1.The effect of hydroxyethyl starch on capillary permeability in a rat model of acute lung injury

Objective To evaluate the effect of 6% hydroxyethyl starch (HES) 200/0.5 on the pulmonary capillary permeability in oleic acid induced acute lung injury (ALI) and investigate the possible mechanism of the therapeutic effect of HES. Methods Forty male SD rats weighing 280-370 g were randomly divided into four groups: group I balanced solution (control); group II 6% HES 200/0.5; group III gelatin and group IV dextran 40. The animals were anesthetized with intraperitoneal 3% pentobarbital 30-40 mg?kg-1 . Right external jugular vein was cannulated. Oleic acid 0.06 ml?kg-1 was given iv.10 ml?kg-1 of the above-mentioned different solutions was infused over 1.5 h in the four groups. Then 0.5% Evan's blue 1ml was injected iv. Four hours later the animals were sacrificed and lungs removed for pathologic examination under microscope, and determination of lung water content [ (wet weight -dry weight)/wet weight ? 100% ] . Lung Evan's blue content was determined by formamide extracting method. Lung MDA content and MPO activity were also determined.Results Lung water and Evan's blue contents were significantly lower in group II than those in any other groups. MPO activity and MDA level in group II were lowest among the four groups. Conclusions HES can decrease pulmonary capillary permeability, attentuate pulmonary edema and lung injury, and depress MPO activity and MDA level. The underlying mechanism may be due to its depressant effect on PMN aggregation and lipid peroxidation.
2.Primary culturing and biological identification of neonate rat myocardial cells

Objective To find out a way for culturing the myocardial cells of neonatal rats in vitro, and to study their biological characters. Methods Cardiomyocytes from the heart of 1-3 days old Sprague-Dawley rats were prepared by a modified protocol. Hearts were harvested and cut into pieces of about 1mm3 in size, and placed into cell culture bottles with nylon without pre-treatment of digestive enzyme. DMEM supplemented with 10% (v/v) FCS was used for culture, with thymidine (6mg/ml) added to inhibit fibroblast growth. Cells in monolayer were formed on the gauze and the bottom of the culture flask, and then cells were isolated by 0.25% trypsin digestion for 30 seconds. Cell suspension was transferred to 100cm2 cell culture flasks at a density of 104 cells /cm2 for identification, viability test and morphologic observation. The cells obtained were stained with monoclone anti-a-sarcomeric actinin and FITC to evaluate the purity of the myocardial cell preparation by flow cytometry, AO-PI fluorescein stain was used to evaluate the viability and Giemsa staining for examining the morphology of the myocardial cells. Results 24-48 hours after culture, the myocardial cells became adherent to bottle wall to form a sheet, and began to pulsate. The purity of myocardial cells in culture cell population was 94.13%, the ratio of viable myocardial cells was 95.3%, and 95%CI was 91.6%-99%. The output of myocytes from each neonatal heart was 3.3?106 and 95%CI is 2.1? 106- 4.5?106. Conclusion Neonatal myocytes culture in this way can generate large amount of viable single myocardial cells suitable for myocardial research in a short period.

4.Short term results of laparoscopic versus open complete mesocolic excision for right-sided colon cancer

Objective To compare the clinical efficiency between laparoscopic and open complete mesocolic excision for right-sided colon cancer.Methods Between January 2011 and August 2012,a total of 134 patients with right-sided colon cancer who underwent CME at Fuzhou General Hospital of Nanjing Military Command were divided into laparoscopic (71) and open (63) groups.The intraoperative parameters,pathology,postoperative course and short-term outcomes were compared between groups.The chi-test and the student t test were used for statistics.Result There were no significant differences in the length of distal (P=0.427) and proximal margin (P=0.515),tumor diameter (P =0.440) and number of lymph nodes dissected (P =0.377).Postoperatively patients were followed for up to 12 months,no significant differences were found in local regional recurrence rates (4.2% vs 1.6%) (P =0.622) and distal metastasis rates (5.6% vs 3.2%) (P =0.684) between the two groups.The mean operative time (P =0.134) and postoperative complication rate (P =0.977) were similar.The mean intraoperative blood loss was less (P =0.000),bowel flatus passed earlier (P =0.000) and hospital stay shorter (P =0.000) in laparoscopic than that in open group.Conclusions Laparoscopic CME has the same oncologic clearance effects and short-term follow-up result compared with open CME for right-sided colon cancer,and laparoscopic CME is minimally invasive,less bleeding,less pain and quick recovery.
5.Gravity frequency and its monitoring application of EEG spectrum in the vigilance operation.

It is an important means to study the electrical activity of the brain's nerve cells by exploring physiological information of the EEGs from the frequency domain. The gravity frequency is one of the global parameters with using this method. We used the multitaper spectrum method (MTM) spectrum estimation method of good performance to calculate the EEG spectrum and its gravity frequency of subjects under vigilance and vigilance decrement state. The results showed that the gravity frequency of vigilance state was higher than that of vigilance decrement state, the gravity frequency became smaller along with the vigilance decrement, and the location of the gravity frequency shifted to the left in the spectrum. Finally, the monitoring curve of the gravity frequency was acquired by designing an algorithm, and it was used to online monitoring vigilance operators.

6.A restrospective analysis of adverse events during peri-anesthesia period in patients scheduled for thoracic surgery

From August 2006 to June 2011 among consecutive 18 294 patients underwent thoracic surgery from Shanghai Chest Hospital,41 developed adverse events during peri-anesthesia period and the incidence was 0.224%,and the constituent ratios of the predictable and unpredictable events were 15% and 85%,respectively.Cardiac arrest and massive hemorrhage were the main clinical manifestation of the adverse events and the constituent ratios were 37% and 24%,respectively.Among the inducements for adverse events,patient's factor,surgical factor,anesthesia factor,patient-surgery factor,patient-anesthesia factor and patient-surgery-anesthesia factor accounted for 12.2%,48.8%,12.2%,7.3%,7.3% and 12.2%,respectively.The ratio of death from adverse events was 17% (7 cases),and among the inducements for adverse events causing death,surgical factor,patient-surgery factor and patient-surgery-anesthesia factor accounted for 43%,43% and 14%,respectively.The incidence of adverse events was 1.093% in the patients underwent operation on trachea,which was significantly higher than that in the patients underwent operation on lung (0.223%),mediastinum (0.236%) and esophagus (0.194%).In conclusion,although the adverse events which occurred during peri-anesthesia period in the patients underwent thoracic surgery were rare,they threatened the safety of patients.Surgical factor was not only the main inducement,but also the risk factor for death,and cardiac arrest and massive hemorrhage were the main clinical manifestation of the surgery-related adverse events.For cardiac arrest,as long as it was found in time and treated appropriately,the serious consequences could be avoided.For massive hemorrhage,more attention should be paid due to be the main reason of death,and the prevention depended on the surgeon's improvement of diagnosis and surgery.It was difficult to predict patient-related the adverse events and careful monitoring was required.For the anesthesia-related adverse events,they were mostly due to the poor airway management,so preoperative airway assessment should be strengthened.
7.Design, Synthesis and Antiviral Activity of Novel EV71 3C Protease Inhibitors

Objective:To explore novel compounds with anti-EV71 activity. Methods: A series of oligopeptides were designed based on the active groove of the EV71 3C protease using molecular simulation technology. Furthermore, some oligopeptides were syn-thesized and tested for the inhibition ability against EV71 in vitro. Results:Pentapeptides had higher binding affinity to the EV71 3C protease than the other oligopeptides, such as compounds 25 and 16. Therefore, compounds 25 and 16 were synthesized and tested in vitro for the anti-EV71 activity, and compound 25 exhibited potent anti-EV71 activity(EC50 =1. 36 μmol·L-1, CC50 =211. 94 μmol ·L-1, SI=155. 84), while compound 16 had no evident effect on EV71. Conclusion: It is worthy of further investigation on com-pound 25 as a novel scaffold of anti-EV71inhibitor.
8.Ventilation modes and factors influencing tidal volume in nonventilated lung during one-lung ventilation in patients undergoing thorascopic surgery:monitoring using electrical impedance tomography

Objective To evaluate the ventilation modes and factors influencing the tidal volume(VT)in the nonventilated lung during one-lung ventilation(OLV)in patients undergoing thorascopic surgery using electrical impedance tomography.Methods Thirteen American Society of Anesthesiologists physical statusⅠ or Ⅱ patients of both sexes,aged 45-64 yr,weighing 45-80 kg,undergoing elective pulmonary surgery performed via video-assisted thoracoscope,were enrolled in the study.After anesthesia induction,a double lumen tube was placed,and correct tube placement was confirmed with a fiberoptic bronchoscope.Two-lung ventilation and OLV were performed sequentially when in supine position with a fixed VT of 8 ml/kg and respiratory rate(RR)of 12 breaths/min.When the patients were turned to lateral position(with the operated lung on the upper side),correct placement of the tube was reconfirmed with a fiberoptic bronchoscope.Bilateral lungs were ventilated with VT of 8 ml/kg and RR of 12 breaths/min,and unilateral lungs were then ventilated in the following modes in sequence:VT 8 ml/kg and RR 12 breaths/min for the lung on the upper side;VT 8 ml/kg and RR 12 breaths/min for the lung on the lower side;VT 8 ml/kg,RR 12 breaths/min and positive end-expiratory pressure(PEEP)4 cmH2O for the lung on the lower side;VT 6 ml/kg and RR 16 breaths/min for the lung on the lower side;VT 4 ml/kg and RR 24 breaths/min for the lung on the lower side.Each ventilation mode stabilized for 2 min.At 2 min of OLV in each mode,electrical impedance tomography was used to record the ventilation mode in the nonventilated lung,anesthesia machine was used to record VT in the nonventilated lung,and the percentage of VT on nonventilated side in VT on ventilated side(VT-non/VT-ven%)was calculated.When the unilateral lung on the lower side was ventilated in lateral position,logistic regression analysis was used to analyze the correlation between VT on ventilated side(no PEEP)and that on nonventilated side.Results Opposite ventilation was found in the nonventilated lung during OLV.VT-non/VT-ven% was significantly higher in lateral position than in supine position(P<0.05).VT-non/VT-ven% was significantly higher when the lung on the lower side was ventilated than that when the lung on the upper side was ventilated(P<0.05).Four cmH2O PEEP exerted no effect on VT-non/VT-ven%.When the lung on the lower side was ventilated in lateral position,there was a linear positive correlation between VT on ventilated side(no PEEP)and that on nonventilated side(r=0.899,P<0.05).Conclusion During OLV in patients undergoing thorascopic surgery,there is opposite ventilation in the nonventilated lung,and VT is influenced by body positions and VT in contralateral lungs.
9.The protective effects of bifidobacterial adhesin on ischemic reperfusion injury of intestine in rats

ObjectiveTo investigate the protection effect of bifidobacterial adhesin for intestine ischemia/reperfusion (I/R) injury on gut barrier function in rat.MethodsSeventy-two male SD rats were randomly divided into sham operation group (n =24), I/R model group (n =24) and pretreatment group of bifidobacterial adhesin (pretreatment group, n = 24).Six rats were anatomized at 6 h, 1 d, 4 d and 7d after inducing I/R model in each group, respectively.The pathological changes of the terminal ilea and the blood levels of TNFα, IL-6, IL-10, diamine oxidase (DAO), and the activity and content of D-lactic acid were observed.ResultsThe blood levels of TNFα, IL-6, DAO and D-lactic acid in I/R model group were significantly higher than sham operation group at all time points (P <0.05) , while the blood level of IL-10 was no significantly change.The activity of IL-6 and DAO in pretreatment group was significantly lower than I/R model group at all time points (P < 0.05), the blood level of TNFαt in pretreatment group was significantly lower than I/R model group at 1 d, the blood level of D-lactic was significantly lower than I/R model group at 4 d and 7 d (P < 0.05). Intestinal pathological damages were obviously milder in pretreatment group than I/R model group at all time points (Chiu's pathological scores: 6 h, 3.22 ±0.22 vs 3.57 ±0.20;1 d,3.77 ±0.13 vs 3.90 ±0.12;4 d,2.93 ±0.23 vs 3.07 ±0.21;7 d,2.10 ±0.30 vs 2.22 ±0.17,all P < 0.05).ConclusionThe pretreatment of bifidobacterial adhesin could protect the intestinal mucosa from I/R injury, and alleviate intestinal ischemic reperfusion injury.
10.Main points of complete mesoeolic excision for right-hemi colorectal cancer and comparation with traditional surgery in the efficacy

Objective To compare the clinical safety and efficacy of complete mesoeolic excision (CME) for right-hemi colorectal cancer with traditional surgical treatment.Methods Collectting the clinical data of 69 cases,who randonly divided into two groups,A group induced 38 cases and were implemented regular CME,and B group induded 31 cases which were implemented traditional surgery of right-hemi colorectal cancer patients from the Department of General surgery,Fuzhou General Hospital of Nanjing Military Region from Jan.1 2010 to Oct.1 2010,and comparing the safety and efficacy.Results The differences between and traditional surgery in terms of operation time,blood loss,exhaust and defecation time,hospitalization time,the rate of postoperative complication,the number of removed lymph node of Ⅰ and Ⅱ phase,3-year recurrence rate of Ⅱ phase,3-year survival rate of Ⅱ phase and the number of removed lymph node of Ⅲ phase in mesentery root were not significant.The differences in terms of the number of removed lymph node of Ⅲ phase(22.76 ± 6.94,15.11 ± 7.85),3-year recurrence rate of Ⅲ phase(5.88%,45.45%),3-year survival rate of Ⅲ phase (94.12%,63.64%),the total number of removed lymph node(20.56 ± 7.11,13.92 ± 6.62),3-year overall recurrence rate(7.89%,29.03%)and 3-year overall survival rate (94.74%,77.42%) were significant.Conclusions Comparing with traditional surgery,CME significantly benefits the patients of right-hemi colorectal cancer with stage Ⅲ,but not for patients with stage Ⅰ and Ⅱ.
