The immune mechanism of Graves' diseases (GD) and the roles of regulator T cells were investigated. In 32 patients with GD (GD group) and 20 healthy volunteers (control group), flow cytometry was used to detect the proportion of CD4+CD25+ cells, MACS to isolate CD4+ CD25+ cells,RT-PCR to assay the expression of FOXP3, and ELISA to test the level of IL-10, respectively. It was found that there was no significant change in the proportion of CD4+CD25+ T cells between GD group and control group (P>0.05), while secretion of IL-10 and expression of FOXP3 in GD group were lower than control group (P<0.01 and P<0.05, respectively). In conclusion, though the proportion of regulatory T cells of peripheral blood lymphocytes in the patients with GD, the functions of them were significantly weakened, which might be a pathogenic factor in GD.

Objective To explore the localization diagnosis method of lumbar intervertebral foramen stenosis by multi-di-mensional MRI scans of lumbar nerve roots. Methods Twenty-one patients with lumbar intervertebral foramen stenosis were fol-lowed up from June 2006 to June 2011 postoperatively, 10 cases of male, 11 cases of female;36 to 65 years old, average 45.6 years. The medical history is six to thirty six months, an average of 9.4 years;5 cases have low back pain with unilateral leg pain and 16 cases showed unilateral leg pain only. The height of intervertebral space and foramen intervertebrale were measured on the X-rays of lumbar lateral position. Lumbar nerve roots MR imaging at the position of axial, coronal and sagittal scan were performed separately to the patients who were clinically suspected to suffer from lumbar intervertebral foramen stenosis. A definitive diagno-sis of the location of nerve root compression and structural changes surrounding the nerve root can be obtained. Surgical operation was performed to confirm the accuracy of the MRI imaging diagnosis. Results There were 9 cases of lumbar intervertebral fora-men stenosis caused by lumbar disc herniation. The other 12 cases are caused by zygapophyseal joint hyperplasia. All cases of lumbar intervertebral foramen stenosis located at the low back. By comparing MR images of lumbar intervertebral foramen stenosis with surgical procedure, the surgical observation of 21 patients completely coincided with the preoperative localization diagnosis, coincidence rate was 100%(21/21). After surgical treatment, 20 cases achieved a complete remission of leg pain and 1 case was not satisfactory. Conclusion MRI imaging at the position of axial, coronal and sagittal scan for lumbar nerve roots were useful to rigorous localization diagnosis of lumbar intervertebral foramen stenosis, and can provide accurate radiological evidence for sur-gery program.

BACKGROUND:There are several reports about the application of fresh bone marrow aspirate being injected directly to repair partial ligament injury, but the application about fresh bone marrow aspirate directly being planted on scaffolds to build tissue-engineered ligament is rarely mentioned.
OBJECTIVE:To evaluate the feasibility of applying fresh bone marrow aspirate planted directly on scaffolds to construct tissue-engineered ligament
METHODS:We constructed fibroin fiber/smal intestinal submucosa composite scaffold, then planting fresh bone marrow directly to built bone marrow seeding group and planting seed cel s (bone marrow mesenchymal stem cel s) on the scaffold to built cel seeding group. The control group had no treatment. After that, we detected the density of cel adhesion, cel proliferation ability and extracel ular matrix secretion. Then, the composite in the bone marrow seeding group was implanted into the broken anterior cruciate ligament in rabbits, and material biocompatibility in vivo was evaluated after 12 weeks.
RESULTS AND CONCLUSION:After 4 hours of incubation, bone marrow seeding group was significantly higher than the cel seeding group in cel adhesion density and proliferation rate (P<0.05). Bone marrow seeding group and cel seeding group showed higher type I, III col agen secretion compared with the control group (P<0.05), but the col agen secretion of bone marrow seeding group and cel seeding group showed no significant difference. Composite cel scaffold implantation in vivo did not cause fatal immune rejection and severe inflammatory reaction, and no significant ligament regeneration and vascularization occurred. These findings indicate that fresh bone marrow aspirate can be seeded directly on scaffolds to construct tissue-engineered ligament, and the short-term biocompatibility in vivo is good.

Objective To examine the gene expression profile of SRY-type high-mobility-group box-9(SOX9) during entochondrostosis of mice and investigate the effects of transfection of the pDC316-SOX-9 on mice mesenchymal stem cells(MSCs) in vitro. Methods cDNA microarray technique with 34 000 genes was used to analyze the gene expression profiles during entochondrostosis in the limbs of mice embryo from E10 to E14. Pathway analysis of SOX9 was performed with GCOS1.2 software. The recombined expression vector pDC316-SOX-9 was constructed and transfected into mice MSCs by lipofectamine. The phenotype changes of cells were observed with cell energometry, HE stain, immunohistochemical method, RT-PCR and ELISA.Results The gene expression of SOX9 during the critical phase of chondrogenesis in mice embryo limbs at E12 was increased evidently. SOX9 might promot chondrogenesis. As compared with vector and blank group, the chondrocytes of the SOX9 transfected group had the tendency of enhanced differentiation. Conclusion SOX9 may promote chondrogenesis. The transfection of SOX9 gene into mice MSCs can promot MSCs differentiate into chondrocyte, which may provide some experimental data for cartilage histoengineering.

Objective To observe the effect of sitagliptin on carotid intima-media thickness (c-IMT) and serum hs-CRP and IL-6 levels in the patients with newly diagnosed type 2 diabetes mellitus. Methods 65 patients with newly diagnosed type 2 diabetes mellitus who were admitted to the outpatient clinic and were hospitalized in our hospital were selected. They were randomly divided into sitagliptin phosphate tablet group (experimental group) with 35 cases and metformin hydrochloride tablet group (control group) with 30 cases. All patients were given health education about diabetes, diet guidance and exercise. All patients were given drawing of venous blood at a fasting condition ≥10 h before treatment and 6 months after treatment to determine the following indicators: FPG, 2 hPG, hs-CRP, IL-6, TC, TG, HDL-c, LDL-c, HbA1 c, hs-CRP, IL-6, etc, and c-IMT was determined. Results There was no statistically significant difference in blood glucose, blood lipids, inflammation index and c-IMT between the two groups before treatment (P>0.05); after treatment, FPG, 2 hPG and HbA1 c were significantly lower than those before treatment in both groups, and the difference was statistically significant (P<0.01). There were statistically significant differences in FPG and HbA1 c between the two groups (P<0.01). There was no statistically significant difference in 2-hour postprandial blood glucose between groups (P>0.05). The c-IMT, hs-CRP and IL-6 levels in the experimental group after treatment were significantly lower than those before treatment, and the differences were statistically significant (P<0.05). There was no significant change in c-IMT in control group after treatment (P>0.05). hs-CRP and IL-6 had a decreasing trend compared with those before treatment, and the difference was not statistically significant (P>0.05). There were statistically significant differences in c-IMT, hs-CRP and IL-6 between the two groups after treatment (P<0.05). Spearman linear correlation analysis: c-IMT was positively correlated with hs-CRP levels and IL-6 (r=0.6585, P=0.000; r=0.571, P=0.000). There was no significant correlation between fasting blood glucose and glycosylated hemoglobin (P>0.05). Conclusion The DPP-4 inhibitor of sitagliptin has a clear hypoglycemic effect and can reduce c-IMT, hs-CRP and IL-6. The anti-inflammatory effects besides hypoglycemic effect may be involved in the reduction of atherosclerosis in the patients with diabetes mellitus.
