1.Comparison of the results of displaced fernoral neck fractures with internal fixation and total hip replacement

[Objective]To evaluate the results of displaced neck fractures with internal fixation(AO compress cannulated screws) and total hip replacement.[Method]Form 1998.5~2003.10,forty-four patients(mean age 75.2 years) who had an acute displaced femoral neck fracture were treated with internal fixation(21 hips) and total hip replacement(23 hips).The hip function,including the pain,movement and walking,was evaluated by Charnley Score(Charnley improved Merle d' Aubign? score).The health-related quality of patients' life was rated with use of the EQ-5D index.[Result]Forty patients were followed up at least twentyfour months.The ratio of complications in fixation group,defined as nonunion,osteonecrosis and peri-implant fracture,was 36.8%.The ratio of complications in total hip replacement group,defined as dislocation,periprosthetic fracture,or radiographic signs of loosening of the femoral or acetaular component,was 4.7%.There was significance different between two group(P
2.Biomechanical comparison of three internal fixation methods for intercondylar fracture of humerus

Objective A biomechanical comparison of three kinds of internal fixation for intercondylar fracture of humerus was, made so as to provide a theoretical basis for clinical practice. Methods 18 fresh humeri were used to reproduce T-type intercondylar fracture, and three different fixation appliances were applied, including cross Kirschner wire with addition of “8” wire, single “Y” type plate, 1/3 tubular plate with reconstruction plate. Biomechanical tests were performed on these specimens. Result Under the same load condition, 1/3 tubular plate with reconstruction plate yielded the best results, while crossing Kirschner wire with “8” wire was the poorest. Conclusion The results showed that 1/3 tubular plate with reconstruction plate was a good internal fixation appliance. It has a proper design and stability, but it is not easy to apply. Therefore, an optimal method should be chosen according to local condition in clinical practice.
3.Establishment and evaluation of a rabbit model of femoral delayed union using external fixation

BACKGROUND: Mechanisms related to bone fracture and nonunion have been clarified, but the healing time and treatment strategy of delayed union remain controversial. OBJECTIVE: To establish a reliable model of femoral delayed union using external fixation in the New Zealand white rabbit. METHODS: Seventy-two New Zealand white rabbits were randomized to three groups. After removal of 10-mm femoral middle bone cortex with internal and external periosteum and bone marrow, a uniplanar external fixator was used. There was a 1 mm gap between broken ends (group A), and the broken ends were sealed by bone wax (group B). For controls, only bone cortex was cut and then fixed to contact the broken ends. The gross, radiological and histological observations were performed to observe the bone healing at 4, 8 and 12 weeks after modeling. RESULTS AND CONCLUSION: The control group achieved bone healing at the 4th week, which was considered as the normal time of bone healing. The groups A and B achieved healing at the 12th week, which was remarkably longer than that in the control group. The rabbit femoral delayed union model showed the typical X-ray imaging and pathological features of delayed union. Our results suggest that the model is reasonable and reliable, which can provide reference for clinical research.
4.Dynamic observation on bone mineral density of unsexed rabbits with QCT

Objective: The purpose of this tudy was to dynamicly observe the weight and the bone mineral density (BMD) of the unsexed rabbits with a few self-made standardized phantoms. Methods:The eighteen healthy adult female rabbits were measured for their weight and BMD in preunsexed and postunsexed 5 months, 10 months with quantitative CT(QCT). Results:There were 61.1% of rabbits whose weight and BMD decreased after 5 months of the postunsexed and 100% of rabbits whose weight and BMD decreased after 10 months of the operation. Conclusion:QCT can be used to dynamicly observe curative effect of drugs in various periods as well,and it is a good method to study osteoporosis.
5.Radiographic analysis of treatment of inter-trochanteric fractures using proximal femoral nails

Objective:To establish a reliable approach for measuring proximal femoral 3 dimensional anatomy, and to compare post-operative differences of proximal femoralanatomy in the inter-trochanter fractures with two kinds of antegrade nailings.Methods: Some computer assisted design ( CAD ) soft-wares, e.g.Mimics, were used to establish a reliable approach for measuring proximal femoral 3 dimen-sional (3D) anatomy.Intra-class correlation coefficient ( ICC) was used to test the reliability of intra-and inter-observers.The post-operative pelvic CT data of 19 cases of inter-trochanter fracture patients treated with InterTAN nailing and 21 cases of inter-trochanter fracture patients treated with proximal femo-ral nail anti-rotation ( PFNA) were retrospectively analysed and used to measure bilateral proximal femo-ral anatomical parameters, including 2D and 3D femoral neck-shaft ( NS) angle and femoral neck ante-version (NA) angle, and 2D and 3D anteversion angles of the intramedullary (IM) nailings.ICC was used to test the consistency of the NA angles in the different groups, and the paired student T-test was used to test the differences of the paired quantitative data.Results:The established measurement method hasdexcellent consistency within the intra-and inter-observers, with all the ICCs higher than 0.9.The paired student T-test showed no significant difference between the post-operative bilateral 2D or 3D NA angles.The ICCs results showed that there were no consistency between the post-operative bilateral 2D or 3D NA angles (P values were 0.099 and 0.055, respectively), but the excellent consistency between the 2D injured side NA angle and 2D IM nailing’ s NA angle, or between the 3D injured side NA angle and 3D IM nailing’s NA angle (the ICCs were 0.81 and 0.8, respectively, P values <0.001).In PF-NA group, 57%of the differences between the 2D post-operative injured side’s and intact side’s NA an-gles were higher than 15°, which was more than 15.78%in InterTAN group.The paired student T-test showed no significant difference between the post-operative injured side’ s 2D or 3D NS angles and the in-tact side’s respective 2D or 3D angles in PFNA group (P values were 0.925 and 0.367, respectively), but in InterTAN group, the post-operative injured side’s 2D or 3D NS angles were significantly smaller than the intact side’s respective angles (P values were 0.033 and 0.009, respectively).Conclusion:By analyzing and comparing bilateral proximal femoral anatomical parameters after two kinds of IM nailings procedures, the differences between the bilateral post-operative NA angles in PFNA group were significantly larger than those in InterTAN group.There was significant correlation between the NA angles of the injured sides and NA angles of IM nailings in both the groups.
6.Investigation of microRNA expression on Uygur cervical squamous cell carcinoma in southern Xinjiang

Background and purpose:Cervical cancer is one of the most common cancer in Xinjiang, especially for Uygur from southern Xinjiang and its pathogenesis is not clear. MicroRNA (miRNA) is a class of small non-coding RNA playing an important regulatory role. Its expression and dysfunction is closely related to the development of tumors. In this study, we screen and preliminary analyse expression of miRNA in cervical squamous cell carcinoma samples with human papillomavirus (HPV) 16 positive of Uygur patients. The target genes of miRNA were predicted.Methods:miRNAs were pre-screened by using miRNA microarray technology in 5 cases of HPV16 positivity Uygur patients from southern Xinjiang with cervical squamous cell carcinoma. Fifteen cases specimens were examined by qRT-PCR for preliminary veriifcation, and 83 cases of cervical cancer were detected and analysed the expression of miRNA; Targeted genes were predicted by using four softwares of target scan, miRwalk, miRanda and Pictar.Results:Eighteen differentially expressed miRNAs were selected by SAM software in 5 cases of HPV16 positivity southern Xinjiang Uygur cases with cervical squamous cell carcinoma.miRNA-138 and miRNA-720 were found expressed signiifcantly different by initial veriifcation. Contrasted with 40 normal cases, miR-138 and miR-720 were down-regulated in 83 Uygur patients from southern Xinjiang with cervical squamous cell carcinoma (P<0.05),and correlated with lymph node matastasis and vascular invasion (P<0.05), no correlation with age and the range of cervical wall involvement and HPV16 (P>0.05). miRNA-720 was correlated with clinical stage and tumor size (P<0.05); And the commonly targeted gene between miRNA-138 and miRNA-720 was EZH2.Conclusion:miRNA-138 and miRNA-720 were downregulated in Uygur patients from southern Xinjiang with cervical squamous cell carcinoma, and the common target gene was EZH2.The expression of miR-720 and miR-138 were correlated with relevant risk factors of invasion and metastasis.
7.The expression and the diagnostic value of NOD2 in hepatocellular carcinoma

Objective To investigate the expression of nucleotide-binding oligomerization domain protein 2 (NOD2)in serum of patients with hepatocellular carcinoma (HCC),and to analyse the roles of NOD2 in HCC development and its clinical diagnostic value.Methods This study including 66 patients with HCC in the hospi-tal from March 1,2013 to December 31,2014 and 61 healthy controls.Serum NOD2 levels were determined by enzyme-linked immunosorbent assay (ELISA).Analysis of significance was performed with rank sum test using SPSS statistical 16.0 software.Results Serum levels of NOD2 in HCC patients were 171 pg/ml,significantly higher than that of healthy controls(95 pg/ml,Z =-5.00,P =0.00),and the serum NOD2 levels were correla-ted with clinical stage of HCC (H=56.26,P =0.00).Compared with the serum NOD2 levels in stageⅠ,Ⅱpatients (106 pg/ml)and healthy controls (95 pg/ml),the serum NOD2 level in stage Ⅲ and Ⅳ (220 pg/ml) were significantly increased (χ2 =31.24,P =0.00;χ2 =47.23,P =0.00),but the expression of NOD2 in stageⅠandⅡwere nearly equal to that of the healthy controls (χ2 =0.36,P =0.83).The ROC analysis revealed that the best diagnostic cutoff-point of serum NOD2 levels for predicting the Ⅲ and Ⅳ stages of HCC was 148.78 pg/ml,meanwhile corresponding sensitivity was 89.1% and specificity was 77.0%.Additionally,corre-lation analysis demonstrated that there was no significant correlation between NOD2 and alpha-fetal protein (r =0.44,P =0.14).Survival curves obtained that the survival time of HCC patients with NOD2 serum concentrations≥ 200 pg/ml was significantly less than that <200 pg/ml (χ2 =15.32,P <0.05).Conclusion NOD2 is highly expressed in the serum of HCC patients,especially in advanced patients,which is possibly involved in the development of HCC and has the potential to become an effective marker used for HCC diagnosis.

9.Growth state and adenovirus infection efficiency of human umbilical cord-derived mesenchymal stem cells in 3 different culture systems

BACKGROUND: In vitro culture condition and culture efficiency are different in reported umbilical cord-derived mesenchymal stem cells, and lacked of unified standards. Different derived mesenchymal stem cells have different biological properties. Therefore, it is very necessary to establish a simple and high-performance culture system for umbilical cord-derived mesenchymal stem cells. OBJECTIVE: To observe the growth state of human umbilical cord-derived mesenchymal stem cells in different culture systems in vitro and adenovirus infection efficiency. METHODS: Mesenchymal stem cells were separated from healthy full-termed delivery fetus using collagenase digestion method and purified by adherent culture. These cells were cultured and amplified in DMEM (low glucose), MesenPRO RS~(TM) Medium and STEMPRO~(R) MSC SFM in vitro. The 3-5 passage mesenchymal stem cells were infected by the Ad5-EGFP, Ad5/11-EGFP, Ad5/35-EGFP as multiplicity of infection (MOI)=1, 10, 100. Viral infection and green fluorescence expression were observed at post-infection 24, 56 and 72 hours using inverted fluorescence microscope. RESULTS AND CONCLUSION: The cell morphology in STEMPR~(R) MSC SFM was different from other two culture system and these cells were not easy to adherent after trypsin digestion. Cell doubling time in the MesenPRO RS~(TM) Medium was shorter than other two groups. Mesenchymal stem cells were infected by Ad5/35-EGFP with higher efficiency than other two kinds of adenovirus, but part of cells appeared apoptosis. The infection efficiency of Ad5/11-EGFP was highest. The fluorescence intensity was gradually increased with increased MOI.
10.Effects of Ethanol Extracts of Several Traditional Chinese Medicinal Herbs on Tyrosinase Expression and Melanogenesis in Guinea Pig Skin

Objective To explore the effects of traditional Chinese medicinal herbs (TCMHs) on the expression of tyrosinase gene, melanogenesis and proliferation of melanocytes and elucidate the mechanism of TCMHs in promoting melanogenesis. Methods Seven TCMHs including Herba Ecliptae, Spica Prunellae, Caulis Spatholob, etc, which were known to be effective in activating tyrosinase in vivo, were selected. Brownish guinea pigs were selected as the experimental model. The mRNA in situ hybridization (ISH), Schmorl-staining and dopa-oxygenase staining were performed to observe the effects of TCMHs on gene expression of tyrosinase, melanogenesis and melanocyte proliferation. Results The mRNA ISH showed that these seven drugs, especially Herba Ecliptae,Spica Prunellae and Tribulus terrestris could significantly increase the number of positive cells and the intensity of hybridization signal in the treated group as compared with that in the control group (P 0.1). Conclusions These results suggested that these 7 TCMHs including Herba Ecliptae can upregulate the gene expression of tyrosinase, enhance the melanogenesis and promote the proliferation of melanocytes.
