Objective To compare the distance of left ventricular along long axis measuring under real-time movement and Doppler tissue imaging.Methods Titanium nips were implanted into anterior wall and anterior septum of left ventricule of 7 open-chest canine models.Quantitative study was made on time velocity integral(TVI) of the implanted myocardium and left ventricular long axis in 7 canine models,and 7 patients with replaced metal mitral valve.Digital subtraction angiography was adopted to record the real-time movement of the titanium nips in 7 canine models and the metal valve in 7 patients.And comparison was made between the results.Results The maximum values of displacement measuring with TVI were close to the distance under real-time movement. Conclusions The displacement measuring with TVI can reflect the character of the myocardial displacement truly.

Objective To investigate the effect of simvastatin on the mobilization and migration of endothelial progenitor cells(EPCs).Methods EPCs were harvested from the bone marrows of two rabbits,cultured with M199,and identified by immunohistochemistry.The identified EPCs were then treated with simvastatin with different concentrations(0,0.01,0.1,1.0 ?mol/L),and their migration induced by simvastatin was determined with Transwell chamber assay.Six rabbits models of cranial bone defect were established and divided into control and experiment groups(3 in each).In order to elicit the effects of simvastatin on mobilization of EPCs,simvastatin was embedded in polylactic acid compound,and implanted into the cranial bone defect area in the experiment group.Meanwhile,polylactic acid was implanted in the control animals.After 10 days,the expression rate of CD34+/CD133+ EPCs in the rabbit peripheral blood was counted by flow cytometry to determine the motivating effect of simvastatin.Results In Transwell experiment,16 hours after adding simvastatin(0,0.01,0.1 or 1.0 ?mol/L),the cell migration ability was obviously increased showing a dose-dependent trend(OD value:0.077?0.014 in control group and 0.075?0.013 in 0.01 ?mol/L group vs 0.097?0.011 in 0.1 ?mol/L group and 0.099?0.019 in 1.0 ?mol/L group,P

Objective To investigate the effect of simvastatin-polylactic acid compound on critical calvarial defects in rats.Methods Twenty male SD rats(150 g?10 g),were used to establish critical cranial defect(10 mm in diameter)model.The animals were randomly divided into control and experiment groups(10 in each).In the control group,40 mg of polylactic acid were implanted into the defect area;whereas in the experiment group,simvastatin-polylactic acid compound were used(20 mg simvastatin and 40 mg polylactic acid).Four and eight weeks after the implantation,the defect area of the rats was observed by X-ray and toluidine blue staining.Results Eight weeks after the operation,X-ray examination showed high-density regions in the defect area in the experiment group,while low-density regions in the control group.The radiopacity of cranial defects were 27.33%?2.54% in the control group,and 74.63%?2.42% in the experimental group(n=5,t=-30.148,P=0.000).Toluidine blue staining showed a few new bone tissues at 4 weeks and fully filled bone defect at 8 weeks in the experiment group.Meanwhile,in the control group,only a small quantity of new bone tissue could be seen on the edge of the cranial defects.Conclusion Locally implanted simvastain-polylactic acid compound is a promising method for the treatment of bone defect owing to its high osteogenic ability.

Objective To quantify the morphometric characteristics of the pedicles of the infantile thoracolumbar spine and to determine the feasibility and safety of pedicle screw fixation in very young children. Methods A total of 52 patients younger than three years of age underwent standard spiral computed tomography (CT) were enrolled in this study. The patients were grouped according to age, group 1 (0 ～ 18months of age) and group 2 (18 ～ 36 months of age). Images were reformatted, and multiplanar reconstructions were used to attain images of thoracic and lumbar pedicles on sagittal, coronal and transverse planes.The measurements included the inner and outer pedicle diameters on the transverse plane, pedicle sagittal diameter, pedicle length and the pedicle angle on the transverse. Characteristics of the pedicles of the immature thoracic and lumbar spine were studied. Results Pedicle diameters on the transverse plane decreased gradually from T1 to T5 and increased gradually from T6 to L5. The shortest transverse diameter of the thoracic pedicle was T5 or T6. The inner diameter of the segment of L4 in infant (younger than eighteen months) was less than 3.0 mm and the external diameter less than 3.5 mm from T3 to T9. The inner diameter of the segment from T5 to T8 in infant (more than eighteen months) was less than 3.0 mm and the external diameter from T4 to T7 was less than 3.5 mm. The sagittal diameter was significantly more than the transverse diameter. Pedicle diameters on the transverse plane, the sagittal diameter, and the length of the pedicle changed significantly with age, but the pedicle angle did not changed significantly with age. Conclusions The pedicles in the segment from T3 to T9 of less than eighteen months and from T4 to T7 of more than eighteen months were too small, there was no feasibility and safety of pedicle screw fixation. So pedicle screw fixation was feasible and safe for below T9 segment in the infants who are younger than three years old.

Objective To investigate the retention level and its influencing factors for male nurses,and explore effective strategies to increase retention level of male nurses.Methods A sample of 610 male nurses from many hospitals in Tianjin were investigated with the self-designed demographic questionnaire,Retention Scale and Perceived Organization Support.The results underwent analysis.Results Male nurses' retention score was (20.71±4.00) points.Single factor analysis found that marriage,age,number of children,time engaged in nursing work,the working years in present position,the purpose of the work had statistically significant influence on retention scores of the male nurses.Multiple linear regression analysis showed that marriage,professional and technical titles,the purpose of work,work support and value identification were the predictive effect on the degree of the retention.Conclusions The retention of male nurses in Tianjin was in the medium level.Hospitals and all relevant departments should take effective measures,improve the professional sense of worth of the male nurses and job satisfaction,in order to enhance the man nurses' retention rate,promote the sustainable development of the nursing team construction.

Objective We aim to objectively evaluate the influence of six-character formula on patients with stable chronic obstructive pulmonary disease (COPD).Methods The randomized controlled trials (RCTs) which investigated the influence of six-character formula on patients with COPD were electronically searched in the databases such as China National Knowledge Infrastructure (CNKI),Chinese Science Technology Periodical Databases (VIP),Wanfang Data,PubMed,The Cochrane Library (Issue 6,2014) and ELSEVIER.According to the inclusion and exclusion criteria,the references was screened,the data were extracted and the methodological quality of the included articles was assessed.Then,Meta-analy-sis was performed using RevMan 5.3 software.Results A total of 10 studies involved,578 patients were included.The result of Meta-analysis showed that compared with the control group,six-character formula significantly improved six-minute walk distance,forced expiratory volume in a second (FEV1) and FEV1 predicted value,reduced the score of activity limitation and the impact of the disease in St George's respiratory questionnaire (SGRQ).the value of MD and 95％CI were 22.62 (10.49-34.75),0.10 (0.01-0.18),3.08 (0.18-5.97),8.69 (4.09-13.83),7.60 (2.34-12.85).However,there was no statistical difference in the score of symptom.Conclusions Six-character formula plays a significant role in the treatment of stable COPD.However,more studies in large scale and high quality are needed to verify the result.

OBJECTIVE:To analyze the cost-effectiveness of two regimens in the treatment of chronic hepatitis B fibrosis. METHODS:112 cases of chronic hepatitis B fibrosis were divided into Compound biejia ruangan tablet group (group A,n=56) and Anluo huaxian pill group(group B,n=56). Both groups received Entecavir dispersible tablets combined with relevant Chinese patent medicine. The liver fibrosis index and transient elastography of 2 group were detected before and after treatment,TCM symp-tom score and effective rate calculation were conducted to compare the cost-effectiveness of 2 groups. RESULTS:The cost,effec-tive rate and cost-effectiveness ratio of group A were 9 227.10 yuan,74.11% and 12 451;those of group B were 8 202.90 yuan, 69.28%and 11 840;incremental cost-effectiveness ratio was 21 205. Group B showed a better cost-effectiveness. Result of sensitiv-ity test was same to that of cost-effectiveness analysis. CONCLUSIONS:The cost-effectiveness of Anluo huaxian pill combined with Entecavir dispersible tables is better than Compound biejia ruangan tablet combined with Entecavir dispersible tables in the treatment of chronic hepatitis B fibrosis.

Objective To investigate the causes of misdiagnosis and improve the level of diagnosis and treatment for endocrine myopathy,through the study the clinical data of 20 patients with endocrine myopathy who were misdiagnosed as polymyositis were analyzed.Methods 20 cases were retrospectively analyzed who suffered from proximal weakness as the main performance and misdiagnosed as polymyositis,through the method of supervision to track,review of the observation and methods of medical record information query.Results The main types of diseases which were misdiagnosed including 18 patients of thyroid diseases(13 patients of hypothyroidism,5 patients of hyper-thyroidism),2 patients of parathyroid disease(1 patient of hyperparathyroidism,1 patient of hypoparathyroidism). Conclusion Mastering polymyositis and endocrine myopathy in the identification of main points,improving the ability of differential diagnosis,so that doctor can avoid misdiagnosis.

OBJECTIVE: To study the efficacy of treatment with microsurgery in combination with local injection of type A botulinum toxin for vocal process granuloma. METHOD: 28 patients with vocal process granuloma received endotracheal intubation under general anesthesia. The lesion was removed with micro-scissor and CO2 laster under a self-retaining laryngoscope and microscope. The incision and mucous membrane surrounding the wound was closed with 8-0 absorbable suture. 4-point injection of botulinum toxin type A 8-15 u was then performed along the thyroarytenoid muscle and arytenoid muscle of the same side. Postoperative medication was administered based on disease causes. RESULT: All patients experienced vocal cord dyskinesia of the injected side 2-3 days after surgery. At 1 month after the surgery, wound healing was good in all the 28 patients, and the vocal cord movement was limited at the injected side. At 3 months, movement of the bilateral vocal cords was normal, and the vocal cord process mucosa was smooth. Patients were followed up for more than a year, and only one patient had recurrence in 2 months after surgery. The cure rate was 96. 4%. CONCLUSION Combination of laryngeal microsurgery and type A botulinum toxin local injection can shorten the treatment course of vocal process granuloma.
