1.Experimental methodology of simultaneous determination of carbamazepine, phenytoin, and phenobarbital in serum by high-performance liquid chromatography

Objective To establish a high-performance liquid chromatography (HPLC) method with diode array detection to simultaneously determine carbamazepine,phenytoin,and phenobarbital in serum.Methods Extraction solvent (800μl ethylene acetate) and sample (0.2 ml) was mixed,extracted for 2 min,and centrifuged (3500 r/min,4 minutes).A volume (600 μl) of extract liquor was volatilized to dryness in water bath with the volatilization temperature 75 ℃,then was redissolved with 1.0 ml mobile phase.Analysis conditions was column temperature 30°,mobile phase (methanol∶ water =40∶60),and detection wavelength of 254 nm.Three metabolites were effectively separated.Results Under the optimized condition,calibration curves of three metabolites were linear in the ranges of (1.52 ~ 120 mg/L) and the correlation coefficients were not less than 0.999.The detection limits (S/N =3) were in the range of 0.4 ~ 1.5 mg/L.The spiked recoveries were in the range of 91.3% ~ 111% with relative standard deviations (RSD) less than 5%.Conclusions The optimal pretreatment condition for the sample was established.The chromatographic separation and the detection condition were optimized.The method was sensitive and accurate,and could meet the need of monitoring serum drug concentration.
2.Effects of Perspective-taking on HIV-Related Stigma Intervention among Medical College Students

Objective:To explore the effect of perspective-taking on HIV-related stigma intervention among medical college students.Methods:192 medical college students selected from one medical college were randomly divided into experimental group and control group.Perspective-taking intervention was implemented in the experimental group.HIV-related Stigma Questionnaire was used to measure the level of HIV-related stigma before and after the intervention for two groups.Results:Before intervention,there was no significant difference in the total score of HIV-related stigma between the two groups (P > 0.05);there was significant difference in the total score of HIV-related stigma between the two groups after the intervention (P < 0.05).Empathy had a partial mediating effect on the relationship between perspective-taking and HIV-related stigma,and the ratio of mediation effect to total effect was 0.260.Conclusion:The perspective-taking effectively can reduce the level of HIV-related stigma through the mediation effect of empathy.
3.A novel missense mutation in MIP gene resulted in polymorphic cataract.

OBJECTIVETo map the disease locus for a congenital cataract family, and detect the disease-causing gene.
METHODSAn autosomal dominant congenital cataract family was genotyped by genome wide scan using 382 autosomal microsatellite markers from ABI-MD10. Two-point linkage analysis was carried out by the MLINK program.
RESULTSThe disease locus of this family was mapped at 12p11.2-q15. Sequence analysis of a candidate gene-major intrinsic protein (MIP) revealed a novel missense mutation G-->A at the nucleotide 702 in exon 4, which resulted in a substitution of arginine to lysine at codon 233 (p.R233K).
CONCLUSIONThe mutation G-->A at nt702 in MIP gene was associated with the binocular polymorphic congenital cataract in the family. This transition occurring at the C-terminus of MIP might decrease the stability of the C-end of the protein and impact the function of the protein.

4.Factors related to neonatal skin infection in community

Total 1 452 newborns were home visited within 28 d after birth during January 2016 to December 2016,and 27 cases of skin infections were found.Umbilical inflammation was more common (11 cases),followed by folliculitis (9 cases) and impetigo (7 cases).Samples were collected for bacterial culture and identification from the skin of infected newborns,the hands and nasal cavity of family baby caretakers,and household articles,including faucets,mobile phones,toilet buttons and so on.The pathogens identified were Staphylococcus aureus in 20 cases (including 2 methicillin-resistant strains) and Escherichia coli in 7 cases.The isolation of Staphylococcus aureus in household articles was significantly correlated with newborns infected with Staphylococcus aureus (x2=678.808,P<0.01);and the isolation of Staphylococcus aureus from family baby caretakers was also significantly associated with neonatal Staphylococcus aureus infection (x2=820.396,P<0.01).In 7 cases of Escherichia coli infection,the Escherichia coli were detected from samples of baby caretakers or household articles.The study suggests that the awareness of baby caretakers and home hygiene are important to reduce neonatal skin infections.
5.Clinical research of eight areas of trigeminal nerves with electrical stimulation

Objective To study the distribution rule of sense and motor fibers in the normal trigeminal nerve roots by electrophysiologic monitoring.Methods Twenty patients accepted microvascular decompression because of primary mimetic convulsion, collected in our hospital from January 2011 and December 2013, was studied their normal trigeminal nerve roots through electroneurophysiology;during the operation, the trigeminal nerve roots were divided into 8 areas: Ⅰ area is the external lateral area, Ⅱ area is the external upper area, Ⅲ area is the middle upper area, Ⅳ area is the internal upper area, Ⅴ area is the internal lateral area, Ⅵ area is the internal bottom area, Ⅶ area is the middle bottom area, and Ⅷ area is the external bottom area;and then, one mm diameter bipolar stimulator was adopted to give 0.2 mA electrical stimulation on these areas, respectively;and the compound nerve action potential (CNAP) at the foramina supraorbitale (V1), foramina infraorbitale (V2) and foramina mentale (V3) was recorded as the CNAPV1, CNAPV2 and CNAPV3 respectively;the compound muscle action potential (CMAP) at the masseter muscle (MM) and temporal muscle (TM) was recorded as CMAPMM and CMAPTM respectively;and finally, the distribution rules of the sense and motor fibers inside the trigeminal nerve roots were judged according to the laws of CNAP and CMAP emergence.Results During the operation, all patients could get stable CNAP and CMAP wave patterns at periphery according to the electrical stimulation on trigeminal nerve roots;the CNAPV1, CNAPv2 and CNAPV3, CMAPMM and CMAPTM seldom appeared in the Ⅰ, Ⅱ and Ⅷ areas of trigeminal nerve roots, but more often appeared in the Ⅳ, Ⅴ and Ⅵ areas, enjoying significant differences (P<0.05);motor fiber located in the internal lateral area of the nerve, and the sense fiber located in the middle and external lateral area.By stimulating the sense taproot fiber, CNAP could be recorded, and by stimulation the motor taproot fiber, CMAP and several CNAP from V1, V2 and V3 could be recorded.Conclusion Electrical stimulation on motor taproot implies that there are anastomosing fibers existting in both motor and sense fibers;according the 8 areas being divided, motor fiber locating in the internal lateral area of the nerve, and sense fibers locating in the middle and external lateral areas can be concluded.
6.Eeffect of ultra-low dose CT scanning on the diagnosis of ankle fracture and the quality of a three-dimensional printing model

Objective:To investigate the effect of ultra-low dose (ULD) computed tomography (CT) scanning on the diagnosis of ankle fractures and the quality of a three-dimensional printing (3DP) model.Methods:This study was a prospective study. A total of 61 patients with clinical ankle fractures treated conservatively in Zhuhai Hospital of Guangdong Hospital of Traditional Chinese Medicine from November 2019 to January 2022 were included in this study. Patients underwent standard dose (SD) CT scan and ultra-low dose (ULD) CT scan, respectively. The tube voltage/tube current of SD and ULD were 120 kV/100 mAs and 80 kV/10 mAs, respectively. Two senior radiologists evaluated the presence of ankle fractures. The effective radiation dose ( E), noise, signal-to-noise ratio (SNR), contrast signal-to-noise ratio (CNR), and CT value of bone cortex minus CT value of peripheral fat (CTc) were compared. The radiologists also evaluated the discoverability, diagnosability, and overall image quality of the fracture line according to Likert′s 5-point scoring method. Two senior orthopedists subjectively evaluated the quality of each 3DP model (model clarity and operation guidance). A score ≥ 3 indicated that the quality of the CT diagnostic image and 3DP model were acceptable. Results:The interval between the two CT scans was (9.23 ± 1.92) d. A total of 94 fracture sites were found. There were no missed diagnosis or misdiagnosis based on the SD and ULD scans.Noise, SNR, and CNR were better on the SD CT scanning ( F=5.92, 9.70, 8.32, P=0.00), however, CTc was higher on the ULD scans ( F=27.55, P<0.01). The image scores of the SD and ULD scans were (4.97 ± 0.18) and (4.21 ± 0.71), and the quality scores of the 3DP model (4.99 ± 0.01) and (4.87 ± 0.34), respectively. The SD scans were better than the ULD scans with respect to CT image quality and 3DP model quality ( Z=-6.88, -2.91, P<0.01), but both were considered suitable to meet clinical needs (all ≥ 3 points). The E associated with SD and ULD scannings were (34.68 ± 4.96) μSV and (1.04 ± 0.10) μSV, respectively. The latter was thus significantly better than the former ( F=38.77, P =0.00). Conclusions:The E value of ULD scanning is about 3.00% of SD scanning E, which can meet the needs of clinical diagnosis of ankle fracture and 3DP model printing diagnosis.
7.The feasibility and short-term curative effect of the genicular arterial embolization treatment for moderate to severe knee pain secondary to osteoarthritis

Objective:To investigate the safety and efficacy of transcatheter genicular artery embolization (GAE) for moderate to severe knee osteoarthritis (KOA).Methods:This prospective study included 13 patients (17 knees) with KOA who were treated with GAE from October 2020 to March 2021. The Kellgren-Lawrence (K-L) grade was 2-3 for 11 knees, and 4 for 6 knees. The Western Ontario and McMaster Universities osteoarthritis index (WOMAC) and the Whole-Organ Magnetic Resonance Imaging Score (WORMS) assessments were performed for all the subjects before operation. The success rate, clinical efficacy and complications were recorded after operation. Clinical outcomes were evaluated at 1 day, 1week and 1, 3, 6 months after the operation.Results:The success rate of GAE in 17 cases was 100%, and the success rate of target artery superselection was 98.4%(63/64). The baseline WOMAC pain score was 11(10, 13) and total score was 44(38, 58) for 17 knees. Post-operation follow-up WOMAC pain score were 4(3, 7), 2(1, 5), 2(1, 6) and 4(2, 6) at 1 day, 1 week, 1 month, and 3 months. Post-operation follow-up WOMAC total score were 22 (15, 34),20 (12, 24),17 (12, 26) and 20 (12, 31) at 1 day, 1 week, 1 month, and 3 months. There were 16 knees with 6 month follow-up assessment, with the WOMAC pain score of 2.5(2, 5), and the total score of 15(12, 26). Significant difference was found in the WOMAC pain score between baseline and the 1 day, 1 week, 1, 3 and 6 months follow up ( Z=-3.631, -3.623, -3.622, -3.622, -3.532, all P<0.001); also, the total score was statistically significant different between the baseline and the 1 day, 1 week, 1, 3 and 6 months follow up ( Z=-3.639, -3.634, -3.646, -3.527, -3.532, all P<0.001). At 3 months follow-up, there was 1 knee recognized clinical failure. Post-operative adverse reaction in this group included skin ecchymosis in femoral artery puncture area ( n=3), knee joint stiffness and pain within 1 week ( n=4),and clanging joints during postoperative activities ( n=6). Conclusion:GAE is a feasible and safe procedure with obvious short-term curative effect, which can alleviate pain symptoms and improve restricted movement in patients with KOA.
8.Double targets percutaneous radiofrequency thermocoagulation in treatment of primary trigeminal neuralgia

Objective To discuss the curative effect of double targets percutaneous radiotfrequency thermocoagulation (PRT) on primary trigeminal neuralgia (PTN) and its safety.Methods Sixty patients with PTN were randomly divided into single target treatment group and double targets treatment group.CT guided stereotactic location was used to navigate PRT;patients in the single target treatment group only accepted one target damage and those in the double targets treatment group accepted target damage with 3 mm of deep and shallow interval.The curative effects and complications were compared one week,3 months,one and 2 years after the surgery.Results The percentage of patients with good curative effect in double targets treatment group was significantly higher than that in single target treatment group one and 2 years after surgery,with statistically significant difference (86.7% vs.60%,70% vs.45%,P<0.05).Recurrence rates of double targets treatment group 3 months,one and 2 years after surgery were 0%,3.3% and 6.7%,which were lower than those in single target treatment group (3.3%,10% and 16.7%),without significant difference (P>0.05).Ipsilateralfacial hypaesthesia of different degrees appeared in both groups,but complications as keratohelcosis,diplopia,chewing weakness or difficulty in opening mouth did not appear.And the degree of facial numbness in double targets treatment group was a little higher than that in single target treatment group without statistical significance (P>0.05).Conclusion Double targets PRT used in PTN treatment can obviously improve the postoperative pain relief and do not apparently increase the degree of postoperative facial numbness as compared with single target PRT.
9.Simultaneous Determination of 7 Effective Components in Yao Medicine Yueli Yaomi Spray by QAMS

OBJECTIVE:To establish QAMS method for simultaneou s determination of 7 effective components in Yao medicine Yueli yaomi spray ,such as α-cyperone,α-pinene,β-pinene,limonene,β-elemene,caryophyllene oxide and ligustilide , so as to provide method reference for the quality control of the preparation. METHODS :GC method was adopted. The determination was performed on DB- 1701P capillary column ,using nitrogen as carrier gas. The temperature of the hydrogen flame ion detector was 240 ℃. The temperature was programmed ,the inlet temperature was 240 ℃,the injection volume was 1 μL and the split ratio was 20 ∶ 1. Using limonene as internal reference ,the relative correction factors of other 6 components were calculated,the contents of them were calculated with relative correction factors ,and then compared with the results of internal standard method (using naphthalene as internal standard ). RESULTS :The mass concentration linear range of α-cyperone, α-pinene,β-pinene,limonene,β-elemene,caryophyllene oxide and ligustilide were 0.008 9-1.110 0,0.028 3-3.540 0,0.020 5- 2.560 0,0.023 0-2.880 0,0.016 3-2.035 0,0.013 1-1.640 0,0.008 3-1.040 0 mg/mL(all r>0.999 0);the limits of quantification were 0.005 6,0.013 1,0.011 4,0.018 6,0.010 8,0.008 9,0.004 5 mg/mL;the detection limits were 0.001 9,0.004 1,0.003 7, 0.006 2,0.003 5,0.002 9,0.001 5 mg/mL;RSDs for precision ,stability(24 h),and repeatability tests were all less than 2% (n=5 or n=6); the average recoveries were 98.48% , 014) 101.37%,97.96%,99.80%,102.79%,97.77%,102.14%, and RSDs were all lower than 2%(n=9),respectively. The average relative correction factors of α-cyperone,α-pinene, β-pinene,β-elemene,caryophyllene oxide and ligustilide were 1.045 8,0.621 0,0.488 5,0.382 9,0.708 9,0.956 9 respectively,and the RSDs were all lower than 2%(n=6). There wa s no statistical significance in contents of 7 components between QAMS method and internal standard method (P>0.05). CONCLUSIONS :The established QAMS method is simple , accurate,stable and reproducible ,and can be used for simultaneous determination for 7 components in Yueli yaomi spray.
