1.Imaging Diagnosis of Spinal Teratomas

Objective To study the imaging characters of spinal teratomas.Methods 10 cases of spinal teratomas were analyzed with clinical and radiography. 8 cases intramedullary, 2 cases extramedullary. 10 cases had MR examination. Results The location of spinal teratomas in 5 cases was in lumbar, 3 cases in thorax, 1 case in thoracolumbar, 1 case in cervical. 7 cases were multicyst type, 3 cases were singlecyst type. 10 cases of MR image showed mix signal intensity. The contrast enhancement T 1WI with fat saturation presented disappear fat signal of spinal teratomas. The teratomas heterogeneous enhancement was found in 10 cases with MR examination. 3 cases lumbosacral area of spinal teratomas united tethercord. Conclusion Neuroradiological features of spinal teratomas has specific manifestation. The T 1WI fat saturation scan is a availability nicety method for spinal teratomas.
2.The imaging analysis of intracranial malignant teratomas

Objective To analyse the imaging characters of intracranial malignant teratomas (IMT). Methods The imaging appearances of IMT were analyzed in 15 pathologically proved IMT. MRI were performed on all the 15 cases, CT on 8 of them. Results Thirteen IMT showed as a lobular cyst solid mass with clear borders. The solid portion of IMT displayed iso (6/13) to hypointense(7/13) on T 1WI and iso (2/13) to hyperintense(11/13) on T 2WI. The heterogeneous enhancement was found in 11 cases, homogeneous in 2. Two IMT exhibited as the intrasellar solid lesions involving thalamencephalon. Calcification was found in 8 IMT on CT scan. No fat was demonstrated in the tumor on both CT and MR. Conclusion IMT displays relatively typical appearances on imaging studies. Together with the CT scan, The MR study may make the diagnosis before the operation.
3.Imaging Diagnosis of Skull Base Chondromas

Objective To evaluate the value of CT MRI in diagnosing skull base chondromas.Methods 8 cases of skull base chondromas were analyzed with etiology,pathology,imaging fetures and differential diagnosis.5 cases were female,3 cases were male,age ranged from 25 to 48 years.The clinical presentation of 8 cases of skull base chondromas had neurologic syndrome.And 3 cases were examined with CT,MR imaging ,4 cases with MR imaging,1 case with CT scanning.Results In this series of 8 chondromas,3 located in the cavernous sinus,2 in the clival region,2 in the region of jugular foramen,1 in the frontal cranial fossa.Intratumoral calcification was found in 4 cases with CT examination.Heterogeneous enhancement was found in 4 cases with CT examination.The margin of chondromas on CT was well defined.Conclusion CT have a very important diagnosis values for detecting tumors on chondromas.MRI is superior to CT for delineating the growth style of the tumor and its relationship to the surrounding anatomic structures.

5.Imaging Diagnosis of Cranial Extradural Empyema

Objective To evaluate CT and MRI for the diagnosis of cranial extradural empyema.Methods The imaging features in 4 patients with cranial extradural empyema were analyzed.Results 2 cases in frontal,1 case in frontalparietal,1 case in posterier cranial fossa,in this series of 4 cranial extradural empyemas was found homogenous enhancement of dural,and thickened meninges surrounding the empyema.In the series of 1 case show bony thickening and thin.Conclusion The CT and MR of cranial extradural empyema can well demonstrate the morphological and pathological evidence of ivolved menings.Therefore,CT and MR is the most diagnostic value in cranial extradural empyema.

7. Ultrasonic monitoring of carotid blood flow in interposed abdominal pulling-pressing cardiopulmonary resuscitation

Objective:
To explore the difference in ultrasonic monitoring in carotid blood flow, resuscitation effects and prognosis between interposed abdominal pulling-pressing cardiopulmonary resuscitation (IAPP-CPR) and standard cardiopulmonary resuscitation (STD-CPR).
Methods:
Seventy-five cardiac arrest (CA) patients admitted to emergency department of Shijingshan Teaching Hospital of Capital Medical University from June 2015 to December 2017 were enrolled. The patients were divided into STD-CPR group and IAPP-CPR group according to the treatment orders of them and the desire of relatives. All patients were given persistent external compression, airway open, tube intubation, and mechanical ventilation, vasoactive drugs application, defibrillation if required. STD-CPR group was operated according to the 2015 American Heart Association (AHA) CPR guidelines. On the basis of the standard CPR, IAPP-CPR group was recovered using abdominal lifting and compressing CPR instrument to press down to lift the upper abdomen continuously, when the chest compressing relaxed (frequency 100 times/min, down and lift time ratio 1:1, compressing strength 50 kg, lifting strength 30 kg). The patients' gender, age and CA etiology were recorded in the two groups. The vital signs and blood flow of carotid artery were monitored with ultrasonic Doppler during the CPR. The return of spontaneous circulation (ROSC) rate and 48-hour survival rate were observed in patients. The influence factors of ROSC were screened by Logistic regression analysis.
Results:
The data of 75 patients with CA were enrolled finally, with STD-CPR group of 38 patients and IAPP-CPR group of 37 patients. There were no significant differences in patients' gender, age or CA etiology between the two groups. Comparing with STD-CPR group, the peak blood flow velocity of carotid artery in IAPP-CPR group was speeded up significantly (cm/s: 107.16±13.75 vs. 78.99±14.77,

RESULTS:
The data of 75 patients with CA were enrolled finally, with STD-CPR group of 38 patients and IAPP-CPR group of 37 patients. There were no significant differences in patients' gender, age or CA etiology between the two groups. Comparing with STD-CPR group, the peak blood flow velocity of carotid artery in IAPP-CPR group was speeded up significantly (cm/s: 107.16±13.75 vs. 78.99±14.77, P < 0.01), the overall blood flow volume of carotid artery was increased significantly (mL/min: 989.06±115.88 vs. 751.62±118.92, P < 0.01), but there was no significant difference in inner diameter of carotid artery between the two groups (mm: 4.55±0.25 vs. 4.61±0.21, P > 0.05). During the CPR, the mean arterial pressure (MAP) and the transcutaneous oxygen saturation (SpO2) in IAPP-CPR group were significantly higher than those of STD-CPR group, but no significant difference was found in heart rate between the two groups. Four patients in STD-CPR group got ROSC, and 3 survived over 48 hours (1 myocardial infarction patient died of ventricular fibrillation) while 6 patients in IAPP-CPR group got ROSC and survived over 48 hours. There was no significant difference in ROSC rate or 48-hour survival rate between the two groups, but data of IAPP-CPR group was slightly higher than that of STD-CPR group [ROSC rate: 16.22% (6/37) vs. 10.53% (4/38), 48-hour survival rate: 16.22% (6/37) vs. 7.89% (3/38), both P > 0.05]. Multivariate Logistic regression analysis showed that the higher the MAP during CPR, the greater the possibility of ROSC was [odds ratio (OR) = 1.361, 95% confidence interval (95%CI) = 1.182-1.669, P = 0.030].
CONCLUSIONS
IAPP-CPR was superior to traditional STD-CPR in improving arterial blood flow and resuscitation effect, but no superiority was found in ROSC rate and survival rate, which may be relate to the small number of patients that included in this study. More clinic trials are needed.

A fungal carotid aneurysm is an infrequently occurring infectious aneurysm that is usually treated by
antifungal therapy plus surgical debridement of the infected vessel. We herein report an extremely
rare case involving a patient with a medical history of bladder cancer treated by Bacillus Calmette–
Guérin (BCG) who developed a fungal aneurysm of the internal carotid artery and thrombosis of
the superior ophthalmic vein caused by maxillary Aspergillus sinusitis. The patient was successfully
treated by antifungal, anticoagulant, and antiplatelet drugs combined with internal carotid artery
ligation therapy. Internal carotid artery fungal aneurysm is associated with a high mortality rate, but
the present case suggests that it can be successfully treated by antifungal therapy combined with a
less invasive endovascular therapy.
