1.Anatomical base of radial nervetransfer operation

[Objective]Through the anatomical observation and measurement on middle and distal humerus and radial nerve,To investigete a new radial nerve transfer operation.[Method]By means of anatomical methods,radial nerve's stretching branch,the relationship between radial nerve and deep brachial vessel in 30 upper climbs were observed and measured on 15 adults fixed in 10% formalin.The anatomical structures of humerus of one hundred specimens were also observed.This data were made by designing and performing the radial transfer operation after statistical analyzing.[Result](1)Length of humerus: The left side was (30.60?1.46)cm and the right side(31.37?1.23)cm.The length of sulcus for radial nerve: the left side was(5.65?1.01)cm and the right side(6.57?0.58)cm.The stretch angle of radial nerve: the left was 13.00??1.08?,the right13.86??0.97?.(2)The stretch angle of radial nerve: the radial nerve in the sulcus for radial nerve went from superomidial region to lateral lower region and the angle open to the inner part was formed.The angle is obvious in 29 sides,that was 96.67%,and not obvious only in 1 side,that was 3.33%.(3)The position of the radial nerve superficial and deep branches: The radial nerve in 17 sides branches were above interepicondylar line,which was 76.67%.Below the line were 10 sides,cover 33.33% and on the line 3 sides,cover 10.00%.(4)The curve distance between the exit of radial nerve and the brarnches was(14.26?1.01)cm.(5)The relationship between radial nerve and deep brachial vessel: the radial nerve lies inside of the deep brachial vessel were 12 sides,lies the outside of the deep brachial vessel were 10 sides and cross the deep brachial vessel were 8 sides.[Conclusion]The design of radial nerve transfer operation is reasonable,it conforms to the radial nerve's anatomy and physiology and provides much wider operation space for the performing of all kinds of internal fixation on fractures of middle and distal humerus.It reduces the injury of the radial nerve in the course of the operation and alter the operation while taking the internal fixation substance out.It doesn't increase the difficulty and injury.
2.The impact factors analysis of hospitalized patients with coronary heart disease accompanying depression disorders

Objective To explore the status and impact factors of hospitalized patients with coronary heart disease(CHD) accompanying depression disorders.Methods Adopt HRSD,SDS and general questionnaire for survey tools to evaluate the 300 hospitalized patients with CHD.Using SPSS13.0 software to build database,and the data was analyzed by descriptive statistics analysis and correlation analysis.Result There were 148 people accompanying depression disorders among the hospitalized·patients with CHD(47.30% ),and 68 people were treated (47.89%).Multivariate logistic regression indicated that the age ( OR =-0.415,P < 0.05,CI =0.443 ~0.984),smoking history( OR =0.384,P < 0.05,CI =1.118 ~ 1.928 ),course of disease ( OR =-0.250,P <0.05,CI =0.608 ~ 0.996),myocardial infarction ( OR =0.676,P < 0.05,CI =1.082 ~ 3.576 ),family history ( OR =-0.744,P < 0.05,CI =0.231 ~ 0.978 ) were related to coronary artery disease accompanying depression disorders.Multivariate correlation analysis indicated that anxiety factor and gender were closely related,weight factor and age,educational level,myocardial infarction had closed relationship,cognitive factors were closely associated with the course of disease,day and night changes were in relation to age,block factor and gender,age,education,drinking history,smoking history were related closely,sleep factor and gender,onset form,hypertension relations closed,despair factor have closed relationship with gender,age,educational level,course of disease.Conclusion Age,smoking history,course of disease,myocardial infarction,family history were important risk factors of depression disorders in hospitalized patients with CHD.The HRSD's seven symptoms factors are closely associated with multipling influence factors.
3.Detect of P53,bax and caspase 3 genes expression after optic nerve injury in rats with SYBR green I fluorescence quantitative PCR

Objective Previous study showed that the histopathological basis of visual function damage caused by optical nerve injury is apoptosis of retinal ganglion cells(RGCs). This procedure is regulated by P53, bax and caspase 3 genes. Present study aimed to observe the expression of bax, P53 and caspase 3 mRNA in RGCs after traumatic optic nerve damage in the rats by SYBR green I fluorescence quantitative PCR method. Methods The animal model of optic nerve injury was established in the right eyes of 56 adult Wistar rats by a fluid percussion brain injury device (FPI) . Animal were killed on days 1, 3, 5, 7, 9, 14, 28 days separately after injury. Other 16 Wistar rats were divided into normal control group and sham operation group. The total RNA was isolated from rat fresh retina tissue by Trizol method and was treated by reverse transcription to cDNA using 01igo(dt) 18 as primer and then amplified. The target fragments of bax, P53 and caspase 3 cDNA were linked with carrier pTZ57 R/T to construct recombined plasmids which were transformated to E. Coli DH5α by T/A clone method. Recombined plasmids were extracted with alkaline lysis method and the plasmids were selected in white colonies by ampicillin screening, EcoR I restrictive enzyme analysis, and their specificity was evaluated using DNA sequencing. The standard curves were created by plasmid DNA and the precise expression level of target genes in samples were determined using software. The results were expressed as the ratios of target gene mRNA to GAPDH mRNA. Results The standard curve drawn by pTZ57R/T and target gene presented a good linear tendency with the higher sensitivity and specificity. The expression of P53 and bax mRNA began to increase on the third day after the injury of optic nerve and peaked on the fifth day and started to decline on the seventh day. The expression of caspase 3 mRNA increased from the fifth day through the ninth days after injury and declined on the fourteenth day. The significant differences were found in the expression of P53, bax and caspase 3 between model group and control group (P < 0. 05) . Conclusion The pro-apoptotic protein P53, bax and caspase 3 play an important role in RGCs apoptosis.
4.Expression and significance of S100A4 and E-cadherin in colorectal carcinoma

Objective To investigate the expression and significance of S100A4 protein and Ecadherin in coloreetal carcinoma. Methods S-P immunohistochemical method was used to detect the expression of S100A4 and E-cadherin in 87cases with colorectal carcinoma and 87 cases with adjacent colorectal tissue, and the expression of S100A4 and E--cadherin were analyzed with relation to clinicopathologic factors and post-operative five-year survival. Results There was no expression for S100A4 protein in glandular epithelium of adjacent colorectal tissues. The positive expression rate of S100A4 was 64.4 %(56/87) in colorectal carcinoma. There was a significant difference between eolorectal carcinoma and adjacent group(P <0.01). The expression of S100A4 was positively correlated with the clinical stages, lymph node metastasis and five-year survival (P <0.05), but not with other clinicopathalagic factors (P >0.05). There was 100 % expression for E-cadherin in adjacent colorectal tissues. The positive expression rate of Ecadherin was 62.1%(54187) in colorectal carcinoma. There was a significant difference between colorectal carcinoma and adjacent group (P <0.01). The expression of E-cadherin was positively correlated with the clinical stages, lymph node metastasis, tumor site and five-year survival (P <0.05), hut not with other clinicopathologic factors (P >0.05). The expression of S100A4 was negatively correlated to E-cadherin in colorectal carcinoma without statistical meaning(r =-0.087, P >0.05). Conclusion S100A4 and E-cadherin are closely related with colorectal cancer invasion, metastasis and prognosis; S100A4 might be an important predictor of the clinicopathologic features and prognosis of eolorectal carcinoma.
5.Absorbable rod versus Herbert screw for radial head fractures:therapeutic effects and treatment costs

BACKGROUND:Mason II type and III type radial head fractures with massive bone are mainly treated by open reduction and metal fixation, but metal implants have to be removed in the second operation, which increased patients’ travail and financial burden. Absorbable screw/rod also can be used to treat radial head fractures, and has its special advantages due to the low price and no second operation. OBJECTIVE:To compare the clinical therapeutic effects of absorbable rod and Herbert screw for the treatment of Mason II or III type radial head fractures. METHODS:A total of 80 patients with Mason II and III type radial head fractures were equal y assigned to experimental and control groups. They received open reduction and internal fixation. The experimental group was treated with absorbable rod and the control group received Herbert screw. RESULTS AND CONCLUSION:A total of 79 patients were fol owed up for averagely 34 months. No significant difference in average operative time, fracture healing time, Broberg and Morrey elbow scores and incidence of complications was detected between the experimental and control groups (P>0.05). However, treatment costs were lower in the experimental group than in the control group (P<0.05). Results indicated that the therapeutic effects between absorbable rod and Herbert screw for Mason II or III type radial head fractures were similar. However, absorbable rod for radial head fractures can avoid the second operation for removal of the implant. Therefore, we recommend absorbable rod in the choice of internal fixation materials.
6.Radial head replacement versus open reduction and internal fixation for comminuted radial head fractures:more advantageous?

BACKGROUND:At present, the methods of treating unstable comminuted radial head fractures contain open reduction and internal fixation and metal prosthesis replacement. There were success cases treated by the two methods, but some shortcomings simultaneously existed.
OBJECTIVE:To compare the clinical therapeutic effects of replacement of the radial head with metal prosthesis with open reduction and internal fixation for the treatment of unstable comminuted radial head fractures.
METHODS:A prospective randomized control ed analysis was performed in 45 cases of unstable comminuted radial head fractures. These cases received open reduction and internal fixation and metal prosthesis replacement. This study compared the Broberg and Morrey elbow joint function score and the incidence of complications after fixation, and performed statistical analysis.
RESULTS AND CONCLUSION:The subjects were fol owed up for 1-5 years, averagely 2.8 years. According to Broberg and Morrey scores, the average score was 90.1 and the incidence of complications was 13.6%in the prosthesis replacement group. The average score was 76.8 and the incidence of complications was 47.9%in the open reduction. Significant differences were visible between the two groups (P<0.01). Compared with the open reduction group, prosthesis replacement for unstable comminuted radial head fractures obtained better joint function and lower incidence of complications.
7.Percutaneous mechanical thrombectomy for the treatment of acute massive pulmonary embolism: its preliminary clinical application

Objective Acute massive pulmonary embolism(PE)is a clinical emergency requiring rapid and supportive measures.With the development of interventional technology and devices,percutaneous mechanical thrombectomy(PMT)is considered to be an alternative for the treatment of PE,though there is still relative lack of clinical experience.The purpose of this study is to evaluate the clinical efficacy and safety of PMT in the management of acute massive PE.Methods The clinical data of massive PE patients treated with interventional methods were collected and analyzed in a retrospective way.From Jan.2003 to Jan.2008,6 patients(5 males and one female,with a mean age of 62 years)with acute massive PE,which was initially diagnosed by computed tomography and finally confirmed by pulmonary angiography,were treated with percutaneous catheter fragmentation and/or Straub Rotarex thrombectomy device.Results The improvement of clinical status and restoration of blood flow in the main branches of pulmonary artery were obtained in all patients.Oxygen saturation(SaO2)increased from preoperative(79.5±5.3)%to postoporative (92.8±3.4)%,with P<0.01.Partial arterial oxygen pressure(PaO2)increased from preoperative(58.0±9.8)mmHg to postoperative(88.7±4.1)mmHg(P<0.01).After PMT treatment,the mean pulmonary artery pressure(PAP)decreased from preoperative(40.8±7.8)mmHg to postoperative(29.8±8.0)mmHg (P<0.01).Miller index decreased from preoperative 0.54±0.03 to postoperative 0.18±0.07(P<0.01).During a clinical follow-up period ranged from 1 to 5 years,four patients showed no recurrence of PE,the other two patients lost touch with the authors.Conclusion The preliminary experience in onr series suggests that PMT is an easy,effective and safe therapy in the clinical management of acute massive PE,especially when thrombolysis is contraindicated.
8.Electronically aided rehabilitation exercise to improve the cardiac function of patients with chronic hear failure

Objective To study the effect of assistive functional rehabilitation exercise on cardiac functioning of patients with chronic heart failure (CHF). Methods Sixty CHF patients were divided randomly into a treatment group (the rehabilitation group) and a control group, with 30 in each group. All the patients were administered routine therapy. In the treatment group, the patients were administered rehabilitation exercises with the assistance of a electric equipment made by the authors, daily for 5 days a week for a total of 3 months. The New York Heart Association (NYHA) cardiac function grading, the left ventricular ejection fraction(LVEF), the left ventricular end diastolic diameter (LVEDD) and the brain natriuretic peptide (BNP) level in plasma as well as the 6 min walking range were observed in both groups before and after treatment. Results After 3-months of treatment, the NYHA grading, LVEF, LVEDD, BNP level in plasma and 6 min walking range were all significantly improved in both groups when compared with those before the treatment, with the treatment group improved to a significantly larger extent than the control group ( p<0.05 ). Conclusion Assistive rehabilitation exercise in addition to the routine therapy can significantly help improve the cardiac function in CHF patients.
9.Correlation between cognitive function and P50 sensory gating in patients with drug-na¨ive first-episode schizophrenia

Objective:To explore the relation of cognitive function correlates to P50 sensory gating in patients with schizophrenia.Methods:Totally 106 patients with drug-na¨ive first-episode schizophrenia and 86 healthy con-trol subjects matched for age,sex and education were recruited.All patients met the Diagnostic and Statistical Man-ual of Mental Disorders,Fourth Edition (DSM-IV)criteria for schizophrenia.Their cognitive function were assessed with the MATRICS Consensus Cognitive Battery (MCCB).The P50 auditory gating potential were recorded in all subjects using conditioning-testing stimulus paradigm and stimulus train paradigm.The P50 components were meas-ured in S1 S2 latency,S1 S2 amplitude and P50 suppression ratios.Results:Compared with the controls subjects, the patients with schizophrenia had significantly longer S1[60.8 ±7.8)ms vs.(56.3 ±7.0)ms,P <0.05]S2 la-tency [(59.1 ±10.2)ms vs.(55.7 ±10.7)ms,P <0.05],lower S1 amplitude [(2.8 ±1.8)μV vs.(3.5 ±1.6)μV,P <0.05],less amplitude difference (S1-S2)[(1.5 ±1.8)μV vs.(2.2 ±1.4)μV,P <0.05]and higher P50 suppression ratios (S2 /S1 )[(59.7 ±65.6)vs.(37.6 ±30.0),P <0.05].Schizophrenia patients had lower score than control subjects in all cognitive domains of MCCB (Ps <0.05).No differences in neurocognitive performance were found between subjects in the normal P50 ratio and the abnormal in healthy controls or patients with schizo-phrenia (Ps >0.05).No significant correlation was found between P50 gating measures (P50 ratio and P50 ampli-tude difference)and neuropsychological measures in MCCB scores (Ps >0.05).Conclusion:There may be no as-sociation between P50 deficits and cognitive measures in patients with drug-na¨ive first-episode schizophrenia.
