1.Nursing cooperation of implantation of modified capsular tension ring and foldable posterior chamber intraocular lens combined with phacoemusification in treatment of Marfan syndrome accompahied with lens subluxation

Objective To explore the nursing care of patients with Marfan syndrome accompanied with lens subluxation, and the clinical efficacy of phacoemusification combined with the implantation of modified capsular tension ring and foldable posterior chamber intraocular lens. Methods 11 patients (11 eyes) were performed the surgery. Uncorrected visual acuity preoperatively was (0.13±0.09). Best corrected visual acuity preoperatively was (0.20±0.17). Preoperative preparation, intraoperative observation and postoperative guidance were executed. The prognosis of these patients were observed. Results The operations were performed successfully. No intraoperative complications were founded. Mean uncorrected visual acuity at 2 year postoperatively was (0.60±0.28), and mean best corrected visual acuity was (0.70±0.28). Conclusions Careful preparation before surgery, close cooperation during surgery and the correct management after surgery were the important measures for the success of surgery.
2.Perioperative nursing care to patients with open angle glaucoma undergoing EX-PRESS drainage device implantation

Objective To investigate the key points in the care of patients with open angle glaucoma undergoing EX-PRESS drainage device implantation during perioperative period.Method Eighteen patients with open angle glaucoma underwent EX-PRESS glaucoma drainage device implantation and received individualized nursing care during perioperative period.Results Drainage device implantation was successfully performed in 18 patients.The mean operation time was(30.0±5.0)mins,ranged 25 to 45 mins.The postoperative intraocular pressure was stable.Conclusions EX-PRESS glaucoma drainage device implantation is an innovative and effective strategy in the treatment of open angle glaucoma.Comprehensive nursing during perioperative period is important to ensure the clinical efficacy.
3.Comparative study of efficacy of different types of artificial liver treatments in management of hepatitis B-associated subacute severe hepatitis

Objective To observe the clinical efficacy of plasma exchange(PE),PE combined with plasma bilirubin absorp-tion(PE+PBA),and PE combined with double plasma molecule absorption system(PE+DPMAS),to investigate the best treat-ment options for the patients of hepatitis B-associated subacute severe.Methods Totally 140 patients who had hepatitis B-associat-ed were randomly divided into a PE group,a PE+PBA group,and a PE+DPMAS group.The dinical symptoms and blood rontine, electrolytc and the main biochemical indexes were recorded both before and after treatment and compared among the three groups. Results The total effective rate was higher in the PE +DPMAS group than in the PE group and PE+PBA group,which were 70.8%,60.9%,67.4% respectively,but there was no significant difference(P >0.05).compared with before treatment,serum total bilirubin (TBIL),alanine aminotransferase (ALT),aspartic acid amino shift enzyme (AST),valley aminoacyl transfer peptidase (GGT),alkaline phosphatase (ALP),globulin (GLB)and other biochemical indexes decreased significantly after treatment in 3 groups (P <0.001).PA was increased in the PE group and decreased in the PE+PBA and PE+DPMAS group after treatment,the differences were significant(P <0.05).After treatment,PT was shortened in the PE group,the differences were significant(P <0.05),but there were no significant difference between the PE+PBA and PE+DPMAS groups(P >0.05).Serum K+ and Cl- was obviously declined after treatment in the PE group,there was significant difference(P <0.001).Serum Na+ was obviously declined after treatment between the PE + PBA and PE + DPMAS groups (P < 0.001 ),but the difference was no significant in the PE groups.Serum Ca2+ was significantly decreased in the three groups of patients after treatment(P <0.001).WBC,Hb and PLT were significantly statistical difference after treatment(P <0.05).Conclusion The three groups also can improve liver function and the treatment rate for the patients of subacute hepatitis B-associated severe hepatitis.PE+PBA and PE+DPMAS groups can effective-ly reduce two thirds of the overall usage of the plasma.PE+DPMAS groups that the quantity of Cl- ,Ca2+ ,Hb decline much least in the three groups show larger value in the clinical application.
4.Predictive value of plasma N-terminal-pro-brain natriuretic peptide in weaning patients from mechanical ventilation

Objective To evaluate the predictive value of N-terminal-pro-brain natriuretic peptide (NT-proBNP) in weaning patients from mechanical ventilation (MV).Methods Data of 42 patients supported with MV in intensive care unit (ICU) admitted to the Rui Jin Hospital from January through December in 2014 were retrospectively analyzed,and the causes for MV were recorded.According to the outcomes of weaning from MV after 48 hours,the patients were divided into two groups namely success group and failure group.Comparisons of fluid balance in 72 hours before spontaneous breathing trial (SBT),and comparisons of NT-proBNP1 levels at admission,NT-proBNP2 levels before SBT,NT-proBNP3 levels after 48 hours after SBT between two groups were carried out.And the receiver operating characteristic (ROC) curve for predicting weaning rate was plotted to find the optimal cut-off point of NT-proBNP2.Results In the total of 42 patients,there were 27 cases in success group and 15 cases in failure group.There were not statistically differences of NT-proBNP1 levels between success group and failure group (P =0.121).However,the NT-proBNP2 levels and NT-proBNP3 levels in failure group were significantly higher than those in success group (P =0.01,0.003).The area under curve (AUC) of the ROC curve of NT-proBNP2 levels to predict the failure of weaning was 0.862 (95% CI:0.753-0.971).When the optimal cut-off point of NT-proBNP2 was 715.5 pg/mL,the sensitivity and specificity were 93.3% and 74.1%,respectively.Conclusion The NT-proBNP2 levels before SBT have predictive value in weaning rate,and it can be used as one of the screening indicators for weaning.

Endothelial dysfunction was closely related with AS , NO bioavailability ( production and utilization of endothelial NO ) was decreased by oxidative stress , lipid infiltration , inflammatory factor expression , vascular tone alteration and so on , which play an important role in endothelial dysfunction .Enhanced arginine , activityand asym-metric dimethylarginine together with increased hyperhomocysteinemia all promote AS by intervening NO bioavail -ability.Diabetes mellitus, obesity, chronic kidney disease , smoking and so on also involved in AS via influencing NO bioavailability and NO level .
6.The analysis of psychological status and related factors for patients with chronic prostatitis

Objective To investigate the psychological status of patients with chronic prostatitis , and analyze the relat-ed factors. Methods Selected 200 cases of chronic prostatitis patients hospitalized ,applied the SCL-90 compared with the national norm, and chose 200 healthy males as healthy control group,the SAS,SDS score and anxiety,depression, and related factors analysis for existence anxiety mood ,depression in patients with chronic prostatitis were compared. Results The chronic prostatitis patients positive for SCL-90 project scale,anxiety,depression,the number of forced symptom,terror scores were respectively significantly higher than the norm group (P<0.05),in patients with chronic prostatitis SAS, SDS scores were significantly higher than that in healthy control group(P<0.05). 200 cases of chronic prostatitis patients appear anxiety accounted for 44.00%,Depression accounted for 36.00%. respectively,were signifi cantly higher than that in healthy control group (P<0.05). The common factors of unmarried, low cultural level, farmer, doctor many times,at their own expense for the occurrence of anxiety and depression in patients with chronic prostatitis. Conclusion The patients with chronic prostatitis have anxiety ,depression and other negative psychological status,its causes are related closely to the unmarried patients with low education level,farmer,doctor many times,at their own expense.
7.The mediating effect of self-efficacy on mindfulness and job burnout in psychological counselors

Objective To explore the mediating role of self-efficiency between mindfulness and job burnout in psychological counselors. Methods 201 psychological counselors were tested by the demograph-ic data Questionnaire,Maslach Burnout Inventory(MBI-GS),General Self-Efficacy Scale(GSES),Mindful-ness attention and mindfulness scale(MAAS). The structural equation model was used to explore the media-ting role of self-efficacy in the relationship between mindfulness and job burnout. Results Mindfulness in psychological counselors was negatively correlated with emotional exhaustion and depersonalization(r=-0.50,-0.43;P<0.01),while there was a significant positive correlation with personal accomplishment(r=0.30,P<0.01). Self-efficiency was negatively correlated with emotional exhaustion and depersonalization(r=-0.30,-0.32;P<0.01),while there was a significant positive correlation with personal accomplishment(r=0.40,P<0.01). Mindfulness was positively correlated with self-efficiency(r=0.31,P<0.01). The mediating effect showed that self-efficacy played a partial mediating effect on the relationship between mindfulness and job burnout(effect value:-0.071,95%CI= -0.027--0.141),and the mediation effect accounted for 14.82%of the total effect. Conclusion Self-efficiency plays a partial mediating role between job burnout and mind-fulness in psychological counselors. Improving the level of mindfulness and self-efficiency have positive effect on job burnout of the psychological counselors.
8.Alteration of circulating endothelial cells from acute promyelocytic leuke-mia patients before and after treatment and its influential factors

AIM:To determine the biological feature of circulating endothelial cells (CECs) in acute promye-locytic leukemia ( APL) patients before and after treatment , and to analyze the relationship between CECs and the clinical characteristics .METHODS: The CECs were sorted from peripheral blood by magnetic-activated cell sorting and then counted by 3-color flow cytometry.The cells were identified by immunofluorescence staining for the expression of CD 146, CD31, CD144, VEGFR-2, CD45 and CD133.The CECs were cultured in vitro, and the tube formation and colony-forming rate were determined .RESULTS:Increased quantity of CECs was observed in CD 34 positive group and group with WBC >10 ×109/L (P<0.05).The quantity of CECs had a significant difference among low risk , medium risk and high risk groups (P<0.05).The positive rate of CD133 and quantity of CECs significantly reduced in 32 APL patients when they gain complete remission after treatment (P<0.05).The amount of tube formation and colony-forming rate were significant-ly reduced after treatment (P<0.05).The ratio of CECs quantity from APL patients after treatment to that before treatment had a negative correlation with arsenic concentration in urine on day 7 during As2O3 treatment (P<0.05).CONCLU-SION:Accurately counting CECs may be helpful for evaluating prognosis and designing treatment strategy .
9.Objective sleep assessment in patients with chronic insomnia using cardiopulmonary coupling analysis and its correlation with cognitive function

Objective To evaluate the objective sleep status of patients with chronic insomnia by cardiopulmonary coupling (CPC) technique, and evaluate the characteristics of cognitive dysfunction to explore the correlation between objective sleep and cognitive dysfunction in patients with chronic insomnia. Methods Forty-three patients with chronic insomnia, admitted to our hospital from October 2017 to April 2019, were enrolled in our study;15 age-, gender-and education-matched healthy volunteers were recruited as control group. All subjects followed their daily routine at home and completed CPC examination. Montreal Cognitive Assessment (MoCA), Auditory Vocabulary Learning Test (AVLT), Trail Making Test (TMT) and Stroop Color Word Test were used to evaluate the general and single cognitive functions, respectively. The correlation of objective sleep with cognitive function was analyzed. Results (1) As compared with those in the control group, high frequency coupling (stable sleep) ratio was significantly decreased, low frequency coupling (un-stable sleep) ratio and extremely low frequency coupling (rapid-eye-movement sleep/waking) ratio were significantly increased, and latency of high frequency coupling was significantly prolonged in chronic insomnia group (P<0.05). (2) Chronic insomnia group had significantly lower MoCA total scores than control group (P<0.05), specifically manifested as decrement of visuospatial ability and execution and attention abilities; specific cognitive test showed that chronic insomnia group performed worse in immediate recall, and had delayed recall of AVLT, longer time consumption in TMT-B, smaller number of wired arrival numbers, and longer time consumption in Stroop color word test than the control group, with significant differences (P<0.05). (3) There was a correlation between CPC sleep structure and Cognitive Function Scale scores in patients with chronic insomnia. Conclusion In patients with chronic insomnia, stable sleep is reduced, un-stable sleep and rapid-eye-movement sleep/waking are increased; the impaired cognition domains are visual space and executive function, attention and memory; disturbed sleep structure aggravates the memory and execution impairment of patients with chronic insomnia.
10.Clinical analysis of 3 adult-onset patients with genetically diagnosed familial neuronal intranuclear inclusion disease

Objective:To analyze the clinical characteristics of adult-onset patients with familial neuronal intranuclear inclusion disease (NIID).Methods:The clinical data of 3 patients with familial NIID genetically diagnosed in Department of Neurology, Sixth Affiliated Hospital of Guangzhou Medical University in August 2021, January 2022, and August 2022 were collected. Their clinical manifestations, imaging features, pathological features, Notch2 N-terminal-like C ( NOTCH2NLC) gene mutation characteristics, treatment methods and prognoses were summarized retrospectively. Results:The age of these 3 patients was 73, 67, and 65 years, and the onset age was 68, 64, and 56 years, respectively. The clinical manifestations are highly heterogeneous. In patient 1, the nervous centralis, peripheral nerves and autonomic nerves were involved, appearing dementia, epilepsy, Parkinson's syndrome, muscle weakness and uremia; in patient 2, only the nervous centralis were involved, presenting symptoms of Parkinson's syndrome; in patient 3, peripheral nerves and autonomic nerves were involved, prominently presenting with repeated vomiting. Skull diffusion weighted imaging (DWI) showed asymmetric high signal at the dermo-medullary junction in 3 patients. Acidophilic inclusion bodies in some sudoriferous duct epithelial cells, and vascular endothelial nucleus were found in the skin biopsy of 2 patients. All 3 patients completed NOTCH2NL gene test, and all had GGC repeat amplification mutations with mutation frequency>134. These 3 patients were mainly treated symptomatically, and the disease was still progressed gradually. Conclusion:The clinical manifestations of familial NIID are highly heterogeneous; skull MRI characteristic changes and skin biopsy can help to diagnose NIID and NOTCH2NL gene detection can diagnose NIID.
