1.Inhibitory effects of adrenomedulin on expression of vascular cell adhesion molecule-1 in rats with myocardial ischemia-reperfusion injury

BACKGROUND: Calcitonin gene-related peptide(CGRP) can relieve myocardial ischemia-reperfusion injury. Because adrenomedulin(Adm) and CGRP share certain structural homology, it is assumed that Adm might have protective effect on myocardium.OBJECTIVE: To investigate Adm' s inhibitory effect on vascular cell adhesion molecule-1 (VCAM-1) expression in ischemia-reperfusion rats and its protective effect on myocardial ischemia.DESIGN: A randomized paired design using ischemia-reperfusion model of SD rats.MATERIALS: The experiment was conducted in the Experimental Animal Center of Xianning Medical College from December 2003 to May 2004. Twenty-four healthy male SD rats were selected.METHODS: The hearts of the rats were removed to make into ischemia-reperfusion model and then randomized to control group, Adm1-50(1× 10-9) mol/L group, Adm1-50(1 × 10-8) mol/L group and Adm1-50(1 × 10-7) mol/L group. After the hearts underwent ischemia for 60 minutes, the hearts in control group were reperfused with oxygenation Kerbs-Henseleit bicarbonate(KHB) for 60 minutes while those in the other three groups had reperfusion with oxygenation KHB and Adm1-50( 1 × 10-9),(1 × 10-8), and(1 × 10-7) mol/L, respectively, for 15 minutes and KHB perfusion for another 45 minutes. The liquid flowing from the coronary artery was collected, and the content of creatine kinase isoenzyme was detected. Myocardium in the left ventricle was collected for RNA extraction, and the expression of VCAM-1m RNA was determined with reverse transcription-polymerase chain reaction method.MAIN OUTCOME MEASURES: The expression of myocardial VCAM-1mRNA in control group and groups of different concentrations of Adm1-50.RESULTS: After electrophoresis, each group was found to have an obvious amplified band at 194bp, which was GADPH mRNA amplified segment, and the expression of GADPH mRNA in each group was the same. VCAM-1 mRNA amplified segment with strong brightness and clear border was found at 334bp in control group and Adml-50 (1 × 10-9) mol/L group. The brightness of VCAM-1 mRNA amplified band decreased significantly in Adm1-50(1 × 10-8) mol/L group whereas slight brightness and obscure amplified band was found in Adm1-50(1 × 10-7) mol/L group. The gray value of amplified VCAM-1mRNA and GAPDH mRNA in Adm1-50(1 × 10-8)group and(1 × 10-7) mol/L group was 0.6 ±0.31 and 0.5 ±0.36, respectively, which was obviously lower than that in control group( 1.2 ± 0. 52) ( P< 0. 05).CONCLUSION: Adm1-50 may inhibit the expression of myocardial VCAM-1mRNA in myocardial ischemia-reperfusion in a dose-dependent manner.
2.Clinical observation of the patients with malignant coelom effusion treated with pseudomonas aeruginosa injection combined with ulinastatin by body cavity injection

Objective To evaluate the effect and safety of pseudomonas aeruginosa injection (PA-MSHA) combined with ulinastatin (UTI) injection in the treatment of patients with malignant pleural effusion and/or ascites.Methods 52 patients were randomly divided into PA-MSHA group and PA-MSHA combined with UTI group,each group including 26 patients.All patients were given ultrasonic testing before treatment.The single drug group was given PA-MSHA 10 ml intrapleural and/or intraperitoneal injection.The two-drug combination group was given PA-MSHA 10ml and UTI 300 000 U,twice per week.Evaluation of the efficacy and adverse reaction was performed after 4 times.Results The effective rate of single PA-MSHA group was 34.6 % (CR 1 case,PR 8 cases),while the effective rate of PA-MSHA combined with UTI group was 61.5 % (CR 2 cases,PR 14 cases).The effective rate of PA-MSHA combined with UTI group was statistically higher than that of single PA-MSHA group (P < 0.05).8 cases got fever in single PA-MSHA group,3 cases in PA-MSHA combined with UTI group got fever,side effect had no statistical significance (P > 0.05).Conclusion PA-MSHA combined with UTI has better effect in the treatment of patients with malignant pleural effusion and/or ascites compared with single PA-MSHA,and both treatments have low side effects.
3.Protective effect of ischemic-preconditioning under the mild hypothermia against small intestine ischemia-reperfusion injury in rats

AIM: To investigate the protective effect of ischemic-preconditioning under the mild hypothermia against small intestine ischemia-reperfusion injury in rats and its mechanism. METHODS: Thirty-two rats were randomized into 4 groups (8 rats in each group): sham operated group (Sham), ischemia-reperfusion (I/R) group, ischemic-preconditioning (IP) group, mild hypothermia ischemic-preconditioning (MHIP) group. The wet/dry ratio, Ca 2+ -Mg 2+ -ATPase activity in intestine tissue, the malondialdehyde (MDA) content, activities of lactate dehydrogenase (LDH), superoxide dismutase (SOD) and total antioxdase (TAX) in blood were determined. Objective To research the protective effects of totalPanax Japonicus extract on learning memory, antioxidation, and anti-apoptosis of aging mice,and explore the mechanism. Methods Male mice were randomly divided into normal group, model group, Vitamin E (VE) group, Panax Japonicus extract low and high dose group. Except for the normal group, the other groups were injected withD-gal on the back of the neck subcutaneously to establish aging model. Normal group and model group were given a gavage with saline and each treatment group was given a gavage with totalPanax Japonicus extract and VE once a day for 7 weeks after the aging model established. All mice were be measured their learning and memory ability in the eighth week. After the test, the morphological changes of CA1 neurons were observed by HE stain. SOD, GSH-Px, MDA levels in brain tissue were measured by biochemical method, the expressions of Bcl-2 and Bax mRNA were evaluated by RT-PCR.Results Mice inPanax Japonicus extract low and high dose group could spend less time in searching for the platform, improve the learning and memory ability. TotalPanax Japonicus extract increased the activity of SOD and GSH-Px, while decreased the content of MDA. In addition, it could increase the expression level of Bcl-2 mRNA and reduce the expression level of Bax mRNA as well.Conclusion TotalPanax Japonicus extract has anti-aging effect.
6.Transverse fascia repair with fishing net methods for inguinal hernia under laparoscopic

Objective To explore the feasibility of fishing net repairing transverse fascia method for inguinal hernia (type Ⅰ , Ⅱ ) using laparoscopic surgical procedure. Methods A retrospective analysis of clinical data between the method of fishing net repairing transverse fascia surgery for 145 cases of inguinal hernia (typeⅠ,Ⅱ ) using laparoscopic surgical procedure from May 2004 to May 2008 (laparoscopic group) and the method of open repairing surgery 143 cases (open group) at the same period were conducted. The differences in the operative time, rehabilitation activities time, length of stay, cost of hospitalization and 0comphcations, recurrence rate were compared. Results The laparoscopic group was significantly better in the operative time [ ( 14.8 ± 11.5) min ], found hiding oblique hernia ( 15 cases), rehabilitation activities time[ ( 16.5 ± 14.3) h], use of analgesics(5 cases), scrotal edema(1 case), length of stay[ (4.2 ± 1.5) d], than those of the open group [ ( 37.6 ± 25.4) min, 0, (52.7 ± 12.6) h, 13, 14, ( 8.4 ± 2.6 ) d respectively ] ; but the recurrence rate was no significantly different. Conclusion Method of fishing net repairing transverse fascia for inguinal hernia (type Ⅰ , Ⅱ ) using laparoscopie surgical procedure is feasible.
7.Impact of RelA antisense oligonucleotides on laryngeal carcinoma Hep-2 cell proliferation.

OBJECTIVE:
To study the impacts of RelA antisense oligonucleotides on proliferation in laryngeal carcinoma Hep-2 cell.
METHOD:
RelA antisense oligonucleotides was designed, which was transferred into laryngeal carcinoma Hep-2 cell. MTT was used to detect the growth-inhibiting ratio at different transferred timepoints. Hep-2 cell which was transferred 48 h was used to do colony assay, and expression of RelA was detected by Reverse Transcription PCR and Western blot.
RESULT:
MTT results showed that RelA antisense oligonucleotides could significantly suppress the proliferation of Hep-2 cell, and the suppression-ratio elevated with time. There were statistical difference compared with control groups. The number of cells colony was reduced in RelA antisense oligonucleotides group compared with control groups, which had statistic significance. RT-PCR and Western blot results demonstrated that RelA antisense oligonucleotides could significantly inhibit the expression of messenger RNA and protein in Hep-2 cell.
CONCLUSION
RelA antisense oligonucleotides can inhibit the expression of messenger RNA and protein, and induce the cell proliferation and increase the number of cells colony in Hep-2 cell.

8.Research on the effect of curriculum ideological and political teaching model of Fundamentals of Nursing based on outcome based education concept

Objective:To explore the teaching effectiveness of curriculum ideological and political teaching model for Fundamentals of Nursing based on outcome based education concept.Methods:Using the convenient sampling method, a total of 194 undergraduates of grade 2019 from School of Nursing at University of South China were selected as the control group from February to December 2021, and 146 undergraduates from 2020 were selected as the experimental group from February to December 2022. The control group was taught in accordance with the conventional teaching methods, and the experimental group was taught in the ideological and political teaching model of Fundamentals of Nursing based on outcome based education concept. After the intervention, the total course scores of nursing undergraduates in the two groups were compared, and the cognitive attitude, implementation effect and teaching satisfaction of the experimental group students on "ideological and political curriculum" were compared.Results:After the intervention, the total course score of nursing undergraduates in the experimental group was (85.65±7.31), and that in the control group was (77.71±7.28), and the difference between the two groups was statistically significant ( P<0.01). The cognitive attitude score of undergraduate nursing students in the experimental group towards "ideological and political curriculum" was (32.62±2.15), and the implementation effect score of "ideological and political curriculum" was (22.78±1.78). The satisfactory form of ideological and political cases for undergraduate nursing students in the experimental group was news reports and hot events, and the satisfactory ideological and political teaching method was experiential teaching method and case-based teaching method. Conclusions:The curriculum ideological and political teaching model of Fundamentals of Nursing based on outcome based education concept can improve the academic performance of nursing undergraduate students and provide reference for the construction of ideological and political teaching courses in nursing basic courses. In the future, this teaching model can be applied to the teaching reform of other nursing courses in order to accelerate the progress of ideological and political construction of professional courses.
9.Value of dual-input perfusion of 320 row CT on the efficacy evaluation of small cell lung cancer

Objective To discuss the value of dual-input perfusion of 320 row CT on the efficacy evaluation of small cell lung cancer (SCLC).Methods A total of 18 patients with SCLC confirmed by pathology who received cisplatin plus etoposide chemotherapy between June 2016 and June 2018 in the 8th Medical Center of Chinese PLA General Hospital were collected.All patients received 320 row CT perfusion scan at 3 time points before chemotherapy,after 2 cycles and 4 cycles of chemotherapy.Tumor size,perfusion pseudo color map and bronchial arterial blood flow (BF),pulmonary flow (PF) and perfusion index (PI) were obtained.The efficacy and adverse reactions were evaluated.The single factor analysis was used to make the group comparison.Pearson test was used to make correlation analysis.Results Two patients after 2 cycles of chemotherapy had complete remission (CR),another 2 patients after 4 cycles of chemotherapy had CR,and 3 patients of the above 4 cases with CR had abundant BF;after 4 cycles of chemotherapy,7 cases had partial remission (PR),6 cases had stable disease (SD),1 patient had progression of disease (PD).Dual-input perfusion of 320 row CT showed that 10 cases had the tumor area < 15 cm2 and 8 cased had the tumor area >15 cm2 before the treatment.There was a negative correlation between PI and the tumor area (r =-0.694,P =0.026) on patients with the tumor area < 15 cm2 before the treatment,and no correlation was found in patients with tumor area >15 cm2 (P > 0.05).One case had Ⅳ degree of bone marrow suppression,and obvious adverse reactions were not seen in the rest of the patients.Conclusion Dual-input perfusion of 320 row CT based on the simple imaging can make an accurate quantitative judgement of the effect of SCLC according to perfusion parameter,which provides a new basis for curative effect evaluation on SCLC.
10.Retrospective study on the types and characteristics of shock in polytrauma patients at different stages after trauma

Objective:To investigate the types, incidences, and clinical characteristics of shock in polytrauma patients at different stages after polytrauma.Methods:A retrospective study was conducted on polytrauma patients admitted to multiple trauma centers from June 2020 to December 2021. The inclusion criteria were patients >18 years old and treated due to polytrauma. Exclusion criteria included an admission time of more than 48 h after trauma, a history of malignancy, or metabolic, consumptive, and immunological diseases. The early stage was defined as the period of ≤48 h after polytrauma, and the middle stage was defined as the period between 48 h and 14 days. The patient’s medical history, clinical manifestations, laboratory tests, imaging examination, injury severity score (ISS), and Glasgow coma scale (GCS) were collected. The types, incidences, and clinical characteristics of shock in different stages after polytrauma were analyzed, according to the diagnostic criteria of each type of shock. The differences between the groups were compared by Student’s t test, χ2 test or Mann-Whitney U test. Results:The incidence of the early and middle stage shock after polytrauma were 73.1% and 36.4%, respectively, with statistically significant difference between stages ( P<0.01). There were significant differences in the incidence of hypovolemic shock (83.6% vs. 28.4%), distributed shock (13.7% vs. 80.9%) and cardiogenic shock (3.5% vs. 6.6%) between stages (all P<0.05). The incidence of obstructive shock (8.4% vs. 9.7%, P>0.05) was similar between stages. The incidence of undifferentiated shock was 1.6% and 1.2%, respectively. There were 9.5% patients with multifactorial shock in the early stage and 14.4% in the middle stage. Totally 7 combinations of multifactorial shock were found in different stages after polytrauma. In the early stage, the combination of HS and DS accounted the highest ratio (42.3%) and followed by HS and OS for 28.8%. In the middle stage, the combination of HS and DS was the most common (48.6%) and followed by DS and OS (24.3%). Conclusions:The incidence of shock in polytrauma patients is high. Different types of shock can occur simultaneously or sequentially. Therefore a comprehensive resuscitation strategy is significant to improve the success rate of treatment.
