1.Comparison of the excursion of blood glucose in type 2 diabetes injected glargine and human isophane insulin

Objective To compare the excursion of blood glucose (BG) in the type 2 diabetes mellitus treated with oral antidiabetic drugs (OADs) plus glargine or human isophane insulin (HII). Methods A 1 : 1 randomization schedule assigned 30 type 2 diabetics inadequately controlled on OADs (fasting BG>9.0 mmol/L and HbA1C > 8.5%) to 2 groups additionally treated with glargine or HII. The insulin dose was titrated to achieve fasting capillary BG<6.0 mmol/L. Montoring BG with continuous glucose monitoring system, then the standard deviation of BG (SDBG), maximal excursion of BG (LAGE) and coefficient of variation (CV) of fasting plasma glucose (FPG) were calculated. Results SDBG (1.49±0.35 vs 1.73±0.46), LAGE (3.23±0.76 vs 3.73± 1.00) and CV-FPG (17.26±2.24 vs 20.33±3.21) were lower in glargine group than those in HII group (P< 0.05). No difference could be found in hypoglycaemia between two groups. Conclusion OADs plus glargine could make blood glucose more stable than OADs plus HII without increasing the incidence of hypoglycaemia.
2.Impact of high-load atorvastatin on autoimmunity in patients with unstable angina

Objective To explore the impact of high‐load atorvastatin on T cell subsets in patients with unstable angina (UA) after percutaneous coronary intervention (PCI) .Methods One hundred and eighty patients with UA were randomly divided into high‐load atorvastatin group ,ordinary‐load atorvastatin group and routine group ,60 cases in each group .The ratios of CD4+ T cell , CD8+ T cell and the frequencies of CD4+ CD25+ Treg were detected in 3 groups 1 day before PCI and 1 week ,1 month and 6 months after PCI by flow cytometry analysis .Results Different doses of atorvastatin reduced the ratio of CD4+ T cells and in‐creased the ratio of CD8+ T cells and also the frequencies of CD4+ CD25+ Treg after PCI for 1 week ,1 month and 6 months .The longer the time to take atorvastatin ,the more obvious the effect was(P<0 .05) .The ratios of CD4+ ,CD8+ T cells and the frequen‐cies of CD4+CD25+ Treg after PCI for 1 month and 6 months in high‐load atorvastatin showed significant difference compared with those in ordinary‐load atorvastatin group and routine group(P<0 .05) .Conclusion Atorvastatin could regulate the balance of T cell subsets in patients with UA ,and thus it may reduce the UA onset and the treatment effect of high‐load atorvastatin is more sig‐nificant .
3.Effects of Buzhong Yiqi Decoction on the expressions of MRP, LRP and P-gp in transplantation tumor of A549 tumor-bearing nude mice

Objective:To investigate the effects of Buzhong Yiqi Decoction on the expressions of multidrug resistance-related protein (MRP), lung resistance-related protein (LRP), and P-glycoprotein (P-gp) in transplantation tumor of A549 tumor-bearing nude mice.Methods:A549 cells were cultured to establish xenograft models of tumor-bearing nude mice. According to the random number table method, they were divided into blank control group, cisplatin group, cisplatin + Buzhong Yiqi Decoction low-dosage group, cisplatin + Buzhong Yiqi Decoction medium-dosage group, cisplatin + Buzhong Yiqi Decoction high-dosage group. After 25 days of corresponding drug intervention, the mRNA expressions of MRP, LRP and P-gp in transplantation tumors were detected by Real-Time PCR, and Western blot was used to detect the expressions of MRP, LRP and P-gp proteins in xenograft tissues in each group.Results:Compared with blank control group, the mRNA and protein expressions of MRP, LRP and P-gp in cisplatin group were significantly down-regulated ( P<0.05). Compared with cisplatin group, the mRNA and protein expressions of MRP, LRP and P-gp in the cisplatin + Buzhong Yiqi Decoction low-dosage group, cisplatin + Buzhong Yiqi Decoction medium-dosage group, cisplatin+Buzhong Yiqi Decoction high-dosage group decreased ( P<0.05). Conclusion:Buzhong Yiqi Decoction can inhibit the expressions of MRP, LRP and P-gp in transplantation tumor of A549 tumor-bearing nude mice and without dosage dependence.
4.Clinical analysis of the correlation between the expression of soluble B cell maturation antigen and the efficacy of chimeric antigen receptor T cell targeting B cell maturation antigen in patients with multiple myeloma

Objective:The effect of bone marrow soluble B cell maturation antigen (sBCMA) expression on the efficacy and side effects of chimeric antigen receptor (CAR) -modified T-cell-targeting B cell maturation antigen (BCMA) in patients with multiple myeloma (MM) .Methods:This study involved 29 patients with relapsed or refractory MM (RRMM) who received humanized anti-BCMA CAR-T cell clinical trials from January 2018 to December 2021. The expression of sBCMA in bone marrow before and after anti-BCMA CAR-T cell treatment was detected by flow cytometry and compared.Results:①Two months after BCMA CAR-T cell treatment, 20 patients (68.97%) achieved an overall response (OR), whereas nine patients had stable disease (SD) or miner emission (MR). ②The expression of sBCMA in the bone marrow of 20 patients with OR was higher before treatment than after [26 926 (18 215, 32 488) ng/L vs 9 968 (6 634, 11 459) ng/L; P<0.001]; no significant difference was observed in patients with MR and SD [41 187 (33 816, 47 046) ng/L vs. 33 954 (31 569, 36 256) ng/L; P=0.145]; sBCMA expression in patients with OR before CAR-T cell treatment was lower than in patients with MR and SD ( P=0.005). ③No significant linear correlation was found between the peak value of CAR-T cells and sBCMA expression in the bone marrow of all 29 patients with RRMM ( R2=0.035, P=0.330). ④No significant difference in sBCMA expression was found between grades 0-1 CRS group (13 patients) and grades 2-4 CRS group [16 patients; 32 045 (18 742, 40 801) ng/L vs 29 102 (24 679, 38 776) ng/L, P=0.879], nor between grade 0 ICANS group (22 patients) and grade 1-3 ICANS group [seven patients; 30 073 (19 375, 40 065) ng/L vs 33 816 (22 933, 43 459) ng/L, P=0.763]. Conclusion:sBCMA expression in the bone marrow is related to the efficacy of BCMA CAR-T cell therapy in patients with RRMM, but is not significantly correlated with the severity of adverse events. It may serve as a predictive biomarker for the efficacy of BCMA CAR-T cell therapy in these patients.
5.Analysis on clinical effect of photodynamic therapy combined with CO2 laser in treatment of vaginal intraepithelial neoplasia

Objective To analyze the clinical efficacy and safety of photodynamic therapy(PDT)com-bined with CO2 laser in treating vaginal intraepithelial neoplasia(VaIN)to provide reference for the treatment scheme of the patients with VaIN.Methods A total of 80 patients with definitely diagnosed VaIN by colpo-scopic biopsy in Henan University People's Hospital from June 2022 to February 2023 were selected and di-vided into the PDT combined CO2 laser group and CO2 laser group by the random number table method,40 cases in each group.The follow up lasted for 6 months.The cure rate,negative conversion rate of human papillomavir-us(HPV),adverse reactions,recurrence and progression were compared between the two groups.Results After 3,6 months of treatment,the cure rates of VaIN in the PDT combined CO2 laser group were 77.50%and 82.50%,respectively,which in the CO2 laser group were 75.00%and 70.00%respectively,and the differ-ences between the two groups were not statistically significant(P>0.05).There was no statistically signifi-cant difference in the cure rates of low-grade squamous intraepithelial lesion(LSIL)and high-grade squamous intraepithelial lesion(HSIL)after 3,6 months of treatment between the two groups(P>0.05).There was no statistically significant difference in the negative conversion rate of HPV after 3 months of treatment be-tween the PDT combined with CO2 laser group and CO2 laser group(71.79%vs.56.41%,P>0.05).How-ever,after 6 months of treatment,the negative conversion rate of HPV in the PDT combined CO2 laser group was significantly higher than that in the CO2 laser group(82.05%vs.58.97%,P<0.05).In the CO2 laser group,one case experienced vaginal bleeding during treatment process,two cases appeared mild vaginal wall adhesion.No obvious adverse reaction was observed in the PDT combined with CO2 laser group.After 6 months of treatment,there was no case with progression of lesion in both groups;there were one case(3.23%)of relapse in the PDT combined CO2 laser group and three cases(10.00%)of relapse in the CO2 laser group,and the difference was not statistically significant(P>0.05).Conclusion PDT combined with CO2 laser for treating VaIN has a high cure rate and HPV clearance rate,which is a safe and effective method for treating VaIN.
