1.Application of naso-jejunal feeding tube in early enteral nutrition for patients with severe acute pancreatitis

Objective To investigate the method and nursing care of early enteral nutrition through naso-jejunal feeding tube for patients with severe acute pancreatitis.Methods We retrospectively studied 37 patients with severe acute pancreatitis,who were given early enteral nutrition through naso-jejunum nutrition tube.Results Abdominal distention occurred in 5 patients,and 3 patients got mild diarrhea.No aspiration and SAP relapse occurred among 37 patients.All the patients were discharged from hospital after recovery.Conclusions Enteral nutrition through naso-jejunum nutrition tube is the main treatment and nutritional support measure for patients with severe acute pancreatitis.Daily care of the naso-jejunum nutrition tube,and prevention and treatment of complications are the critical work in SAP patients' nursing care.
2.Cloning of 5'flank upstream regulation region of LCRG1 and the measurement of promoter activity

Background and purpose:The molecular regulation mechanism of the LCRG1 gnen is unclear;The study was designed to clarify the regulatory elements of LCRG1 gene in its 5'flanking region.Methods:Bioinformatics approaches were adopted and a putative promoter region was found in 5'flank upstream fragment of LCRG1 gene,5'flank upstream regulation fragment 1.776 kb(from-1 191 bp to +585 bp)of LCRG1 gene was amplified by PCR using genomic DNA as template,construct was obtained by cloning DNA fragments into pGL3 reporter vector.The construct was then introduced into COS7 cells by Lipofectemin method for transient expression of reporter gene,and luciferase activities was measured by luciferase assay.Results:The sequence of 5'flank upstream regulation fragment 1.776 kb(from-1 191 bp to +585 bp)of LCRG1 gene was successfully cloned and proved to be correct by DNA sequencing,the activity of pGL3-1776 was about 0.16-fold higher than that of pGL3-control cotransfection with PRL-TK and 35-fold higher than that of pGL3-basic cotransfection with PRL-TK.Conclusions:5'flank upstream regulation region 1.776 kb(from-1 191 bp to +585 bp)of LCRG1 was cloned successfully,the fragment presented promoter activity.

5.Influence of gonadotropin releasing hormone agonist on the expression of mRNA of nerve growth factor and its receptors in eutopic endometrial stromal cells

Objective To investigate the influence of gonadotropin releasing hormone agonist (GnRH-a) on the expression mRNA of nerve growth factor (NGF) and its receptors (TrkA and P75NTR) in normal and eutopic endometrial stromal cells (ESC).Methods From January to April 2009,3 patients with endometriosis undergoing surgery in Peking Union Medical College Hospital were obtained eutopic endometrium as study group matched with eutopic endometrium from 3 parents with teratoma as control group.ESC were incubated with different concentration of GnRH-a (0,5 × 10-11,5 × 10-10,5 × 10-9,5 ×10-8,5 × 10-7 g/ml).The expression of mRNA of NGF,TrkA and P75NTR were measured by real-time-PCR.Results At concentration of 0 g/ml,the levels of NGF,TrkA and P75NTR mRNA in ESC were 6.32,8.55,8.08 in study group,which were significantly higher than 0.94,0.67,1.08 in control group (P <0.05).Treated by the following concentration of GnRH-a (5 × 10-11,5 × 10-10,5 × 10-9,5 × 10-8,5 ×10-7 g/ml),the median expression of NGF,TrkA and P75NTR mRNA was 1.00,0.96,1.05; 1.09,0.82,1.27 ; 1.04,0.52,0.81 ; 1.00,0.55,0.64; 0.78,0.49,1.02 in study group.Compared with the expressions of those untreated by GnRH-a in study group, they showed significantly lower trends (P <0.05).In control group,the median expression of NGF,TrkA and P75NTR mRNA was 0.98,0.37,0.92; 0.70,0.45,1.15; 1.55,0.80,1.35; 1.09,0.41,1.35; 0.90,0.82,1.18.Compared with the expressions of those untreated by GnRH-a in control group,there were no statistically differences ( P >0.05).And treated by the same concentration of GnRH-a,the expressions of NGF,TrkA and P75NTR mRNA did not show statistically difference between the two groups ( P > 0.05 ).Conclusion The expression of NGF.TrkA and P75NTR mRNA were suppressed by GnRH-a.
6.The study of childhood trauma experience and family environment with obsession

Objective This study aimed to explore the relationship between the traumatic experience in childhood and onset of obsession in adulthood,and the character of family environment of obsession patients.Methods Using Y-BOCS and CAT and FES-CV to evaluate 41 cases with OCD and 36 normal subjects respectively.Baaed on these scales,an edited interview schema for a semi-structured interview was choosed with both groups for 50 minutes respectively.Results ①Tbe whole score and scores of two subscales on punishment,negative-environment in CAT were all obviously higher in patients with obsession [(50.16± 21.03),(12.03±4.24),(19.77±9.74),for punishment:P<0.05,for others:P<0.01],and there were a significant difference in the two groups[(35.35±14.78),(9.92±2.51),(12.54±8.14)].The negative environment was chosen in regress equation,and could explain 14.67% of outcome of obsession. ②The scores of 4 ubacaies like cohesion,expressiveness,intellectual-cultural orientation,active-recreational orientation [(5.13±2.28),(4.10±2.12),(2.71±1.87),(2.52±2.34)] ,were all obviously lower than the normal control in FES-CV.While the subscore of conflict (5.03±2.21) was higher than the normal control(3.50±2.40),and all these differences were significant.When two factors of active-recreational orientation and cohesion in FES-CV were put into regression equation( R2 =0.2078 ,R2 =0.2784),they contribute 27.84% and the disease,and the active-recreational orientation factor can explain 20.78%.③The result of interview found significant difference between obsession groupand control group in parental personality,own personality,childhood experience,and family environment.Condusion The traumatic experience in childhood and unhealthy family environment do have association with onset and development of obsession,also they have a close relationship with the development of personality after adulthood.
7.Effect of 3M transparent dressing on complications after peripherally inserted central catheter

Objective To discuss effect of different kinds of dressings on complications after periph-erally inserted central catheter(PICC). Methods Patients with skin hypersensitiveness after PICC adopt-ed sterile bandage and Anshutuo IV3000 transparent dressing to carry out local dressing change respective-ly. Incidence of catheter infection,slippage and complications were compared. Results The rate of catheter infection was 19.4%( 0 cases) and catheter slippage was 6.5%(7 cases) by using sterile bandage.While the rate of catheter infection was 3.2%(1 case) and catheter slippage was 3.2%(1 case) by using Aashutuo IV3000 transparent dressing. Conclusions Using Anshutuo IV3000 transparent dressing on local dressing change solved the shortcomings of common dressings and sterile bandage,reduced the inci-dence of complications after PICC and prolonged the using time of catheter.
8.Effect of audio and video feedback teaching on the rehabilitation of patients with ankylosing spondylitis

Objective To investigate the effect of the use of audio and video feedback on the rehabilitation of patients with ankylosing spondylitis (AS). Methods 60 AS patients were divided into two parts (32 in the experimental group and 28 in the control group) by random number table method, and two groups of patients were treated with the same drug treatment. Patients in the control group were instructed by oral instruction, in the experimental group, the use of audio and video feedback teaching guide patients family exercise, compared with the two groups of patients with four months of follow-up, family exercise compliance, joint function and quality of life, and other indicators. Results Four months after the intervention, the experimental group patients family exercise compliance was better than 84%(27/32), higher than the control group of 36% (10/28), the difference was statistically significant (χ2=9.238, P < 0.01). Patients in the experimental group in four months after the intervention, the health questionnaire to investigate the physiological function, role limitation due to physical problems, body pain, general health, vitality, social function, role limitation due to emotional problems, mental health eight dimension scores respectively were (72.7±8.4), (81.5±9.5), (57.2±8.4), (65.5±8.4), (62.9±7.2), (78.4±7.4), (67.3±6.2), (77.2±7.6) points while in the control group were (60.8±9.1), (72.1±9.6), (50.3±9.4), (60.8±7.0), (58.2±9.1), (74.4±8.7), (60.2±7.2), (74.3±7.4) points, the difference between the two groups had statistical significance (t=1.493-5.266, P < 0.05 or 0.01). Four months after the intervention, the scores of Bath Ankylosing Spondylitis Functional Index (BASFI) of patients with AS in the experimental group were (1.1± 0.8), (1.7±0.2), (1.8±1.3), (1.6±1.0), (3.1±2.1), (1.0±0.6), (1.9±1.6), (3.8±2.5), (2.6±1.7), (2.7±1.6) points, while (3.8±1.6), (2.8±1.5),(2.6±1.5)、(2.8±1.2),(4.4±2.2),(2.2±1.0),(3.3±1.6),(6.5±2.5),(5.6±2.6), (4.3 ± 1.5)control group, the two groups the difference was statistically significant (t=0.282-8.427, P<0.05). Conclusions Application of audio and video feedback teaching in patients with AS rehabilitation guidance, help patients to master the family training methods, improve the joint function of patients, improve the quality of life.

Diabetes mellitus has become the third threaten diseases second only to cardiovascular disease and tumor.Many re-searches demonstrated that the microRNAs(miRNAs)in blood related to the occurrence and development of diabetes melli-tus.Because of the non-invasion and repeatability of circulating miRNAs,the circulating miRNAs have a broad application prospect of diagnosis and prognosis evaluation for various types of diabetes.This overview arms to introduce the source and characters of circulating miRNA,and also the progress of circulating miRNA related to diabetes mellitus,so as to lay a foun-dation for further research.
10.Research of model for cultivating practical talent in local medical college

After taking above measures, students' practical ability was significantly enhanced and quality of teaching was improved.
