1.Cloning of 5'flank upstream regulation region of LCRG1 and the measurement of promoter activity

Background and purpose:The molecular regulation mechanism of the LCRG1 gnen is unclear;The study was designed to clarify the regulatory elements of LCRG1 gene in its 5'flanking region.Methods:Bioinformatics approaches were adopted and a putative promoter region was found in 5'flank upstream fragment of LCRG1 gene,5'flank upstream regulation fragment 1.776 kb(from-1 191 bp to +585 bp)of LCRG1 gene was amplified by PCR using genomic DNA as template,construct was obtained by cloning DNA fragments into pGL3 reporter vector.The construct was then introduced into COS7 cells by Lipofectemin method for transient expression of reporter gene,and luciferase activities was measured by luciferase assay.Results:The sequence of 5'flank upstream regulation fragment 1.776 kb(from-1 191 bp to +585 bp)of LCRG1 gene was successfully cloned and proved to be correct by DNA sequencing,the activity of pGL3-1776 was about 0.16-fold higher than that of pGL3-control cotransfection with PRL-TK and 35-fold higher than that of pGL3-basic cotransfection with PRL-TK.Conclusions:5'flank upstream regulation region 1.776 kb(from-1 191 bp to +585 bp)of LCRG1 was cloned successfully,the fragment presented promoter activity.
2.Analysis of susceptible factors of neonatal nosocomial infection in primary hospital and the control countermeasures

Objective To understand the clinical characteristics and susceptible factors of neonatal nosocomial infection in primary hospital,and discuss the control measures in order to effectively reduce the neonatal nosocomial infection rate.Methods A retrospective analysis about clinical characteristics and susceptible factors of neonatal nosocomial infection was carried out from October 2010 to September 2012.Results Premature rupture of fetal membranes,premature infant,low birth weight,invasive operation were high risk factors for nosocomial infection.Infection most often occurred in respiratory tract,skin and soft tissue.Conclusions Neonatal nosocomial infection is influenced by many factors,in which the awareness of prevention and control among the medical staff,the aseptic operation,hand hygiene compliance,the ward disinfection management level and education during the pregnancy are crucial,which are the key link in the control of neonatal nosocomial infection.
3.Cloning and Identification of Promoter of Human LCRG1 Gene

Laryngeal carcinoma related gene 1(LCRG1) is a candidate tumor suppressor gene of Laryngeal carcinoma. To further investigate its transcriptional regulation, the transcriptional start sites for LCRG1 gene have been identified by 5′ RACE (rapid amplification of cDNA ends) based on the bioinformation analysis of LCRG1. Then eleven luciferase expression vectors which contained potential human LCRG1 gene promoter were constructed. Luciferase reporter assay indicated that LCRG1 promoter region was mainly located in -169~+127 region nearby the major transcriptional start site. These results suggested that the region (-169~+127) includes an essential promoter for human LCRG1 gene transcription.
4.A Clinical Study on Repairing the Unilateral Cleft Lip with Millard Method and the Improvement of Operative Approach

Objective To analyze the efficacy of repairing unilateral cleft lip (UCL) with Millard method, and explore the individual operative procedure. Methods According to the difference of lip height between non-cleft and cleft sides, MillardⅠ, MillardⅡ or modified Millard Ⅱ operative approach was chosen to repair UCL, then the clinical effect of operation was evaluated. Results The ideal operative effects were obtained in 208 cases of UCL using various operative approaches based on the individual deformity. There were consistent lip height between both sides, normal vermillion bulk and satisfactory facial appearance after operation. Conclusion The satisfactory efficacy could be obtained by choosing Millard method's operative approaches based on the individual deformity. The modified Millard operative approach could repair all kinds of UCL.
5.Current situation of myopia among primary school students in Jinhua city and the intervention effect of knowledge, belief and practice

AIM:To investigate the current situation of myopia in a primary school in Jinhua city and the intervention effect of knowledge, belief and practice.METHODS: From September to December 2022, a total of 1 482 primary school students who met the inclusion criteria were selected for myopia test and education intervention based on cluster sampling method. The questionnaire was designed based on relevant references. The main contents of the questionnaire include students' general information, low vision and knowledge, belief and practice before and after intervention and prevention of myopia.RESULTS: A total of 1 482 primary school students were investigated and tested, among which myopia was detected out of 657 students, with a myopia rate of 44.33%. Specifically, mild myopia accounted for 54.64%(359); moderate myopia accounted for 23.90%(157); severe myopia accounted for 21.46%(141). A total of 1 468 valid questionnaires were collected before intervention, and 1 457 valid questionnaires were collected after intervention. The overall knowledge rate of myopia prevention and control knowledge before intervention was 62.57%(7348/11744), and it was 81.20%(9465/11656)after intervention, with statistical significance before and after intervention was statistically significant(P<0.001). The behavior attendance rate before intervention was 82.48%(9687/11744), and it was 93.73%(10925/11656)after intervention, with statistical significance comparison before and after intervention(P<0.001).CONCLUSION: The results of this survey showed that the myopia situation of primary students in Jinhua city was not optimistic, and some students were not aware of prevention knowledge and correct behavior before intervention. After intervention, the knowledge, belief and practice of myopia prevention and control have achieved remarkable results.
6.Status of the poor vision among 3 014 primary school students in Wucheng district of Jinhua city and analysis of the influencing factors

AIM: To explore the prevalence and influencing factors of poor vision among 3 014 primary school students in Wucheng District, Jinhua City, in order to provide evidence for the intervention measures of poor vision.METHODS: Using a cross-sectional study, a self-designed questionnaire was done, including general conditions and influencing factors of vision. The international standard logarithmic visual acuity chart was used for visual acuity detection. The risk factors that were statistically significant among the single factor groups were used as independent variables for multivariate Logistic regression analysis.RESULTS: A total of 1 367 students with poor vision were detected, the detection rate was 45.36%, among which mild poor vision rare was 26.08%; moderate rate was 11.51%; and severe rate was 7.76%. Multivariate Logistic regression analysis showed that senior grades, parental myopia, daily insufficient sleep and outdoor activity, overuse of electronics or computers and no eye exercise were risk factors for poor vision.CONCLUSION: The rate of poor vision among the primary school students was severe, schools should take measures to improve and prevent students' poor vision by increasing outdoor activity time, correcting poor reading and writing posture, and reducing the time spent on electronic products.
7.Clinical analysis of transdermal fentanyl patches for pain management in the terminal cancer patients with abnormal hepatic and renal function and ascites

Objective To investigate the clinical efficiency,safety,adverse reactions and healthrelated life quality of transdermal fentanyl patches for pain management in terminal cancer patients with hepatic and renal dysfunction and ascites.Methods 98 terminal cancer patients with moderate to serve pain combined with abnormal hepatic and renal dysfunction were randomly divided into two groups.Group Ⅰ (n=56)received transdermal fentany patches.Group Ⅱ,the control group (n=42),received morphine controlledrelease tablets.Terms of pain intensity,function of renal and liver,adverse reactions and heath-related quality of life were assessed before and after treatment.Results Patients were satisfied with their pain management in both groups,and no significant difference in pain release was observed between the groups (x2 =0.01,0.07,0.01,0.04,P > 0.05).The incidences of constipation and dysuresia were significantly less in the group Ⅰ compared with that of the group Ⅱ (x2 =7.32,3.96,P < 0.05).The incidences of hypersomnia,dizzy,nausea and vomiting were similar betwecn the two groups (x2 =0.12,0.54,0.54,0.02,P > 0.05).Most of the adverse reactions would relieve or disappear after symptomatic treatment.Values of BUN,CR,ALT,AST were similar before and after treatment in group Ⅰ (t =1.43,1.67,0.91,0.11,P> 0.05).However,in group Ⅱ,these values were significantly increased after treatment (t =17.59,49.17,42.12,36.23,P < 0.05).The heathrelated life quality (appetite,spirit,somnus,fatigue,daily life and countenance) were significantly improved after treatment in both groups (the group Ⅰ t =3.37,4.40,2.07,5.66,4.48,P < 0.05; the group Ⅱ t =2.03,2.27,3.59,4.16,2.79,P < 0.05),the spirit group Ⅰ improved more obviously compared with group Ⅱ (t =2.93,P < 0.05).Conclusion Transdermal fentanyl patches provides equal pain relief compared with Morphine controlled-release tablets in the terminal cancer patients with abnormal hepatic and renal function and ascites.Transdermal fentanyl patches significantly improve health-related quality with less hepatic and renal function influence and low incidence of adverse reaction.It should be recommended in the treatment terminal cancer patients with moderate to serve pain combined with abnormal hepatic and renal function and ascites.
8.Study on the gene expression profile of poorly differentiated gastric carcinoma using cDNA microarray

Background and purpose:The molecular mechanism of the pathogenesis of poorly differentiated gastric carcinoma is unclear, because the key genes related to poorly differentiated gastric carcinoma have not been identified. The study was designed to establish the gene expression profile of poorly differentiated gastric carcinoma, isolate gastric carcinoma-related genes, and investigate the relationship between gastric carcinoma-related genes and gastric carcinoma. Methods:The changes of gene expression profile between poorly differentiated gastric carcinoma and adjacent normal tissue of gastric epithelia were analyzed by cDNA microarray which represented approximately 10 000 known genes that would be tested in the assay. Immunohistochemistry were employed to validate the relationship between gastric carcinoma-related genes and gastric carcinoma.Results:212 genes that were differentially expressed in cancer and non cancerous tissues were identified; 169 genes were highly expressed in cancer tissues by more than 2.0 fold, 43 genes were lower expressed in cancer tissues by more than 2.0 fold. EMS1 protein was located in cytoplasma. 9.A novel role for tanshinoneⅡA in modulation of dendritic cells maturation and function

Objective:Bone marrow-derived dendritic cells ( DCs) were extracted and gave tanshinone ⅡA to intervene ,and we observed the change of DCs function ,which investigate the effects of tanshinone ⅡA in immune system.Methods:Extract bone mar-row-derived DCs,and cells were cultured in complete medium with 10 ng/ml GM-CSF and IL-4.On day 5,magnetic cell sorting was used to purify DCs,and the purify must be up to 90%.Then,a certain concentration of tanshinone ⅡA or LPS was gave to the cell cul-ture,and cells and supernatant were collected for following experiments .We used flow cytometry to detect phenotype , and enzyme-linked immunosorbent assay(ELISA) was used to detect cytokine production of TNF-αand IL-12.Also,allogeneic mixed lymphocyte reaction was used to detect the ability of DCs to induce T cell proliferation and polarization .Results:When the concentration of tanshi-none ⅡA was 500 ng/ml,the inhibition of secretion of TNF-αwas maximal.So we chose that concentration .Compared with the control group,DCs in experimental group had reduced expression of CD 80,CD86 and MHCⅡ(P<0.05).Compared with the control group ,DCs in experimental group displayed the reduced level of IL-12 and TNF-α(P<0.05).Compared with the control group,DCs in experimen-tal group had reduced ability to induce T cell proliferation (P<0.05).Compared with the control group ,DCs in experimental group in-duced T cell to secret increased level of IL-4,and reduced level of IFN-γ(P<0.05).Conclusion:TanshinoneⅡA can inhibit the den-dritic cells maturation induced by LPS ,which takes part in immune system and autoimmune diseases .
10.Characterization of Key Regulatory Elements of LCRG1 Promoter

LCRG1(laryngeal carcinoma related gene1,LCRG1),a new candidate tumor suppressor gene of laryngeal carcinoma.However,it is known little about the possible regulatory mechanisms of LCRG1 gene expression.Restriction endonuclease digestion was used to obtain a set of the 5',or 3'deletion mutants from the region(-169~+127) of the LCRG1 gene.It has been found that the minimal promoter of the LCRG1 gene is mapped at the region from-169~-57.Linker scanning mutational analysis in the region(-169~+127) of the LCRG1 gene was used to identify the crucial cis-elements within the promoter region,The key cis-elements are within the region from-137~-122.SP1,E2F1/DP1,EKLF and ZF9 transcription factor binding site sites were predicted in the region by bioinformatics analysis.Co-transfection with each of a panel of the expression plasmids of the known transcription factors with the relevant reporter construct indicates Sp1 is potent transcription factor for enhancement of the promoter activity,SP1 can also up-regulate the endogenous expression of LCRG1 gene.Electrophoretic mobility shift assay(EMSA) was applied to verify that the key cis-elements of LCRG1 gene exist sequence of Sp1 binding sites.The findings,which showed that the key cis-elements within the region from 137~-122 play an important role in expression of the LCRG1 gene,provide a novel evidence for further study of the function of LCRG1 gene.
