1.Effects of Ramipril on the ventricular remodeling and GLUT4 in diabetic rats complicated with myocardial infarction

Objective To investigate ventricular remodeling and glucose transporter 4(GLUT4) expression in rat with type 2 diabetes mellitus(DM) complicated with myocardial infarction (MI), as well as the effects of ramipril on ventricular remodeling and CLUT4 expression. Method Diabetic animal model (DM) was produced by cholesterol-diet and intraperitoneal injedtion of streptozotocin (STZ,30 mg/kg). The successful model was identified based on the blood gulcose value > 6.7 mmol/L. Left anterior descending coronary artery was ligated to produce the model of MI. 46 wistar rats were randomly(random number) divided into 5 groups: DM + MI + placebo group(placebo group, n = 9), DM + MI + ramipril group(ramipril group, n = 10), DM + sham-operation group (DM group, n = 10),non-DM group + MI (MI group, n = 9), and non-DM + sham-operation group (n = 8). After six weeks, echocardiographic studies was performed ; Left ventricular mass index (LVMI), myocardial infarct size and myocardial collagen volume fraction(CVF) of non-infarction area were also detected. GLUT4 mRNA in the myocardium away from infarction region was measured with real-time fluorescent quantitative RT-PCR and the expression of GLUT4 protein was measured with western blot. Analysis of variance was used to compare the groups. P -values< 0.05 were considered to indicate statistically significant differences. Results Compared with Ml group and DM group,EF[(32 ±3)% vs. (65 ±4)%, P <0.01 and (32 ±3)% vs. (38 ±4)%, P <0.05 respectively] significantly decreased,while E/A ratio (P < 0.01), LVMI (P < 0.05) and CVF (P < 0.05) obviously increased in placebo group. The expression of GLUT4 mRNA and protein were similar between placebo group and DM group, however, much lower than those in MI group(P<0.01).In rampiril group, E/A ratio (P < 0.01), LVMI(3.1±0.2 vs.4.2±0.4, P <0.01) and CVF(P < 0.05) significantly decreased compared with placebo group, while EF[(45±3)% vs.(32±3)%, P <0.01] and the expression of GLUT4 mRNA and protein (P < 0.05) increased. Conclusions Rampiril could improve left ventricular hypertrophy and systolic/di-astolic dysfunction, increase the expression of GLUT4 mRNA and protein in diabetic rats complicated with MI.
2.Application of AIDET communication mode in the therapy of modified electroconvulsive

Objective To evaluate the effects of AIDET ( Acknowledge, Introduce, Duration, Explanation, Thanks ) communication mode in the modified electroconvulsive therapy. Methods Firstly, according to the procedure of the modified electroconvulsive therapy, we established the standard phrases of AIDET communication mode. Secondly, 100 cases of hospitalized psychiatric patients from August to October 2013 accepted the modified electroconvulsive therapy and were divided into the control group and the observation group, with 50 cases in each. The control group used the traditional lecture-style communication, while the observation group was given the AIDET communication processes and standard terms. The anxiety of two groups before and after the treatment was compared by the Hamilton Anxiety Scale ( HAMA ) , in addition with the treatment adherence and the satisfaction of nursing. Results Comparison of the HAMA scores in the two groups before the treatment was not significantly different (P >0. 05). After using the two different communication modes, the HAMA scores of somatic anxiety, mental anxiety and the total score of the observation group were significantly lower than those in the control group (t=5. 99, 6. 51, 10. 87, 10. 23, 6. 19, 7. 94, respectively;P<0. 05). The treatment adherence and the satisfaction with nursing of the observation group were significantly higher than those in the control group (Z =5. 77,χ2 =7. 16, respectively;P <0. 05). Conclusions The AIDET mode of communication can effectively reduce the patient′s anxiety, and improve the patients′adherence, treatment compliance and satisfaction.
3.Hypolipidemic effect and mechanism of Semen Astragali Complanati on hyperlipidemia rats with kidney-yang deficiency pattern

Objective To explore the effect of water extracts from Semen Astragali Complanati (Flatstem Milkvetch Seed,hereinafter“SAC”)on lipid metabolism in hyperlipidemia model rats with kidney-yang deficiency pattern,so as to try to discover the mechanism.Methods Female 10-week-old SD rats were randomly divided into six groups (each n =10):normal group,model group,estrogen group,SAC low-dose group,SAC mid-dose group and SAC high-dose group.The rat model was induced by bilateral ovariectomization combined with high-fat diet feeding.And rats of normal group and model group were in-tragastrically (i.g.)administered with saline,those of estrogen group were i.g.with estradiol valerate (0.2 mg/kg),and three SAC groups were i.g.with SAC extract at the dose of 34,68 and 136 mg/kg for consecutive 8 weeks.The body weight of rats were recorded,serum TC,TG,HDL-C and LDL-C were measured and serum E2 ,FSH and estrogen receptors (ERα,ERβ)mRNA in liver tissue were de-tected.Results Compared with the normal group,the body weight of rats of model group increased sig-nificantly after three weeks’high-fat feeding (P <0.001 );from Week 5 the level of total cholesterol (TC)and low density lipoprotein cholesterol(LDL-C)increased significantly (P <0.001 or P <0.01), and from Week 7 triglyceride(TG)and high density lipoprotein cholesterol(HDL-C)also did (P <0.001 or P <0.05);serum E2 significantly decreased (P <0.01),and follicle stimulating hormone(FSH)in-creased (P <0.05),the expression of ER-αmRNA in liver significantly decreased(P <0.05).High dose of SAC significantly reduced the serum level of TG and TC from Week 7 (P <0.05 or P <0.001), middle dose of SAC reduced the level of LDL-C and increased HDL-C significantly at the same time point (P <0.05,P <0.01),and a more week later,the low dose increased the serum level of HDL-C (P <0.05).Despite the levels of E2 and FSH of model rats showed no significant difference after treatment by SAC (P >0.05),the expression of ER-αmRNA in liver tissue increased in all SAC groups (P <0. 05).Conclusion The effect of regulating blood lipid metabolism of SAC had been proved.The poten-tial mechanism could be via upregulating the expression of estrogen receptors to amplify the signals trans-mitted by the receptors,which may make up the low estrogen level to direct at the disorder of lipid metab-olism during peri-menopausal period.
4.Application and effect of the specialized and standardized training among psychiatric nurses

Objective To investigate the methods and effect of specialized and standardized training, and provide evidence for the revision of the standardized training program.Methods Cluster sampling method was used to recruit all the psychiatric nurses who started their career in 2013 by“needs-based and tutorial manner”cultivating model. These nurses received a three-year standardized training. Competency inventory for registered nurse(CIRN)were used to evaluate the effect of the training program before and after the intervention. Results After three-year standardized training,the total score of CIRN was improved from(114.34±17.12) points to(147.37±14.87)points(t=-21.176,P<0.01). The dimension score of critical thinking/scientific research,clinical care,leadership ability,interpersonal relationship,legal/ethical practice,professional development and education/consultation were respectively(22.60±5.38),(17.46±3.49),(29.09±2.79), (23.69±3.00),(24.29±3.75),(12.74±2.65),(17.51±4.27)after the training,and they were all higher than (14.09±5.14),(10.11±3.21),(27.80±3.54),(22.29±3.03),(23.17±3.670),(8.54±4.50),(8.34±2.81) before the training(t=-13.896,-15.960,-3.277,-3.790,-3.545,-7.727,-14.192;P<0.01).Conclusions “Needs-based,tutorial manner”standardized training model has a positive effect on improving the core competence of new mental health nurses.
5.Application of extended nursing intervention on schizoprene ’ s rehabilitation in community

Objective To explore the effect of extended care on the social function of schizophrenia patients in community rehabilitation.Methods Eighty four cases of schizophrenia were randomly divided into intervention group (Beitang District,40 cases) and control group( Chong’an District, 41cases).All patients received regular drug treatment and health education, and got guildance when discharged.Patients in the intervention group were recived follow-up intervention and telephone interview for 12 months, and bulided the extended care data.Social Disability Screening Schedule ( SDSS ) was used to evalue the social function of patients, and relapse rate and readmission rate were compared between the two groups.Results When discharged 6 months and 12 months, medication adherence and medication management form of the patients in the intervention group were better than that in the control group, the relapse rates in the intervention group were 12.5%, 27.5%, and readmission rates were 7.5%, 17.5%,which lower than 34.1%, 58.5%,26.8%, 48.8%in control group, and the difference was significant (P<0.05).Total scores and partial factors scores of SDSS were lower in the intervention group when dischared 12 months (P<0.05).Conclusions Extended nursing intervention could improve the social function of patients, and reduce relapse rate and readmission rate.
