomedical Literature Database, Chinese Scientific Journals Databases and Chinese Medical Association Journals to collect randomized controlled trials of ganciclovir to prevent and cure CMV infection after renal transplantation (up to June, 2009). Two reviewers extracted data independently using a designed extraction form. The quality of included trials was evaluated according to the Cochrane Handbook. RevMan 5.0 software was used for data analysis. Results Twelve randomized controlled trials were included. The results of meta-analysis showed that: ①Compared with no receive antiviral agents, ganciclovir couldn't lower CMV infection rate and disease rate in 3 months and 6 months after renal transplantation, but could lower CMV disease rate in 12 months. The delay between transplantation and CMV infection was significantly longer. ②Either valaciclovir or ganciclovir could lower CMV infection rate and disease rate after renal transplantation, without statistical difference. ③Compared with acyclovir, ganciclovir could lower CMV disease rate in 6 months after renal transplantation. ④Compared with CMV-IgG and valganciclovir, ganciclovir didn't have statistical difference in decreasing CMV disease rate (P=0.93;P=0.14). Conclusions Longer prophylaxis by ganciclovior may prevent CMV infection after renal transplantation. Its curative effect is similar to valaciclovir, CMV-IgG and valganciclovir, but better than acyclovir.
5.Determination of Alkaloids in Honey by Dispersive Solid Phase Extraction and High Performance Liquid Chromatography-Tandem Mass Spectrometry

A method was developed for simultaneous determination of pyrrolizidine alkaloids and isoquinoline alkaloids in honey by dispersive solid phase extraction and high performance liquid chromatography-tandem mass spectrometry ( QuEChERS-HPLC-MS/MS) . The honey samples were extracted with acetonitrile solution and cleaned up with PSA absorbent. Agilent Poroshell 120 SB-C18 chromatographic column was used to separate alkaloids with high sensitivity and satisfactory resolution. The identification and quantification were achieved by using electrospray ionization in positive ion mode ( ESI+) with multiple reaction monitoring (MRM). Matrix-matched calibration curves with good correlation coefficients (R2>0. 99) were obtained in the concentration range of 0. 1-100 μg/L. The recoveries of the spiked samples at 1-100 μg/kg were in the range of 70% to 110% with the RSD of intra-day and inter-day lower than 15% and 20%, respectively. The limits of detection ( LOD) and limits of quantification ( LOQ) for all alkaloids were 0. 3 and 1. 0 μg/kg, respectively. This method was successfully applied to the simultaneous determination of pyrrolizidine alkaloids and isoquinoline alkaloids for quantification and confirmation in honey samples.

7.Investigation and Analysis of Satisfaction Degree of Hospitalized patients on Nursing in Primary Hospitals

Objective:To comprehensively improve the quality of clinical nursing, survey the hospitalized pa-tient′s satisfaction and analyze the low satisfaction issues.Methods:Adopt the self-made questionnaire in four pri-mary hospitals.Results:Through the questionnaire analysis, understand the hospitalized patients to improve grass-roots hospital trusteeship overall satisfaction, but satisfaction to nursing technical operation, the life care is low. Logistic stepwise regression analysis, according to the results of nursing technical operation and health education are the main factors influencing the patients′satisfaction.Conclusion:Nursing managers should pay attention to the factors influencing patient satisfaction, and take corresponding corrective measures, improve the patient′s satisfac-tion, to further improve the reform of hospital custody.
8.Transparent cap-fitted endoscopy in diagnosis of Barrett's esophagus

Objective To evaluate the feasibility of transparent cap-fitted endoscopy in improving diagnostic yield of Barrett's esophagus(BE).Methods A total of 168 patients with endoscopieally suspected BE were randomly divided into cap group(n=60)and control group(n=108).A transparent cap-fitted endoscopy Was applied in cap group to take biopsy,while a routine one was used in control group,and 2 biopsies from suspected lesions were collected in each patient.BE was diagnosed in the presence of columnar epithelium and the diagnostic rate was compared between 2 groups.Results The diagnostic rates of BE were 83.3%(50/60)and 69.4%(75/108)in cap and control group,respectively(P<0.05).Conclusion The lesions at cardia Call be exposed clearly with capped endoscopy,which facilitates localized biopsy and BE diagnosis.
9.Safety and efficacy of single balloon enteroscopy for small intestinal disorders

Objective To evaluate safety and clinical significance of single balloon enteroscopy (SBE) for small intestinal diseases. Methods Data of 83 patients with suspected or known small intestinal diseases, who underwent SBE from March 2009 to July 2010, were reviewed in terms of preparation time,procedure time, detection rate and complication occurrence. Results The 83 patients included 37 cases of digestive tract bleeding, 38 chronic abdominal pain, 1 chronic diarrhea, 2 fever and 5 incomplete ileus. A total of 94 procedures of SBE were performed, including oral route in 46 patients, anal route in 26 and both routes in 11. Excluding 6 cases with endoscopic therapy, the mean procedure time of oral approach was 29.6 ± 10. 3 min, and that of anal route was 57.1 ± 15.6 min. Abnormalities were detected in 57 ( 68.7% )of the 83 patients, with detection rate of 81.1% (30/37) in digestive tract bleeding with unknown reason,57. 8% (22/38) in chronic abdominal pain of unknown reason, 50. 0% (1/2) in fever of unknown reason and 80. 0% (4/5) in incomplete ileus. Peutz-Jeghers syndrome was diagnosed in 6 patients and endoscopic polypectomy was performed, with complicated bleeding in one patient. No other procedure-related complications were observed. Conclusion SBE is well-tolerated and safe for diagnosis of small intestine diseases,with easy manipulatiou, short procedure time, high detection rate and satisfactory location of intestinal hemorrhagic lesions.
10.Characteristics of achalasia:a high-resolution manometry study

Objective To evaluate the clinical and manometric characteristics of achalasia.Meth-ods Patients diagnosed as having achalasia from July 2010 to July 2014 at our hospital were enrolled.High resolution manometry(HRM)results were analyzed.Data of Eckardt scale,MDADI and SF-36 were ana-lyzed.Results All subjects had dysphagia,35.6% (37 /104)accompanied with regurgitation,26.9%(28 /104)with heartburn and 17.3%(18 /104)with weight loss.According to HRM results and Chicago classification criteria,16.35%(17 /104)of the subjects were classified as type Ⅰ,76.92%(80 /104)as type Ⅱ and 6.73%(7 /104)as type Ⅲ.27.9%(29 /104),19.2%(20 /104)and 24.0%(25 /104)of the subjects finished Eckardt scale,MDADI and SF-36,respectively.Eckardt score was positively correlated with integrated relaxation pressure(IRP)(r =0.421,P <0.05)and MDADI physical score was negatively with IRP(r =-0.530,P <0.05).Conclusion Dysphagia often occurs as the chief complaint among acha-lasia patients.And type Ⅱ is the most common.IRP is an indicator of the severity of clinical symptoms and impairment of quality of life.
