1.The study of childhood trauma experience and family environment with obsession

Objective This study aimed to explore the relationship between the traumatic experience in childhood and onset of obsession in adulthood,and the character of family environment of obsession patients.Methods Using Y-BOCS and CAT and FES-CV to evaluate 41 cases with OCD and 36 normal subjects respectively.Baaed on these scales,an edited interview schema for a semi-structured interview was choosed with both groups for 50 minutes respectively.Results ①Tbe whole score and scores of two subscales on punishment,negative-environment in CAT were all obviously higher in patients with obsession [(50.16± 21.03),(12.03±4.24),(19.77±9.74),for punishment:P<0.05,for others:P<0.01],and there were a significant difference in the two groups[(35.35±14.78),(9.92±2.51),(12.54±8.14)].The negative environment was chosen in regress equation,and could explain 14.67% of outcome of obsession. ②The scores of 4 ubacaies like cohesion,expressiveness,intellectual-cultural orientation,active-recreational orientation [(5.13±2.28),(4.10±2.12),(2.71±1.87),(2.52±2.34)] ,were all obviously lower than the normal control in FES-CV.While the subscore of conflict (5.03±2.21) was higher than the normal control(3.50±2.40),and all these differences were significant.When two factors of active-recreational orientation and cohesion in FES-CV were put into regression equation( R2 =0.2078 ,R2 =0.2784),they contribute 27.84% and the disease,and the active-recreational orientation factor can explain 20.78%.③The result of interview found significant difference between obsession groupand control group in parental personality,own personality,childhood experience,and family environment.Condusion The traumatic experience in childhood and unhealthy family environment do have association with onset and development of obsession,also they have a close relationship with the development of personality after adulthood.

Objective To study the expression of smad2/3 protein and the effects of flunarizine on the its expression in braln tissue following transient cerebral ischemie reperfusion in gerbils. Methods A cerebral ischemia-reperfusion model in gerbils was established by clamping both common carotids. Thirty-five gerbils were randomly divided into three groups, sham operation group, cerebral ischemia-reperfusion group and flunarizine treatment group. The expression of smad2/3 protein in brain tissue was detected by immunohistochemistry technique. Results Experimental results revealed that smad2/3 protein was expressed in neuroeyte in 35 gerbil brain. Compared with sham operation group, the expression of smad2/3 protein in neurecytes of cerebral isehemia-reperfusion group was evidently increased at the lst day, 3rd day and 7th day (P <0. 01). Compared with cerebral ischemia-reperfusion group, the expression of smad2/3 protein in neurecytes of gerbils in flunarizine treatment group was evidently decreased at these time point (P < 0.05). Condusions Smad2/3 protein was expressed in nettrcvytes of gerbils. Expression of smad2/3 protein in neuroeytes of gerbils was evidently increased following cerebral ischemic reperfusion, and its expression in flunarizine treatment group was evidently decreased.
4.Effect of Tripterygium wilfordii glycosides combined with gestrinone on endometriosis and serum cytokine expression.

To study the effect of Tripterygium wilfordii glycosides and gestrinone on endometriosis and serum cytokine expression, 135 cases of endometriosis patients were divided into treatment group(n=69) and control group(n=68). The observation group was orally given with T. wilfordii glycosides, 20 mg, tid, for 4 weeks. Then, the dose decreased to 10 mg/time, tid. T. wilfordii glycosides combined with gestrinone capsule(2.5 mg) were given in the 1st and 4th day of a menstrual cycle. Later, the administration was fixed at two times every week. The course of treatment lasted for 3 months. The control group was treated with gestrinone capsule(according to the same intake method). The serum-related cytokine levels before and after treatment were determined, and the clinical efficacy was observed. The results showed the total effective rate of the observation group was 89.71%, which was obviously higher than that of the control group(74.63%), with statistically significant differences(P<0.05). After treatment, TDS showed varying degrees of decreases, with a better effect in the observation group (P<0.01). Before treatment, serum TGF-β, IL-10 and IL-4 level had no significant difference. After treatment, all of these cytokines decreased, particularly for the observation decreased(P<0.01). Before and after treatment, serum IL-17 had no obvious difference between the two groups. This study suggested that the integrated traditional Chinese medicine and western medicine has an obvious clinical efficiency in endometriosis. Its mechanism may be related to the effective regulation of cytokines.
5.Evaluation on the use of detection of hepatitis C core antigen for screening blood donor.

OBJECTIVETo investigate the value of detection of hepatitis C virus core antigen (HCV-cAg) for screening blood donor by using the internal reagent enzyme immunoassay (EIA) and anti-HCV antibody.
METHODSThe first and repeat assays were performed for detection of serum anti-HCV and HCV-cAg ELISA in 3972 donor's serum specimens from August to October of 2004. Twenty-five donors positive for anti-HCV were tested with HCV-cAg EIA kits and the results were compared with the results of HCV RNA determination with RT-PCR method.
RESULTSIn 3972 donor's serum samples, only 1 serum specimen was positive for HCV RNA identification among 10 specimens which were positive for anti-HCV in first assays, and only 1 serum specimens was positive for HCV RNA identification among 12 specimens positive for anti-HCV in repeat assays, only 2 serum specimens were positive HCV RNA identification in 3 specimens which were positive for HCV-cAg assays.
CONCLUSIONThe sensitivity of HCV-cAg ELISA is similar to HCV RT-PCR, but it is much cheaper. Therefore, HCV-cAg ELISA and anti-HCV may be used together to screen blood donor.

8.Application of femoral I.D.E.A.L localization in anterior cruciate ligament reconstruction with single bundle anterior cruciate ligament.

OBJECTIVE:
To evaluate the effect of femoral I.D.E.A.L localization in single bundle anterior cruciate ligament reconstruction (ACLR).
METHODS:
From January 2019 to October 2022, 122 patients with anterior cruciate ligament injury were treated with ACLR, including 83 males and 39 females. The age ranged from 23 to 43 years old, with an average of (32.19 ±8.55) years old. The course of disease ranged from 1 week to 6 months. According to the different surgical schemes, the patients were divided into two groups, namely the traditional group, which adopted the over-the-top femoral lateral positioning scheme, including 64 patients. The I.D.E.A.L group adopted the I.D.E.A.L femoral lateral positioning scheme, including 58 patients. The patient has pain and dysfunction of knee joint before operation. MRI of knee joint indicates anterior cruciate ligament injury. The visual analogue scale(VAS), International Knee Documentation Committee(IKDC) scoring system and Lysholm scoring system were used to evaluate the knee joint function of the patient. KT-2000 was used to detect the recovery of knee joint after operation and to count the postoperative complications.
RESULTS:
The wounds healed well after operation. One hundred and twenty-tow patients were followed up for 15 to 46 months, with an average of (25.45±9.22) months. The knee joint stability of patients after operation was significantly increased. The VAS at 1 day and 1 week after operation of patients in the I.D.E.A.L group was significantly lower than that in the traditional group(P<0.05). The IKDC score and Lysholm score of patients in the I.D.E.A.L group were significantly higher than those in the traditional group(P<0.05). In the traditional group, there were 6 cases of short-term (<1 month) complications and 19 cases of long-term (≥1 month)complicatios. In the I.D.E.A.L group, there were 3 cases of short-term complications and 7cases of long-term complications(P<0.05).
CONCLUSION
The single bundle anterior cruciate ligament reconstruction and femoral I.D.E.A.L positioning can achieve better early postoperative effect and reduce early postoperative pain.

9.Correlation between metabolic syndrome and adverse events after internal fixation of femoral intertrochanteric fracture in the elderly.

OBJECTIVE:
To explore the effect of metabolic syndrome on 15 days postoperative adverse events of femoral intertrochanteric fractures with internal fixation.
METHODS:
From January 2011 to June 2019, 986 elderly patients with intertrochanteric fracture were treated with internal fixation, including 312 males and 674 females, with an average age of(77.71± 7.58) years old. And there were 97 patients with metabolic syndrome and 889 patients without metabolic syndrome. Through the electronic medical record system, the patient's age, gender, fracture type, cause of trauma, body mass index, smoking history, preoperative comorbidities, preoperative bloodtransfusion, operation timing, ASA classification, anesthesia method, internal fixation type, operation duration, and 15 days postoperative adverse events, which include surgical site infection, acute heart failure, acute respiratory failure, pulmonary infection, acute renal failure, DVT, embolism in important organs, urinary tract infection, death, and blood transfusion after surgery were collected. The differences of preoperative and intraoperative baseline datas and 15 days postoperative adverse events between the two groups were compared and analyzed by t text or univariate
RESULTS:
There were statistical differences in age, body mass index, history of cardiac insufficiency, history of COPD, history of renal insufficiency, operation timing, ASA classification, operation duration, surgical site infection, acute heart failure, DVT, urinary tract infection and blood transfusion between two groups (
CONCLUSION
Elderly patients with intertrochanteric fracture with metabolic syndrome had higher postoperative surgical site infection rate, DVT incidence rate, urinary tract infection rate, and postoperative blood transfusion rate. Therefore, the orthopedic treatment team should give more attentionand optimize the treatment plan during the perioperative period with the cooperation of internal physician and anesthesiologist.

10.Aberrant function connectivity and grey matter volume within default mode network in patients with type 2 diabetes mellitus by functional MRI

Objective To explore early aberrant change of the function and grey matter volume within default-mode network in patients with type 2 diabetes mellitus (T2DM) by Independent component analysis and voxel-based morphometry.Methods Twenty-nine T2DM patients and thirty-three healthy control subjects were recruited in this study between May 2014 and January 2015,and the demographic and clinical data (fasting plasma glucose and HbA1c level) were obtained.General cognition was assessed by MMSE and MoCA in all subjects.Resting-state fMRI and T1-weighted imaging were performed in all subjects.Then the z-maps of default-mode network were obtained from Resting-state fMRI by independent component analysis,and the grey matter volumes were extracted by VBM.Finally,two-sample t-test was performed in the function and grey matter volume of DMN between two groups,and pearson correlation coefficients among zFC values extracted from significantly different brain regions,grey matter volumes and durations of T2DM patients were calculated.Results Patients with T2DM showed decreased Z value around the right anterior cingulated cortex and the right medial pre-frontal cortex(cluster=121,t=-3.81,P< 0.05).There was no significant difference in grey matter volume between the two groups,and no correlation between Z value or duration and grey matter volume was found.Conclusions It suggests the abnormality of connectivity of default-mode network may precede the abnormality of grey matter volume in type 2 diabetes mellitus patients.The DMN might be a new potential tool in monitoring the change of cognition in patients with type 2 diabetes mellitus.
