1.Correlation of plasma N-terminal pro-B-type natriuretic peptide with heart function indicators in patients with hypertension and paroxysmal atrial fibrillation

Objective To investigate the correlation of plasma N‐terminal pro‐B‐type natriuretic peptide (NT‐proBNP) level with echocardiographic indicators and P wave terminal force of lead V 1 (PtfV1) in the patients with hypertension and paroxysmal atrial fibrillation(PAF) .Methods Fifty‐six outpatients and inpatients with hypertension were divided into the PAF group (n=26) and the sinusrhythm group (n=30) .Thirty age‐matched and gender‐matched healthy volunteers were taken as the control group . The plasma NT‐proBNP level was determined .Left ventricular enddiastolic diameter (LVEDD) ,left atrial diameter(LAD) and left ventricular ejection fraction(LVEF) were examined by echocardiography .the 12‐lead electrocardiogram was routinely performed Pt‐fV1 was calculated .Results The plasma NT‐proBNP level in the PAF group was higher than that in the sinusrhythm group and the control group ,the difference was statistically significant (P<0 .05) .The plasma NT‐proBNP level in the PAF group was de‐creased significantly after successful cardioversion .The plasma NT‐proBNP level in the PAF group was positively correlated with LVEDD(r=0 .543 ,P<0 .05) and LAD (r=0 .606 ,P<0 .01) .The plasma NT‐proBNP level was negatively correlated with LVEF (r= -0 .750 ,P<0 .01) and positively correlated with the PtfV 1 absolute value (r= 0 .513 ,P< 0 .01) .Conclusion The plasma NT‐proBNP level can better reflect the heart structure and function in the patients with atrial fibrillation .Detecting the plasma NT‐proBNP level combined with echocardiographic indicators and PtfV 1 is conducive to comprehensively assess the heart function in the patients with hypertension and PAF .
2.Prevalence of hepatitis C virus infection in volunteer blood donors in Huzhou area

Objective To estimate the prevalence of anti-hepatitis C virus (HCV) positivity among blood donors from Huzhou,and to determine risk factors associated with infection.Methods Data were collected from volunteer blood donors between 2009 and 2013.Anti-HCV test and the confirmation test with RIBA and RT-PCR were performed.Information about voluntary blood donors was colleted,then donors who were confirmed HCV seropositive were retrospectively analyzed by statistical comparison.Result The prevalence of anti-HCV positivity was (0.84 ± 0.04) ‰.The predominant genotype currently circulating in Huzhou area was 1 b.There were obvious difference in the incidence among different volunteer blood donors (P < 0 01).Conclusion The rate of HCV infection was much lower in Huzhou area,and there were obvious difference in the incidence and genotype among different volunteer blood donors.
3.Customized orthotic insoles improve the gait and balance of hemiplegic stroke survivors more effectively than an ankle-foot orthosis

Objective:To observe the effect of customized orthotic insoles on the gait and balance of hemiplegic stroke survivors.Methods:Sixty stroke survivors with gait abnormalities were randomly divided into a group fitted with ankle foot orthoses (AFO) ( n=30) and a group who received customized orthotic insoles ( n=30). All received conventional rehabilitation training for 4 weeks. Before the fitting, as well as 8 hours and 4 weeks afterward, both groups were evaluated using the Tinetti gait scale (TGS), the plantar pressure balance index, the difference in length between their right and left step, step width, the Timed Up and Go test (TUGT), the Fugl-Meyer lower extremity assessment (FMA-LE), the 6-minute walk test (6MWT), a trunk impairment scale (TIS), the Berg Balance Scale (BBS) and the Barthel Index (BI). Results:At 8 hours after the fitting all of the insole group′s measurements were better than those of the AFO group, on average, but the differences were not statistically significant. After 4 weeks the average TGS, balance index barefoot and wearing the orthosis, step length difference, BBS and BI of the insoles group were significantly better than the AFO group′s averages. The other indicators were not significantly different.Conclusions:Customized orthotic insoles are more effective than an AFO in relieving the biomechanical abnormalities in hemiplegic patients′ feet and ankles, and enhancing their balance and gait.
4.Construction and validation of a risk model for colorectal cancer prognosis based on N6-methyladenosine modification and immune infiltration

Objective To investigate the prognostic value of N6-methyladenosine(m6A)modifi-cation related genes and immune infiltration in colorectal cancer(CRC)and construct a risk model for predicting outcome of patients.Methods The transcriptome data and matched clinical information of CRC patients were downloaded from The Cancer Genome Atlas(TCGA)and Gene Expression Omni-bus(GEO)database.The prognostic value of m6A modification related genes and immune infiltration were investigated using the consensus clustering method and single sample gene set enrichment analy-sis.The weighted gene co-expression network analysis(WGCNA)was used to identify prognostic genes related with m6A modification and immune infiltration.Lasso regression analysis was used to construct a multi-gene risk model.The expression differences of prognostic genes identified were fur-ther validated through expression differential analysis in the Gene Expression Profiling Interactive Anal-ysis(GEPIA)database.Finally,the Kaplan-Meier was used to evaluate the predicting performance of the model in different subgroups and external validation cohorts.Results Both the m6A modification and immune infiltration phenotype could effectively stratify the prognosis of CRC patients from the TCGA cohort.Most m6A modification related genes were significantly correlated with immune infil-tration in CRC tissues.Four following prognostic genes were selected using the WGCNA method combined with Lasso regression analysis:intelectin-1,lymphocyte antigen 6 complex locus G6D,atonal homolog l and matrix metalloproteinase 28.In colorectal cancer tissues,the expression levels of lymphocyte antigen 6 complex locus G6D and matrix metalloproteinase 28 exhibited significant differences compared to adjacent non-cancerous tissues(P<0.05).The risk model constructed based on the above prognostic genes can effectively identify the potential risk population with poor prognosis in different clinical subgroups of the TCGA cohort and the GEO validated cohort.Conclu-sion A risk model based on m6A modification and immune infiltration could effectively predict the clinical outcome of CRC patients,and related prognostic genes have potential to be developed as mo-lecular targets for CRC therapy.

6.Effect of baseline LH/FSH ratio in PCOS on IVF-ET outcomes: a retrospective cohort study

Objective:To exlplore the association between the baseline luteinizing hormone/follicle stimulating hormone (LH/FSH) ratio of polycystic ovary syndrome (PCOS) and in vitro fertilisation-embryo transfer outcomes.Methods:This was a retrospective cohort study. A total of 2 868 PCOS patients were enrolled, all of the participants were patients in The First Affiliated Hospital of Anhui Medical University Hospital from October 2015 to October 2021. Propensity score matching (1∶2.5) was conducted to regulate the non-random allocation of patients. Data were extracted from the hospital′s medical records. Patients with baseline LH/FSH ratio>2 were deemed as study group, patients with baseline LH/FSH ratio≤2 were deemed as control group. Single factor analysis was applied to compare the differences of pregnancy outcomes between two groups.Results:After propensity score matching (1∶2.5), there were no statistically significant differences in baseline data between the two groups (all P>0.05), indicating that the data were comparable. In the study group, the total dose of gonadotropin (Gn) and duration of Gn were lower than those of the control group ( t=4.989, P<0.001; t=3.267, P=0.001), the rate of in vitro maturation was higher than that of the control group ( χ2=4.938, P=0.026), the number of retrieved oocytes and cleavage were higher than those of the control group ( t=-2.305, P=0.021; t=-2.816, P=0.005), but there were no differences in the number and rate of high-quality embryos between the two groups ( t=-1.636, P=0.102; t=-0.123, P=0.902). The incidence of moderate to severe ovarian hyperstimulation syndrome in the study group was significantly higher than that in the control group ( χ2=17.277, P<0.001). Regardless of fresh embryo transfer or frozen-thawed embryo transfer cycles, the incidences of gestational diabetes mellitus in the study group were higher than those in the control group ( χ2=9.174, P=0.002; χ2=4.204, P=0.040) of singleton pregnancy. In the fresh embryo transfer cycle, the clinical pregnancy rate [30.30% (20/66) vs 47.75% (53/111)] and delivery rate [30.30% (20/66) vs 46.85% (52/111)] in the study group were lower than those in the control group ( χ2=5.198, P=0.023; χ2=4.695, P=0.030). In the frozen-thawed embryo transfer cycle, the delivery rate in the study group was higer than that in the control group [59.41% (423/712) vs 55.04% (1 053/1 913); χ2=7.526, P=0.023]. The clinical pregnancy rate and delivery rate of fresh embryo transfer cycle in the study group were significantly lower than those of frozen-thawed embryo transfer cycle ( χ2=21.308, P<0.001; χ2=20.871, P<0.001), but there were no significant differences in the control group (all P>0.05). Conclusions:PCOS patients with a higher basal LH/FSH ratio are more likely to develop moderate to severe ovarian hyperstimulation syndrome after controlled ovarian stimulation and have a higher incidence of gestational diabetes mellitus. Better pregnancy outcome could be obtained by frozen-thawed embryo transfer.
7.Study on the mechanism of Poria cocos polysaccharides on the regulation of gluconeogenesis in liver of type 2 diabetic mellitus model rats

OBJECTIVE To investigate the effect and mechanism of Poria cocos polysaccharides on the regulation of blood glucose in type 2 diabetes mellitus (T2DM)model rats by phosphatidylinositol 3-kinase(PI3K)/protein kinase B (Akt)/forked box transcription factor O 1(FoxO1)pathway. METHODS SD rats were randomly divided into blank control group (no modeling ,no administration),model group (modeling,no administration ),metformin group (modeling,200 mg/kg)and P. cocos polysaccharide low-dose,medium-dose and high-dose groups (modeling,100,200,400 mg/kg),8 in each group. Except for blank control group , other groups were given high fat diet combined with streptozotocin to construct the model of T 2DM rats. At the same time , administration groups were given relevant dose of medicine intragastrically ,and blank control group and model group were given constant volume of water intragastrically ,once a day ,for consecutive 42 days. During the experiment ,general condition and bodyweight of rats were observed every day ;fasting blood glucose (FBG)of rats were collected ,and oral glucose tolerance test were conducted and area under curve (AUC)was calculated the day before last administration. After last medication ,the heart ,liver, kidney organ index were calculated ;the levels of HbA 1c,TC,TG,MDA,SOD,GSH-Px and hepatic glycogen content were detected. HE staining was used to observe the pathological changes of liver and pancreatic tissue ,and the pathological grade score was calculated. Western blot assay was used to detect the protein expressions of p-PI 3K,p-Akt,p-FoxO1, PEPCK and G 6Pase in liver tissues. RESULTS Compared with blank control group ,the rats of model group suffered cc1965@163.com from polydipsia ,polyphagia and polyuria ;the body weight , the levels of SOD and GSH-Px ,the protein expressions of p-PI 3K,p-Akt and p-FoxO 1 were significantly decreased (P<0.05);liver and kidney organ index ,blood glucose level at 0,0.5 and 2 hours after intragastric administration of glucose solution ,AUC, FBG,HbA1c,serum levels of MDA ,TC,TG and hepatic glycogen content ,liver and pancreatic pathological grade score ,the protein expressions of PEPCK and G 6Pase were all increased significantly (P<0.05). Compared with model group ,the general condition of rats in P. cocos polysaccharide groups were all improved ,and all of above indicators had been reversed to varying degrees. CONCLUSIONS P. cocos polysaccharide can downregulate protein expressions of PEPCK and G 6Pase which are key enzymes of gluconeogenesis ,inhibit hepatic gluconeogenesis ,effectively decrease blood glucose levels and regulate glucolipid metabolism in T 2DM model rats by weakening oxidative stress and upregulating PI 3K/Akt/FoxO1 pathway.
