1.Trends in mortality and life lost due to drowning in Huzhou City from 2012 to 2021

Objective:
To investigate the mortality and years of life lost due to drowning in Huzhou City from 2012 to 2021, so as to provide insights into drowning prevention and control.
Methods:
The mortality surveillance data on drowning in Huzhou City from 2012 to 2021 were collected from the Zhejiang Chronic Disease Surveillance Information Management System. The crude mortality and standardized mortality of drowning by the 2010 population census data in 2010, years of potential life lost (YPLL) and working years of potential life lost (WYPLL) due to drowning were calculated. In addition, the annual percent change (APC) was used to analyze the trends in drowning mortality and the rate of YPLL and WYPLL in Huzhou City from 2012 to 2021.
Results:
A total of 1 681 deaths occurred due to drowning in Huzhou City from 2012 to 2021, accounting for 9.11% of total injury deaths. The overall mortality and standardized mortality of drowning were 6.35/105 and 4.70/105, respectively, and mortality and standardized mortality of drowning were 6.95/105 and 5.44/105 in men and 5.76/105 and 3.98/105 in women, respectively. The highest mortality of drowning was found in residents at ages of 65 years and older (16.04/105 to 27.02/105), followed by in residents at ages of less than 5 years (2.54/105 to 16.37/105). The overall mortality of drowning was 5.29/105 to 7.21/105 among residents in Huzhou City from 2012 to 2021, and no significant change tendency was seen (APC=-2.18%, t=-2.085, P=0.071). The standardized mortality of drowning reduced from 6.10/105 to 3.69/105 (APC=-4.88%, t=-4.215, P=0.003). In addition, the YPLL and rate of YPLL, and WYPLL and the rate of WYPLL due to drowning mortality were 22 620.50 person-years, 0.10%, 17 956.50 person-years and 0.08% in Huzhou City from 2012 to 2021, respectively, and the rates of YPLL (APC=-6.95%, t=-3.203, P=0.016) and WYPLL (APC=-7.60%, t=-3.126, P=0.014) both appeared a tendency towards a decline from 2012 to 2021.
Conclusions
The standardized mortality of drowning and rate of YPLL appeared a tendency towards a decline among residents living in Huzhou City from 2012 to 2021, and residents at ages of 65 years and older and less than 5 years are high-risk populations for management of drowning mortality.
2.Evaluation on the Effect of Intervention Measures Enforced by Clinical Pharmacists on Reducing Dosage of Benzodiazepines

OBJECTIVE: To evaluate the effect of intervention measures enforced by clinical pharmacists on reducing dosage of benzodiazepines. METHODS: Clinical pharmacists described benzodiazepine withdrawal scheme for adult patients who were treated with repeated benzodiazepine prescription more than 3 months. 10% of benzodiazepines dose was reduced every two weeks and single blind drug withdrawal was carried out. RESULTS: Consumption of benzodiazepine was reduced significantly by 70.50% after intervening by pharmacists. CONCLUSION: Clinical pharmacists play an important role in benzodiazepine withdrawal, whose intervention is rapid and useful method for physicians reducing dosage of benzodiazepines.
3.Influence of six-character formula on patients with stable COPD: a Meta-analysis

Objective We aim to objectively evaluate the influence of six-character formula on patients with stable chronic obstructive pulmonary disease (COPD).Methods The randomized controlled trials (RCTs) which investigated the influence of six-character formula on patients with COPD were electronically searched in the databases such as China National Knowledge Infrastructure (CNKI),Chinese Science Technology Periodical Databases (VIP),Wanfang Data,PubMed,The Cochrane Library (Issue 6,2014) and ELSEVIER.According to the inclusion and exclusion criteria,the references was screened,the data were extracted and the methodological quality of the included articles was assessed.Then,Meta-analy-sis was performed using RevMan 5.3 software.Results A total of 10 studies involved,578 patients were included.The result of Meta-analysis showed that compared with the control group,six-character formula significantly improved six-minute walk distance,forced expiratory volume in a second (FEV1) and FEV1 predicted value,reduced the score of activity limitation and the impact of the disease in St George's respiratory questionnaire (SGRQ).the value of MD and 95%CI were 22.62 (10.49-34.75),0.10 (0.01-0.18),3.08 (0.18-5.97),8.69 (4.09-13.83),7.60 (2.34-12.85).However,there was no statistical difference in the score of symptom.Conclusions Six-character formula plays a significant role in the treatment of stable COPD.However,more studies in large scale and high quality are needed to verify the result.
4.Evaluation of Immunogenicity and Protection Efficacy of the Recombinant Hypoxanthine-Guanine-Xanthine of Plasmodium falciparum in Mice

Objective To investigate immunogenicity and protection efficacy of the recombinant hypoxanthine-guanine-xanthine (HGXPRT) of Plasmodium falciparum expressed in Pichia pastoris. Methods 35 BALB/c mice were divided randomly into five groups: HGXPRT+ISA720 experiment group, HGXPRT+Freund experiment group, ISA720 adjuvant control group, Freund adjuvant control group, and blank control group. BALB/c mice were subcutaneously immunized three times with the HGXPRT protein formulated by either Freund or ISA720 adjuvants at a three weeks interval. Mice were bled via tail vein at 2 weeks after each immunization. Specific antibodies were detected by ELISA as well as IFAT using cultured parasites. The immunized mice were challenged with 105 P.yoelii 10 days after the third immunization and parasitemia was monitored daily by examining Giemsa-stained thin film. Results Strong immune response was induced by the HGXPRT antigen formulated with the adjuvant. Antibody titers of more than 1∶105 were detected after the third immunization while no specific antibody was detected in the mice immunized with adjuvants only. The antibodies against HGXPRT recognized the cultured parasite by IFAT. Four days after mice were challenged with P.yoelii, high parasitemia appeared in the two control groups, which were 24 h earlier than experiment groups. 5.Prevalence and influencing factors of comorbidity of chronic diseases among hypertensive patients with uncontrolled blood pressure in Huzhou City

Objective:
To investigate the prevalence and influencing factors of comorbidity of chronic diseases among hypertensive patients with uncontrolled blood pressure in Huzhou City, so as to provide insights into community hypertension control.
Methods:
Hypertensive patients with uncontrolled blood pressure at ages of 35 to 74 years were sampled using a cluster random sampling method from 5 districts (counties) of Huzhou City. Participants' demographics, living behaviors, and development of chronic diseases were collected using questionnaires, and the height, body weight, waist circumference and blood pressure were measured. Blood glucose, blood lipid and other biochemical parameters were detected, and the number and combination of comorbidity of chronic diseases were descriptively analyzed. Factors affecting the comorbidity of chronic diseases were identified using a multivariable ordinal logistic regression model.
Results:
A total of 1 215 respondents were included, with a mean age of (60.83±7.76) years, and including 652 men (53.66%) and 563 women (46.34%). The prevalence of dyslipidemia, diabetes, hyperuricemia and cardiac encephalopathy was 45.10%, 30.95%, 23.05% and 5.10%, respectively. The prevalence of comorbidity of chronic diseases was 69.22% among respondents, and there were 497 respondents with one comorbidity (40.91%), 272 with two comorbidities (22.39%) and 72 with three and more comorbidities (5.93%). Hypertension+dyslipidemia (20.74%), hypertension+diabetes+dyslipidemia (9.96%) and hypertension+diabetes+dyslipidemia+hyperuricemia (4.36%) were predominant comorbid combinations. Multivariable ordinal logistic regression analysis showed that participants with overweight (OR=1.782, 95%CI: 1.390-2.286), obesity (OR=2.411, 95%CI: 1.802-3.222), grade 2 hypertension (OR=1.438, 95%CI: 1.077-1.919) had a higher risk of multiple comorbidities than those with normal body mass index and controlled blood pressure, and women (OR=0.563, 95%CI: 0.456-0.696) had a lower risk of multiple comorbidities than men.
Conclusions
The prevalence of comorbidity of chronic diseases was 69.22% among community hypertensive patients with uncontrolled blood pressure in Huzhou City, and the comorbidity of chronic diseases mainly included dyslipidemia and diabetes. Men, overweight, obesity and hypertension resulted in a high risk of comorbidity of chronic diseases.

8.Method for obtaining noise of thoracic CT images based on Canny edge detection algorithm

Objective:To propose a method for obtaining noise from thoracic CT images based on the Canny edge detection algorithm.Methods:A total of 250 pieces of thoracic CT images of male volunteers collected in 2021 were selected. The contours of thoracic CT images were extracted by the Canny adaptive threshold method. The similarity of the Sobel algorithm, the Canny double threshold method, and the Canny adaptive threshold method was compared. The Hoff transform was used to determine the regions of interest. The effects of the selection size of the regions of interest, the reconstructed convolution kernel, and tube current on the noise of thoracic CT images were investigated.Results:The Canny adaptive threshold method preserved more detail, and the continuity and integrity of the edges were improved, indicating that it was more flexible and robust for edge detection and image segmentation. The Canny adaptive threshold method had the highest structural similarity index (0.644) and the lowest root mean square error (0.371). It had the highest similarity in edge contour detection, and the effect was more significant. As the size of the square area of interest increased, the average noise decreased. The noise standard deviation increased in some intervals, especially in larger square regions. In the case of the same reconstructed convolution kernel, the mean noise of the ascending aorta in thoracic CT images was higher than that of the thoracic aorta. The noise standard deviation of the ascending aorta was lower than that of the thoracic aorta. For ascending aorta, the mean ascending aorta noise (41.97 dB) of reconstructed convolutional nucleus E was the lowest, with the highest noise standard deviation (20.64 dB). For the thoracic aorta, the mean noise (30.78 dB) of the reconstructed convolutional nucleus E was the lowest. The mean noise and standard deviation of the thoracic aorta decreased with the increase in tube current.Conclusions:A method based on the Canny edge detection algorithm to obtain noise from thoracic CT images is proposed, which is suitable for detecting thoracic CT images.
9.Long-term outcome of childhood T-cell acute lymphoblastic leukemia treated with modified national protocol of childhood leukemia in China-acute lymphoblastic leukemia 2008

Objective:To analyse the long-term efficacy in childhood T-cell acute lymphoblastic leukemia (T-ALL) cases enrolled in the national protocol of childhood leukemia in China-acute lymphoblastic leukemia (NPCLC-ALL) 2008.Methods:Clinical data of 96 patients diagnosed as T-ALL and treated with NPCLC-ALL2008 protocol between January 2009 and December 2017 in the Department of Hematology-Oncology, the Children′s Hospital, Zhejiang University School of Medicine were analyzed retrospectively. Predictive value of minimal residual disease (MRD) monitored by flow cytometry was analyzed. Kaplan-Meier method was used for long-term survival analysis.Results:A total of 96 evaluable patients with newly diagnosed T-ALL were analysed, including 72 males and 24 females. The age was 9.5 (ranged from 1.0 to 16.0) years. The follow-up time was 5.7 (ranged from 1.0 to 9.7) years. Among 96 patients, 92 (96%) achieved complete remission. The 5-year event free survival (EFS) and overall survival (OS) rates were (61±6) % and (70±5) %, respectively. Relapse occurred in 18 cases and the 5-year cumulative incidence of relapse was (27±6) %. Twenty-four patients died. The 5-year OS rates of patients with MRD>5% on day 15 of induction therapy was significantly worse than those with MRD≤5% ((60±12) % vs. (72±6) %, χ 2=3.904, P=0.048) . The 5-year EFS and OS rates were obviously lower in patients with MRD>10% before the consolidation therapy ((50±35) %). The 5-year OS rates of patients with relapsed disease was significantly worse than those without ((26±13) % vs. (81±5) %, χ 2=18.411, P<0.01). The earlier the relapse, the worse the prognosis. The 5-year OS rates for patients relapsed within 6 months, within 3 years and more than 3 years, were (25±22) %, (30±14) % and (50±35) % respectively (χ 2=13.207, P<0.01). Conclusions:NPCLC-ALL2008 protocol is effective for childhood T-ALL. The MRD guided accurate risk stratification and individualized treatment can reduce the relapse and improve the survival rate of pediatric T-ALL.
10.Advances on correlation of PET-CT findings with breast cancer molecular subtypes, treatment response and prognosis.

In recent years, PET-CT has an increasing importance in the diagnosis and treatment of breast cancer. PET-CT scan can be used as a noninvasive method for molecular subtyping of breast cancer, and prediction of therapeutic effect and prognosis of patients. Studies have revealed that luminal A subtype has a significantly lower maximum standard intake value (SUVmax) than the other subtypes; triple-negative and human epidermal growth factor receptor 2 (HER2) positive tumors have relatively high SUVmax than luminal B subtype, but the specificity and sensitivity of SUVmax in diagnosis of molecular subtypes are very low, so its clinical application is limited. In predicting the effectiveness of the treatment and the prognosis of the patients, the decreased uptake of fluorodeoxyglucose (FDG) is correlated with better therapeutic effect. In addition, patients with high FDG uptake have worse survival outcomes. New tracers, such asF-fluoroestradiol (F-FES) and[89Zr]trastuzumab play an important role in molecular subtyping of breast cancer.F-FES PET-CT can effectively evaluate the estrogen receptor (ER) status of breast cancer and the response to endocrine therapy.[89Zr]trastuzumab PET-CT can evaluate the expression of HER2 and localization of HER2-overexpressing tumors, but their specificities and sensitivities are also low. In this article, we review the recent advances on the correlation of PET-CT findings with molecular subtypes, treatment response and prognosis of breast cancer.
