1.Improved resistance index in renal allograft with reversed diastolic flow

Objective To study the accuracy of improved resistance index (RI) in judging the prognosis in renal allograft with reversed diastolic flow. Methods According to the transplant nephrectomy, patients with reversed diastolic flow in renal allograft were classified into two groups:surgical group (n = 5) and nonsurgical group (n = 19). The differences in improved RI between two groups were compared by using Student's t test. Improved RI was defined as a ratio of peak systolic velocity plus peak diastolic velocity divided by peak systolic velocity. Receiver operating characteristic (ROC) curve was constructed for improved RI to evaluate diagnostic accuracy in judging the prognosis in renal allograft with reversed diastolic flow. Results Improved RI in surgical group ( 1.57 ± 0. 26)was higher than in nonsurgical group (1.22 ± 0. 08) (P<<0. 05). Areas under ROC curve for improved RI was 0. 979. An improved RI threshold of 1.31 had 100 % sensitivity, 90 % specificity, 71%positive predictive value, and 100 % negative predictive value for renal allograft with reversed diastolic flow loss as the maximum Youden index was 90 %. Applying this cutoff value to predict the function of renal allograft with reversed diastolic flow recovery, the accuracy was 92 % (maximum) or 83 %(minimum). Conclusion Improved RI can not only serve as a useful noninvasive index to predict renal allograft with reversed diastolic flow loss, but also to predict the function of renal allograft with reversed diastolic flow recovery.
2.Value of EEG,TCD and SEP on evaluating brain function in coma patients

Objective To investigate the value of electroencephalogram(EEG), transcranial Doppler sonography (TCD), somatosensory evoked potentials (SEP) on evaluating brain function in coma patients. Methods The brain function in 72 cases of coma(mild coma 12,moderate coma 17,open eyes coma 4 and severe coma 39 cases) were examined using EEG,SEP ,TCD and scored. Results For the first time of exam, the results of EEG in 25 cases were electrical silence; no blood stream signalthe were found in 28 cases by TCD; the wave of P14 were disappeared in 26 cases by SEP and all of them belonged to severe coma group. For the second time exam, the results of finally EEG in 17 cases were electrical silence. The results of TCD in other 24 cases were surge waves and nail waves spectra eumorphism. Brain function grading: the EEG grade V in 42 cases, grade Ⅳ in 10 cases, grade Ⅲ in 14 cases were affirmed; TCD grade V in 28 cases, grade Ⅳ in 9 cases and SEP grade Ⅲ in 27 cases were affirmed. All the 28 cases of death and the most of 25 cases abandoned treatment for many reasons were belong to severe coma group. Conclusions The more severe of coma, the higher grade of EEG,TCD and SEP, the worse of prognosis. EEG,SEP,TCD testing can provide an objective indicator not only for evaluating brain function of coma patients, but aslo for estimating prognosis.
3.Method for Japanese encephalitis virus NS3 protease activity analysis and high-throughput screening assay for inhibitors.

Japanese encephalitis virus (JEV) is a single-stranded and positive-sense RNA, which has a single ORF (open reading frame), encoding a polyprotein precursor. Non-structural protein 3 (NS3) plays an important role in processing the polyprotein precursor and has become an important drug target of flavivirus. In this study, NS2BH-NS3 gene was amplified by PCR and subcloned to the prokaryotic expression plasmid, resulting pET30a-NS2BH-NS3. The fusion protein was expressed in Escherichia coli BL21 (DE3) in soluble form after induction by Isopropyl beta-D-1-Thiogalactopyranoside (IPTG). The recombinant protein was purified by Ni-NTA affinity column. Then a fluorescence resonance energy transfer (FRET) method was used to determine enzymatic activity and the assay conditions were optimized. After screening 113 compounds, we found two compounds inhibiting the activity of NS2BH-NS3. This study provides a convenient and cost-effective method for screening of JEV NS3 protease inhibitor.

4.One Network Resource Sharing Architecture of Traditional Chinese Medicine Industry

Currently, the basic network resource of the entire traditional Chinese medicine (TCM) industry is fragmented. Issues of data unify are not easily resolved. It seems that there is no new breakthrough in key technologies, especially on the problem of data sharing and resources' integration. In this context, a method of medical information data sharing is advocated to solve the problem. We use the RESTful style to define resources of the pharmaceutical industry, which is based on the UDDI Web services standards. The objective of this paper is to create a method of TCM resource URL defined and exchanged. Finally, this paper provided an easy way for TCM data network sharing.
5.The value of CSEP intraoperative monitoring in evaluating spinal cord function and predicting prognosis

Objective To explore the value of somatosensory cortex evoked potential(CSEP)monitoring in objective evaluation of spinal cord function and integrity as well as prognostication of neural function after surgery.Methods CSEP monitoring was performed before the operation,during surgical procedure and after the operation in 56 spinal cord surgery patients to observe and analyze latency and amplitude of cortical potential.The significant changes were amplitude reduction≥150%or latency prolongation≥10%.Results There were 71.43%(40/56)true negative finding,both latency and amplitude without abnormal changes during the operation and without new neural functional deficit after operation.There was 0.18%(1/56)false negative finding,latency and amplitude without abnormal changes during surgical procedure,but with new neural functional deficit after operation.There were 26.79%(15/56)true positive findings,where latency transient prolongation<10%and amplitude reduction<50%during surgical procedure were observed.After suspending procedure or changing direction during surgical procedure,the waveform restored or improved.Conclusion CSEP monitoring during surgical procedure could reflect the sensitiveness to distracting,ischemia and injury of spinal cord.A timely intervention might improve the CSEP.CSEP could reflect the function and integrity of spinal cord.CSEP possess an important value in preventing spinal cord injury and evaluating the prognosis of neural function.
6.Application of placental mesenchymal stem cells

BACKGROUND:Placental mesenchymal stem cels are becoming a new source of seed cels because of wide range of sources, low immunogenicity and not involving ethical issues.
OBJECTIVE:To elaborate the sources, biological characteristics and latest application of placental mesenchymal stem cels.
METHODS:Literature search was performed in PubMed, ScienceDirect, OvidSP, CNKI databases for relevant literatures published from 2003 to 2015. The key words were “placenta, mesenchymal stem cels, placenta mesenchymal stem cels, cel transplantation, application mechanism” in Chinese and English, respectively. Then, 57 papers were further analyzed and reviewed in line with the theme.
RESULTS AND CONCLUSION:Placental mesenchymal stem cels have been isolated and cultured successfuly, and confirmed to have multi-differentiation potential. A large number of placental mesenchymal stem cels have been used in the experimental animal and clinical researches, and they have a great potential in bone tissue engineering, revascularizaion and nerve repair. However, the specific mechanism underlying the application of placental mesenchymal stem cels is not clear. In order to ensure the safety and effectiveness, there are stil many problems to be further studied before placental mesenchymal stem cels are widely used in clinic.
7.Risk factor analysis of severe acute pancreatitis complicated by pancreatic encephalopathy

OBJECTIVE: To study the risk factors for severe acute pancreatitis (SAP) complicated by pancreatic encephalopathy (PE). METHODS: Clinical data from 255 patients with SAP from January 2005 to December 2006 were reviewed. Thirty-one SAP patients had PE, and 224 SAP patients did not. Clinical characteristics of SAP patients in both PE group and non-PE group were analyzed. RESULTS: Ranson scale and the incidence rates of acute respiratory distress syndrome (ARDS), renal failure, hypoproteinemia, hypocalcemia and hyperglycosemia in PE group were higher than those in non-PE group (P<0.05). There were no significant differences in acute physiology and chronic health evaluation II and CT severity index scales, the activities of amylase and lipase, the incidence rate of liver function failure, the infection rate and the operability between the PE group and the non-PE group (P>0.05). Multivariate logistic regression analysis showed that ARDS and hyperglycosemia were high risk factors. Cure rate in PE group was higher than that in non-PE group. CONCLUSION: Nosogenesis of PE is the result of multiple factors. ARDS and hyperglycosemia may be the high risk factors for PE.
8.Test-retest reliability of the intra abdominal pressure by using pressure biofeedback

Objective To assess the test-retest reliability of pressure biofeedback in measuring intra abdominal pressure in the lumbar region for normal subjects .Methods We choosed 33 subjects (20 to 27 years old) and asked them to do four movements (movement A :prone position ;movement B :supine position with flexion hip and knee joint ;movement C :supine position with single flexion hip and knee joint ;movement D :prone position with flexion shoulder joint ;) by using pressure biofeedback twice totally ,and the last measurement should be done after 24 h .The indicator of test-retest reliability of joint position was intraclass correlation co-efficient (ICC) .Results The test-retest reliability of intra abdominal pressure was modest in the four movements in the lumbar re-gion ,and the values of ICC were from 0 .786 - 0 .848 .The test-retest reliability of intra abdominal pressure was good in Movement A [ICC(95% CI)= 0 .817(0 .630 - 0 .910)] ,Movement B [ICC(95% CI)= 0 .848 (0 .692 - 0 .925)] ,Movement D [ICC(95% CI)= 0 .841(0 .678 - 0 .921)] .And the test-retest reliability of intra abdominal pressure was good in Movement C [ICC (95% CI) =0 .786(0 .566 - 0 .894)] .Conclusion The test-retest reliability of intra abdominal pressure was modest and good in the lumbar re-gion ,and pressure biofeedback could be useful to assess intra abdominal pressure .
9.Analysis on dissolved content of main constituents of Maxing Shigan Decoction and its powder

AIM:To analyze the dissolved content of main constituents of Maxing Shigan Decoction(Herba Ephedrae,Semen Armeniacae amarum,Gypsum Fibrosum,etc.) and powder applied in experiment.The experiment was carried out simulantly in vitro;i.e.drug passed through metabolically man-made stomach liquid and stomach-intestine liquid. METHODS:The experiment of decoction and its powder of Maxing Shigan Decoction were carried out respectively.main ingredients of component drugs in compound Maxing Shigan Decoction were chosen to analyze the dissolved content by the aid of GC and Complexometry. RESULTS:There was a distinctive difference in dissolved content of main ingredients. CONCLUSION:The law of dissolved content change provides a quick and efficient scientific basis for clinical dosage and determination of quality standard.
10.Influence of caveolin-1 on the ET-1-induced VSMC proliferation

AIM: To investigate the effect of caveolin-1 on the endothelin-1 (ET-1)-induced vascular smooth muscle cells (VSMC) proliferation. METHODS: The -thymidine (TdR) incorporation, immunofluorescence assays and western blotting were used in this study. RESULTS: The ETA receptor specific antagonist BQ123 inhibited the increase in TdR incorporation in response to ET-1 on VSMC. Immunofluorescence assays showed that caveolin-1 was mostly distributed in plasmalemma of VSMC. After 24 h treatment of VSMC with ET-1, the expression of caveolin-1 in VSMC was significantly decreased. Western blotting showed that ET-1 inhibited the expression of caveolin-1 in VSMC, BQ123 reversed the effect of ET-1. CONCLUSION: Caveolin-1 was mostly distributed in plasmalemma of VSMC. ET-1 downregulated caveolin-1 expression in VSMC via ETA receptor.
