2.A Normal Pregnancy and Delivery by Assisted Activation Following Roundheaded Spermatozoa (Globozoospermia) Injected into Oocytes.

Globozoospermia is a rare type of teratozoospermia. It occurs in 0.1% of all andrological patient's and used to be considered sterile. Globozoospermic patient has 100% round headed spermatozoa, but the spermiogram is normal. The spermatozoa show oval-shape head, the lack of a nuclear envelope, acrosome, and post acrosomal sheath. OBJECTIVE: To report that a couple with infertility secondary to globozoospermia received ICSI treatment. MATERIAL AND METHOD: Case report. RESULTS: In the first trial, fertilization was failed. In the second trial, 40% of oocytes were fertilized and all of these embryos were cleaved, but pregnancy did not achieved. In the third trial, sperm injected oocytes were exposed to 10 micrometer calcium ionophore for 15 min. All of the injected oocytes were fertilized and proceeded to develop. Triplet pregnancy was achieved after the transfer of six embryos in their third cycle. One embryo vanished and the remaining twins (female) were delivered at 33 weeks of gestation by Caesarean section. CONCLUSION: This result shows that assisted activation following ICSI may overcome infertility associated with globozoospermia.

4.Sleep Apnea Syndrome and the Cardiovascular Diseases.

Sleep apnea syndrome (SAS) has known to be associated with obesity, hypoxemia, sympathetic activation, and hypertension. In addition, SAS is associated with various mechanisms involved in cardiovascular diseases, such as oxidative stress, dyslipidemia, smoking, and psychological stress. In recent prospective observation studies, SAS is known to be a risk factor for cardiovascular diseases, i.e., heart failure, stroke, and coronary artery diseases. The association with heart failure was most evident. In some participants in the longitudinal study applied with continuous positive airway pressure (CPAP) treatment, SAS was not associated with prognosis. With the viewpoints in the current standard of cardiovascular medicine, there seems to be few evidence showing that CPAP treatment could prevent or improve cardiovascular outcome. Detailed review of the limitations of the prospective observational studies suggests that more sophisticated measurement of the clinical outcomes and adjustment of confounding factors will be required in the future studies. And for the clinical application, more evidence of CPAP treatment on the cardiovascular outcomes are mandatory.

6.Benefits of Intensive Blood Pressure Lowering.

In general, each increase of 20/10 mmHg (systolic to diastolic) blood pressure (BP) above 115 mmHg and 75 mmHg results in a two-fold increased risk for cardiovascular complications. In addition, cerebrovascular events in association with hypertension are more frequent than myocardial infarctions in Asian populations. To prove the correlation between cardiovascular events and hypertension observed in the observational studies, BP-lowering studies should be performed to demonstrate the preventive effect on cardiovascular events. However, no randomized clinical studies to date have been able to prove the preventative effects of BP lowering in the blood pressure around 140/90 mmHg or mild hypertension. Despite the fact that many guidelines, including Joint National Committee-7 guidelines, recommend BP lowering for those ranges of blood pressure. In recent years, there have been many contradictory hypotheses about the asymmetry between the results from observational studies and clinical trials, which have led to the establishment of representative studies to set new target BPs. Such studies have proven the benefits of BP lowering closer to optimal blood pressure. Therefore, a discussion about applying these findings to patient care is needed.

8.Angiosarcoma of the Scalp: A Case Report and the Radiotherapy Technique.

Cutaneous angiosarcomas are uncommon malignancies which account about 1% of sarcomas. They are found most commonly in the head and neck regions, frequently on the scalp. Although preferred treatment has been combined surgery and postoperative radiation therapy, the extensiveness and multiplicity of the lesions set limits to such an approach and the patient is often referred for radiotherapy without surgery. As the entire scalp usually needs to be treated, radiation therapy is a challenging problem to radiation oncology staffs. We report a case of angiosarcoma of the scalp, which was treated successfully by radiation therapy with a simple and repeatable method using mixed photon and electron beam technique. Using a bolus to increase the surface dose of the scalp and to minimize dose to the normal tissues of the brain desirable but difficult technically to be well conformed o the three dimensional curved surface such as vertex of the head. A helmet made of thermoplastics filled with paraffin was elaborated and used for the treatment, resulting of the relatively uniform surface doses along the several points measured on the scalp, the difference among the points not exceeding 7% of the prescribed dose by TLD readings.
