2.Formulation Optimization of Budesonide Sustained-release Tablet by Central Composite Design-response Surface Method

OBJECTIVE:To optimize the formulation of Budesonide sustained-release tablet. METHODS:Using the cumula-tive releases in 2,4,8 h as investigation indexes,central composite design-response surface method was used to optimize the amount of hydroxypropylcellulose L(HPC-L),amount of soybean phosphatides,and filler(fixed total 200 mg)lactose- micro-crystalline cellulose mass ratio in the formulation of Budesonide sustained-release tablet,and the verification test was conducted. The release behaviors of prepared sustained-release tablet and original preparation in pH 7.2,7.0,6.8 phosphate buffer were com-pared. RESULTS:The optimal formulation was as follow as budesonide of 9 mg,HPC-L of 46.49 mg,soybean phosphatides of 9.23 mg,filler lactose-microcrystalline cellulose mass ratio of 1:2.9;the cumulative releases in 2,4,8 h were 21.9%,50.1%, 99.5%,the relative errors with predicted values (22.0%,50.0%,98.5%) were 0.45%,0.20%,1.02%(n=3),respectively. Compared with cumulative release of original preparation,the f2 was higher than 50. CONCLUSIONS:Budesonide sustained-re-lease tablet is successfully prepared,which shows similar release behavior to original preparations.

Objective To study the expression of matrix metalloproteinase (MMP) and the tissue inhibitor of matrix metalloproteinase (TIMP) in hepatocellular carcinoma (HCC) and its relation with the prognosis of patient. Methods MMP 1,MMP 2,MMP 9 and TIMP 2 in human HCC tissue and HCC cell line were measured and quantified with immunohistochemistry, Northern blot hybridization and image analysis. Results MMP 1,MMP 2 and MMP 9 were localized in the cytoplasm of tumor cells. Objective To investigate the effects of "a determinant variants in chronic hepatitis B(CHB)patients on the expression of hepatitis B surface antigen(HBsAg) and anti-HBs antibodies (HBsAb). Methods Eight hundred sixty-six chronic hepatitis B patients were enrolled, which HBs Ag carriage was beyond a 6 month period.77 patients(8.9%)concomitantly carried both HBs Ag and anti-HBs antibodies,789 patients(91.1%)were only HBs Ag positive. Selection criteria for patients with both HBs Ag and anti-HBs were mainly focused on anti-HBs titers at least three times above the analytical threshold of the technique(10 U/L)on at least three consecutive visits.14 patients were selected from77 patients, who presented both markers(group Ⅰ),and 12 patients from another 789 patients who positive for HBs Ag only(group Ⅱ)were randomly selected as controls. The HBs Ag-encoding gene was amplified and cloned, and at least 15clones per patient were sequenced and analyzed. Results The number of residue changes within the S protein group Ⅰ was 2.7 times more frequently than that in group Ⅱ patients, and "a" determinant of the major hydrophilic region(MHR)occurred mostly. Ten patients (71%)from group Ⅰ and three patients(25%)from group Ⅱ presented at least two residue changes in the MHR. The most frequent changes in group Ⅰ patients were located at positions s145,s129,s126,s144, and s123 as described for immune escape variants. Conclusions In CHB patients, the coexistence of HBsAg and HBsAb is associated with an increase of "a" determinant variability, suggesting a selection of HBV immune escape mutants during chronic carriage. The consequences of this selection process play an important role in vaccine efficacy, diagnosis and clinical therapy.
5.Onset time of cough variant asthma

The characteristics of onset time of cough variant asthma (CVA) match with the human hypnagogic and arousal time nodes, which are also in accordance with the time of defensive qi entering yin and exiting yang of human body. Reviewing the generation and physiological function of defensive qi, we hold that the onset of CVA is related to the lung, the spleen and the kidney. Deficiency of the three viscera leads to defi ciency of defensive qi defi ciency, and blockade of the circulation pathway of defensive qi on body surface, thus interaction of the two factors results in the special time nods of CVA’s onset, which is also the basic pathomechenism of CVA.
6.Influence of Bushenqingtou Decoction on serum IL-2,IL-4,IL-10,TNF-α and IFN-γin chronic hepatitis B patients with positive HBeAg

Objective To observe the changes of peripheral blood interleukin-2(IL-2) ,interleukin-4(IL-4) ,interleukin-10(IL-10) ,tumor necrosis factor-alpha(TNF-α) and interferon gamma (IFN-γ) in chronic hepatitis B(CHB) patients with positive e-anti-gen(HBeAg) treated by Bushenqingtou Decoction .Methods 60 cases of CHB with positive HBeAg were randomly divided into the treatment group and the control group ,30 cases in each group .The CHB treatment group received Bushenqingtou Decoction with the treatment course of 48 week ,while the CHB control group did not received the medication therapy .In the beginning and at 48 weeks of treatment ,the blood in the two CHB groups were collected for detecting HBV DNA ,IL-2 ,IL-4 ,IL-10 ,TNF-αand IFN-γ. Results Compared with before treatments ,the levels of IL-2 ,TNF-αand IFN-γafter treatments in the CHB treatment group were obviously increased ,while the levels of HBV DNA ,IL-4 and IL-10 were remarkably decreased (P<0 .01);however ,there were no statistical differences of the various indexes after the treatments in the CHB control group (P>0 .05) .Conclusion Bushenqingtou Decoction could inhibit the replication of HBV DNA in the CHB patients with positive HBeAg and improve the body immune func-tion .
7.Surgical treatment of vascular injury

Objective To study the management of vascular injury.Methods Retrospective analysis was made on the clinical data of 59 cases of vascular injury,including 55 cases of vascular injury in neck and(extremity) and 4 cases of portal vein and vena cava injury.Among them,21 cases had femoral artery injury with infection and 4 cases had vascular injury due to intervention therapy.All patients with vascular wound of extremity or neck had undergone hemostasis by compression and antishock treament before hospital admission.All cases of femoral artery injury with infection underwent hemostasis by arterial ligation and incision and(drainage) of abscess.Vascular anastomosis was performed in 11 cases,vascular grafting in 12 cases,and(vascular) repair in 14 cases.Results There were 2 deaths.5 cases had amputation(including a case of(femoral) embolism due to intervention trerapy).Postoperative intermittent claudication,decreased skin(temperature) and other signs of ischemia occurred in 21 cases of femoral artery injury with infection,but none developed limb gangrene. The other cases were discharged in good health.Conclusions In the treatment of vascular injury,wound hemostasis and antishock treatment should be done first to save the patient′s life and the management of the vascular injury depends on the situation,with the aim to try by all means to save the extremity.Vascular reconstruction is the main method for treatment of vassular injury.Vascular ligation can be done in cases of femoral artery injury with infection.
8.Diagnosis and treatment progress of liver injury after allogeneic hematopoietic stem cell transplantation

Allogeneic hematopoietic stem cell transplantation (allo-HSCT) is a curative method for various hematological diseases. With the optimization of transplantation technology, the clinical application of allo-HSCT is more and more mature. Post-transplant liver injury is one of the common postoperative complications, which seriously affects the quality of life and long-term survival of patients. The causes of liver injury after allo-HSCT can be divided into non-infectious and infectious factors, which show similar clinical manifestations and different treatment principles. Timely diagnosis of post-transplant liver injury and the identification of the disease cause will be beneficial for early prevention or targeted treatment, thereby improving patients' prognosis. This review focuses on the etiology, clinical features, and treatment options of liver injury after allo-HSCT, aiming to deepen the understanding of hematologists on liver injury after allo-HSCT.
9.Quantification of bcr/abl mRNA expression level in leukemia cells by FQ-RT-PCR.

OBJECTIVETo establish a fluorogenic probe quantitative RT-PCR (FQ-RT-PCR) method for detection of bcr/abl mRNA fusion gene expression level in leukemia cells, and provide a useful tool for leukaemia diagnosis and minimal residual disease inspectation.
METHODThe conventional RT-PCR was used to amplify bcr/abl gene from cultured K562 cells, the quantitative standard template was constructed with A-T clone method. The fluo-rogenic quantitative RT-PCR method by using Applied Biosystems 7700 Sequence Detector for detecting the expression of bcr/abl fusion gene was successfully. The sensitivity, stability and repetitiveness of this method was determined. The peripheral blood samples from 14 CML patients, one of whom before and one month after bone marrow transplantation (BMT) and 4 cases of ALL in the early stages were detected.
RESULTSThe sensitivity of FQ-RT-PCR for detecting bcr/abl fusion gene was about 10(-5) micro g RNA from K562 cell and 10 copies recombined plasmid. The repetition CT value (cycle threshold) and the coefficient variation (CV) among tubes and batches were 2.0% and 3.7%, respectively. The median bcr/abl fusion gene expression level of 14 CML patients was 5.15 x 10(4) copies/ micro g RNA. The products analyzed by electrophoresis showed that 11 cases were b2a2 and 3 cases b3a2. 1.2 x 10(5) copies/ micro g RNA in one CML patient before BMT was changed to 2.3 x 10(3) copies/ micro g RNA one month after BMT. B2a2 was observed in one of the four (25.0%) patients with ALL, and its level of expression was 8.2 x 10(5) copies/ micro g RNA.
CONCLUSIONThe established FQ-RT-PCR method is sensitive, specific, reliable, accurate and good at repetitiveness. The results expressed in copies were easy for evaluation and comparation. Two different bcr/abl fusion gene form - b2a2, b3a2 can be detected by the method. It can be widely applied to diagnosis and detection of minimal residual disease for CML and some ALL patients.

10.Clinical effect of minocycline in treating severe ocularrosacea

Objective:To evaluate the effect of minocycline on severe ocular rosacea.Methods:Twenty-three patients with severe ocular rosacea were recruited and received 8 weeks treatment. Oral minocycline 100 mg was given daily for the first 2 weeks, and 50 mg daily minocycline daily for the next 6 weeks. The best corrected visual acuity and the eye and body skin conditions before and after treatment were recorded. The visual acuity and the degree of skin and body inflammation were compared before and after treatment to evaluate the efficacy.Results:After 2 weeks of minocycline treatment for severe rosacea eye type, 14 patients had improved eye conditions and dermatitis subsided, with an effective rate of 60.87%; 23 patients had an effective rate of 100% after 8 weeks of treatment. No adverse drug reactions were seen during treatment. The visual acuity before treatment was 0.20±0.09, and the visual acuity was 0.14±0.07 8 weeks after treatment. The patients were followed up for 18 months without recurrence.Conclusions:Minocycline is safe and effective in treating severe ocular rosacea and can control recurrence.
