1.Exercise and Fas mediated apoptosis in cardiomyocytes,skeletal muscle cells and lymphocytes

OBJECTIVE: To analyze the possible mechanism of Fas mediated apoptosis, investigate the correlation of exercise with apoptosis of cardiomyocytes,skeletal muscle and lymphocytes, and explain the changes of histocyte injury under exercise training from a new level of recognition, so as to provide scientific theoretical evidences for the prevention and treatment of overtraining.DATA SOURCES: A computer-based online search of Springlinker and Pubmed databases was undertaken to identify articles related to Fas mediated apoptosis of cardiomyocytes, skeletal muscle and lymphocytes published in English from January 1994 to December 2004 by using the keywords of "Fas, apoptosis, exercise, skeletal muscle, cardiomyocyte, lymphocyte". Meanwhile, Chinese relevant articles published from January 1994 to December 2004 were searched in the Chinese journal full-text database with the keywords of "apoptosis, exercise, skeletal muscle, cardiomyocyte, lymphocyte" in Chinese.STUDY SELECTION: The data were checked firstly. Inclusive criteria:studies related to Fas mediated apoptosis of cardiomyocytes, skeletal muscle and lymphocytes. The repetitive ones were excluded.DATA EXTRACTION: Totally 58 literatures were collected, 29 repetitive studies were excluded, and the other 29 were accorded with the inclusive criteria.DATA SYNTHESIS: Apoptosis is a gene-controlled process of spontaneous death for cells, and it is the physiological cell death. As a death factor inducing apoptosis, Fas is of important biological significance in regulating apoptosis. The study on exercise and the mechanism of apoptosis signal conduction will explain the changes of histocyte injury under exercise training from a new level of recognition, and provide scientific theoretical evidences for the prevention and treatment of overtraining, Besides,Fas plays an important role in various tumors, diseases of immune system and spinal cord injury.CONCLUSION: The deeper investigation on exercise and Fas mediated apoptosis may disclose the mechanism for the treatment of disease, and introduce new methods of exercise treatment, which undoubtably has important practical significance in accelerating healthy of human being.
2.Suppression of antisense-tissue inhibitor of metalloproteinase-1 transfection on bleomycin induced pulmonary fibrosis,morphologically

Objective To observe the morphologic change in rats with pulmonary fibrosis induced by bleomycin through antisense-human tissue inhibitor of metalloproteinase-1(hTIMP-1)transfection.Methods Thirty rats were divided into five groups randomly(six rats in each group),namely,hTIMP-1 transfection group,anti-sense hTIMP-1 transfection group,empty retroviral vectors transfection group,pulmonary fibrosis group,and the normal group.On the 1st,3rd,7th,14th,28th,60th,90th day of BLM administration to the hTIMP-1,anti-sense hTIMP-1 and retroviral vectors transfection group,the hTIMP-1 cDNA retroviral vectors containing sense and anti-sense ones and the empty vectors were conducted into the lungs of rats with pulmonary fibrosis,which were sacrificed on the 28th days.The pulmonary fibrosis group was given just BLM for the same period of time.The rats in the normal control groups were sacrificed on the same day.Observe the role of anti-sense hTIMP-1 transfection and the change of hydroxyproline.Results The degree of fibrosis and content of HYP were extenuated by antisense hTIMP-1 transfection on 1st and 3rd day after BLM exposure compared with the same time of fibrosis and transfection by empty retroviral vectors group,morphologically.Conclusion Transfection with antisense-TIMP-1 on the first and third day can suppress the process of pulmonary fibrosis induced by BLM to some extent.
3.Related factors to the competence of informed consent in new admitted patients with schizophrenia

Objective:To investigate the competency of informed consent in new admitted schizophrenia pa-tients.Methods:Fifty-three patients who were consecutively admitted to the hospital and diagnosed as schizophrenia according to the International Statistical Classification of Diseases and Related Health Problems,Tenth Revision (ICD-10)were recruited and assessed in seventy-two hours.The competence of informed consent was assessed by the MacArthur Competence Assessment Tool-Treatment (MacCAT-T).The insight was assessed by the Insight and Treatment Attitude Questionnaire.The severity of illness was assessed by the Brief Psychiatric Rating Scale (BPRS).Correlations between the competence of informed consent and the factors mentioned above were discussed.Results:Forty-five (84.9%)patients had lower MacCAT-T scores,especially the scores of understand-ing dimension (1.58 ±1.85)and appreciation dimension (2.40 ±1.47).The competence of informed consent was significantly correlated with insight (r =0.73,P <0.01)but not correlated with age,education,duration of illness and BPRS scores.Logistic regression demonstrated that insight (OR =0.69)was a protective factor of competence of informed consent while thought disturbance (OR =1.39)was a risk factor.As to the involuntary admitted pa-tients,34.1% of them had competence of informed consent.Conclusion:The competence of informed consent in new admitted schizophrenia patients is widely impaired especially in understanding and appreciation dimension.The competence of informed consent is correlated with insight and thought disturbance.Voluntary or involuntary hospi-talization does not reflect the level of competence of informed consent.
4.Revision and psychometric evaluation of the Chinese version of Depletion Sensitivity Scale

Objective:To validate the Chinese Version of Depletion Sensitivity Scale (DSS) and test its validity and reliability.Methods:A sample of 499 postgraduates coming from a university in Beijing was investigated with preliminary questionnaire,249 of them were assessed for exploratory factor analysis(EFA) and 250 of them were assessed for confirmatory factor analysis (CFA) and testing of validity and reliability.The criterion validity was tested with the Self-Regulatory Fatigue Scale (SRF-S),Dual-Mode of Self-Control Scale (DMSC-S) and Adolescent Self-rating Life Events Check-list (ASLEC).Totally 280 students were retested for test-retest reliability with 2 week interval.Results:DSS was composed of 8 items in two factors,which were cognitive factor and behavioral factor.The two factors explained 60.7% of the variances.The confirmatory factor analysis identified a two factor model (x2/df=4.18,RMSEA =0.09,NFI =0.95,NNFI =0.94,CFI =0.96,IFI =0.96,GFI =0.96,SRMR =0.05).The scores of depletion sensitivity were positively correlated with the scores of cognitive fatigue,behavioral fatigue,emotional fatigue poor control,punished,loss,relationship pressure and adaption problem (r =0.13-0.55,Ps <0.001).The Cronbach α coefficients were 0.80 for the total questionnaire and 0.79 and 0.73 for the two factors.The test-retest reliability were 0.52 for the total questionnaire and 0.46 and 0.58 for the two factors.Conclusion:It suggests that the Chinese version of Depletion Sensitivity Scale (DSS) is reliable and valid enough to be applicable to measure the depletion sensitivity of Chinese graduate students.
5.Development of the ego depletion aftereffects scale in postgraduates

Objective To develop the ego depletion aftereffects scale (EDA-S) and test its validity and reliability.Methods Based on literature analysis and the open questionnaire survey this study compiled the preliminary questionnaire.A sample of 801 subjects coming from a university in Beijing was investigated with preliminary questionnaire and 406 students were retested for test-retest reliability with 2 week interval.Results The EDA-S was composed of 38 items in nine factors,which were emotional regulation difficulty,social withdrawal,low efficacy,working memory impairment,low processing fluency,work burnout,fatigue,somatic distress and low adherence.The eleven factors explained 72.09% of the variances.Furthermore,the result of CFA indicated that the model fitted the data with well construct validity(x2 =1744.62,df=629,x2/df =2.77,RMSEA =0.06,NFI =0.98,NNFI =0.98,CFI =0.98,IFI =0.98,GFI =0.85,SRMR =0.05) and its factors scores were positively correlated with the SFR-S scores(r=0.31-0.59,P<0.001).The Cronbach α coefficients were 0.95 for the total questionnaire and 0.73-0.91 for the eleven factors.The test-retest reliabilities were 0.60 for the total questionnaire and 0.26-0.57 for the nine factors.Conclusion It suggests that the EDA-S is reliable and valid enough to be applicable to measure the depletion degree of adolescents.
6.Impact of obstructive sleep apnea on nonalcoholic fatty liver disease

Obstructive sleep apnea (OSA) may induce chronic intermittent hypoxia, which may lead to the disorders of multiple systems and organs and even sudden cardiac death in severe cases. Besides respiratory, cardiovascular, endocrine, and metabolic diseases, OSA is also closely associated with nonalcoholic fatty liver disease (NAFLD). This article briefly describes the current status of OSA and NAFLD, introduces the impact of OSA on NAFLD, and reviews the mechanisms of OSA in NAFLD. It is pointed out that clarifying the mechanisms of OSA in affecting NAFLD and discovering potential prevention and treatment targets for NAFLD from the aspect of OSA are of great significance in delaying and even blocking the progression of NAFLD.
7.The status quo and influencing factors of humanistic care provided by psychiatric nurses in Beijing

Objective? To understand the status quo of humanistic care quality of psychiatric nurses in Beijing and its influencing factors. Methods? From May to August 2018, This study selected all the psychiatric hospitals in Beijing and stratified by the tertiary, secondary and first-level psychiatric hospitals. Through random number sampling, 2 to 3 hospitals were selected from each group to conduct cluster surveys. To analyze the humanistic care quality of psychiatric nurses and its influencing factors, 890 psychiatric nurses were surveyed using general information questionnaire, Nursing Caring Characters Assessment Tool (NCCAT), Perceived Organizational Support Scale (POS) and Nurses Professional Values Scale-Revised (NPVS-R). 861 valid surveys were returned with the response rate of 96.74%. SPSS 18.0 was used for data input and statistical analysis. Results? In 861 psychiatric nurses, NCCAT score was 86.36% (125.22±19.79); POS total score was 72.16% (45.46±10.87); NPVS-R score was 81.73% (106.25±21.31). Univariate analysis showed that there were statistically significant differences in the scores of nurses of different levels of hospitals, education backgrounds, professional titles, job satisfaction, parental support, voluntariness, family support, and humanistic training (P<0.05). Pearson correlation analysis showed that the dimensions of humanistic care of psychiatric nurses were positively correlated with organizational support (r=0.593,P<0.01), and professional values (r=0.712,P<0.01). Multiple linear regression analysis showed that hospital level, education, voluntariness, humanistic training, organizational support and professional values were the influencing factors of psychiatric nurses' humanistic care quality (P< 0.01). Conclusions? The quality of humanistic care of psychiatric nurses in Beijing is at a high level. Through humanistic training, strengthening on-the-job education, increasing organizational support, and improving professional values can improve the quality of psychiatric nurses' humanistic care.
8.Clinical study of self-designed Yiqi Huoxue Huayu Decoction combined with western conventional therapy in the treatment of chronic heart failure complicated with atrial fibrillation

Objective:To observe the effects of self-designed Yiqi Huoxue Huayu Decoction on cardiac function and serum endothelin (ET-1) and matrix metalloproteinase-9 (MMP-9) levels in patients with chronic heart failure (GHF) complicated with atrial fibrillation.Methods:A total of 100 patients with GHF complicated with atrial fibrillation treated in our hospital from January 2019 to June 2021 were selected as the study subjects, and divided into experimental group and control group according to random number table method, with 50 patients in each group. Patients in both groups were given cardiotonic, diuretic, vasodilator and other conventional treatment, and patients in the experimental group added self-designed Yiqi Huoxue Huayu Decoction. TCM syndrome score, left ventricular end diastolic diameter (LVEDD), left ventricular end systolic diameter (LVESD) and left ventricular ejection fraction (LVEF), serum ET-1 and MMP-9 levels, clinical efficacy, safety and incidence of adverse reactions were observed and compared between 2 groups before and after treatment.Results:The total response rate was 82.0% (41/50) in the observation group and 62.0% (31/50) in the control group, and the difference was statistically significant ( χ 2=4.96, P=0.026). After treatment, the symptoms scores of palpitation, shortness of breath, fatigue, chest and hypochasm pain in the observation group were significantly significantly lower than those in the control group ( t=5.28, 5.29, 5.62, 5.42, P<0.01). After treatment, the LVEDD[(51.23±6.59)mm vs. (55.65±6.17)mm, t=3.46], LVESD[(43.10±4.76)mm vs. (45.99±5.31)mm, t=2.87], serum ET-1[(65.79±8.29)μg/L vs. (79.83±10.08)μg/L, t=7.61], MMP-9 [(175.86±24.81)ng/L vs. (189.49±26.13)ng/L, t=2.68] in experimental group were significantly lower than those in the control group ( P<0.05), while LVEF [(50.01±7.6)% vs. (46.25±6.96)%, t=2.57] was significantly higher than that of the control group ( P<0.05). There were no significant differences in GPT, GOT, UA, SCr levels between both groups before and after treatment ( P>0.05). There were no obvious adverse reactions betewwn both groups. Conclusion:The self-designed Yiqi Huoxue Huayu Decoction can relieve the clinical symptoms, improve the heart function and serum ET-1 and MMP-9 levels in patients with GHF complicated with atrial fibrillation safely.
9.A case of nasopharyngeal carcinoma with autonomic nervous dysfunction and literature review

Objective:A case of advanced nasopharyngeal carcinoma with autonomic dysfunction was reported and its pathophysiological mechanism was discussed.Methods:The diagnosis and treatment of a nasopharyngeal carcinoma patient with autonomic nervous dysfunction such as paroxysmal syncope was summarized, and the pathophysiological mechanism of this case was analyzed by searching related literature.Results:Nasopharyngeal carcinoma characterized by autonomic dysfunction was rare and had a poor prognosis. Autonomic dysfunction caused by nasopharyngeal carcinoma was associated with carotid sinus syndrome, parapharyngeal space syncope syndrome, glossopharyngeal nerve reflex and paraneoplastic neuropathy.Conclusions:Early detection and treatment is a key factor affecting the prognosis of nasopharyngeal carcinoma. Clinicians should consider nasopharyngeal carcinoma as one of the differential diagnoses in the diagnosis and treatment of patients with autonomic nervous dysfunction combined with cranial nerve damage.
10.Bone marrow mesenchymal stem cell derived exosomes miR-21-5p promotes proliferation, migration and invasion of prostate cancer PC-3 cell by downregulating PHLPP2

[Abstract] Objective: To investigate the effects of exosome originated from bone marrow mesenchymal stem cell (BMSCs) on proliferation, migration and invasion of prostate cancer PC-3 cell and its mechanism. Methods: qPCR was used to detect the expression level of miR-21-5p in prostate cancer cell lines. The morphology of exosomes isolated from BMSCs was observed with an electron microscope. Western blotting was used to detect the expressions of exosome surface markers and the epithelial-mesenchymal transition (EMT)-related proteins (E-cadherin, N-cadherin and Vimentin). Dual luciferase reporter gene experiment was used to detect the targeted regulation relationship between miR-21-5p and PH domain leucine-rich repeat protein phosphatase 2 (PHLPP2). PC-3 cells were co-cultured with 10 μl BMSCs exosomes suspension (Exo group), transfected with sh-PHLPP2 or antagomiR, then CCK-8 and Transwell experiments were used to detect changesinproliferation,migrationandinvasionofPC-3cell.Results: miR-21-5p was highly expressed in prostate cancer PC-3 cell line. The exosomes in the supernatant of BMSCs culture fluid were successfully isolated, and the typical vesicle-like structures of exosomes were observed under transmission electron microscope. Exosomes expressed specific proteins such as CD9, CD63 and CD81. In the Exo group, the proliferation, invasion, migration, as well as the expressions of N-cadherin, Vimentin and miR-21-5p in PC-3 cells were significantly higher than those in the control group (all P<0.05). PHLPP2 is a target gene of miR-21-5p. Compared with the control group, the expression of PHLPP2 in PC-3 cells of Exo group and sh-PHLPP2 group was significantly reduced (0.66±0.09, 0.42±0.05 vs 1.09±0.08, all P<0.01); cell viability, invasion and migration were significantly improved (all P<0.01); and E-cadherin expression level was significantly reduced while N-cadherin and Vimentin expressions were significantly increased (both P<0.05). Conclusion: miR-21-5p is highly expressed in prostate cancer PC-3 cell line. BMSC exosome miR-21-5p can increase the proliferation, migration and invasion ability of PC-3 cells through targeted down-regulation of PHLPP2.
