1.Epithelioid angiomyolipoma of the kidney

Objective To investigate the clinicopathologic features of epithelioid angiomyolipoma of renal(REAML). Methods Six cases of REAML were analyzed by histopathological and clinical characteristics.Of the 6 cases,4 cases were solitary tumors and 2 cases were multiple lesions.The diameter of tumors was about 9 cm in average.One case had a family history of nodular sclerosis.Three cases were found adipose tissue in CT scan and diagnosed for RAML,the other 3 cases were diagnosed for renal cancer.All cases were undergone surgical approach,3 cases were undergone resection of tumors only and the other 3 were performed nephrectomy. Results Pathological characters:tumor was mainly composed of epithelioid cells presented with invasive hyperplasia of atypical pleomorphism,hyperchromatic nuclei with frequent mitotic figures,giant neoplasic cells and extensive hemorrhagic necrosis immunohistochemcial staining showing a positive HMB45 and negative of EMA,CK in most neoplasic cells.All cases were followed up for 10 to 44 months,5 cases did not recurrence and metastases;the other had lung metastasis after operation 18 months later,without any treatment the patient died 10 months later. Conclusions Most of REAMLs are benign and often misdiagnosed for renal cancer by CT scan.HMB45 is positive in immunohistochemcial staining available for diagnose.The minority of REMAL is malignant potentially and should be followed up closely.Operation is major method.
2.Diagnosis and management of tumors primary in renal sinus

Objectlve To discuss the characters and management of renal sinus tumors. Methods The clinical data of 3 tumors occurring in tenal sinus were reviewed.The first patient was a 33-year-old man with the chief complaint of lumbago in the right side for 6 months.B-ultrasound showed low echo in the right pelvis and CT scan showed that a mass measured about 3.5 cm in diameter in the right kidney collective system.Intravenous urography showed the upper collective system was tompressed.He was diagnosed for renal sinus tumor.The second case was a 34-year-old woman with the chief complaint of sudden lumbago in the right for 3 d.Color ultrasonography found hyperechoic in the right kidney.CT scan showed a mass measured 8 cm×6 cm × 8 cm in the right renal sinus,its CT value was about-70 HU.Intravenous urography showed the lower collective system was compressed.She was diagnosed for renal angiomyolipoma of right kidney.The third case was a 55+year-old woman with the chief complaint of lumbago for 1 year.B ultrasound showed hyperechoic in the leftrenal hilum.CT scan showed a nlass of 8 cm×5 cm×5 am in the left renal sinus with CT value of 50 HU.Intravenous urography showed the collective system of the left kidney was compressed.Mag-netic resonance imaging showed high signal on T1 WI and T2WI but low signal in fat suppression ima-ging.She was diagnosed for lipoma of the left renal sinus. Results All the 3 cases had undergone surgical approaches.The first case underwent surgical exploration of renal sinus tumor.Fast pathologic diagnosis was benign tumor,only the tumor was resected.Postoperative pathology confirmed the diagnosis of angioleiomyoma.There was no recurrence during follow-up of 3 years.The second case was scheduled for tumor enucleating,but nephrectomy was perform because of serious bleeding and damaged renal pelvis.Pathological report was angiomyolipoma.The third case was scheduled for lipoma enucleating but nephrectomy was performed because of tumor encapsulated renal pedicle vascular.Pathological report was lipoma.During the follow-up for 4 years there was no relapse. ConclusionsTumors occurred in renal sinus are rare, most of which are benign.CT scan, MRI and intravenous urography are the best imaging examination methods for differential diagnosis.Surgical operation is the major approach, while for the tumor radical nephrectomy less than 4 cm watchful waiting could be a choice.Tumor resection with nephron sparing is feasible while tumor is larger than 4 cm.Radical nephrectomy should performed for the malignant tumor.
3.Psychological assessment and research to the wounded of 96 cases in Yushu,Qinghai

Objective Provided the basis on psychological treatment for the earthquake wounded, through investigated the psychological healthy condition of them in Yushu, Qinghai Province. Method To investigated the 102 cases earthquake wounded from 15 - 18 April 2010 who transported from Yushu, Qinghai to West China hospital of Sichuan University; Accept standard: The wounded (96 cases) who could cooperated this investigative; Adopted the Hamiton anxiety scale( HAMA) to had a review investigation for the wounded in the from of talk, and used the SPSS statistics software to have the Wilcoxon Mann-Whitney test and correlation analysis for the investigation data. Results There were 55 of the 96 cases have been confirmed anxiety, HAMA > 14 points; 16 cases had serious anxiety (HAMA > 29 points); The average score in all of them equal to (17.3 ±10.8) points. And there was a remarkable difference between male and female. The age is possible have related degree with the wounded anxious. What the main in their characteristics of them was Psychic anxiety. Conclusions There was apparent anxiety of them whom came from Yushu, Qinghai, and they needed proactive psychological treatment, female、 middle-aged and elderly need more.
4.Surveillance of hepatitis C among five high-risk populations in Inner Mongolia Autonomous Region from 2013 to 2021

Objective:
To investigate the prevalence of hepatitis C virus (HCV) infections among five high-risk populations in Inner Mongolia Autonomous Region, so as to provide insights into improvements in the control strategy for hepatitis C.
Methods:
The detection of anti-HCV antibody was collected from patients receiving renal dialysis, patients receiving invasive diagnosis and treatment in hospitals, physical examination populations, unpaid blood donors and subjects admitted to family planning clinics in national hepatitis C surveillance sentinels in Inner Mongolia Autonomous Region from 2013 to 2021, and the year-, gender- and age-specific prevalence of anti-HCV antibody was analyzed.
Results:
The mean prevalence of anti-HCV antibody was 2.19%, 1.81%, 0.05%, 0.10% and 0.03% among 3 600 patients receiving renal dialysis, 3 600 patients receiving invasive diagnosis and treatment in hospitals, 18 000 physical examination populations, 18 000 volunteer blood donors and 3 600 subjects admitted to family planning clinics, respectively. The prevalence of anti-HCV antibody appeared a tendency towards a decline among patients receiving renal dialysis (χ2trend=49.065, P<0.001) and volunteer blood donors (χ2trend=11.419, P=0.001). The prevalence of anti-HCV antibody was higher among male patients receiving invasive diagnosis and treatment in hospitals than among females (2.34% vs. 1.36%; χ2=4.826, P=0.028), and no gender-specific prevalence of anti-HCV antibody was seen among other four high-risk populations (all P>0.05). The highest prevalence of anti-HCV antibody was detected among patients receiving renal dialysis (3.30%) and patients receiving invasive diagnosis and treatment in hospitals at ages of 50 to 59 years (3.35%), while the highest prevalence was found among physical examination populations at ages of 60 years and greater (0.18%).
Conclusion
The prevalence of anti-HCV antibody was high among patients receiving renal dialysis and patients receiving invasive diagnosis and treatment in hospitals and low among physical examination populations, volunteer blood donors and subjects admitted to family planning clinics in Inner Mongolia Autonomous Region from 2013 to 2021. Periodical monitoring of anti-HCV antibody is recommended among the elderly.
5.Assessment of the expression profile of SRY-type high-mobility-group box-9 during chondrogenesis of mice and the effects of transfection of recombinant rat sty-type high-mobility-group box-9 on mice mesenchymal stem cells in vitro

Objective To examine the gene expression profile of SRY-type high-mobility-group box-9(SOX9) during entochondrostosis of mice and investigate the effects of transfection of the pDC316-SOX-9 on mice mesenchymal stem cells(MSCs) in vitro. Methods cDNA microarray technique with 34 000 genes was used to analyze the gene expression profiles during entochondrostosis in the limbs of mice embryo from E10 to E14. Pathway analysis of SOX9 was performed with GCOS1.2 software. The recombined expression vector pDC316-SOX-9 was constructed and transfected into mice MSCs by lipofectamine. The phenotype changes of cells were observed with cell energometry, HE stain, immunohistochemical method, RT-PCR and ELISA.Results The gene expression of SOX9 during the critical phase of chondrogenesis in mice embryo limbs at E12 was increased evidently. SOX9 might promot chondrogenesis. As compared with vector and blank group,the chondrocytes of the SOX9 transfected group had the tendency of enhanced differentiation. Conclusion SOX9 may promote chondrogenesis. The transfection of SOX9 gene into mice MSCs can promot MSCs differentiate into chondrocyte, which may provide some experimental data for cartilage histoengineering.
6.Vascular endothelial growth factor gene transfection to enhance the repair of avascular necrosis of the femoral head of rabbit.

OBJECTIVETo explore a new method for the therapy of avascular necrosis of the femoral head.
METHODSThe recombinant plasmid pCD-hVEGF165 was mixed with collagen and was implanted in the necrotic femoral head. The expression of vascular endothelial growth factor (VEGF) was examined by RNA dot hybridization and immunohistochemical techniques. Repair of the femoral head was observed by histological and histomorphometric analysis.
RESULTSThe expression of VEGF was detected in the femoral head transfected with the VEGF gene. The femoral head transfected with the VEGF gene showed a significant increase in angiogenesis 2 and 4 weeks after gene transfection and a significant increase in bone formation 6 and 8 weeks after gene transfection on histomorphometric analysis (P < 0.01).
CONCLUSIONSTransfection of the VEGF gene enhances bone tissue angiogenesis. Repair of osteonecrosis could be accelerated accordingly, thus providing a potential method for therapy of osteonecrosis.

7.miR-128a is up-regulated in hepatocellular carcinoma and promotes tumor cell proliferation by targeting RND3.

OBJECTIVETo investigate the expression of microRNA-128a (miR-128a) and its role in hepatocellular carcinoma (HCC).
METHODSNineteen pairs of fresh surgical specimens of HCC and adjacent tissues were examined for miR-128a expression using qRT-PCR. A miR-128a mimics or inhibitor was transfected into HCC cells, and the cell viability was analyzed by MTT assay. RND3, one of the potential targets of miR-128a, was predicted by bioinformatics software and demonstrated by dual luciferase reporter assay. The expression of RND3 after transfection was detected using qRT-PCR and Western blotting, and the cell cycle-related proteins were determined with Western blotting.
RESULTSThe expression of miR-128a were significantly up-regulated in HCC tissues as compared to the adjacent tissues (P<0.05). In cultured HCC cells, miR-128a promoted the cell proliferation and resulted in down-regulated RND3 mRNA and protein expressions by targeting RND3' 3'UTR (P<0.05) and also in the down-regulation of cyclin B1, cyclin D1 and CDK4 protein expressions.
CONCLUSIONmiR-128a is up-regulated in HCC and promotes HCC cell proliferation by targeting RND3.

8.Experimental study of vascular endothelial growth factor gene therapy for avascular necrosis of the femoral head.

To explore a new method for the therapy of the avascular necrosis of the femoral head, the recombinant plasmid pCD-hVEGF165 was mixed with collagen and was implanted in the necrotic femoral head. The results showed that the expression of VEGF was detectable in the femoral head treated with VEGF gene. Angiogenesis in these femoral heads was more abundant than the control. Bone repairing was augmented in the femoral head treated with VEGF gene. The results suggest that angiogenesis in bone tissue could be augmented by gene transfection of VEGF and bone repairing would be accelerated accordingly.

9.Exploration and practice of integrated digestive system course for medical students

Integrated medical curriculums are becoming more and more popular in medical colleges at home and abroad. In this article, the methods and teaching of integrated digestive system course at Shanghai Jiao Tong University School of Medicine was thoroughly introduced from the theoretical course, PBL course, trainee course, assessment method and effect evaluation. The successful experience of practice of the integrated digestive system curriculum was summarized as follow: strong guarantee from systems, adequate reserves of teaching faculty and good mechanism of feedback. Also, the problems in the practice of the curriculum were presented, which include the shortage of corresponding textbook and of deep running-in between teachers.
10. Whole-genome and quasispecies sequences analysis of three hepatitis A virus isolates in China

Objective:
To analyze the genetic characteristics of whole-genome and quasispecies sequences from three hepatitis A virus (HAV) strains in China.
Methods:
Serum samples from acute hepatitis A patients were collected and viral RNA extraction, transcription, nested PCR, sequencing and assembling were performed to gain near full-length sequences; cloning-based sequencing of the full-length VP1-2 A region was also performed.
Results:
Genotyping showed that the nucleotide and amino acid identities among three strains on VP1-2 A junction region were both 100% and all belonged to subgenotype IA; the nucleotide and amino acid identities on whole-genome region were 99.9%-100% and 100% respectively, and shared the highest identities with AH2 strain from GenBank of 98.5% in nucleotide and 99.7% in amino acid level; no amino acid variation was found among published neutralizing antigenic sites. Within cloning sequences from each strain, the nucleotide and amino acid identities were 99.0%-100% and 98.1%-100%, while among all cloning sequences were 99.0%-100% and 97.2%-100%. The variation rate of nucleotide and amino acid in VP1-2 A junction region were both higher than that of partial VP1 region.
Conclusions
Sequences among three strains in VP1-2 A region were identical, the nucleotide and amino acid identities in both whole-genome region and among quasispecies sequences were relatively high to deduce that they were from the same outbreak. This study provides new insight for identification of HAV transmissions and tracing investigations.
