1.Research of model for cultivating practical talent in local medical college

Measures were taken to reform the current teaching mode including establishing cur-riculum system which embodies medical characteristics,exploring probation mode suitable for local med-ical college,setting up students' autonomous learning platform,improving performance evaluation meth-ods,teaching students in accordance with their aptitude,etc. Meanwhile,new partnership between school and hospital was explored though improving teaching management system. After taking above measures, students' practical ability was significantly enhanced and quality of teaching was improved.
2.Mierosphere internal iliac arterial embolizrtion therapy in cases of late pelvic malignant tumors

Transcatheter bilateral internal lilac arterial embolization therapy (TAE) using CDDP-mierosphere was performed in 19 patients with advanced or recurrent pelvic malignant tumors. The complete response (CR) of turner to the therapy in one patient, partial response (PR) in 12, miner response (MR) in 5, and no changes (NC) in 1, thus the response rate (CR+PR) reached 68% (13/19). The colporrhagia, pain of vulva and lump of buttock were evidently remitted. In the present survived 13 cases, with 1-year for 4 and half-year for 6. The important complications after embolization including mild lower limb paresthesia and hypodynamia in 10 cases, severe lower limb dyscinesia and sensory disturbance in one, pain of buttock in 16, erythema of buttock in two, pain of hip in one and hematochezia in one. The complications have been crucially discussed.
3.An association study on Gilles de la Tourette's syndrome and catechol-O-methyl transferase gene polymorphism

ObjectiveTo investigate the association between catechol-O-methyl transferase (COMT)gene polymorphism and Gilles de la Tourette' s syndrome(GTS).MethodsUsing Amplification Refractory Mutation System(ARMS) PCR genotyping assay method,a polymorphism (val158met) of COMT gene was genotyped in 112 of all GTS patients ( total GTS group) including 54 GTS-alone patients group,48 GTS + ADHD patients group among of them and 71 healthy controls.The correlation between positive association of polymorphism (val158met)of COMT gene in GTS and the age of onset in patients with GTS was also analyzed.ResultsCompared with healthy controls group,genotype of val158met did not differ in total GTS patients group or alone-GTS patients group (χ2 =0.56,P=0.756;χ2 =1.05,P=0.600 respectively).There was also no significant difference (P>0.05)in allele distribution of val158met in total GTS patients group or alone-GTS patients group compared with controls group respectively (χ2 =0.18,P=0.669;χ2 =0.29,P=0.593 respectively).However,genotype distribution of val158met was significantly different between GTS + ADHD patients group and controls group( χ2 =6.35,P =0.041 ).The frequency of the val allele of this locus was significantly higher in GTS + ADHD patients group than those in controls group ( χ2 =5.49,P =0.019 ).The mean age of onset (6.80 ± 1.54 ) in 36 children within GTS + ADHD patients group with the val/val geantype of COMT gene val158met polymorphism was significantly earlier than the mean age of onset (8.04 ± 1.54)in 12 children in val/val genotype (P =0.016 ).ConclusionPolymorphism (val158met) of COMT gene may be associated with GTS children with comorbid ADHD,which may play an important role to make the age of onset in children with GTS become earlier.
4.Primary synovial sarcoma of the kidney: a case report

The diagnosis and treatment of a case of primary renal synovial sarcoma was reported. The patient was diagnosed as malignant tumor of left kidney before operation and underwent laparoscopic radical nephrectomy. Renal synovial sarcoma was confirmed by pathological examination and gene test. After 6 courses of doxorubicin chemotherapy plus heavy ion radiotherapy for 15 days, no tumor metastasis or recurrence was found during the 14 months of follow-up. The diagnosis of this disease depends on molecular genetics. Surgery and chemotherapy are commonly used at present. Most patients have a poor prognosis.
5.Expression changes of miR-100 in maturation and cancerous rat liver

Purpose To explore the effect of miR-100 in rat liver development and the occurrence of liver cancer. Methods The liv-er tissues of neonate SD rats, adult SD rats and 2-AAF/PH rats were collected, then real-time fluorescence quantitative polymerase chain reaction ( RT-PCR) and FISH methods were used to detect the expression of miR-100 in the liver tissues. Results The expres-sion level of miR-100 in adult SD rats liver tissues was significantly higher than that in neonate SD rats liver tissues (P<0. 05). Com-pared with the adult rats, the expression level of miR-100 in 2-AAF/PH rats liver tissues was obviously decline (P<0. 05). Conclu-sion miR-100 can as a maker of liver mature, and its deletion maybe related to the tumor formation.
6.Genetic Polymorphism of 17 STR Loci in Uygur Population of Akesu in Xinjiang.

OBJECTIVES:
To investigate the genetic polymorphisms of 17 STR loci in Uygur population of Akesu in Xinjiang Uygur Autonomous Region.
METHODS:
Blood samples from 10 094 unrelated individuals of Uygur population of Akesu in Xinjiang were amplified by using the 17+1 PCR amplification kit, and 17 STR loci typing were detected. Allele distribution and population genetic parameters of each locus were calculated, and compared with those of Chinese Han population, and Uygur population of Yili, Hotan and Turpan regions.
RESULTS:
In the 10 094 unrelated Uygur individuals, 252 alleles were detected. All loci meet the Hardy-Weinberg equilibrium using a chi-square goodness-of-fit test expectation except vWA loci. Fourteen out of the 17 STR loci, including D13S317, showed high power of discrimination. A significant difference on the allele distribution of all the 17 STR loci was observed between Uygur population of Akesu and Chinese Han population, and the difference of some loci existed between Uygur population of Akesu and the other three Uygur populations.
CONCLUSIONS
The 17 STR loci are highly polymorphic genetic markers, and the polymorphic information could serve as reference data for forensic individual identification and paternity identification in Akesu.

7.Prediction model of mid-term fatality risk after radical resection in patients with hepatocellular carcinoma based on ALBI-grade.

OBJECTIVE:
To establish a clinical prediction model of the mid-term fatality risk after radical resection in patients with primary hepatocellular carcinoma (HCC) based on the albumin-bilirubin (ALBI) grade and to assess its prediction value.
METHODS:
Clinical data of 533 patients who received HCC radical resection in Jinhua Hospital of Zhejiang University, Jinhua People's Hospital, Jinhua Hospital of Traditional Chinese Medicine and Jinhua Guangfu Hospital from January 2010 to August 2016 were retrospectively reviewed. In the training group ( =407), Cox model was used to screen the clinical risk factors of postoperative death, and a predictive model based on ALBI grade was established and then examined in the validation group ( =126). The value of the prediction model was assessed by ROC curve and calibration curve; the prediction results of the model were visualized by the nomogram for the convenience of clinical use.
RESULTS:
Cox model showed that ALT ≥ 80 U/L, tumor maximum diameter ≥ 5 cm, portal vein tumor thrombus and ALBI grade 2 were independent risk factors for the prognosis of patients with HCC radical resection. The prognosis index (PI) was 0.550×ALT+0.512×ALBI grade+0.872×maximum tumor diameter+1.377×portal vein tumor thrombus. The AUCs for predicting the risk of death in 12, 36 and 60 months were 0.872, 0.814 and 0.810, respectively (all < 0.01), and the goodness of fit ( ) of the established model were 0.953, 0.976 and 0.994. AUC of the established model for predicting risk of death in 36 months after resection was 0.814, which was higher than those of ALBI (AUC=0.683), BCLC (AUC=0.713), CLIP (AUC=0.689), Child-Pugh (AUC=0.645), TNM (AUC=0.612) ( < 0.05 or < 0.01).
CONCLUSIONS
ALT ≥ 80 U/L, maximum tumor diameter ≥ 5 cm, portal vein tumor thrombus and ALBI grade 2 are independent risk factors of patients after HCC resection, and ALBI grade-based prediction model is satisfactory in prediction of mid-term death risk of the patients.

8.Biomechanical changes and characteristics of impact temporal bone fractures

Objective To investigate the relationship of impact stress distribution and stress wave dissemination with temporal bone fracture. Methods Twelve donated fresh corpse were selected and impacted with driving force of 800 kPa, 1 200 kPa and 1 400 kPa to establish the model of impact temporal bone fractures. With aid of three-dimensional stereochemical structure image of 64-slice CT, the vitodynamic distribution and dissemination of impacted temporal bone was calculated by using computer simulation. Results (1) The instant velocity, acceleration and energy of impact as well as displace-ment scope of the hammer increased with higher driving force in the tempus. (2) Instantaneous impact of the temporal bone resulted in Von Mises stress concentration area around the impact point. The stress was weakened suddenly in the skull-brain tissue coupling place but later accumulated in the skull base. (3) Only ipsilateral linear fracture occurred when the driving force of impact was 800 kPa. While impact dif-ferent degrees of depressed fractures could be seen and one case showed secondary fracture of the opposite side when the driving force of impact was 1 200 kPa. The impact with driving force for 1 400 kPa begot complex ipsilateral fractures, secondary fracture of the opposite side and skull basal fracture. (4) Frac-ture line of the temporal bone accorded with impact stress concentration area. Conclusions All pa-rameters, stress distribution and dissemination of impact are in accordance with temporal bone fracture during impaet process of the temporal bone, which is of vital significance for diagnosis and protection of impact temporal bone fractures.
9.Single-stage repair of infected or contaminated abdominal wall defects and abdominal hernias with biological meshes

ObjectiveTo evaluate the safety and efficacy of biological meshes (human aceUular dermal matrix mesh) in single-stage repair of infected or contaminated abdominal abdominal wall defects and abdominal hernias. MethodsSeventeen patients with abdominal wall defects or abdominal hernias were enrolled. The wounds of all these patients were infected or contaminated due to the existence of enterocutaneous fistula or stoma, wound infection and synchronous colonic resection. The diagnosis included enterocutaeneous fistula 8 cases, incisional hernia 6 cases, incarcerated inguinal hernia 1 case and cylindrical abdominoperineal resection for rectal cancer for 2 cases. The sizes of abdominal defects ranged from 3 cm × 2 cm to 6 cm × 17 cm, and all the cases were repaired with human acellular dermal matrix mesh(RENOV(R)). Most of the patients were repaired with intraperitoneal onlay mesh technique( IPOM, for 12 cases), and other methods included Lichtenstein operation for 1 case, inlay repair for 2 cases and sublay for 2 cases. Results All the 17 patients recovered uneventfully. For 12 patients, the wounds were sutured at operation and only one case of delayed healing occurred due to fat liquefaction. For the other 5 patients, the wounds were left open and healed after vacuum assisted closure (VAC) therapy or wet- to- dry dressing changes. On follow up for 8.3 ±4.5 months ( 1 to 15 months), no occurrence of incisional hernia or recurrence was found. laxity of abdominal wall occurred in one case. A patient complained intermittent pain of the site of suture for mesh fixing two months after operation and the pain resolved spontaneously one month later. ConclusionsThe biological mesh, acellular dermal matrix mesh, could be used in single- stage repair of infected or contaminated abdominal wall defects safely and effectively, although the long-term outcome still needs further evaluation.
10.Expression of SATB2 and P53 in colorectal cancer and their clinical significance

Objective To investigate the expression of SATB2 and P53 protein in colorectal cancer and to analyze their relationship with the clinical pathologic characteristics. Methods The expression of SATB2 and P53 protein in 30 normal colorectal tissues and 120 colorectal cancer tissues were detected by immunohistochemical method,then analyzed their correlation. Results The expression of SATB2 in colorectal cancer was lower than that in normal colorectal tissues(P<0.05);and its expression correlated with the tumor size,differentiation,inva-sion,depth,lymph node metastasis and TNM stages(P<0.05),while not with age and gender.The expression of P53 in colorectal cancer was significantly higher than that in normal colorectal tissues(P<0.01),and its expres-sion correlated with differentiation,invasion depth and TNM stages(P<0.05),while not with age,gender,tumor size and lymph node metastasis. The expression of SATB2 and P53 in colorectal cancer have negative correlation (P < 0.01). Conclusion Compared with in normal colorectal tissues,the expression of SATB2 declined and the expression of P53 increased in colorectal cancer,while they have negative correlation. The expression levels of SATB2 and P53 protein have important significance to evaluate the prognosis of colorectal cancer.
