1.Efficacy analysis of gasless robotic surgery via transaxillary approach for unilateral N1b PTC.

Objective:To compare the efficacy of gasless robotic surgery via transaxillary approach and combined axillary-retroauricular approach for unilateral N1b PTC, and to explore the safety and effectiveness of gasless robotic surgery via transaxillary approach for unilateral N1b PTC. Methods:Unilateral N1b PTC patients who underwent surgery in the Department of Otolaryngology, Sun Yat Sen Memorial Hospital, Sun Yat sen University between July 2016 and December 2024 were included and analyzed. According to the inclusion and exclusion criteria and the differences of surgical approaches, the patients were divided into the transaxillary approach(TA) group and the combined axillary-retroauricular approach(TARA) group. The demographic data, operation time, intraoperative blood loss, postoperative drainage volume, postoperative complications, shoulder function evaluation, postoperative visual analogue scale(VAS) of neck aesthetics and recurrence of the two groups were statistically analyzed. Results:A total of 88 patients undergoing gasless robotic surgery were included in this study, including 23 cases in the TA group and 65 cases in the TARA group. The proportion of males in the TA group was significantly higher than that in the TARA group(56.5% vs 21.5%, χ²=9.776, P=0.002). The total operation time in the TA group was significantly lower than that in the TARA Group(180.00[155.00, 220.00]min vs 220.00[177.50, 272.50]min, z=-2.775, P=0.006), and the postoperative blood loss in the TA group was significantly lower than that in the TARA Group(30.00[20.00, 50.00]ml vs 50.00[30.00, 60.00]ml, Z=-2.127, P=0.033). The proportion of area Ⅱ-Ⅴ in the TA group and the TARA group was 87.0% and 70.8%, respectively, and there was no significant difference between the two groups(P>0.05). There was no significant difference in lateral cervical lymph node dissection and central lymph node dissection between the two groups(P>0.05). During the follow-up period, no recurrence was found in the two groups, and there was no significant difference in the incidence of complications between the two groups(P>0.05). According to the stratification of dynamic recurrence risk assessment, it can be seen that the proportion of curative effect satisfaction in the TA group was as high as 95.7%, and that in the TARA group was as high as 81.5%, with no significant difference between the two groups. There was no significant difference in VAS score of neck, Constant Shoulder Score and NDⅡ scale between the two groups(P>0.05). Conclusion:Gasless robotic surgery via transaxillary approach for unilateral N1b PTC is safe and feasible, and the amount postoperative lymph node acquisition is equivalent to that of combined axillary-retroauricular approach, which can provide a new choice for the treatment of unilateral N1b PTC patients.

2.Efficacy of transfer learning artificial intelligence model based on ultrasound in evaluating the probability of malignancy of partially cystic thyroid nodule

Objective To investigate the feasibility and accuracy of an ultrasound-based transfer learning artificial intelligence model in predicting the malignancy probability of partially cystic thyroid nodules(PCTN).Methods A retrospective analysis was conducted on 246 patients with PCTN who had definitive pathological results and were admitted to Weihai Municipal Hospital,Cheeloo College of Medicine,Shandong University from January 2021 to December 2023.Patients were randomly divided into training and test cohorts at a ratio of 7:3.Ultrasonic image features of PCTN were evaluated,and independent risk factors were identified using multivariate logistic regression analysis,with the area under the curve(AUC)subsequently calculated.Additionally,five different pre-trained models-Inception_v3,EfficientNet,VGG19,ResNet50,and DenseNet121-were selected for transfer learning after data preprocessing using the PyTorch framework in Python.The AUC values of these models were calculated and compared.Results Solid portion greater than 50%,eccentric acute angle,ill-defined margin,spiculated or microlobulated margin,rim calcification,and microcalcification exhibited statistically significant differences(P<0.05)in distinguishing between benign and malignant PCTN.The AUC value derived from these independent risk factors was 0.843.Furthermore,among the five transfer learning models evaluated,the ResNet50 model demonstrated the highest diagnostic efficiency,achieving an AUC value of 0.903 2.Conclusion The ultrasound-based transfer learning artificial intelligence model demonstrated superior performance compared to traditional ultrasound image evaluation methods,enabling accurate prediction of the nature of PCTN and thereby reducing unnecessary ultrasound-guided fine needle biopsies.
3.A comparative study of gasless transoral vestibular robotic surgery and traditional open surgery for resection of thyroglossal duct cysts

Objective:To compare the clinical efficacies of gasless transoral vestibular robotic surgery and open surgery for the treatment of thyroglossal duct cysts.Methods:A retrospective analysis was conducted on patients with thyroglossal duct cysts who underwent surgical treatment in the Department of Otolaryngology at Sun Yat-sen Memorial Hospital, Sun Yat-sen University from August 2020 to October 2023. According to the differences in surgical methods, patients were divided into a robotic surgery group and an open surgery group. Statistical analysis was conducted on demographic data, surgical time, bleeding volumes, drainage volumes, postoperative complications, scar conditions, postoperative aesthetic scores, and recurrence rates of the two groups of patients. For normally distributed measurement data, inter-group comparison was done via independent-sample t-test. For non-normally distributed data, Mann-Whitney U test was applied. Comparisons of composition ratios or rates were performed using the χ2 test or Fisher′s exact probability method. Results:A total of 44 patients with thyroglossal duct cysts who met the inclusion and exclusion criteria were included, including 22 males and 22 females, aged from 3 to 73 years old. Both the robotic surgery group and the open surgery group had respectively 22 cases. The maximum diameter of tumors of the open surgery group was 3.45(2.50, 4.00) cm, while the robotic surgery group measured 2.50 (2.10, 3.20) cm, with the open group demonstrating significantly larger tumor dimensions compared to the robotic group ( Z=-2.329, P<0.05). Compared to the open surgery group, the robotic surgery group showed significantly more surgical time [105.00 (95.00, 135.00) min vs. 65.00(58.75, 76.25) min, Z=-5.377, P<0.05], postoperative hospitalization time [4.00 (3.75, 5.00) days vs. 3.00(2.00, 4.00) days, Z=-3.202, P<0.05] and bleeding volume [20.00 (10.00, 20.00) ml vs. 5.00 (5.00, 10.00) ml, Z=-4.769, P<0.05], but had less scar length [0 cm vs. 4.00 (2.00, 5.00) cm, Z=-6.097, P<0.05] and better postoperative cosmetic satisfaction [10.00 (10.00, 10.00) vs. 8.00 (6.75, 8.00), Z=-5.221, P<0.05]. With follow-up of 10.0-48.0 months, both the robotic surgery group and the open surgery group showed no recurrence. There was no significant difference in the follow-up time between the two groups ( Z=-0.224, P=0.823). Conclusion:Gasless transoral vestibular robotic surgery is safe and feasible for the treatment of thyroglossal duct cysts, with efficacy comparable to traditional surgery, which can provide a new option for the treatment of thyroglossal duct cysts.

5.ICU nurses' real experience of caring for patients undergoing shallow sedation and mechanical ventilation: a Meta-synthesis

Objective:To systematically evaluate the qualitative study of ICU nurses' experience in caring for patients undergoing shallow sedation and mechanical ventilation, and provide reference for improving the quality of shallow sedation nursing.Methods:Qualitative studies about ICU nurses' experiences of caring for patients undergoing shallow sedation and mechanical ventilation were systematically searched in Chinese and English databases such as Cochrane Library, Web of Science, Embase, PubMed, Ovid, Wanfang Data, China National Knowledge Infrastructure, China Biology Medicine disc, and VIP. The search period was from database establishment to June 30, 2024. Included literature was assessed according to the Joanna Briggs Institute Center for Evidence-Based Health Care quality assessment criteria for qualitative research. The results were summarized and analyzed through the aggregative meta-synthesis method.Results:A total of nine articles were included. Eight subcategories and were synthesized into three integrated results of difficulties and challenges, positive coping, and facilitation strategies for sedation management.Conclusions:ICU nurses encounter difficulties and challenges in caring for patients undergoing shallow sedation and mechanical ventilation. Although nurses themselves are able to respond positively, nursing managers need to provide additional sedation management support strategies to improve the quality of nursing.

Trigeminal neuralgia is a manifestation of orofacial neuropathic pain disorder,always deemed to be an insurmountable peak in the field of pain research and treatment.The pain is recurrent,abrupt in onset and termination similar to an electric shock or described as shooting.A poor quality of life has been attributed to trigeminal neuralgia,as the paroxysms of pain may be triggered by innocuous stimuli on the face or inside the oral cavity,such as talking,washing face,chewing and brushing teeth in daily life.The pathogenesis of trigeminal neuralgia has not been fully elucidated,although the microvascular compression in the trigeminal root entry zone is generally considered to be involved in the emergence and progression of the pain disorder.In addition,orofacial neuropathic pain restricted to one or more divisions of the trigeminal nerve might be secondary to peripheral nerve injury.Based on current hypotheses regarding the potential causes,a variety of animal models have been designed to simulate the pathogenesis of trigeminal neuralgia,including models of compression applied to the trigeminal nerve root or trigeminal ganglion,chronic peripheral nerve injury,peripheral inflammatory pain and center-induced pain.However,it has not yet been possible to determine which model can be perfectly employed to explain the mechanisms.The selection of appropriate animal models is of great significance for the study of trigeminal neuralgia.Therefore,it is necessary to discuss the characteristics of the animal models in terms of animal strains,materials,operation methods and behavior observation,in order to gain insight into the research progress in animal models of trigeminal neuralgia.In the future,animal models that closely resemble the features of human trigeminal neuralgia pathogenesis need to be developed,with the aim of making valuable contributions to the relevant basic and translational medical research.

Alzheimer's disease(AD)is a disease with clinical manifestations of learning and memory impairment,cognitive dysfunction,and language dysfunction,the pathogenesis of AD is complex,of which Aβ theory covers various mechamisms such as oxidative stress,inflammation,apoptosis and other mechanisms.Based on the Aβ mechanism and related signaling pathways,this study discusses the overview of typical Chinese medicines and their active ingredients in the prevention and treatment of AD.The aim is to provide insights and references for the development of traditional Chinese medicines for the prevention and treatment of AD.
10.Implementation and interpretation for proficiency testing of microbiological examination

Objective To understand the testing performance of drug manufacturers and testing institute in Yunnan province by organizing proficiency testing of microbiological examination of non-sterile drugs,and to improve the testing performance and quality management level of participants by interpretation of unsatisfactory results.Methods According to the requirements of relevant China National Accreditation Service for Conformity Assessment (CNAS),preparation values of positive or negative was determined as assigned values for qualitative items,and the median value was as assigned value for enumeration items.Using robust statistical methods and Z-score for assessment,if all the 4 items were satisfactory,it would be assessed as overall satisfactory performance,and contrariwise,it would be assessed as unsatisfactory performance.If partial item were satisfactory,it would be assessed as partially satisfactory performance.Results 152 laboratories participated in this proficiency testing program,116 laboratories obtained satisfactory performance and the satisfactory rate was 76.31%;33 laboratories obtained partially satisfactory performance,and the partially satisfactory rate was 21.71%;3 laboratories obtained unsatisfactory performance and the unsatisfactory rate was 1.97%.To classify the participants with type,the satisfactory rate for drug control institute was 88.23% and the satisfactory rate of drug manufacturers was 74.81%.Conclusion The testing performance of participating laboratories in microbiological examination of non-sterile drugs is generally good,which can ensure the smooth implementation of the program and the accuracy and effectiveness of the results.The testing capacity and quality management level of a few laboratories need to be improved.
