1.Diagnostic Value of Multi-slice Spiral CT in Non-small Cell Lung Cancer Preoperative TNM Staging

PurposeTo evaluate the diagnostic value of multi-slice spiral CT (MSCT) in preoperative TNM staging in non-small cell lung cancer (NSCLC) by comparing with corresponding postoperative pathology.Materials and Methods Ninety-two cases of surgically resected lung cancer were included. Clinical and pathological staging were made and compared, and to evaluate the sensitivity, specificity and accuracy of the MSCT in diagnosing lymph node metastasis.Results MSCT made correct T staging in 83.7% of the cases with excellent agreement (Kappa=0.727,P<0.05). Accurate N staging was in 79.3% of the patients with acceptable agreement (Kappa=0.635,P<0.05). The TNM staging was 75.0% accurate with moderate agreement (Kappa=0.680,P<0.05). The sensitivity, specificity, positive predictive value, negative predictive value and accuracy of CT in diagnosis of lymph node metastasis were 71.1%, 85.2%, 77.1%, 80.7% and 79.3%, respectively. CT accuracy in the diagnosis of groups 4, 5, 6, 10 lymph nodes metastasis was relatively low, in which group 4 had highest false positive, and group 10 had the highest false negative.Conclusion The TNM staging of NSCLC based on MSCT scan is valuable. It can be used as the main basis to evaluate the patient's ability to choose the best treatment plan and predict the prognosis.
2.Research progress of traditional Chinese medicine intervention in pre-diabetes

With the continuous improvement of people's living standard, more and more people are in pre-diabetes state. Pre-diabetes is the key to the development of diabetes, and early intervention can reduce the incidence of diabetes, and prevent transforming pre-diabetes to diabetes in order to maintain the health status of the patient. By retrieving the Chinese Biomedical Literature Database of nearly five years on pre-diabetes intervention literature, it was found that traditional Chinese medicine interventions in pre-diabetes have a relatively new understanding. Through the traditional Chinese medicine, acupuncture, acupoint massage, and combine traditional Chinese and western medicine, medicinal food, eight brocade etc intervention therapy in patients with pre-diabetes, the occurrence and development of diabetes and its complications can be effectively prevented.
3.Progress of long non-coding RNA DANCR in thyroid cancer

Thyroid cancer is the most common head and neck malignant tumor, and its incidence is increasing year by year. Therefore, it is of great significance to explore the mechanism of the occurrence and development of thyroid cancer, and develop the diagnostic markers and therapeutic targets of thyroid cancer, so as to improve the curative effect of thyroid cancer and prolong the survival time of patients. The previous study has found that long non-coding RNA (lncRNA) DANCR has abnormal high expression in thyroid cancer, which could promote the expression of leucine-rich repeat kinase 2 (LRRK2) by affecting miRNA-185-5p and transcription factor SMARCB1, and indirectly participate in autophagy and regulate the development of thyroid cancer. LncRNA DANCR is expected to become a new target for diagnosis and treatment of thyroid cancer. This paper introduces the possible regulatory mechanism of lncRNA DANCR in thyroid cancer, and provides new ideas for the basic research and clinical diagnosis and treatment of thyroid cancer.
4.Relationship of subjective health complaints and health promotion lifestyles among nursing college students

Objective To explore the relationship of subjective health complaints and health promotion lifestyles among nursing college students.Methods A total of 775 nursing students in a college in Guangzhou city were surveyed using Subjective Health Complaints Scale and Health Promoting Lifestyles-Ⅱ (HPLP-Ⅱ),and then analyzed the datum of the investigation.Results The incidence rate of subjective health complaints was 57.9%(449/775).The symptoms of the top five were fatigue [19.2%(149/775)],eye discomfort [18.5%(143/775)],gastrointestinal upset [18.2%(141/775)],sleep disorders [16.6%(129/775)] and poor interpersonal relationship [11.0%(85/775)].The mean score of the HPLP-Ⅱ was 133.74±19.10,both scores of the HPLP-Ⅱ and the subscales in the nursing students without complaints were significantly higher than the others,P < 0.01.Further multinomial Logistic results suggested HPLP-Ⅱ negatively related with the occurrence of subjective health complaints.Conclusion Health promotion lifestyle can prevent or improve the complaints status common existed among nursing college students.
5.Effects of dexmedetomidine on action potential in isolated rabbit sinoatrial node cells

Objective To evaluate the effects of dexmedetomidine on action potential in isolated rabbit sinoatrial node cells.Methods Twenty-four healthy New Zealand rabbits of both sexes,weighing 1.5-2.5 kg,were sacrificed.Their hearts were removed and sinoatrial nodes were dissected and placed in Tyrode solution.Sixty isolated sinoatrial nodes were randomly divided into 6 groups (n =10 each):control group (group C),0.5 ng/ml dexmedetomedine group (group D1),5.0 ng/ml dexmedetomedine group (group D2),dexmedetomidine 5.0 ng/ml + yohimbine 1 μmol/L (alpha 2-adrenoreceptor antagonist) group (group D2 + Y),dexmedetomidine 5.0 ng/ml + 2 mmol/L CsCl (blocker of pacemaker current) group (group D2 + C),and dexmedetomidine 50.0 ng/ml group (group D3).After perfusion with the Tyrode' s solution for 60 min,C group was perfused with Tyrode' s solution for another 40 min,D1-3 groups were perfused with Tyrode's solution cont(a)ining dexmedetomedine 0.5,5.0 and 50.0 ng/ml,respectively,for40 min,and D2 + Y and D2 + C groups were perfused with Tyrode' s solution containing 1 μmol/L yohimbine and 2 mmol/L CsCl,respectively,for 20 min,and then 5.0 ng/ml dexmedetomedine was added and sinoatrial nodes were perfused for 20 min.Maximal rate of depolarization (Vmax),amplitude of action potential (APA),action potential duration at 50% and 90% repolarization (APDso and APD90),velocity of diastolic (phase 4) depolarization (VDD),and rate of pacemaker firing (RPF) were recorded at 60 min of perfusion with Tyrode' s solution (T1),20 min of perfusion with yohimbine or CsCl (T2) and termination of perfusion with dexmedetomedine (T3).Results Compared with group C,APA,VDD and RPF were gradually decreased at T2,3 in groups D1-3 (P < 0.05),and there was no significant difference in Vmax,APD50 and APD9o among C and D1-3 groups (P > 0.05).VDD and RPF at T2 in group D2 + C and APA,VDD and RPF at T3 in groups D2 + Y and D2 + C were significantly lower than those at T1 (P < 0.05).APA,VDD and RPF in group D2 + Y and APA in group D2 + C were significantly lower at T3 than at T2 (P < 0.05).Conclusion Dexmedetomidine can decrease the sinoatrial node autorhythmicity in a concentration-dependent manner in rabbits,and the mechanism may be related to the inhibition of pacemaker current,but not alpha 2-adrenoreceptor.
6.Practice and experience of training model based on automated breast ultrasound system

The new ultrasound device, automated breast ultrasound (ABUS), has the advantages of repeatable reading of standardized whole breast images, and can separate ultrasound scanning and diagnosis. ABUS is more suitable for breast ultrasound training. This study compared the breast ultrasound imaging training model with ABUS diagnosis practice as the core and the traditional breast ultrasound imaging training model with handhold ultrasound operation and diagnosis as the core, and found that the new training model with ABUS diagnosis practice as the core had a short training period. The diagnostic level of doctors have improved rapidly, and the new training model could efficiently train more technically qualified ultrasound doctors for breast disease.
7.Effects of iron overload on cognitive function and autophagy of hippocampal neurons in rats

Objective:To explore the effect of iron overload on the cognitive function of rats and its possible internal mechanism.Methods:Thirty 8-week-old male Sprague Dawley rats of SPF degree were randomly divided into 2 groups, iron overload group(IO group) and control group(Sham group), with 15 in each group.The rats in IO group were injected intraperitoneally iron dextran(100 mg/(kg·d)) for 28 days.The cognitive function of rats was detected by Morris water maze method. Western blot method was used to detect the expression of TfR1 and autophagy-related protein p-AMPK, LC3 and Beclin1 in the hippocampus of rats. Immunofluorescence was used to observe the expression of LC3 and Beclin1 in the hippocampus of rats. HE staining was used to observe the morphological changes of neurons in the hippocampus. Transmission electron microscopy was used to observe the number of autophagosomes and the morphology of endoplasmic reticulum in hippocampus.The software of SPSS 20.0 was used for repeated measurement ANOVA and t-test. Results:Morris water maze test showed that there were significant interaction between the group factor and training time factor of escape latency( F=3.55, P<0.01). And the simple effect analysis showed that compared with the Sham group((28.09±18.41)s, (21.42±15.53)s, (16.96±8.35)s, (10.24±3.75)s), the average escape latency of rats(2nd-5th day) in IO group((56.68±30.65)s, (58.21±36.09)s, (36.58±13.54)s, (27.29±14.30)s )were significantly longer ( t=8.57, 6.81, 9.51, 7.12, P<0.01). The platform was removed on 6th day of the space exploration experiment, compared with the Sham group ((41.89±3.89)%), the percentage of time spent in the target quadrant of IO group ((25.46±3.56)%) was significantly decreased( t=24.06, P<0.01). Western blot showed that the relative expression levels of (TfR1 (2.10±0.48), p-AMPK (0.74±0.10), LC3 (1.11±0.40), Beclin1 (1.05±0.20)) in IO group in the hippocampus of the rats were significantly higher than those of the Sham group(TfR1(0.11±0.18), p-AMPK(0.19±0.02), LC3(0.22±0.11), Beclin1(0.17±0.02))( t=1.58, 14.58, 10.06, 20.65, P<0.01)). HE staining showed that compared with the Sham group, the neuron in the hippocampus of the IO group were sparsely arranged, morphologically irregular, and the number of the neurons was significantly reduced. Transmission electron microscopy showed that compared with the Sham group, the number of autophagosomes in the hippocampus of IO group was increased. Conclusion:Iron overload may exert its neurotoxic effect by increasing the level of autophagy in the hippocampus, causing cognitive dysfunction.
8.Application of MoCA and MMSE in screening for cognitive impairment in acute ischemic stroke

Objective To compare the applicability of the Beijing Version of the Montreal Cognitive Assessment (MoCA) and the Mini Mental State Examination (MMSE) in screening for cognitive impairment in patients with acute ischemic stroke for 2-3 weeks.Methods MoCA and MMSE were conducted in 201 patients with acute ischemic stroke within 2 to 3 weeks after the onset of stroke.With MoCA<23 and MMSE <26 as the cut off value,we assessed the clinic effect of the MoCA and MMSE and explored the correlation between two instruments.Results The average scores of MoCA and MMSE scale were (20.5±4.3) and (25.4±3.5) points.The prevalence of cognitive impairment evaluated with MoCA and MMSE were 57.2%and 43.3%,respectively.MoCA showed significant correlation with MMSE score (Pearson's correlation coefficient=0.833,P<0.001),and an agreement with Kappa values of 0.532 (P<0.01) in screening for cognitive impairment.Conclusions The prevalence of cognitive impairment assessed with MoCA is higher than that of with MMSE when using MoCA<23 and MMSE<26 as the cut off values.Both instruments show a good agreement for screening cognitive impairment in acute ischemic stroke within 2 to 3 weeks following the disease onset.
9.A clinical study on capecitabine maintenance treatment after combination chemotherapy to the patients with recurrent and metastatic breast cancer

Objective To investigate the therapeutic effect,safety and its prognostic factors of capecitabine as maintenance treatment agent for prolonging the PFS of patients with recurrent and metastatic breast cancer after they received combination chemotherapy.Methods From January 2011 to June 2013,38 cases with recurrent and metastatic breast cancer were collected in the department of medical oncology of the First Hospital of Lanzhou University.All the 38 patients received NX scheme (vinorelbine combined capecitabine chemotherapy),and some patients among of them had stabile disease after chemotherapy and were administered X scheme (capecitabine,twice a day,2 000 mg/m2 daily,withdrawal for 7 days after a consecutive intake of 14 days,21 days as a cycle,at least 2 cycles) until disease progressed or toxicity could not be tolerated.Adverse reactions and PFS were observed and recorded.Single factor chi square test and multivariate COX proportion hazard model were used to evaluate the relationships between clinic features and RR,PFS.Results The overall response rate (CR + PR) was 55.26% (21/38),clinical benefit patients rate (CR + PR + SD) was 84.2% (32/38),with 4 patients of CR (4/32),17 patients of PR,11 patients of SD,6 patients of PD.Thirty-two no progressived patients were administered capecitabine until PD.The median PFS was 10.0 months.Stratification analysis showed that patients whose Karnofsky (KPS) ≥80 had an average PFS of 14.1 months,while an average PFS of 6.8 months for patients whose KPS < 80,with a statistical significance (x2 =6.251,P =0.000).Cox proportion hazard model also showed that age (RR =3.561,95% CI:1.372-5.216,.x2 =4.025,P =0.031),menopausal status (RR =1.895,95 % CI:1.124-4.452,x2 =5.725,P =0.048),KPS score (RR =4.553,95% CI:1.131-7.703,x2 =11.205,P =0.005),the number of metastasis (RR =5.781,95% CI:2.321 ~11.243,x2 =3.925,P =0.011) were important prognostic factors for the patients with breast cancer.Major treatment-related adverse reaction was grade Ⅰ-Ⅱ hand-foot syndrome.One patient discontinued treatment because of grade Ⅲ hand-foot syndrome.Conclusion Capecitabine as maintenance treatment can significantly prolong the PFS of patients with recurrent and metastatic breast cancers at remission or stable stage after combination chemotherapy with a better tolerance.Age of patients,menopausal status,KPS score,the number of metastasis are the prognostic factors for the efficacy of NX-X regimen.
10.Correlation of Fatigue with Health-promoting Lifestyle in Undergraduate Students

Objective To explore the correlation of fatigue with health-promoting lifestyle in the udnergraduate students, so as to provide scientific evidence for the intervention. Methods A cross-section trial was carried out in the undergraduate students from a medical university in Guangzhou. By a self-designed questionnaire, the incidence of fatigue in the undergraduate students was figured out after data processing and statistical analysis. Results A total of 6 100 questionnaires were released, and 5 843 were responded, with a respondent rate being 95.79%. The number of effective questionnaires arrived 5 676. The results showed that the prevalence of fatigue among undergraduate students was 49.6%. The female had a higher risk than the male ( 51.6% vs 45.8%, P<0.01). The prevalence of fatigue in grade 1-3 students was 49.4%, 48.1% and 52.1% , respectively, the difference being insignificant among the three grades (P>0.05). The students with poor level of health-promoting lifestyle had a higher risk of suffering from fatigue (OR=6.647, 95%CI: 4.913-8.993). Conclusion Fatigue is prevalent among the undergraduate students. The incidence of fatigue can be reduced by increasing the level of self-actualization, physical exercise and pressure management.
