1.Causes of Complaint Events in Outpatient Dispensary and Countermeasures

OBJECTIVE:To reduce and prevent the incidence of complaint events in outpatient dispensary.METHODS:The causes of patients' complaints were analyzed and the countermeasures were put forward.RESULTS & CONCLUSIONS:The complaint events mainly induced by the absence of rules and regulations for pharmacists,lack of related information and professional knowledge,unsoundness of internal management,patients' enhancement in self-protection consciousness as well medical knowledge.The countermeasures available include the following:strictly following rules and regulations and operating instruction,reinforcing legal consciousness and self-protection consciousness,shifting one's ideas and concepts,strengthen-ing service consciousness etc.
2.Voluntary blood donation intention of students in middle vocational school in Jinhua and its influencing factors

【Objective】 To explore the intention of voluntary blood donation in students from a middle vocational school in Jinhua and its influencing factors. 【Methods】 A total of 400 students were enrolled from a middle vocational school in Jinhua by convenience sampling, and a self-designed questionnaire was issued to each student to investigate the cognition, intention and attitude of voluntary blood donation. The influencing factors of voluntary blood donation intention were analyzed by Logistic regression. 【Results】 A total of 394 valid questionnaires(98.50%) were collected. For 394 surveyed students, the average score of cognition of voluntary blood donation was (7.27±2.69) points, and the overall rate of intention to voluntary blood donation was 21.32%. Logistic regression analysis equation consisted of family support, better cognition, feeling happy about blood donation and worrying about blood donation, with risk ratios(OR) as 31.78, 188.69, 26.27 and 0.01, respectively(all P<0.05). 【Conclusion】 Students from a middle vocational school showed poor congnition and intention of voluntary blood donation.Family support for voluntary blood donation, better cognition of voluntary blood donation and happiness for blood donation were positive factors, and the fear of blood donation was a negative factor.It is necessary to strengthen the publicity of blood donation among students in middle vocational school to improve their congnition and intention to voluntary blood donation.
3.Participation of Anticoagulant Professional Pharmacist in the Medication for a Pulmonary Embolism Pa-tient with Warfarin Resistance

Objective: To discuss the therapeutic regimen for warfarin resistant patients with the help of clinical pharmacists. Methods:The pharmacist analyzed the cause of warfarin resistance and provided the dose adjustment for a pulmonary embolism pa-tient. Results:The genotype of the patient was identified as VKORC1-1639AG by gene test. The patient was diagnosed as hereditary warfarin resistance excluding other causes. With the assistance of pharmacist, the problem of warfarin resistant was solved. Conclu-sion:For warfarin resistant patients, clinical pharmacists must definite the reason of warfarin resistant firstly and then carry out the therapeutic strategy. Gene detection may be used as an important guide for warfarin resistance.
4.Explore the correlation between childhood obesity and asthma

Objective To investigate the relationship between childhood obesity,asthma and lung function.Method Based on body weight and whether with asthma the 45 children were divided into normal control group,asthma group,obesity and asthma group,15 cases each.Each child was measured leptin levels and pulmonary function was measured with a spirometer.Each asthmatic children was surveyed by Asthma Control Test (C-ACT) questions.Results Asthmatic children have varying degrees harm of lung function.Obesity and asthma group compared with asthma group of children is more decreased in lung function.Children with asthma compared with normal control group of children is more higher in serum leptin concentration.Obesity and asthma children is highest in serum leptin concentrations.Asthmatic children C-ACT average score than obesity and asthma score,the difference was statistically significant (P < 0.01).Conclusion Lung function and leptin abnormalities are exist in children with asthma.Also in the case of asthma,the abnormal levels of lung function and leptin in obese children compared with normal-weight children are more apparent,C-ACT score are more lower,and asthma is more severe.Weight control in obese children with asthma is favorable measures to control asthma attacks.
5.Dorsal plate assisted fixation of dorsal lunate fossa fracture of distal radius.

OBJECTIVE:
To explore clinical efficacy of dorsal plate assisted fixation of dorsal lunate fossa fracture block of distal radius.
METHODS:
From January 2019 to January 2022, 30 patients were treated with dorsal plate assisted fixation of dorsal lunate fossa fracture of distal radius, including 13 males and 17 females, aged from 42 to 68 years old with an average of (48.7±5.6) years old;According to Doi fracture classification, 24 patients were type 3 blocks and 6 patients were type 4 blocks. The degree of palmar angle of anterior and posterior distal radius was fixed by dorsal steel plate during operation. Fracture healing and functional recovery of wrist were observed after operation. Functional evaluation was performed by Gartland and Werley scoring system at 12 months after operation.
RESULTS:
All patients were followed up from 12 to 13 months with an average of (11.3±0.9) months. All fractures healed for 4 to 5 months with an average of(4.7±0.8) months. Median palpal inclination of anterior and posterior distal radius fixed by dorsal plate was 5.30°(4.85°, 6.03°), 12.45°(11.98°, 13.43°) respectively, and had statistical difference( P<0.01). Gartland and Werley scores was (1.1±0.4) at 12 months afteropertaion, and 27 patients got excellent result and 3 good.
CONCLUSION
Dorsal plate assisted fixation of dorsal lunate fossa fractures is beneficial to reduction and stabilization of displaced dorsal fractures and restoration of palmar inclination.

6.How COVID-19 epidemic and anxiety affect the blood donation behaviors of college students

【Objective】 To explore how the awareness and prevention status of COVID-19 epidemic, as well as anxiety affect blood donation behaviors of college students. 【Methods】 From June 7 to 13, 2020, students from two colleges in Jinhua were given self-made questionnaires through Wenjuanxing (a platform providing functions equivalent to Amazon Mechanical Turk), general demographic information, voluntary blood donation, core knowledge and prevention measures of COVID-19, as well as general anxiety were investigated. 466 valid questionnaires were collected, and the influencing factors of voluntary blood donation were analyzed by Logistic regression. 【Results】 College students donated(88.1±19.8)% were more likely to conduct qualified protections against COVID-19 than non-donation students(83.3±19.0)%(P<0.05), and be less anxious (9.68±4.80 vs 10.86±4.74, P<0.05). During COVID-19 outbreak, male donors was 3.732 times females(P<0.05). The donation rate of students with good awareness of COVID-19 protection, getting 60.0% correct or more, was 4.663 times students getting 60.0% below correct (P<0.05), and students with anxiety total score below 10 points was 2.227 times those above 10 points(P<0.05). 【Conclusion】 Gender, awareness of protections against COVID-19, and anxiety level are important factors affecting the blood donation behaviors of college students during COVID-19 epidemic, therefore strengthening psychological counseling and improving protection measures against COVID-19 are conductive to promote blood donation of college students during COVID-19 epidemic.
7.Effect on the pain and tear film stability in patients after pterygium excision treated with intradermal needling.

OBJECTIVE:
To analyze the effects of intradermal needling for pain and tear film stability in patients after pterygium excision.
METHODS:
A total of 76 patients (98 affected eyes) with primary pterygium were randomly divided into an observation group (38 cases, 53 affected eyes) and a control group (38 cases, 45 affected eyes).In the control group, only pterygium resection was performed, in the observation group, intradermal needling after pterygium resection was applied at Cuanzhu (BL 2), Yuyao (EX-HN 4), Taiyang (EX-HN 5), Sibai (ST 2), Hegu (LI 4), removed after 24 h and changed three times a week. The pain level of 3 days after surgery, dry eye symptoms, the basic tear secretion test (Schirmer-Ⅰ), and the tear-break time (BUT) changes before surgery, 2 weeks after surgery and 4 weeks after surgery were compared between the two groups, and the clinical efficacy was evaluated.
RESULTS:
The pain level of 3 days after surgery in the observation group was significantly lower than that in the control group (<0.05). The dry eye symptom scores at 2 weeks and 4 weeks after surgery in the two groups were significantly lower than those before surgery (all <0.05), and the dry eye symptom scores in the observation group were significantly lower than those in the control group (both <0.05). The Schirmer-Ⅰ test at 2 weeks and 4 weeks after surgery was significantly prolonged than that before surgery(all <0.05), and the Schirmer-Ⅰ test in the observation group was significantly longer than that in the control group (both <0.05). The BUT at 2 weeks and 4 weeks after surgery in the two groups was significantly longer than that before surgery (all <0.05), and the BUT in the observation group was significantly longer than that in the control group (both <0.05). The total effective rate in the observation group was 89.5% (34/38), which was higher than 71.1% (27/38) in the control group (<0.05).
CONCLUSION
Intradermal needling can effectively reduce the pain level of patients after pterygium resection, improve dry eye symptoms, promote the secretion of tears and improve the tear film stability.

8.Changes in the flux of hexosamine biosynthesis pathway in skeletal muscle of rats with insusulin resistance induced by high fat diet

Objective To investigate the effect of hexosamine biosynthesis pathway on the development of insulin resistance induced by high fat diet.Methods Normal male SD rats were randomly divided into three groups:control(fed with normal chow),high fat(fed with high fat diet for 13 weeks),and rosiglitazone (intragastric administration with rosiglitazone for 5 weeks)groups.After 13 weeks,all the rats were sacrificed,serum and muscle triglycerides(TG),serum total cholesterol(TC),and serum and muscle free fatty acids(FFA) were measured.Insulin sensitivity wss evaluated by insulin sensitivity index(ISI)and glucose infused rat(GIR) with the hyperinsulinemic englycemic clamp technique.The flux of HBP in skeletal muscle was detected with the expression level of glutamine-fructose-6-phosphate transaminase(GFAT)mRNA(RT-PCR),the content of UDPGlcNAc(HPLC)and the level of O-GlcNAc glycosylation in skeletal muscle proteins(Western blot). Results Compared with control group,senlm TG,TC,FFA and muscle TG,FFA levels of high fat group increased(aII P<0.01).both ISI and GIR decreased(both P<0.01),and the leveIs of GFAT mRNA(0.51±0.05 vs 0.18±0.02),UDP-GlcNAc[(6.18±0.86 vs 2.42±0.36)nmol/g],and O-GIcNAc glycosylation of skeletal muscle proteins in high fat group were raised(all P<0.01).In rosiglitazone group,serum and muscle TG.FFA welc deceased(all P<0.01).insulin sensitivity was increased(P<0.05)and the flux of HBP[GFAT mRNA 0.27±0.03,UDP-GIcNAc(2.62±0.32)nmol/g]was reduced(all P<0.05)as compared with high fat group. Conclusions High fat diet-induced insulin resistance in rats is correlated with the increased flux of HBP in skeletaI muscle.which is decreased by rosiglitazone.
9.Qualitative Study on Training Demand among Clinical Medical Postgraduate Students

10. Clinical Study on Timing of Re-treatment of Endoscopic Selective Varices Devascularization for Esophageal and Gastric Varices (Sarin Type GOV1 and GOV2)

Background: Liver cirrhosis complicated with esophageal and gastric variceal bleeding is a commonly seen critical illness. Gastrointestinal endoscopy is widely applied for bleeding control and prevention of rebleeding. Aims: To investigate the timing of re-treatment of endoscopic selective varices devascularization (ESVD) for treatment of esophageal and gastric varices presenting as Sarin type GOV1 and GOV2. Methods: Forty-eight cirrhotic patients with GOV1 or GOV2 varices and a history of bleeding admitted from October 2018 to September 2019 at Jinhua Municipal Central Hospital were enrolled and underwent ESVD therapy for secondary prevention. After the first ESVD procedure, patients were randomly allocated into two groups, and received the re-treatment 2 weeks (Group A) and 4 weeks (Group B) later, respectively. All patients were followed up for 6 months and the efficacy of ESVD, as well as the rebleeding rate and the mortality rate were compared between the two groups. Results: There was no significant difference in devascularization of varicose veins between Group A and Group B (76.9% vs. 81.8%, P>0.05). The incidence of rebleeding in Group A was significantly lower than that in Group B during the 6-month follow-up (11.5% vs. 36.4%, P<0.05). No death occurred in both groups. Conclusions: ESVD is effective for secondary prevention of GOV1 and GOV2 variceal bleeding. With regard to the timing of re-treatment, short-term (2 weeks postoperatively) re-treatment might reduce the probability of rebleeding because it could deal with the high risk and vulnerable bleeding vessels earlier.
