2.Medical Ethical Analysis of Management Mode of Correspondence of Potencyscale- Taking the Department of Neurology as an Example

Objective:To explore the effect of management mode of correspondence of potencyscale on impro-ving the nurses′job satisfaction and patient satisfaction in the department of neurology and analyze the medical eth-ics. Methods:We selected 37 nurses in the department of neurology. From July 2013 to March 2014, they were provided with the traditional management mode, whereas from March 2014 to June 2015 with management mode of correspondence of potencyscale. The questionnaire survey was conducted to measure the job satisfaction of nurses and the satisfaction degree of patients. Results:After the implementation of management mode of correspondence of potencyscale, the research indexes were significantly higher than before. Conclusion:The management mode of correspondence of potencyscale can improve the nursing staff′s work enthusiasm, patient satisfaction, and the quali-ty of care. Meanwhile, it is the specific practice of nursing ethics.
3.Preparation and application of polyclonal antibody against ARIP1

Objective To prepare anti-activin receptor-interacting protein 1 (ARIP1) polyclonal antibody.Methods The GST-ARIP1 fusion protein was expressed in E.coli BL21.A polyclonal antibody against ARIP1 was obtained by immunizing a rabbit with the purified GST-ARIP1 and the localization of mature ARIP1 protein in mouse brain tissues was detected by immunohistochemistry using the prepared anti-ARIP1 antibody.Results Anti-ARIP1 polyclonal antibody could bind specifically with recombinant ARIP1,but not recombinant ARIP2.Immunohistochemical staining showed that ARIP1 mainly expressed in hypothalamus and hippocampus using the prepared anti-ARIP1 antibody.Conclusion The polyclonal antibody against ARIP1 has been successfully prepared and can be used to do immunohistochemical analysis for ARIP1 protein expression.
4.Effect of BmK-9-(2) on cardiac sodium channels in Guinea-pig ventricular myocytes

Objective:To investigate the effect of the BmK-9-(2) on the sodium current in guinea-pigventricular myocytes. Methods:The whole cell patch-clamp technique was used. Results:The BmK-9-(2)increased the magnitude of the peak inward sodium current (27 %) ,peak sodium conductace (17 %) and theprolong time course of sodium channel inactivation (8%) and there was no effects on calcium channels.Conclusion:BmK-9-(2) can activate sodium channel and prolong the inactivated course of myocytes.
5.Association of serum components of GH axis with GHR exon 3 polymorphism in idiopathic short stature children

Objective To investigate the possible association of circulating components of GH-IGFs-IGFBPs system with the GHR-exon 3 genotype in idiopathic short stature (ISS) children. Methods Genomic DNA was extracted and isolat-ed from peripheral leukocytes in 108 ISS children. GHR-exon 3 polymorphism was analyzed with multiplex poly-merase chain reactions (PCR) assay. According to the results of genotype, ISS children were divided into GHRfl group and GHRd 3 group. The height and weight were recorded in two groups. The body mass index (BMI) and BMI standard deviation score (SDS) were measured. The serum levels of insulin-like growth factor (IGF)-1, IGF-binding protein (IGFBP)-3, IGF-1 SDS and IGFBP3 SDS were calculated. GH stimulation test was used to measure the serum GH peak value. Fifty-five ISS chil-dren were treated with recombine human GH [0.15 IU/(kg·d)] for three months to analyse the association of IGF-1 response of GH treatment and genotypes. Results There were 63 GHRfl and 45 GHRd3 in 108 ISS children. There were no signifi-cant differences in BMI, IGF-1, IGFBP3, GH peak, IGF-1 SDS and IGFBP3 SDS between two groups (P>0.05). Multiple stepwise regression analysis showed that age, IGFBP3, lg (BMI) and lg (GH peak) were influencing factors of lgIGF-1 (P<0.05). In 55 ISS children treated with rhGH, there were 34 cases of GHRd3. The differences of △IGF-1 and △IGF-1 SDS were higher in GHRd3 group than those of GHRfl group (n=21). Conclusion The GH sensitivity may be a risk factor in ISS children, which may not be related with GHR polymorphism.
6.Effect of purification of the effective fractions of Sanqi Qilian Decoction on atrophic gastritis

AIM: To investigate the adsorptive and separative properties of 5 kinds of macroporous adsorbing resins to the total saponins and alkaloids, which treated atrophic gastritis with Sanqi Qilian Decoction (Radix et Rhizoma Notoginseng, Radix Astragali, Rhizoma Coptidis.) METHODS: The adsorption capacities, contents and recoveries of the total saponins and alkaloids were selected as the evaluating quotas. The determination of the contents of the total saponins and alkaloids and the investigation into leakage and elution curve were completed with UV-VIS spectrophotometric method. RESULTS: AB-8 resin had the best adsorptive and separative properties to the total saponins and alkaloids in Sanqi Qilian Decoction. The applicable technology was as follows: the concentration of original solution of the extract of Sanqi Qilian Decoction was 40 mg/mL; the largest adsorption capacities on resin to the total saponins and alkaloids were 5.28 mg/mL and 12.38 mg/mL respectively; the eluant was 50% ethanol with 8 times of resin volume, and the elution velocity was 2 mL/min. AB-8 resin could be used for 4 times repeatedly after being reproducted by 95% ethanol and 1 mol/L NaOH. CONCLUSION: Under the process condition which had been selected above, AB-8 resin can be used successfully to adsorb and separate with over 20% of the content of the total saponins Sanqi Qilian Decoction, and 30% of the content of the total alkaloids in the 50% ethanol eluate, and with over 75% of total saponins recovery and 65% of total alkaloids recovery respectively.

Objective:To study the relationship of expressions of E-cadherin and ?-catenin proteins and gastric mucosa intestinal metaplasia and gastric carcinoma.Methods:The immunohistochemical Envision method was used to determine the expressions of E-cadherin and ?-catenin in 30 cases of simple intestinal metaplasia(SIM) ,38 cases of atypia intestinal metaplasia(AIM) and 55 cases of gastric carcinoma.Results:The reduced expression rates of E-cadherin and ?-catenin in gastric carcinoma were significantly higher than that in SIM(P0.05) .Conclusion :E-cadherin and ?-catenin proteins take part in the transformation from intestinal metaplasia to carcinoma in gastric mucosa,AIM is a kind of precancerosis of gastric carcinoma.

Urinary microquantitative albumin (UALb), immunoglobulin G (UIgG) ?2-microglo-bulin (U?2m), and Tamm-Horsfall protein (THP) were measured in 36 chronic B hepatitis (CHB) and 23 liver cirrhosis (LC) patients with radioimmunoassay. 109 healthy normal subjects served as controls. Objective To detect the expression of matrix metalloproteinase-9 (MMP-9) gene in peripheral blood of patients with oral paraquat (PQ) poisoning and evaluate its predictive value for their prognosis.Methods Thirty-seven cases of oral PQ poisoning admitted to Linyi People's Hospital from January 2013 to June 2014 were enrolled,and they were divided into survival group (26 cases) and death group (11 cases) according to the survival sitnation in 28 days after poisoning;a healthy control group included 10 healthy people selected in the same period.The peripheral blood 3 mL was collected from each PQ patient on the 1st and 3rd day after admission,and in the healthy control group,3 mL peripheral venous blood was obtained under fast on the day for physical examination.The MMP-9 gene expression of peripheral blood mononuclear cells (PBMCs) was detected by reverse transcription-polymerase chain reaction (RT-PCR) methods;the serum MMP-9 concentration was determined by using enzyme-linked immunosorbent assay (ELISA);the serum PQ level was detected by high performance liquid chromatography (HPLC),and the amount of poison orally taken was recorded.The correlations between PQ amount orally taken,serum PQ level and MMP-9 expression were analyzed by Spearman correlation analysis.A receiver operating characteristic (ROC) curve was drawn to evaluate the predictive value of peripheral blood MMP-9 level for the 28-day prognosis of PQ poisoning patients.Results After admission the 1 day serum PQ level was (2.60 ± 1.29) mg/L,and the amount of poison taken was 50.0 (7.5,60.0) mL in the 37 patients with oral PQ poisoning.The MMP-9 gene expression level in PBMCs and serum MMP-9 protein level of both PQ poisoning groups were significantly higher than those of healthy control group,and the levels were gradually increased with the extension of poisoning time;the degrees of elevation in death group were more significant [the PBMCs' MMP-9 gene expression (A value):2.84± 1.16 vs.0.95 ± 0.23 on the 1st poisoning day,4.22± 1.75 vs.1.29 ±0.30 on the 3rd poisoning day;serum MMP-9 concentration (μg/L):2791.48± 1 230.88 vs.807.81±279.86 on the 1st poisoning day,4384.21 ± 1 781.97 vs.1 131.14±291.76 on the 3rd poisoning day,all P < 0.05].Correlation analysis showed:there were significant positive correlations of oral PQ amount,serum PQ concentration to the MMP-9 gene expression in PBMCs and serum MMP-9 protein concentration in patients with oral PQ poisoning (all P =0.000).ROC curve analysis showed:the MMP-9 gene expression in PBMCs on the 1st day and the serum MMP-9 content on the 3rd day after admission had predictive value for 28-day prognosis in patients with oral PQ poisoning,and the ROC areas under the curve (AUC) was 0.820 and 0.776 respectively.When the cutoff value of MMP-9 gene expression level on the 1st day after admission was 0.90,the predictive sensitivity and specificity were 80.00% and 63.64% respectively;when the cutoff value of serum MMP-9 protein content on the 3rd day after admission was 904.36 μg/L,the predictive sensitivity and specificity were 80.00% and 72.73% respectively.Conclusion Oral PQ poisoning can lead to the MMP-9 gene expression in PBMCs and elevation of serum MMP-9 protein level in the body,and the MMP-9 gene expression has predictive value for the prognosis of patients with oral PQ poisoning.
10.Effects of pelvic motion in third trimester on the elements and outcomes of childbirth in primipara

Objective:To investigate the effect of pelvic motion in late pregnancy on the outcome of first trimester delivery.Methods:A total of 127 first-borns admitted to the midwifical clinic of Nanjing Maternal and Child Health Hospital from November 1, 2019 to March 10, 2020 were selected as the research objects, and they were divided into the walking group and the pelvic motion group according to the random number table method. In the end, 62 cases were included in the walking group and 65 cases in the pelvic motion group. The walking group did brisk walking in the third trimester. In the pelvic motion group, the motion in the third trimester consisted of the combined pelvic motion involving the two planes of the upright spine and the horizontal spine, and multiple peripelvic joints. The changes of fertility, birth canal and fetus before and after exercise were observed, and the outcomes of childbirth were compared between the two groups.Results:Indicators reflecting productivity: persistence time of "air bike" on the day before delivery was (27.12±7.73) seconds in the pelvic motion group and (20.56±6.60) seconds in the walking group, the difference was statistically significant ( t value was 5.12, P<0.01). The time of contractions after exercise before delivery was (269.38±123.70) seconds in the pelvic motion group and (591.29±201.82) seconds in the walking group, the difference was statistically significant ( t value was 10.89, P<0.01). The indicators reflecting the situation of the maternal birth canal were as follows: "sitting forward flexion level", pelvic motion group was (33.64±5.91) cm, walking group was (29.50±5.28) cm, the difference was statistically significant ( t value was 4.15, P<0.01). The cervical score during regular contractions was 8.01±1.69 in the pelvic motion group and 7.30±1.40 in the walking group, the difference was statistically significant( t value was 2.56, P<0.05). At the beginning of regular contractions, the proportion of fetal "occipital position" was 93.85% (61/65) in the pelvic motion group and 72.58%(45/62) in the walking group, the difference was statistically significant ( χ2 value was 10.41, P<0.01). The "no pain rate after exercise the day before delivery" was 72.31% (47/65) in the pelvic motion group and 29.03% (18/62) in the walking group, the difference was statistically significant ( χ2 value was 25.30, P<0.01). Time of the first labor was (395.84±165.24) min in the pelvic motion group and (574.35±152.23) min in the walking group, the difference was statistically significant ( t value was 6.32, P<0.01). The second stage of labor was (25.98±9.88) min in the pelvic motion group and (33.62±13.94) min in the walking group, the difference was statistically significant ( t value was 3.57, P<0.01). Conclusions:Pelvic motion in middle and late pregnancy can affect labor force, birth canal, fetus and other delivery factors, reduce movement pain and shorten the time of labor.
