1.POT1 Gene Expression in Patients of Acquired Aplastic Anemia with Different TCM Syndromes and Correlation Study

Objective To observe the mRNA expression levels of POT1 in peripheral blood mononuclear cells of acquired aplastic anemia patients with different syndrome, and explore its relationship with acquired aplastic anemia and its TCM syndrome. Methods Peripheral blood mononuclear cells of 52 cases with acquired aplastic anemia and 20 cases as control group were collected to detect mRNA expression of POT1 by using real-time quantitative polymerase chain reaction (RT-qPCR), and its relation with TCM syndrome was analyzed. Results The expression levels of POT1 mRNA in patients with acquired aplastic anemia were lower significantly than control group (P<0.05). The expression levels of POT1 mRNA in patients with deficiency of kidney-yin were lower than patients with deficiency of kidney-yang, and it was lowest in patients with kidney deficiency of both yin and yang. There was significant correlation between the expression levels of POT1 mRNA and age (r=0.374, P=0.038). Conclusion The changes in expression levels of POT1 play a role in the pathogenesis of acquired aplastic anemia. There is correlation between mRNA expression level of POT1 in peripheral blood mononuclear cells and TCM syndrome.

4VO rat models were used. The remedy Zhenxuanyin, composed of Rhizoma Gastrodia, Poria, Radix Lugustici Chuanxiong, divided into 3 dosages, were infused 30 minutes after total cerebral infarction., The cerebralblood flow was marked with 123 I- IMP 24 hours later. Results showed Zhenxuanyin in dosages of 0. 03g/kg, 0. 3g/kg, 1g/kg, 3g/kg all markedly increased cerebral blood flow in which the dose 0. 3g/ kg can restore the blood flow to a normal level.
3.Prevalence of retinopathy of prematurity in 375 cases of preterm infants

Objective To study the prevalence of retinopathy of prematurity (ROP) in preterm infants in Xiamen, Fujian. Design Retrospective case series. Participants From Oct. 2004 to Dec. 2008, 375 infants with birth weights ≤2500g or gestational age ≤32w were screened in Xiamen the First Hospital. Methods Initial examinations with the binocular indirect ophthalmoscopy and the scleral depression began at 4~6 weeks after birth or at 32 weeks postconception. All infants were followed-up until a stable retinal situation was reached. Main Outcome Measures The prevalence of ROP. Results Retinopathy of prematurity was detected in 10.93% of 375 neonates, including 18 eyes of stage 1, 48 eyes in stage 2, 14 eyes in stage 3, 2 eyes in AP-ROP. The prevalence of ROP in the infants with the birth weight ≤1000g, 1001-1500g and 1501-2000g was 45%, 17.07%, 7.01%, respectively; the occurrence rate of ROP in the infants with the gestational age ≤28 weeks, 29-30 weeks, 31-32 weeks, 33-34 weeks was 34.48%, 2.07%, 12.71%, 8.25%, respectively. Conclusions Earlier screening for ROP is very important. Low birth weight and young gestational age are the most important risk factors in the development of ROP.
4.Influence of traditional medicine prescription on Flt3 and N-ras mRNA expression in leukemic stem cells

BACKGROUND:There is a group of leukemic stem cell (LSC) in patients with leukemia. These malignant stern cells, although rare, but self-renewal, with a certain degree of differentiation is the existence of leukemic cells and causes of their continued proliferation. Intervention of leukemia-specific targeted treatment drugs has become a research hotspot in recent years. OBJECTIVE: To investigate the influence of traditional medicine compound on expression of flt3 and N-ras mRNA in LSCs. DESIGN, TIME AND SETTING: A randomized, grouping experiment was performed at the Affiliated Hospital of Shandong University of Traditional Chinese Medicine from September 2007 to December 2008.PARTICIPANTS: A total of 50 cases of acute myelocytic leukemia admitted to Department of Hematology, Affiliated Hospital of Shandong University of Traditional Chinese Medicine and Qilu Hospital of Shandong University between 2006 and 2007 were selected, including 5 cases of type FAB, 15 of M2, 9 of M4 and 21 of M5.METHODS: Original prescription: Huangqi, Hedyotic diffusa, Xiaoji, Taizishen, Banzhilian, Pugongying, Rehmannia dride rhizome, Huangjing, Nuzhenzi, ecliptae herba, Tiandong, Maidong, Baizhu, Fuling, Liquorice root. Fuzheng prescription: Huangqi, Hedyotic diffusa, Xiaoji, Banzhilian, Pugongying, ecliptae herba. Preparation: the above-mentioned three groups of drugs were prepared into 1 g/mL medicine liquid by Laboratory of Chinese Drug Preparation of Affiliated Hospital of Shandong University of Traditional Chinese Medicine. When it was negative in sterility test, the liquid was filtrated and sterilized. 5 mL bone marrow was separately harvested from M1, M2, M4, and M5 leukemia patients, diluted and placed in lymphocyte isolation solution. LSCs were purified by magnetic activated cell sorting and flow cytometry. The cell concentration was adjusted to 2×108/L and divided into 4 groups: control (no treatment), and three experimental groups (treated separately with original, Fuzheng, Quxie prescriptions at final concentration of 100 mg/L). The cells were cultured for 48 additional hours. MAIN OUTCOME MEASURES: RT-PCR was used to detect the mRNA expressions of Flt3 and N-ras in LSCs. RESULTS: The expression of flt3 in three experimental groups was significantly decreased compared with control group (P < 0.05), in particular, original prescription group decreased the most (P<0.05), and no significant difference was found between Fuzheng and the Quxie prescription groups (P>0.05). The expression of N-ras in four groups was similar to Flt3. CONCLUSION: Flt3 and N-ras mRNA were overexpressed in LSCs. Chinese medicine original prescription and its Fuzheng or Quxie prescriptions decreased Flt3 and N-ras mRNA expression.

Endothelial dysfunction was closely related with AS , NO bioavailability ( production and utilization of endothelial NO ) was decreased by oxidative stress , lipid infiltration , inflammatory factor expression , vascular tone alteration and so on , which play an important role in endothelial dysfunction .Enhanced arginine , activityand asym-metric dimethylarginine together with increased hyperhomocysteinemia all promote AS by intervening NO bioavail -ability.Diabetes mellitus, obesity, chronic kidney disease , smoking and so on also involved in AS via influencing NO bioavailability and NO level .
6.Analysis of the clinical characteristics and risk factors of death of accidential injuries in pediatric intensive care unit

Objective:To explore the clinical characteristics and prognostic risk factors of accidental injuries in pediatric intensive care unit(PICU).Methods:The children with accidental injuries admitted to the PICU at Shanghai Children′s Hospital from January 2017 to December 2019 were analyzed retrospectively.The patients were divided into survival group and death group according to outcome.We collected the patients′ clinical data and laboratory indexes, and analyzed the differences between two groups.Multivariate Logistic regression was used to screen the risk factors of death and receiver-operating characteristic(ROC)curve was used to determine the threshold.Results:(1)A total of 253 children were included, accounting for 5.2% of PICU hospitalization during the same period.There were 137 males(54.2%)and 116 females(45.8%), with an average age of 45(19, 96)months.The top three causes of accidental injuries were fall injury in 82 cases(32.4%), traffic accident in 70 cases(27.7%)and foreign body in 39 cases(15.4%). There were 22 cases of deaths, with a fatality rate of 8.7%.(2)The mean arterial pressure at admission in the death group was lower than that in the survival group( P<0.05), and the total length of stay was longer( P<0.001). There were significant differences in Glasgow coma scale, Pediatric Risk of Mortality Scoring Ⅲ(PRISM Ⅲ)between the two groups( P<0.001). In the death group, and the blood lactic acid(LA) significantly increased( P<0.001), hemoglobin and fibrinogen were lower than those in the survival group, the activated partial thromboplastin time and prothrombin time(PT)were significantly prolonged( P<0.001). (3)Multivariate Logistic regression analysis showed that PRISM Ⅲ score, LA and PT were independent risk factors for the death of accidental injuries.The area under curve(AUC)of PRISM Ⅲ score was 0.987(95% CI 0.976-0.999, P<0.001)and the cut-off value was 11 with a sensitivity of 100% and a specificity of 95.7%.The AUC of LA was 0.886(95% CI 0.810-0.961, P<0.001)and the cut-off value was 3.1 mmol/L with a sensitivity of 85.7% and a specificity of 77.3%.The AUC of PT was 0.835(95% CI 0.730-0.941, P<0.001)and the cut-off value was 13.9 s with a sensitivity of 86.6% and a specificity of 72.7%. Conclusion:The main accidental injuries in PICU of our hospital in the past three years are falling injuries, traffic accidents and foreign body injuries.PRISM Ⅲ score, LA and PT at admission are independent risk factors for predicting death.
7.Correlation of Expressions of P170,TOPⅡ and Proliferation in Gliomas

Objective To investigate the correlation of the expressions of P170,topoisomerase Ⅱ(TOPⅡ)and the proliferation in gliomas.Methods Ki-67 was used as the marker of the proliferation in gliomas.The expressions of P170,TOPⅡ and Ki-67 were tested by S-P immunohistochemical technique using monoclonal antibody to their protein in 54 cases of low-grade gliomas(WHO Ⅰ~Ⅱ grade)and 54 cases of high-grade gliomas(WHO Ⅲ~Ⅳ grade),analysis of the clinical and follow-up were performed.Results The expressions of P170,TOPⅡ and Ki-67 had significant differences between low-grade and high-grade gliomas(P<0.01).Correlation analysis showed that the expression of P170 was negatively related with Ki-67(r=-0.276,P=0.019);the expression of TOPⅡ was positively related with Ki-67(r= 0.637,P<0.001);there was no correlation between the expressions of P170 and TOPⅡ.Conclusion The expressions of P170 and TOPⅡ are associated with the malignant prognosis in gliomas.

9.Analysis on performance of primary medical service in Zhejiang′s county-level medical alliances from the perspective of supply and demand balance

Objective:Based on the perspective of supply and demand balance, to analyze the implementation effects of primary-feature medical services measures in medical alliances.Methods:From July to September 2019, two regions with different levels of economic development were chosen, and from each region one leading hospital and two community health service centers/township hospitals were selected from its medical alliance. A total of 213 medical staff and 703 residents were surveyed, to learn their awareness and performance appraisal of these feature services.Frequency and proportion were calculated in a descriptive statistical analysis. The scores of the performance appraisal of both supply and demand sides were tested by independent sample t. Results:In the performance appraisal of such measures, the average scoring by medical staff was 4.39±0.07, and that by residents was 3.85±0.06; the residents were more concerned with service capabilities and contents, while medical staff were more concerned with service content and delivery mode; supply and demand sides were found with consensus in terms of improving the medical service quality of chronic diseases in primary level and service accessibility.Conclusions:The county-level medical alliances should focus on enhancing their capacity of primary medical services, and improving their primary service mode with the help of medical insurance institutions. Meanwhile, it is imperative to optimize the primary medical service policy in view of supply and demand balance.
10.Prevalence and relevant factors of physical and emotional abuse by parents among children with autism spectrum disorder

Objective:
To explore the prevalence and relevant factors of physical and emotional abuse by parents among children with autism spectrum disorder (ASD), so as to provide basis for intervention program of children abuse.
Methods:
A total of 221 ASD children from 3 special education institutions in Tangshan were investigated from March to October in 2021, 395 non ASD children from two kindergartens in urban and rural areas were selected by convenient sampling. Parents of these children were invited for online and on site questionnaire survey. The self designed violence questionnaire, Childhood Autism Rating Scale and Patient Health Questionnaire-9 were used to assess violence, severity of autism, depression of parents. Chi square test, Fisher s exact probability method and Logistic regression were used to analyze the influencing factors of violence.
Results:
About 81.9% of children with ASD and 72.9% of non ASD children experienced violence( P <0.05). The reported rates of physical and emotional violence in ASD children were 74.2% and 73.8% respectively, which in non ASD children were 58.7% and 65.8% respectively. There were significant differences in the 3 types of violence rate between the two groups ( P <0.05). Multivariate Logistic regression analysis showed that boys ( OR =1.70, 95% CI =1.12-2.60), annual per capita income <10 000 yuan( OR =2.43, 95% CI =1.45- 4.08 ), and parental depression ( OR mild =11.01, 95% CI =5.38-22.49; OR moderate =69.97,95% CI =24.25-201.93) were the risk factors for child violence exposure; ASD disease ( OR=1.96,95%CI =1.32-2.92), older age ( OR=1.19, 95%CI =1.01-1.41) and parental depression( OR mild =7.83, 95% CI =3.67-16.74; OR moderate =14.37,95% CI =6.17-33.46) were risk factors for physical violence; boys ( OR =1.62, 95% CI =1.11-2.36), mothers who work in manual labor ( OR=1.68, 95%CI =1.09-2.59) and parental depression ( OR mild =7.69, 95% CI =3.74-15.81; OR moderate =25.37, 95% CI =10.80-59.63) were risk factors for emotional violence( P < 0.05 ).
Conclusion
The reported rate of parental violence against children with ASD is high. Mental health promotion and social support for families with ASD should be strengthened.
