1.Role of fliY gene in pathogenicity-associated chemotaxis and colonization of Campylobacter jejuni.

OBJECTIVETo construct a knockout fliY gene mutant strain of Campylobacter jejuni for determining the role of FliY protein in flagellar movement related to bacterial motility, chemotaxis and colonization.
METHODSThe plasmid pBluescript-II-SK was used to construct the suicide plasmid; according to homologous exchange principle, the suicide plasmid was utilized to generate fliY gene knockout mutant(fliY) in Campylobacter jejuni strain NCTC11168. The fliY mutant strain was identified by PCR, sequencing and Western blotting. The chemotactic and colonizing abilities of fliY mutant were determined by colony migration test and bacterial chemotactic test in vitro, and colonization test in jejunum of mice.
RESULTSThe fliY(-)mutant strain showed a growth curve in medium similar to that of wild-type strain. PCR, sequencing and Western blotting assay confirmed that the fliY gene in fliY(-)mutant was deleted. Compared to the wild-type strain, the colonies of fliY-mutant on semisolid plate were much smaller (P <0.05), the chemotactic ability of fliY mutant towards sodium deoxycholate and bovine bile was significantly attenuated (P <0.05), and the number of fliY mutant (CFU) in jejunal tissue specimens of the infected mice was significantly decreased (P<0.05).
CONCLUSIONThe function of C.jejuni fliY gene refers to controlling flagellar movement, which is involved in bacterial chemotaxis and colonization.

2.Clinical significance of serum lactate dehydrogenase, β2-microglobulin and vascular endothelial growth factor level detection in patients with non-Hodgkin's lymphoma.

The study was aimed to investigate the clinical significance of serum lactate dehydrogenase (LDH), β2-microglobulin (β2-MG) and vascular endothelial growth factor (VEGF) detection in non-Hodgkin's lymphoma (NHL) patients. 58 newly diagnosed NHL patients from January 2010 to December 2011 in our hospital were selected as the study group (NHL group) and 58 healthy individuals were selected at the same time as a control group. Serum LDH level was determined by a biochemical method, while β2-MG and VEGF levels were determined by enzyme-linked immunosorbent assay. The results showed that as compared with control group, the LDH, β2-MG and VEGF levels in NHL group were significantly higher (P < 0.05). In NHL group, the LDH and VEGF levels at different pathological types and clinical stages changed remarkably. Compared with low grade patients, the LDH and VEGF levels of intermediate grade and high grade patients were significantly higher (P < 0.05). As compared with patients at stage I, the LDH and VEGF levels of patients at stage II, III and IV increased significantly (P < 0.05). Out from 58 cases in NHL group, 30 cases (51.72%) showed LDH-positive, 39 cases (67.24%) showed β2-MG positive and 29 cases (50.00%) showed VEGF-positive. It is concluded that some significant changes of serum LDH, β2-MG and VEGF appears in NHL patients, detection of these 3 tumor marker levels may be help to clinically grading patients with NHL.

3.Scutellarin starch microspheres' permeability through nasal mucosa of different animals in vitro.

OBJECTIVETo study scutellarin starch microspheres' permeability through nasal mucosa of different animals in vitro.
METHODThe Franz diffusion cell method was used to experiment the permeability test (n = 4), taking fresh nasal mucosa of dog, swine and domestica in vitro as permeation barrier separately, with scutellarin starch microspheres (scutellarin 0.25 mg) above them, and blank pH 6.8 PBS as absorption liquid to detemine the scutellarin by HPLC.
RESULTThe permeability coefficient of scutellarin starch microspheres through nasal mucosa of dog, swine and domestica in vitro were (5.295 +/- 0.637) x 10(-3) (4.065 +/- 1.140) x 10(-3), (1.855 +/- 0.150) x 10(-3) cm x mL(-1) separately. The permeability coefficient order of scutellarin starch microspheres through nasal mucosa of different animals in vitro is dog > swine > domestica, and there are significant differences between the permeability coefficient of scutellarin starch microspheres through nasal mucosa of dog, swine in vitro, and that through nasal mucosa of swine and domestica in vitro.
CONCLUSIONDrugs in scutellarin starch microspheres could permeate through the above-mentioned nasal mucosa in vitro. There might be different permeability coefficient among different species.

4. Antibiotic resistance analysis of Streptococcus pneumoniae isolates from the hospitalized children in Shanxi Children′s Hospital from 2012 to 2014

Objective:
To investigate the antibiotic resistance status of Streptococcus pneumoniae isolates from hospitalized children in Shanxi Children′s Hospital.
Method:
E-test and Kirby-Bauer methods were applied to determine drug sensitivity of the isolates collected from the body fluid specimens of hospitalized children in Shanxi Children's Hospital from January 2012 to December 2014. The antimicrobial sensitivity and minimum inhibitory concentration (MIC) of Streptococcus pneumoniae to the conventional antibiotics were analyzed, in order to compare the annual trends of non-invasive isolates, while the differentiation of sensitivity from specimens. The comparison of rates was performed by Chi-squared test and Fisher's exact test.
Result:
A total of 671 isolates of streptococcus pneumoniae were obtained, which could be divided as non-invasive isolates(607), invasive isolates from non-cerebrospinal fluid(non-CSF)(40) and invasive isolates from cerebrospinal fluid(CSF)(24). The antimicrobial sensitivity(isolates(%)) of the 671 isolates were respectively vancomycin 671(100.0%), linezolid 671(100.0%), levofloxacin 665(99.1%), penicillin 595(88.7%), ceftriaxone 516(76.9%), cefotaxime 512(76.3%), sulfamethoxazole-trimethoprin(SMZ-TMP) 103(15.4%), clindamycin 28(4.2%), tetracycline 26(3.9%), erythromycin 12(1.8%). From 2012 to 2014, the susceptibility rates of non-invasive isolates to penicillin every year were 95.0%(96/101), 97.3%(110/113), 87.3%(343/393), respectively, and there was significant difference among the three years(χ2=13.266,

Phytoremediation plays an important role in the treatment of heavy metal pollution in soil. In order to elucidate the mechanism of salicylic acid (SA) on copper absorption, seedlings from Xuzhou (with strong Cu-tolerance) and Weifang Helianthus tuberosus cultivars (with weak Cu-tolerance) were selected for pot culture experiments. 1 mmol/L SA was sprayed upon 300 mg/kg soil copper stress, and the photosynthesis, leaf antioxidant system, several essential mineral nutrients and the changes of root upon copper stress were analyzed to explore the mechanism of copper resistance. The results showed that Pn, Tr, Gs and Ci upon copper stress decreased significantly compared to the control group. Meanwhile, chlorophyll a, chlorophyll b and carotenoid decreased with significant increase in initial fluorescence (F0), maximum photochemical quantum yield of PSⅡ (Fv/Fm), electron transfer rate (ETR) and photochemical quenching coefficient (qP) content all decreased. The ascorbic acid (AsA) content was decreased, the glutathione (GSH) value was increased, the superoxide dismutase (SOD), catalase (CAT) and ascorbate peroxidase (APX) activity in the leaves were decreased, and the peroxidase (POD) activity was significantly increased. SA increased the Cu content in the ground and root system, and weakened the nutrient uptake capacity of K, Ca, Mg, and Zn in the root stem and leaves. Spray of exogenous SA can maintain the opening of leaf stomata, improve the adverse effect of copper on photosynthetic pigment and PSⅡ reaction center. Mediating the SOD and APX activity started the AsA-GSH cycle process, effectively regulated the antioxidant enzyme system in chrysanthemum taro, significantly reduced the copper content of all parts of the plant, and improved the ion exchange capacity in the body. External SA increased the content of the negative electric group on the root by changing the proportion of components in the root, promoted the absorption of mineral nutrient elements and the accumulation of osmoregulatory substances, strengthened the fixation effect of the root on metal copper, and avoided its massive accumulation in the H. tuberosus body, so as to alleviate the inhibitory effect of copper on plant growth. The study revealed the physiological regulation of SA upon copper stress, and provided a theoretical basis for planting H. tuberosus to repair soil copper pollution.

6.Clinical comparative study of two kinds of extra-dissection and loose seton operation in the treatment of complex anal fistula complicated with abscess

Objective To compare the clinical efficacy and safety of second -stage and one -stage extra-dissection and loose seton operation in the treatment of complex anal fistula complicated with abscess . Methods From May 2015 to December 2016 ,100 patients with complex anal fistula complicated with abscess were chosen in the First People's Hospital of Wuyi County .The patients were randomly divided into two groups , with 50 patients in each group .The control group received second -stage extra-dissection and loose seton operation ,and the observation group received one -stage extra-dissection and loose seton operation .The clinical effects ,operation time,wound healing time , hospital stay and the incidence of complications after operation of the two groups were compared.Results The total effective rates of the control group and the observation group were 94.00%,96.00%, respectively,there was no statistically significant difference between the two groups (χ2 =8.78,P >0.05).The operation time,wound healing time and hospital stay of the observation group were shorter than those of the control group,and the differences were statistically significant (t=3.72,2.96,3.40,all P<0.05).The incidence rates of urinary retention,bleeding and wound infection after operation of the control group were 2.00%,4.00%,6.00%, respectively,which of the observation group were 4.00%,2.00%,8.00%,respectively,there were no statistically significant differences between the two groups (χ2 =1.04,0.79,1.13,all P>0.05).Conclusion Second-stage and one-stage extra-dissection and loose seton operation in the treatment of complex anal fistula complicated with abscess has the same clinical effects and safety;but one -stage extra -dissection and loose seton operation can efficiently shorten the operation time and speed up the postoperative rehabilitation .
7.Application of 3D printing mirror models and segmentation models in preoperative planning for patellar fractures

Objective:To explore the application of 3D printing mirror models and segmentation models in preoperative planning for patellar fractures.Methods:Retrospectively analyzed were the data of 46 patellar fractures which had been treated at Department of Orthopedics, Liyang City People's Hospital from January 2016 to August 2020 using 3D printing mirror models and segmentation models in preoperative planning. There were 26 males and 20 females, aged from 19 to 79 years (average, 51.5 years). All the fractures belonged to AO type C. According to the design requirements of a solid model, the patients with DICOM CT data of bilateral lower extremities were assigned into the mirror model group (24 cases) while those with DICOM CT data of only a unilateral lower extremity into the segmentation model group (22 cases). In the mirror model group, CT scans of bilateral knee joints and the proximal tibia exceeding 15 cm were required, while in the segmentation model group, CT scans of only the affected knee joint were required. The original DICOM data were extracted and imported into software Mimics 19.0. In the mirror model group, after the "three-level long-short axis control" method was used to judge the symmetry, the original fracture model and the mirror model were created and printed respectively. During the operation, the fracture line drawn by the mirror model was used to find the fracture fragments with a similar shape which were to be assembled and fixated. The fracture fragments in the segmentation model group were divided into independent entities, converted into STL files and printed separately before the bone fragments were assembled and fixated in sequence. X-ray films were taken after operation. Recorded were the simulated operation time, visual analog scale (VAS) of the knee joint at one month after operation, and B?stman scores at one and 6 months after operation.Results:All patients were followed up for 6 to 24 months (mean, 13.6 months). In preoperative planning, only the mirror model was suitable for patients with comminuted fracture with compression and impaction, both models were suitable for fractures without impaction, compression or impaction, and the segmentation model was suitable for patients with bilateral fractures at the same site, poliomyelitis, one limb missing, and previous fracture. In the mirror model group in which the patients were controlled bilaterally using the "three-level long-short axis control" method, there was no significant difference among the 6 sets of data at 3 levels on both sides ( P>0.05). For the segmentation model group and the mirror model group, the time for preoperative simulated operation averaged 11.2 min and 9.2 min respectively, the VAS score at one month after operation was both lower than 3 points, and the B?stman scores at one and 6 months after operation were both larger than 20 points. Conclusions:The mirror image model of the unaffected patella can be used as the fracture recovery model for the affected side, but the symmetry needs to be verified in advance in case of severe degeneration. Both the 3D printed mirror model and the 3D segmentation model are suitable for preoperative planning for patellar fractures as they are complementary. Patients in both groups can obtain good joint function after treatment.

Methods:
In October 2010, with the method of cluster random sampling survey, 531 military personnel stationed in plateau and high cold region were chosen as subject. The fatigue status were evaluated by the Chinese version multidimensional fatigue inventory (MFI-20) , job stress were evaluated by the Job Stress Survey (JSS) , and trait coping style were evaluated by the Trait Coping Style Questionnaire (TCSQ) .

9.Selection strategies of hypoglycemic drug for special population with type 2 diabetes mellitus

The special population with type 2 diabetes mellitus(T2DM)are different from the ordinary T 2DM patients in terms of drug treatment due to age ,gender,physiological and pathological status . Based on the latest evidence of evidence -based medical evidence ,this article systematically reviews the selection strategies of hypoglycemic drug in special population such as T2DM patients with complications , elderly patients , children patients , pregnant/lactating women , etc. To formulate an individualized hypoglycemic treatment plan for T 2DM special population ,it is necessary to reasonably select hypoglycemic drugs according to their different characteristics and the latest evidence -based medical evidence ,so as to precisely reduce blood sugar and prevent and delay the occurrence of diabetic complications . For example ,for T 2DM patients with atherosclerotic cardiovascular disease,heart failure ,kidney disease and liver disease ,sodium glucose co -transporter 2 inhibitors or glucagon-like peptide 1 receptor agonists should be used additionally on the basis of metformin ;for T 2DM patients with gastroparesis ,insulin is recommended ;for T2DM patients with hyperuricemia or gout ,hypoglycemic drugs with uric acid lowering effects should be preferred ,and insulin should be used with caution ;for T 2DM patients with osteoporosis ,hypoglycemic drugs that do not affect bone metabolism or have bone protection effects should be selected ;for the elderly patients with T 2DM,it is recommended to give priority to the drugs with low risk of hypoglycemia alone or in combination .
