1.The Role of Intestinal Fungi and Its Metabolites in Chronic Liver Diseases

Current studies have confirmed that liver diseases are closely related to intestinal microorganisms; however, those studies have mainly concentrated on bacteria. Although the proportion of intestinal microorganisms accounted for by colonizing fungi is very small, these fungi do have a significant effect on the homeostasis of the intestinal microecosystem. In this paper, the characteristics of intestinal fungi in patients with chronic liver diseases such as alcoholic liver disease, nonalcoholic fatty liver disease and cirrhosis are summarized, and the effects of intestinal fungi and their metabolites are analyzed and discussed. It is important to realize that not only bacteria but also intestinal fungi play important roles in liver diseases.
2.Guiding significance of immune factors and T lymphocyte subsets in immuno-modulatory therapy for sepsis

Objective:To investigate the guiding significance of monitoring of immune factors and T lymphocyte subsets in immunomodulatory treatment of sepsis.Methods:Eighty patients with sepsis admitted to Nangyang First People's Hospital from June 2022 to December 2023 were selected as sepsis group,and 80 healthy volunteers who underwent physical examination during the same period were selected as health group.Detection of immune factors[complement C3,IgA,IgG,IgM,IFN-γ,programmed cell death receptor-1(PD-1)]and T lymphocyte subsets(CD4+,CD8+,CD4+/CD8+)on the day of physical examination in healthy group,the day after sepsis group was enrolled,before treatment,and on day 1,3,and 7 after treatment,expressions of immune factors and T lymphocyte subsets were compared between the two groups.All 80 patients with sepsis received comprehensive treatment,during which thymosine-α1 was added for immunoconditioning treatment.Patients were divided into groups according to the improvement of their condition after immunoconditioning treatment[low-risk group(n=31),medium-risk group(n=34),high-risk group(n=15)]and disease outcome[survival group(n=55)and death group(n=25)].To analyze the efficacy of immune factors and T lymphocyte subsets in predicting the condition improvement and disease outcome of patients with sepsis after immune conditioning treatment.Results:Compared with healthy group,expression levels of immune factor complements C3,IgA,IgG,IgM and IFN-γ in sepsis group were decreased,relative expression level of PD-1 mRNA was increased,the values of CD4+T and CD4+T/CD8+T of T lymphocyte subsets were decreased,while CD8+T was increased(P<0.05).Compared with before treatment and 1 day after treatment,expression levels of complements C3,IgA,IgG,IgM,IFN-γ,CD4+T and CD4+T/CD8+T of T lymphocyte subsets were significantly increased in patients with sepsis after treatment on 3 and 7 days,while the relative expression level of PD-1 mRNA and CD8+T were significantly decreased(P<0.05).Condition of sepsis patients improved significantly after immune conditioning treatment,that is,proportion of low-risk patients increased,and proportion of medium-risk patients and high-risk patients decreased(Z=6.954,P<0.05).Levels of C3,IgA,IgG,IgM,IFN-γ,CD4+T and CD4+T/CD8+T in high-risk groups were significantly decreased,while PD-1 and CD8+T were significantly increased in low-risk and medium-risk groups(P<0.05).Levels of C3,IgA,IgG,IgM,IFN-γ,CD4+T and CD4+T/CD8+T in death group were significantly lower than those in survival group,while PD-1 and CD8+were significantly increased(P<0.05).ROC curve analysis showed that AUC of the combination of immune factors and T lymphocyte subsets to predict the improve-ment of the condition of sepsis patients after immune conditioning treatment was 0.923,the sensitivity was 89.90%,and the specificity was 82.31%.The efficacy of predicting the disease outcome of sepsis patients after immune conditioning treatment showed that the pre-dicted AUC was 0.965.The sensitivity was 95.60%and the specificity was 86.57%,which were obviously better than the single item.Conclusion:Sepsis patients have immune dysfunction related to immune factors and abnormal T lymphocyte subsets,immunomodu-latory therapy can help improve immune dysfunction,monitor immune factors upon admission,and levels of T lymphocyte subsets are helpful for early prediction of disease progression and prognosis in sepsis patients after immunomodulatory therapy.
3.Expression of interferon-γ, CD47 and lymphocyte subsets in patients with sepsis and their relationships with prognosis

Objective To investigate the levels of serum interferon-γ (IFN-γ), CD47 and lymphocyte subsets in patients with sepsis and their relationships with prognosis. Methods A total of 180 patients with sepsis who were treated in our hospital were selected as observation group, and 180 healthy volunteers in the same period as control group. Based on the survival status of patients within 28 days of hospitalization, the patients were divided into survival group of 120 cases and death group of 60 cases. The levels of serum IFN-γ and CD47 in patients with sepsis were detected by enzyme-linked immunosorbent assay (ELISA); the level of serum lymphocyte subsets was detected by flow cytometry. Logistic regression analysis was performed on the factors affecting the prognosis of patients with sepsis; the clinical value of the levels of serum IFN-γ, CD47 and lymphocyte subsets in predicting the prognosis of patients with sepsis was analyzed by the receiver operating characteristic (ROC) curve. 4.Study on the Thematic Characteristics of Sleep Disorders Disease Description Texts in Online Health Communities

Purpose/Significance Disease description texts are analyzed to reach a deeper understanding of the current status of on-line consultations for sleep disorders and the thematic characteristics of users with sleep disorders.Method/Process Data about sleep dis-orders from"haodf.com"website is collected by using a web crawler.Furthermore,the main themes about patients'description are i-dentified by the latent Dirichlet allocation(LDA)model.Result/Conclusion The departments of sleep disorders are more dispersed,and the main treatment is drug therapy.Online consultations could improve 83.2%of patients'condition.The themes of patients's disease descriptions include medication and consultation,external environment,description of symptoms,surrogate questions and causes.It is suggested that the platform and doctors should pay attention to the prognosis of patients'medication and mental health status,and pay at-tention to the popularization of comorbidities.

Ovarian cancer is the third-most-common malignant reproductive tumor in women. According to the American Cancer Society, it has the highest mortality rate of gynecological tumors. The five-year survival rate was only 29% during the period from 1975 to 2008 (Reid et al., 2017). In recent decades, the five-year survival rate of ovarian cancer has remained around 30% despite continuous improvements in surgery, chemotherapy, radiotherapy, and other therapeutic methods. However, because of the particularity of the volume and location of ovarian tissue, the early symptoms of ovarian cancer are hidden, and there is a lack of highly sensitive and specific screening methods. Most patients have advanced metastasis, including abdominal metastasis, when they are diagnosed (Reid et al., 2017). Therefore, exploring the mechanism of ovarian cancer metastasis and finding early preventive measures are key to improving the survival rate and reducing mortality caused by ovarian cancer.

6.Epidemiological characteristics of outbreaks of norovirus GII.17P17 acute gastroenteritis in China, 2022

Objective:To analyze the epidemiological characteristics of norovirus (NoV) acute gastroenteritis (AGE) outbreaks caused by GII.17[P17] variant in China, 2022.Methods:Information and specimens of AGE outbreaks between January and December 2022 were collected. NoV RNA was detected in all specimens by real-time RT-PCR. The viral genome of the positive specimens were amplified, sequenced and analyzed.Results:Between January and December 2022, 360 AGE outbreaks were reported cumulatively, of which 266 outbreaks successfully obtained genotype results. GII.17 [P17] was one of the main genotypes and detected in 34 outbreaks (12.78%, 34/266), with the highest number of outbreaks detected in spring (6 outbreaks in March and 7 outbreaks in May), mainly in childcare facilities and primary schools (61.76%, 21/34). According to the result of NoV genotype analysis in different age groups, 14 strains of GII.17 [P17] in this study belonged to Cluster III b and SC III branch of Cluster III (Kawasaki308) in the capsid region and polymerase region, respectively, and both belonged to the same cluster as the variant strain (GZ41621 strain) that caused the NoV AGE outbreaks in China during the 2014/15 season. Compared to reference strains of Cluster I, Cluster II and Cluster III a, Cluster III b was provided with 22 amino acid mutations in VP1. The main amino acid changes in the subgroup of Cluster III b including the virus strains isolated in this study were at T294I and Q299R of antigen epitope A, an insertion mutation occurred at antigen epitope D, H353Q at the site I of the human histo-blood group antigen receptor binding site. The selection pressure analysis detected a large number of negative selection sites, indicating that negative selection plays an important role in the evolution of VP1 genes.Conclusions:GII.17 [P17] was one of the primary genotypes responsible for NoV diarrhea outbreaks in China in 2022. Phylogenetic analysis had revealed that it still belonged to the same cluster as the novel GII.17 [P17] variant (strain GZ41621) that caused NoV epidemics in China during the 2014/15 season, exhibiting minor amino acid variations at the potential epitope.
7. Value of hydrogen proton magnetic resonance spectroscopy in the determination of liver triglyceride in patients with fatty liver disease and its influencing factors

Objective:
To investigate the value of 1H-magnetic resonance spectroscopy (1H-MRS) in determining the content of liver triglyceride in patients with fatty liver disease (FLD), as well as its influencing factors.
Methods:
A total of 124 patients with nonalcoholic fatty liver disease (NAFLD), chronic hepatitis B (CHB), or hepatitis B complicated by FLD who underwent liver biopsy in the Affiliated Hospital of Hangzhou Normal University were enrolled, and the clinical data, serological markers, FibroScan results, and 1H-MRS results were collected. A correlation analysis was performed with the results of liver biopsy as the gold standard, and the influence of factors including hepatitic B virus (HBV) infection and obesity on accuracy was analyzed. A one-way analysis of variance was used for comparison of means between the three groups, and the LSD or SNK test (for homogeneity of variance) or the Tamhane’s or Dunnett’s test (heterogeneity of variance) was used for comparison between any two groups. The t-test was used for comparison of continuous data between groups, and the chi-square test was used for comparison of categorical data. The MRS-PDFF receiver operating characteristic (ROC) curve was plotted, the area under the ROC curve (AUC) was calculated, the optimal cut-off points for the diagnosis of NAFLD were estimated, and sensitivity and specificity were calculated.
Results:

8.Analysis of Chemical Constituents as Flavonoids and Coumarins in Radix Ardisiae from Different Sources

OBJECTIVE:To identif y and analyze the flavonoids and coumarins in Radix Ardisiae from different sources. METHODS:UPLC-QE-HF-MS/MS was adopted. The determination was performed on Zorbax Eclipse-C 18 column with mobile phase consisted of acetonitrile- 0.1% formic acid solution (gradient elution )at the flow rate of 0.3 mL/min. The column temperature was 30 ℃,and the temperature of injector was 4 ℃. The sample size was 2 µL;ESI source was applied in negative and positive scanning ion mode ,the heater temperature was 325 ℃,the sheath gas pressure was 45 arb,the auxiliary gas pressure was 15 arb,the purge gas pressure was 1 arb,the electrospray voltage was 3.5 kV,the capillary temperature was 330 ℃, S-lens RF level was 55%,scan mode was first-order full sca m/z 100-1 500,data-dependent secondary mass spectrometry scanning (dd-MS2,Top N =10),the resolution was 70 000 (first mass spectrometry ) , 17 500 (secondary mass spectrometry),the collision mode was high-energy collision dissociation. Through retrieving foreign and domestic databases as ChemSpider ,mzCloud,mzVault,PubChem,the structure of the compound was identified on the basis of related literatures and reference data ,and the conten ts were compared. RESULTS & CONCLUSIONS:A total of 47 components were separated from Radix Ardisiae of 3 kinds of sources as Ardisia crenata Sims,A. crispa(Thunb.)A. DC. ,A. crenata Sims var . bicolor (Walk)C. Y. Wu et C. Chen. A total of 17 flavonoids were identified ,including 9 flavonols (quercetin 3-O-rhamnoside-7-O-glucoside, myricetin, rutin, mauritanin, kaempferol, quercetin, isorhamnetin, quercetin,mearnsitrin),3 flavan-3-ols [(-)-epigallocatechin,catechin,epigallocatechin gallate )2 dihydroflavonoids [fustin , eriodictyol] and 3 other types [ 3-(2,3-dihydro-benzo[1,4]dioxin-6-yl)-7-hydroxy-2-trifluoromethyl-chromen-4-one,methadone, oriciacridone F] ,10 coumarins {bergenin ,([ 7-hydroxy-4-methyl-2-oxo-2H-chromen-6-yl)oxy]acetic acid ,[7-(carboxymethoxy)- 4-methyl-2-oxo-2hydroxychromo-3-yl]acetic acid ,4,9-dihydroxy-7H-furo[3,2-g]chromen-7-one,6,7-dihydroxy-4-methylcoumarin, esculetin,fraxetin,7,8-dihydroxy-4-methylcoumarin,4-methylumbelliferyl glucuronide ,scoparone}. Results of content analysis showed that in flavonoids and coumarins ,there were 5 common components in Radix Ardisiae from 3 kinds of sources ,i.e. bergenin(peak 2),[7-(carboxymethoxy)-4-methyl-2-oxo-2-hydroxychromo-3-yl] acetic acid (peak 5),methadone(peak 16), quercetin(peak 18),oriciacridone F (peak 26);the contents of common components were significantly different. In addition to 5 common components ,there were 22 different chemical components ,which were compounds corresponding to peaks 1,3,4, 6-15,17,19-25 and 27,respectively. Among them ,compounds corresponding to peaks 3,6,8 and 23 were only found in A. crenata Sims var. bicolor(Walk)C. Y. Wu et C. Chen ;compounds corresponding to peaks 12-15,19 were only found in A. crispa (Thunb.)A. DC. UPLC-QE-HF-MS/MS method can efficiently ,accurately and quickly identify the flavonoids and coumarins in Radix Ardisiae from different sources.
9.Analysis of chemical constituents and components absorbed into plasma of Ardisia crenata based on UPLC-QE-HF-MS/MS

OBJECTIVE To analyze the chemical constituents and components absorbed into plasma of the extract of Ardisia crenata and to elucidate its possible pharmacodynamic material basis. METHODS Overall, 12 rats were randomly assigned to the blank group (n=6) and A. crenata group (n=6) by the paired comparison method. The drug was administered once daily in the morning and afternoon for three days. Serum samples were prepared from serum after redosing on 4th day. The UPLC-QE-HF-MS/ MS was used to analyze and identify the chemical constituents in A. crenata extract and serum samples. Compound Discoverer 3.0 was employed for retention time correction, peak identification, and peak extraction. According to the secondary mass spectrometry information, the Thermo mzCloud online and Thermo mzVault local databases, referring to the relevant literature and control quality spectrum information were used to preliminarily identify the chemical constituents and components absorbed into plasma of A. crenata. RESULTS A total of 34 compounds were identified from the extract of A. crenata, mainly coumarins, flavonoids, organic acids, amino acids, including bergenin, quercetin, gallic acid, L-pyroglutamic acid, etc. Besides, 5 components absorbed into plasma were identified from serum samples: L-pyroglutamic acid, syringic acid, bergenin, cinnabar root saponin A, and mycophenolic acid. CONCLUSIONS L-pyroglutamic acid, syringic acid, bergenin, cinnabar root saponin A, and mycophenolic acid may act as the pharmacodynamic material basis of A. crenata.
