Bacteremia is a common complication in patients with liver cirrhosis,and its incidence rises as the severity of liver disease increa-ses.Liver cirrhosis complicated by bacteremia will lead to poor prognosis.The research progress in liver cirrhosis complicated by bacteremia is reviewed from the following aspects:risk factors,diagnostic points,treatment methods,prevention,and prognosis.It is believed that ear-ly prevention and treatment of bacteremia have great significance for the prognosis of patients with liver cirrhosis.
2.The role of hippocampal γ oscillation abnormality in sepsis-associated encephalopathy

Objective:To explore the role of hippocampal γ oscillation abnormality in sepsis-associated encephalopathy (SAE).Methods:Seventy male Sprague-Dawley rats (2-3 months) were randomly (random number) divided into three groups according to the random digital table method: sham, CLP, and CLP + dopamine 4 (D4) receptor agonists RO-10-5824 group. The SAE animal model was established by cecal ligation and puncture (CLP). On day 10-14 after surgery, the open field, novel object recognition, and fear conditioning tests were performed. After that, the hippocampus was collected to measure expressions of parvalbumin (PV) and D4 receptor. In another set of experiment, CA1 local field potential (LFP) were recorded, and the relationship between LFP and time with novel object was analyzed. Independent sample t-test was used for pairwise comparisons, and multiple comparisons were performed by one-way ANOVA, followed by the Tukey multiple comparisons test. Correlation was analyzed using Pearson correlation. Statistical significance was assumed when P<0.05. Results:Compared with the sham group, hippocampal PV (77.54±4.61)%, D4 expression (56.36±3.88)% and γ oscillation power (41.1±8.62)%, object exposure time (36±3) s, new object recognition rate (49±4)%, and scene stiffness time (56±7) s were decreased significantly ( P<0.05). However, RO-10-5824 treatment could increase hippocaml γ oscillation power (92.3±6.7)%, and reverse the decreased new object exposure time (44±3) s and new object recognition rate (63±4)%. Correlation analysis showed that hippocampal γ oscillation power was positively associated with new object exposure time ( r=0.609 2, P=0.015 9). There was no difference in total distance traveled or time spent in the center among groups ( P>0.05). Conclusion:Hippocampal γ oscillation abnormality might play a key role in cognitive impairment associated with SAE.
3.Quality of life questionnaire for women with breast surgery: evaluation of the Chinese version of BREAST-Q augmentation module

Objective BREAST-Questionnaire (BREAST-Q) was a patient-reported outcome (PRO) instrument designed by the Memorial Sloan-Kettering Cancer Center (MSKCC) and University of British Columbia (UBC).Our study aimed to evaluate the Chinese version of BREAST-Q augmentation module in China.Methods BREAST-Q augmentation module consisted of two parts:pre-operative and post-operative.Pre-operative had 25 items.Post-operative had 88 items.BREAST-Q questionnaires were given to ten breast augmentation patients.All patients completed BREAST-Q questionnaires at two weeks pre augmentation,one day pre augmentation,fourteen weeks post augmentation,and sixteen weeks post augmentation.Results At two weeks pre augmentation,scores for satisfaction with breasts,psychosocial well-being,and sexual well-being were 29.9 + 5.8,16.5 ± 10.2,and 28.2± 9.3,respectively.At sixteen weeks post augmentation,scores for these three domains were statistically increased to be 65.3 ± 10.0,67.5±17.8,and 56.5±22.2,respectively.These differences were all above 20.However,there was no significant difference between pre augmentaion score and post augmentation score for physical well-being.Pre augmentation score was 64.9 ± 11.9,and post augmentation score was 56.0±14.5.Test-retest reliabilities in different domains were all above 0.8 and most were above 0.9.The Cronbach's alpha coefficients were higher than 0.6.Conclusions Chinese version of BREAST-Q augmentation module is reliable and responsive in assessing the quality of life in Chinese women with breast surgery.However,larger sample is needed for future study.
4.Feasibility study of 80 kVp low dose contrast media split-bolus 2-phase computed tomography urography

Objective To evaluate the feasibility of 80 kVp tube voltage combined with adaptive statistical iterative reconstruction (ASiR)in low dose contrast media split-bolus 2-phase CTU.Methods Sixty patients with hematuria were recruited in this study.All the patients (80 kVp Group,n=30;120 kVp Group,n=30)underwent split-bolus 2-phase CTU (80 kVp Group:35 mL and 60 mL;120 kVp Group:50 mL and 70 mL).CTU images were reconstructed with 30% ASiR in 80 kVp Group and with filtered back projection (FBP)in 120 kVp Group.Subjective and objective analysis of CTU images were accomplished by two qualified and independent readers.The radiation dose was evaluated by ED and SSDE.The image score,noise,attenuation value and CNR of urinary tract,and radiation dose were compared by Mann-Whitney rank sum test.Results There was no significant difference in the image quality between two groups (Z =-1.791,P =0.073).The noise of 80 kVp Group were higher than that of 120 kVp Group (Z =-6.299,P <0.001 ), while there was no significant difference of attenuation value between the two groups (Z =-1.204 --0.163,P =0.228 -0.871,except bladder),as well as the CNRs (Z =- 1.818 --0.202,P =0.069 -0.840).The ED and SSDE of 80kVp Group were significantly lower than those of 120 kVp Group (Z =-6.655--6.653,P <0.001).Conclusion The protocol of 80 kVp tube voltage combined with iterative reconstruction in low dose contrast media split-bolus 2-phase CTU is feasible for clinical application,with reduction of radiation dose and acceptable diagnostic image quality.
5.Quality Standard Improvement for Funing Granules

Objective: To improve the quality standard of Funing granules. Methods: TLC was used for the qualitative identifica-tion of Angelicae Sinensis Radix, Chuangxiong Rhizoma, Scutellariae radix, Atractylodis macrocephalae rhizoma, Citri reticulatae peri-carpium, Cyathulae radix, Paeoniae radix alba and Glycyrrhizae radix Et rhizoma. The content of paeoniflorin was determined by HPLC. Results: The TLC spots were clear and well-separated without any interference from the negative control. The linear range of paeoniflorin was 19. 23-2525. 18 ng(r=0. 999 9), the average recovery was 101. 88% ,and the RSD was 0. 62% (n=6). Conclu-sion: The method is simple and accurate with good reproducibility, and can be used for the quality control of Funing granules.
6.Exploration of the serum differential biomarkers for osteoarticular tuberculosis based on matrix-assisted laser desorption/ionization time of flight mass spectrometry

Objective Toinvestigatestatistically significant peptide peaks as biomarkersto diagnose osteoarticular tuberculosis, matrix-assisted laser desorption/ ionization time of flight mass spectrometry (MALDI-TOF MS) was applied to identify the characteristic fingerprint among the serum of patients with osteoarticular tuberculosis, rheumatoid arthritis and healthy adults.Methods Clinical Study. Serum samples of untreatedpatients with osteoarticular tuberculosis and rheumatoid arthritis were collected from August 2018 to December 2018, and serum samples of healthy adults from physical examination were collected as control. After analysis with MALDI-TOF MS, the serum peptide fingerprint datawas imported into software, and protein polypeptide peaks with obvious differences were screened to establish diagnostic models.Results Established the diagnostic model of osteoarticular tuberculosis and healthy adults with m/z 2943.9, 5929.6, 7615.4 and 9033.8 as differential protein polypeptides, the diagnostic model of osteoarticular tuberculosis and rheumatoid arthritis with m/z 4195.6, 5847.6, 5929.6 and 7748.6 as differential protein polypeptides. To these two models, the sensitivity were 95.00% and 97.50%, respectively. The specificity were 85.71% and 88.46%, respectively. The accuracy rates were 89.58% and 92.39%, respectively. The AUC value of ROC curves were 0.8859 and 0.8709, respectively. Conclusions By mass spectrometry and software analysis, the serum protein polypeptides with statistical difference were found successfully. The related diagnostic modelsarealso established, which has certain reference value for auxiliary diagnosis of osteoarticular tuberculosis.
7.Metabolomic analysis of human cerebrospinal fluid in patients with tuberculous meningitis using ultra high performance liquid chromatography tandem mass spectrometry

Objective:To study the non-target metabolomics analysis and to analyze the metabolomic changesof cerebrospinal fluid (CSF) in patients with tuberculous meningitis.Methods:Case-control study. From July 2018 to July 2019, 20 cerebrospinal fluid specimens of diagnosed patients with tuberculous meningitis were collectedin the department of neurology from the first medical center of the PLA general hospital and the eighth medical center of the PLA general hospital and 20 CSF without tuberculous meningitis as the control. Among them, there were 12 males and 8 femalesin the tuberculous meningitis group, aged (37.9±16.1) years; there were 13 males and 7 femalesin the control group, aged (34.7±14.8) years. Using ultra-high performance liquid chromatography-mass spectrometry (UPLC-MS) technology with three different mode, namely reverse phase chromatography positive ion mode, reverse phase chromatography negative ion mode and hydrophilic chromatography positive ion mode,to detectthe metabolic fingerprints of patients′CSF and analyzed by SIMCA software for orthogonal partial least squares discriminant analysis (OPLS-DA). The variable importance projection value of OPLS-DA model (threshold value>1) plus the P value of t-test (P<0.05) was applied to find the differential metabolites in the cerebrospinal fluid of the two groups of patients.Results:Ten differential metabolites were found in CSF, including L-isoleucine, L-phenylalanine, L-kynurenine, L-methionine, L-tyrosine acid, dimethylglycine, L-alanine, L-threonine, L-histidine and L-lysine, and all of them were up-regulated in the tuberculous meningitis group.Conclusion:Changesof the amino acid metabolism found in the cerebrospinal fluid of tuberculous meningitis patients can provide basis for differential diagnosis and basic molecular research of tuberculous meningitis.
8.Effect of Shenling Guchang Prescription on Gestational Diabetes Rats Based on Intestinal Microecology

ObjectiveTo study the mechanism of Shenling Guchang prescription on blood glucose of gestational diabetes mellitus rats by regulating intestinal flora and short chain fatty acids. MethodThe 30 pregnant rats were randomly selected from 36 pregnant rats which were successfully pregnant. The model rats were fed with high-fat and high-sugar diet for 1 week, and 35 mg·kg-1 streptozotocin ( STZ ) was given for 3 consecutive days to construct a gestational diabetes model. After successful modeling, the rats were randomly divided into model group, metformin group, Shenling Guchang prescription low-, medium- and high-dose group. The high dose group of Shenling Guchang prescription was given 18 mg·kg-1, the middle dose group was given 9 mg·kg-1, the low dose group was given 4.5 mg·kg-1 drug solution by gavage, the metformin group was given 52.5 mg·kg-1 drug solution by gavage, the blank group and the model group were given equal volume of normal saline by gavage.At 24 h after the last administration, blood samples were collected from the tail tip of the rats to measure the blood glucose, and blood samples were collected from the abdominal aorta under anesthesia to measure the levels of triglyceride (TG), total cholesterol (TC), high density lipoprotein cholesterol (HDL-C) and low density lipoprotein cholesterol (LDL-C). Lipopolysaccharides (LPS), interleukin-1β (IL-1β), interleukin-6 (IL-6), tumor necrosis factor-α (TNF-α) and insulin (INS) were detected by enzyme-linked immunosorbent assay (ELISA).The intestinal tissue of rats was taken, and the pathological changes of intestinal tissue were observed by hematoxylin-eosin (HE) staining. Fecal samples were collected from rats, 16S rRNA sequencing was used to detect intestinal flora, and short-chain fatty acids were detected by gas chromatography. ResultCompared with the blank group, the incidence of adverse pregnancy outcomes in the model group was significantly increased (P<0.05). Compared with the model group, the incidence of adverse pregnancy outcomes in the Shenling Guchang prescription groups and the metformin group was significantly decreased (P<0.05). Compared with the blank group, the levels of blood glucose, TG, TC, HDL-C, LDL-C, LPS, IL-1β, IL-6 and TNF-α in the model group were significantly increased (P<0.05), and the intestinal tissues had different degrees of inflammatory changes and mucosal damage. Compared with the model group, the levels of blood glucose, TG, TC, HDL-C, LDL-C, LPS, IL-1β, IL-6 and TNF-α in each group of Shenling Guchang prescription and metformin group were down-regulated (P<0.05), and intestinal inflammation and intestinal mucosal damage were improved. Compared with the blank group, the functional structure and diversity of intestinal flora in the model group changed (P<0.05). Compared with the model group, the functional structure and diversity of intestinal flora in the Shenling Guchang prescription groups and the metformin group were reversed, and the trend was close to the blank group (P<0.05).Compared with the blank group, the abundance of pathogenic bacteria such as Escherichia coli, Enterococcus, Klebsiella, and Dustella in the model group was significantly increased, and the abundance of probiotics such as Prevotella, Prevotella, Akmania, Rombustella, and Lachnospiraceae was decreased (P<0.05). Compared with the model group, the abundance of pathogenic bacteria such as Escherichia coli, Enterococcus, Klebsiella, and Dustella was decreased (P<0.05), and the abundance of probiotic bacteria such as Prevotella, Akmanella, Rombustella, and Lachnospira was increased (P<0.05) in the Shenling Guchang prescription groups and the metformin group. Compared with the blank group, the content of short-chain fatty acids in the model group was significantly decreased (P<0.05). Compared with the model group, the content of short-chain fatty acids in each group of Shenling Guchang prescription and metformin group increased (P<0.05). Correlation analysis showed that Proteobacteria was positively correlated with inflammatory factors, blood glucose and blood lipid in gestational diabetes mellitus (P<0.05). ConclusionShenling Guchang prescription has a good regulatory effect on blood glucose, blood lipids and adverse pregnancy outcomes in rats with gestational diabetes mellitus. Its efficacy is comparable to that of metformin sustained-release tablets. Its mechanism may be related to regulating the structure of intestinal flora, increasing the content of short-chain fatty acids, reducing LPS, IL-1β, IL-6, TNF-α, and improving intestinal inflammation.
9.Efficacy of tele-rehabilitation program for preschool children with functional articulation disorders

【Objective】 To observe the effect of tele-rehabilitation program on the articulation resolution of preschool children with functional articulation disorders (FAD), so as to provide reference for the clinical application of tele-rehabilitation in this context. 【Methods】 A total of 66 preschool children diagnosed with FAD in the outpatient department of Child Rehabilitation, Tangshan Maternal and Child Health Hospital from March 2022 to March 2023 were selected into this study, and were divided into tele-rehabilitation group (n=32) and control group (n=34) by random number table method. The control group received daily family rehabilitation guidance, while the tele-rehabilitation group underwent a tele-rehabilitation program lasting for 3 months. All children were assessed using the Chinese phonological ability evaluation lexicon before and 3 months after the treatment. 【Results】 After 3 months of treatment, both the tele-rehabilitation group and the control group showed significant improvements in articulation resolution compared to before treatment (t=12.165、12.986, P<0.05). Notably, the tele-rehabilitation group exhibited significantly greater improvement than the control group (t=2.138, P<0.05). Within the tele-rehabilitation group, children were further divided into three subgroups based on the severity of their dysphonia: mild, moderate, and severe. After 3 months of treatment, the articulation resolution of the mild and moderate groups improved significantly compared to before treatment (Z=2.226, 31.900, P<0.05), whereas no statistically significant improvement was observed in the severe group compared to before treatment (Z=1.857, P>0.05). 【Conclusion】 Tele-rehabilitation program effectively improves articulation resolution in preschool children with FAD, especially for mild to moderate preschool children with FAD.

Background and objective As computed tomography (CT) screening for lung cancer becomes more common in China, so too does detection of pulmonary ground-glass nodules (GGNs). Although anumber of national or international guidelines about pulmonary GGNs have been published,most of these guidelines are produced by respiratory, oncology or radiology physicians, who might not fully understand the progress of modern minimal invasive thoracic surgery, and these current guidelines may overlook or underestimate the value of thoracic surgery in the management of pulmonary GGNs. In addition, the management for pre-invasive adenocarcinoma is still controversial. Based onthe available literature and experience from Shanghai Pulmonary Hospital, we composed this consensus about diagnosis and treatment of pulmonary GGNs. For lesions which are considered as adenocarcinoma in situ, chest thin layer CT scan follow-up is recommended and resection can only be adopt in some specific cases and excision should not exceed single segment resection. For lesions which are considered as minimal invasive adenocarcinoma, limited pulmonary resection or lobectomy is recommended. For lesions which are considered as early stage invasive adenocarcinoma, pulmonary resection is recommend and optimal surgical methods depend on whether ground glass component exist, location, volume and number of the lesions and physical status of patients. Principle of management of multiple pulmonary nodules is that primary lesions should be handled with priority, with secondary lesions taking into account.
