1.Comparison of Phonetic Articulation between Hearing-impaired and Normal Children in 4.5~5.5 Years Old Groups

Objective To study the characteristics and differences of phonetic pronunciation between hearing-impaired and normal children in 4.5~5.5 years old groups. Methods Phonetic articulation of consonants and vowels were tested by three-grade assessors for 20 hearing- impaired children and 20 normal children in 4.5~5.5 years old group using Question Bank for Hearing-impaired Children's Rehabilitation Evaluation of Hearing and Speech. Results and Conclusion For hearing-impaired children, the consonants of /b. m. d. l. j/ were the most articulative, followed with /h. f. r. p. t. n. g. x/, and then /zh. z. k. c. sh. q. s. ch/; for normal children, the consonants of /b. p. f. g. m. d. h/ were the most articulative, followed with /k. j. x. t. r. l/, and then /q. zh. ch. sh. n. z. s. c/. For both groups, the single final was the most articulative, followed with the compound final, and then the nasal. The articulation of these 3 types of vowels were poorer in hearing-impaired children than in normal children (P<0.01)
2.Analysis of the default network function connection of patients with insomnia disorder on resting state functional magnetic resonance

Objective:To investigate the changes of default network functional connectivity (FC) in resting state of patients with insomnia disorder (ID).Methods:Fifty-six patients with insomnia disorder and fifty healthy controls were recruited. All subjects were assessed with Pittsburgh sleep quality index (PSQI), insomnia severity index (ISI), Epworth sleepiness score (ESS) and Hamilton depression scale (HAMD-17). All subjects were scanned with resting state functional magnetic resonance imaging (rs-fMRI). SPM12 and CONN18b were used to preprocess rs-fMRI data on MATLAB (R2013b).The medial superior frontal gyrus was defined as the seed point, and the differences in the functional connection strength of the two groups of subjects were compared. Pearson correlation analysis was performed between the FC value of the brain area with statistical significance and scores of PSQI and ISI.Results:(1)FC analysis showed that compared with the control group, patients with ID had abnormal default mode network (DMN) connection, as follows: FC of left medial superior frontal gyrus and left central anterior gyrus (MNI: x, y, z=-30, -15, 51) and anterior cingulate gyrus (MNI: x, y, z=-6, 24, 36), FC of right medial forehead between the last gyrus and the left central anterior gyrus (MNI: x, y, z=-48, -6, 48), FC of left anterior cuneiform and the left central back (MNI: x, y, z=-54, -18, 54) and left superior occipital gyrus (MNI: x, y, z=-21, -69, 30), FC of right precuneus and left central posterior gyrus (MNI: x, y, z=-60, -21, 48) was enhanced. The FC of left anterior cuneiform lobe and the right middle frontal gyrus (MNI: x, y, z=42, 33, 42) and the right angular gyrus (MNI: x, y, z=54, -51, 45), FC of right precuneus and left inferior temporal gyrus (MNI: x, y, z=-51, -12, -42) was weakened. (2)With the left medial superior frontal gyrus as the seed point, FC values in anterior cingulate gyrus ( r=0.451, 0.338, both P<0.01) and left anterior central gyrus ( r=0.324, 0.402, both P<0.05) were positively correlated with PSQI and ISI scores. With the right precuneus as the seed point, FC value of left posterior central gyrus was positively correlated with PSQI( r=0.333, P=0.013) and ISI scores( r=0.418, P=0.008), while FC value of left inferior temporal gyrus was negatively correlated with PSQI( r=-0.662, P=0.001) and ISI scores( r=-0.402, P<0.01).With the left precuneus as the seed point, FC value of left superior occipital gyrus was positively correlated with PSQI( r=0.438, P=0.001) and ISI scores( r=0.495, P=0.011). Conclusion:Patients with ID may have changes in the functional connection of multiple networks. The decrease of FC in DMN may be one of the potential causes of insomnia. The increase in FC between DMN and the visuospatial attention network may be the core of the mechanism of damage to the brain function network of insomnia link.
3.Inhibitory effect of G1 on the endoplasmic reticulum stress in EA.hy926 endothelial cells

Objective To observe the effect of GPR30 agonist G1 on high glucose-induced endoplasmic reticulum stress ( ERS) in endothelial EA .hy926 cells.Methods EA.hy926 endothelial cells were divided into three groups:nor-mal control group (Con, 17.51 mmol /L glucose), high glucose (HG, 33.3 mmol /L), high glucose +G1 group (HG+G1, HG +1 μmol/L G1).The apoptosis rate of endothelial cells was measured by flow cytometry , the protein expres-sion changes of ERS related molecules Bip , IRE1, PERK and apoptotic molecules Bax , Bcl-2 were measured by Western blot, the mRNA expressions of Bip and CHOP were measured by RT-PCR assay.Results Compared with Con group , the apoptosis in HG group was significantly increased (P <0.01), Bip, IRE1, PERK and apoptotic molecule Bax were upreg-ulateded (P <0.05, P<0.01 or P <0.001), Bcl-2 downregulatted (P <0.01) and Bip mRNA, CHOP mRNA expres-sion were upregulated (P <0.001 and P<0.01).Compared with the HG group, apoptosis rate in HG +G1 group was significantly lower (P <0.05), BIP, IRE1, PERK and apoptotic molecules Ba.0 downregulated ( P <0.05 or P <0.01), Bcl-2 expressions was increased (P <0.05), Bip mRNA and CHOP mRNA expression were decreased (P<0.001 or P<0.01).Conclusion GPR30 agonist G-1 inhibits EA.hy926 ERS in endothelial cells.
4.Clinical characteristics and genetic analysis of a family of febrile seizure with paroxysmal kinesigenic dyskinesia

Objective To discuss clinical,electroencephalogram(EEG) and PRRT2 gene mutation by reporting a febrile seizure (FS) with paroxysmal kinesigenic dyskinesia (PKD) family.Methods Detailed clinical data of the family were collected.The proband (Ⅳ1) and another 4 patients (Ⅲ1,Ⅲ4,Ⅳ2,Ⅳ3)were studied through clinical examinations.Clinical symptoms of Ⅳ2 were not typical,who was diagnosed as a suspected case.Mutation analysis of PRRT2 gene was screened by polymerase chain reaction (PCR) and DNA direct sequencing in 5 patients (Ⅳ1,Ⅲ1,Ⅲ4,Ⅳ2,Ⅳ3) and 4 unaffected family members (Ⅱ2,Ⅲ2,Ⅲ5,Ⅳ4).Results PKD patients had brief involuntary movements in the limbs or trunk induced by sudden voluntary movement when patients were in the stationary state since the teenagers.Two cases (Ⅲ,Ⅲ4) were accompanied by FS.Three cases(Ⅳ1,Ⅲ1 and Ⅲ4)had abnormal EEG records.The PRRT2 gene mutation (c.649dupC mutation) was identified in a healthy member (Ⅳ4) and 4 patients (Ⅳ1,Ⅲ1,Ⅲ4,Ⅳ3).Conclusions FS with PKD family has a PRRT2 gene mutation.The diagnosis is mainly based on family history,typical clinical manifestations and genetic test.This kind of disease may have pre-symptomatic patients.
5.Extracorporeal membrane oxygenation as a bridge to lung transplantation

Objective To evaluate the effect of extracorporeal membrane oxygenation (ECMO) as a bridge to lung transplantation (LTx). Methods The clinical data of 18 patients with end-stage lung diseases was retrospectively reviewed, using ECMO as a bridge to LTx in intensive care unit of Affiliated Wuxi People's Hospital from January 2015 to December 2017. Clinical parameters were obtained from these patients, including gender, age, primary disease, preoperative lactate level, preoperative leukocyte, operation modality (unilateral or bilateral), type of ECMO, ECMO support time as a bridge to LTx, ECMO support time after operation, total usage time of ECMO, ECMO associated complications, primary graft dysfunction (PGD), successful ECMO weaning, and survival. Patients were divided according to type of ECMO, whether successfully weaned from ECMO or not, and primary disease. Clinical data was compared, and the Kaplan-Meier survival of 180-day was studied. Results ① The overall situation showed: A total of 18 patients were enrolled, with 14 males and 4 females, age ranged from 23 to 78 years old. Primary disease included 6 cases of idiopathic pulmonary fibrosis (IPF), 3 cases of idiopathic pulmonary hypertension (IPAH), 8 cases of interstitial pneumonia and 1 case of silicosis. Nine patients received venous-venous (V-V) ECMO and 9 venous-artery (V-A) ECMO as a bridge to LTx; 15 patients received LTx successfully, and failed in 3 cases. The average bridge time was 57.5 (14.5, 116.5) hours. ECMO associated complications included 6 cases with bleeding, 12 cases with renal failure, 2 cases with thrombosis, 2 cases with oxygenator leak, and 1 case with leg ischemia. There were 7 unilateral (5 right lungs and 2 left lungs) and 8 bilateral LTx. Three patients died before LTx due to septic shock. Nine patients died after LTx, 4 for septic shock, 4 for multiple organ failure, and 1 for sudden cardiac death. Six patients survived after LTx.② Group comparison showed: There was no significant difference in gender, age, ECMO support time as a bridge to LTx, ECMO support time after operation, total ECMO usage time, incidence of PGD, successful weaning from ECMO, and 180-day survival rate between V-V ECMO group (n = 7) and V-A ECMO group (n = 8). There was no significant difference in gender, age, primary disease, type of ECMO, operation modality, preoperative leukocyte count between groups of successfully weaned from ECMO (n = 11) and the failed (n = 7). Lower level of preoperative lactate acid (mmol/L: 3.01±1.51 vs. 8.27±3.49, t = -3.770, P = 0.006), shorter total ECMO usage time (hours: 72.82±40.53 vs. 210.71±107.10, t = -3.907, P = 0.001), and higher 180-day survival rate (54.5% vs. 0, P = 0.038) were found in the group of successfully weaned from ECMO, when compared with the failed group. ③ Kaplan-Meier survival analysis showed that postoperative survival rates of 7, 30, 60, and 180 days of 18 patients was 72.2%, 38.9%, 33.3%, and 33.3%, respectively. Among them, the postoperative survival rates of 7, 30, 60, and 180 days in the group of successfully weaned from ECMO (n = 11) were higher than those in group of failed (n = 7; 81.8% vs. 57.1%, 63.6 % vs. 0, 54.5% vs. 0, 54.5% vs. 0, respectively; log-rank test: χ2= 8.009, P = 0.005). The postoperative survival rates of 7, 30, 60, and 180 days in IPF group (n = 6) were lower than those in non-IPF group (n = 12; 33.3% vs. 83.3%, 16.7% vs. 50.0%, 16.7% vs. 41.7%, 16.7% vs. 41.7%; log-rank test: χ2= 4.161, P = 0.041). Conclusions The use of ECMO as a bridge to LTx may provide survival benefit for LTx recipients. V-V ECMO provides effective life support for patients without severe heart failure, and V-A ECMO for patients with unstable hemodynamics. Preoperative lactate level and total ECMO duration time were closely related to ECMO weaning rate. Primary diagnosis may affect prognosis.
6.Curative effect and prognosis analysis of lung transplantation for patients over 65 years old with end- stage lung diseases

Objective To investigate the curative effect and prognosis of lung transplantation for patients over 65 years old with end-stage lung diseases.Methods Clinical data of 24 elderly patients (≥65 years old)undergoing lung transplantation in Affiliated Wuxi People's Hospital of Nanjing Medical University from September 2002 to December 201 1 were retrospectively analyzed.The survival conditions of elderly patients undergoing lung transplantation were studied and the survival conditions of unilateral lung transplantation and bilateral lung transplantation were compared.The complications after lung transplantation were observed and the death causes of patients died during the follow-up period was analyzed.Results All patients completed the lung transplantation successfully.The 1 ,3,5 and 7-year cumulative survival rate of the 24 patients were respectively 75.0%,62.5%,33.3% and 1 2.5%,and those of patients undergoing unilateral lung transplantation were respectively 68.8%,50.0%,31 .3% and 1 2.5% and those of the patients undergoing bilateral lung transplantation were respectively 87.5%,87.5%,37.5% and 1 2.5%.The median survival time of the patients undergoing unilateral lung transplantation and bilateral lung transplantation was respectively 35 months and 51 months,and there was no significant difference (P >0.05 ).The primary postoperative complication was infection with the incidence of 46% (1 1 /24).Seventeen patients died during the long-term follow-up.Among them,9 died of severe infection,1 died of primary graft dysfunction (PGD), 1 died of acute rejection (AR),4 died of bronchiolitis obliterans syndrome (BOS),1 died of heart failure and 1 died of tumor.Conclusions Unilateral lung transplantation and bilateral lung transplantation are both effective methods to treat elderly patients with end-stage lung diseases.Infection is the primary postoperative complication and the cause of death of elderly patients.
7.Effects of ropivacaine on proliferation,apoptosis and cell cycle of MDA-MB-231 cells

Objective To investigate the effects of ropivacaine on proliferation,apoptosis and cell cycle of MDA-MB-231 Cells. Methods The cultured MDA-MB-231 cells were treated with different concentrations of ropivacaine. The proliferation of MDA-MB-231 cells was detected by MTT method. Cell apoptosis and cell cycle were detected by Annexin V-FITC/PI and flow cytometry. Results After ropivacaine administration,MDA-MB-231 cells proliferation inhibition ratio increased significantly.Annexin V-FITC/PI and flow cytometry showed ropiva-caine had no significant effects on cell apoptosis and cell cycle.Conclusion Ropivacaine can inhibit the prolifera-tion of MDA-MB-231 cells,but can′t induce apoptosis and block cell cycle.
8.The influences of sucrose, citric acid and sodium bicarbonate on the adhesion of 3 kinds of adhesives

Objective:To study the influences of sucrose,citric acid and sodium bicarbonate on the adhesion of 3 kinds of adhesives.Methods:60 extracted tooth and 60 zirconia blocks (3 mm × 3 mm × 3 mm) were randomly divided into 3 groups (n =20),namely PULPDENT group,3M ESPE RelyxTM Veneer group and RelyxTM Luting group.Then,the samples of each group fell into 4 subgroups(n=5),namely subgroup A for artificial saliva,subgroup B for 10% sucrose,subgroup C for 0.2% citric acid,and subgroup D for 0.03 % sodium bicarbonate.After completing the adhesion of the specimens with corresponding adhesives,the specimens of subgroups A,B,C and D were submerged into artificial saliva(the control),sucrose,citric acid and sodium bicarbonate solutions for 2 times/day and 5 min/time,respectively.For the rest of time,all the specimens were submerged in artificial saliva.3 months later,shear bond strength of the specimens was tested,the fracture surface was observed under SEM,20 × microscope,and the fracture model was observed by stereoscopic microscope.SPSS 17.0 software was adopted for statistical analysis of the data.Results:The bond strength of PULPDENT,3M ESPE RelyxTM Veneer groups were higher than that of RelyxTM Luting group(P < 0.05);the bond strength of subgroups B,C and D was lower than that of subgroup A(P <0.05);and the difference between the remaining groups was not statistically significant(P >0.05).SEM observation displayed that in group RelyxTM Luting,subgroups B,C and D showed increased crack depth,width and length when compared with subgroup A;there was no obvious difference between the remaining groups and the control group;in groups PULPENT,3M ESPE RelyxTM Veneer and RelyxTM Luting,all samples in their subgroups showed interface failure.Conclusion:Compared with PULPDENT and 3M ESPE RelyxTM Veneer adhesive,RelyxTM Luting is more susceptible to the influence of sucrose,citric acid and sodium bicarbonate,so it is not suitable for bonding zirconia blocks.
9.Analysis of the clinical characteristics of 69 cases of occupational melanosis

Objective To summarize the clinical characteristics of patients with occupational melanosis. Methods Diagnostic data of 69 patients with occupational melanosis was analyzed using retrospective analysis. Results The main occupational hazards for the 69 patients with occupational melanosis were coal tar, petroleum and its fractionated products, pigments and dyes and their intermediates, rubber additives and rubber products. The median length of occupational exposure and disease latency were 8.0 and 6.0 years, respectively, with a highly positive correlation between them (Spearman correlation coefficients=0.962, P<0.01). Skin lesions were mainly found on exposed areas such as the face-to-neck and limbs, prevalence of 94.2% and 75.4% respectively. And 78.3% of patients had skin lesion on more than two sites. The lesions were mostly in the form of irregular flakes (59.4%), with a gray-black color (44.9%). About 43.5% of patients experienced skin itching. Complete blood count, liver function, and kidney function were all within normal ranges. Skin biopsy results showed that epidermal hyperkeratosis, thinning of the spinous layer, liquefaction degeneration of basal cells, increased superficial dermal melanocytes, and infiltration of lymphocytes, histiocytes, and melanocytes around the blood vessels. Reflectance confocal microscopy (RCM) detection showed focal liquefaction degeneration of basal cells in the lesions, with a significant infiltration of melanocytes and inflammatory cells in the dermal papillae and superficial layers. Conclusion The primary target organ of occupational melanocytes is the skin, and no damage to other organs was identified thus far. Results from skin biopsies and RCM examinations can be used for differential diagnosis.
10.Clinical characteristics of patients infected with 2019 novel coronavirus Omicron variant in Zhuhai City

Objective:To explore the clinical characteristics of patients infected with 2019 novel coronavirus (2019-nCoV) Omicron variant in Zhuhai City.Methods:A retrospective study was conducted to compare the clinical characteristics of patients infected with 2019-nCoV Omicron variant (Omicron variant group, 39 cases) from January 13 to January 25, 2022, and those infected with 2019-nCoV non-Omicron variant (non-Omicron variant group, 98 cases) from January 17 to February 17, 2020, in the Fifth Affiliated Hospital of Sun Yat-sen University. Statistic methods were used by Mann-Whitney U test, chi-square test and Fisher exact probability test. Results:Among adults, the age of 21 patients in Omicron variant group was younger than that of 93 patients in non-Omicron variant group (34.0(26.0, 40.5) years vs 50.0(36.0, 62.0) years, Z=-3.81, P<0.001). Patients with underlying diseases in Omicron variant group were fewer than those in non-Omicron variant group (4.8%(1/21) vs 31.2%(29/93), χ2=6.17, P=0.013). The clinical classification of the Omicron variant group was mainly mild (71.4%(15/21)) or common type (28.6%(6/21)), while the non-Omicron variant group was dominated with common type (57.0%(53/93)) and severe type (23.7%(22/93)), and the clinical classification of the two groups had statistically difference ( χ2=31.79, P<0.001). The time of positive nucleic acid in Omicron variant group was longer than that in non-Omicron variant group (14.0(10.5, 16.5) d vs 8.5(4.0, 12.0) d, Z=-3.56, P<0.001). The proportion of pneumonia in Omicron variant group was less than that in non-Omicron variant group (28.6%(6/21) vs 86.0%(80/93), χ2=30.52, P<0.001). Differences were all statistically significant. There was no significant difference of the clinical characteristics in pediatric patients, but the proportion of children infected with 2019-nCoV Omicron variant was significantly higher than that infected with 2019-nCoV non-Omicron variant (46.2%(18/39) vs 5.1%(5/98), χ2=33.65, P<0.001). Conclusions:Adults infected with 2019-nCoV Omicron variant in Zhuhai City present with milder clinical symptoms, less proportion of pneumonia, but the time of positive nucleic acid is longer. The clinical characteristics of children infected with 2019-nCoV Omicron variant are not specific, while the proportion of children infected with 2019-nCoV Omicron variant is significantly higher.
